Ultrasound-guided paravertebral nerve block anesthesia for percutaneous endoscopic laser unroofing treatment of symptomatic simple renal cysts—An innovative ambulatory surgery mode

Objective:This study was designed to evaluate the feasibility,efficacy,and safety of percutaneous ureteroscopy laser unroofing as an ambulatory surgery for symptomatic simple renal cysts under multilevel paravertebral nerve block anesthesia.Methods:From December 2015 to September 2017,33 simple renal cyst patients who had surgical indications were enrolled.Under ultrasound guidance,the T10/T11,T11/T12,and T12/L1 paravertebral spaces were identified,and 7-10 mL 0.5%ropivacaine was injected at each segment.Then a puncture needle was placed inside the cyst cavity under ultrasonic monitoring.A guidewire was introduced followed by sequential dilation up to 28/30 Fr.The extra parenchymal portion of the cyst wall was dissociated and incised using a thulium laser,and a pathological examination was performed.Results:Sensory loss to pinprick from T8 to L1 and sensory loss to ice from T6 to L2 were observed in all patients.None of the patients complained of pain during surgery.No serious complications occurred perioperatively.After the surgery,all patients recovered their lower limb muscle strength quickly,got out of bed,resumed oral feeding,and left the hospital within 24 h of admission.The pathologic diagnosis of all cyst walls was a simple renal cyst.The mean follow-up was 35.8 months.At the end of follow-up,the cyst units were reduced in size by more than 50%compared to the preoperative size,and no patient experienced a recurrence.Conclusion:Multi-level paravertebral nerve block for percutaneous ureteroscopy laser unroofing as an ambulatory surgery mode is feasible,safe,and effective for the treatment of simple renal cysts in selected patients.

Walnut oil is a functional wood oil known to researchers that may potentially be a large source of Chinese edible oils.There are various extraction methods for walnut oil,including traditional(pressing,solvent-and enzymeassisted extraction)and novel methods(microwave,ultrasound,supercritical CO_(2),subcritical and other extraction technologies).Walnut oil is rich in nutrients,including phytosterols,tocopherols,polyphenols,squalene and minerals.It provides many health benefits,such as antioxidant,antitumor,anti-inflammatory,antidiabetic and lipid metabolism-related functions.In addition,the authentication of walnut oil has received much research attention.The present review provides detailed research on walnut oil extraction,composition,health benefits and adulteration identification methods.The path toward further walnut oil improvement in the context of the market value of walnut oil is also discussed.

Opinion consensus incorporating higher-order interactions in individual-collective networks

In the current information society, the dissemination mechanisms and evolution laws of individual or collective opinions and their behaviors are the research hot topics in the field of opinion dynamics. First, in this paper, a two-layer network consisting of an individual-opinion layer and a collective-opinion layer is constructed, and a dissemination model of opinions incorporating higher-order interactions(i.e. OIHOI dissemination model) is proposed. Furthermore, the dynamic equations of opinion dissemination for both individuals and groups are presented. Using Lyapunov's first method,two equilibrium points, including the negative consensus point and positive consensus point, and the dynamic equations obtained for opinion dissemination, are analyzed theoretically. In addition, for individual opinions and collective opinions,some conditions for reaching negative consensus and positive consensus as well as the theoretical expression for the dissemination threshold are put forward. Numerical simulations are carried to verify the feasibility and effectiveness of the proposed theoretical results, as well as the influence of the intra-structure, inter-connections, and higher-order interactions on the dissemination and evolution of individual opinions. The main results are as follows.(i) When the intra-structure of the collective-opinion layer meets certain characteristics, then a negative or positive consensus is easier to reach for individuals.(ii) Both negative consensus and positive consensus perform best in mixed type of inter-connections in the two-layer network.(iii) Higher-order interactions can quickly eliminate differences in individual opinions, thereby enabling individuals to reach consensus faster.

Activation of nuclear factor erythroid 2-related factor 2(Nrf2)by Kelch-like ECH-associated protein 1(Keap1)alkylation plays a central role in anti-inflammatory therapy.However,activators of Nrf2 through alkylation of Keap1-Kelch domain have not been identified.Deoxynyboquinone(DNQ)is a natural small molecule discovered from marine actinomycetes.The current study was designed to investigate the anti-inflammatory effects and molecular mechanisms of DNQ via alkylation of Keap1.DNQ exhibited significant anti-inflammatory properties both in vitro and in vivo.The pharmacophore responsible for the anti-inflammatory properties of DNQ was determined to be theα,β-unsaturated amides moieties by a chemical reaction between DNQ and N-acetylcysteine.DNQ exerted anti-inflammatory effects through activation of Nrf2/ARE pathway.Keap1 was demonstrated to be the direct target of DNQ and bound with DNQ through conjugate addition reaction involving alkylation.The specific alkylation site of DNQ on Keap1 for Nrf2 activation was elucidated with a synthesized probe in conjunction with liquid chromatography-tandem mass spectrometry.DNQ triggered the ubiquitination and subsequent degradation of Keap1 by alkylation of the cysteine residue 489(Cys489)on Keap1-Kelch domain,ultimately enabling the activation of Nrf2.Our findings revealed that DNQ exhibited potent anti-inflammatory capacity throughα,β-unsaturated amides moieties active group which specifically activated Nrf2 signal pathway via alkylation/ubiquitination of Keap1-Kelch domain,suggesting the potential values of targeting Cys489 on Keap1-Kelch domain by DNQ-like small molecules in inflammatory therapies.

Analysis and Prediction Model Reinforced UHPC Shrinkage Property

This paper explores the shrinkage of reinforced UHPC under high-temperature steam curing and natural curing conditions.The results are compared with the existing shrinkage prediction models.The results show that the maximum shrinkage strain of reinforced UHPC after steam curing is 164μεand gradually becomes zero.As for natural curing,the maximum shrinkage strain is 173μεand the value stabilizes on the 10th day after pouring.This indicated that steam curing can significantly reduce shrinkage time.Compared with the plain UHPC tested in the previous literature,the structural reinforcement can significantly inhibit the UHPC shrinkage and greatly reduce the risk of cracking due to shrinkage.By comparing the results in this paper with the existing models for predicting the shrinkage strain development,it is found that the formula recommended in the French UHPC structural and technical specification is suitable for the shrinkage curve in the present paper.

The ternary combination of polymer gel, microsphere and surfactant for conformance control and oil displacement to improve oil recovery in strong heterogeneous reservoir

The reservoir heterogeneity of Suizhong 36-1 oilfield is very obvious,the average permeability reaches 2000mD.Long-term water injection development leads to serious water channeling in high permeability layers,resulting in ineffective circulation of injected water and reduction of oil recovery.It is urgent to form an effective EOR method.In this paper,the oil displacement effect of different combination slugs including polymer gel,polymer microsphere and surfactant is evaluated by means of conformance control and chemical flooding.Three-layer heterogeneous 3-D plate model is used for oil displacement experiment.The experimental results show that the combination slug sequentially composed of polymer gel,polymer microspheres and surfactant have the best oil displacement effect with the oil recovery increment by 21.2% after the first water flooding.In addition,using the best slug combination for two rounds flooding,the recovery factor increased by 28.2% compared with the first water flooding.This paper provides some new insights for enhanced oil recovery in strong heterogeneity reservoir.

Comprehensive evaluation of chemical breakers for multistage network ultra-high strength gel

Polymer gels have been accepted as a useful tool to address many sealing operations such as drilling and completion,well stimulation,wellbore integrity,water and gas shutoff,etc.Previously,we developed an ultra-high strength gel(USGel)for medium to ultra-low temperature reservoirs.However,the removal of USGel is a difficult problem for most temporary plugging operations.This paper first provides new insights into the mechanism of USGel,where multistage network structure and physical entanglement are the main reasons for USGel possessing ultra-high strength.Then the effects of acid breakers,encapsulated breakers,and oxidation breakers(including H_(2)O_(2),Na_(2)S_(2)O_(8),Ca(ClO)_(2),H_(2)O_(2)+NaOH,Na_(2)S_(2)O_(8)+NaOH,and Ca(ClO)_(2)+NaOH)were evaluated.The effects of component concentration and temperature on the breaking solution were studied,and the corrosion performance,physical simulation and formation damage tests of the breaking solution were carried out.The final formulation of 2%-4%NaOH+4.5%-6%H_(2)O_(2) breaking solution was determined,which can make USGel completely turn into water at 35e105C.The combinations of“acid t breaking solution”,“acid+encapsulated breaker”and“encapsulated breaker+breaking solution”were evaluated for breaking effect.The acid gradually reduced the volume of USGel,which increased the contact area between breaking solution and USGel,and the effect of“4%acid+breaking solution”was 23 times higher than that of breaking solution alone at 35C.However,the acid significantly reduced the strength of USGel.This paper provides new insights into the breaking of high-strength gels with complex network structures.

The Correlation Between Note Features and Consecutive Interpreting Quality for English Majors

Note-taking skill is a necessary component in interpreter training programs,and previous research has yielded findings such as note-taking training methods or features of interpreter trainees’notes.However,little research has been done to investigate the changes in note features and correlations between note features and interpreting quality concerning Chinese students’C-E(Chinese-English)and E-C(EnglishChinese)interpreting.Using the framework of Daniel Gile’s Effort Model and Interpretive Theory of Translation,this paper examined how 45 English Majors’notes develop within one semester(seventeen weeks)and the relationship between note features(quantity,form,and language choice of notes)and consecutive interpreting quality.The participants of this study were all beginner interpreting trainees,and the note-taking training was introduced in Week 6.The study employed note manuscripts,interpreting tests,and semi-structured interviews to track the features and changes in students’notes.Correlation analyses and T-tests showed that(a)after the note-taking training,the number of notes increased from Week 8 to Week 17,and it was positively correlated with interpreting quality(fidelity and delivery)for both C-E and E-C interpreting;(b)as for forms of notes,participants primarily employ single Chinese words and the percentages of abbreviations and symbols rose prominently from Week 8 to Week 17 for C-E interpreting.Besides,correlation analyses show that interpreting quality improves with fewer single Chinese words and more abbreviations and symbols.For E-C interpreting,notes were mainly in English,especially single English words and abbreviations.The percentages of single Chinese words and abbreviations ascended whereas those of single English words and symbols decreased.Furthermore,results show that the more abbreviations and symbols,the better target-text fidelity,and fewer abbreviations,the better the targettext delivery;(c)concerning language choice,notes were mainly in source language for both C-E and E-C interpreting a

Deep learning framework for comprehensive molecular and prognostic stratifications of triple-negative breast cancer

Triple-negative breast cancer(TNBC)is the most challenging breast cancer subtype.Molecular stratification and target therapy bring clinical benefit for TNBC patients,but it is difficult to implement comprehensive molecular testing in clinical practice.Here,using our multi-omics TNBC cohort(N=425),a deep learning-based framework was devised and validated for comprehensive predictions of molecular features,subtypes and prognosis from pathological whole slide images.The framework first incorporated a neural network to decompose the tissue on WSIs,followed by a second one which was trained based on certain tissue types for predicting different targets.Multi-omics molecular features were analyzed including somatic mutations,copy number alterations,germline mutations,biological pathway activities,metabolomics features and immunotherapy biomarkers.It was shown that the molecular features with therapeutic implications can be predicted including the somatic PIK3CA mutation,germline BRCA2 mutation and PD-L1 protein expression(area under the curve[AUC]:0.78,0.79 and 0.74 respectively).The molecular subtypes of TNBC can be identified(AUC:0.84,0.85,0.93 and 0.73 for the basal-like immune-suppressed,immunomodulatory,luminal androgen receptor,and mesenchymal-like subtypes respectively)and their distinctive morphological patterns were revealed,which provided novel insights into the heterogeneity of TNBC.A neural network integrating image features and clinical covariates stratified patients into groups with different survival outcomes(log-rank P<0.001).Our prediction framework and neural network models were externally validated on the TNBC cases from TCGA(N=143)and appeared robust to the changes in patient population.For potential clinical translation,we built a novel online platform,where we modularized and deployed our framework along with the validated models.It can realize real-time one-stop prediction for new cases.In summary,using only pathological WSIs,our proposed framework can enable comprehensive stratifications of TNBC
