Squamous cell carcinoma arising from longstanding colocutaneous fistula: A case report

A 60-year-old female patient suffered unhealed wounds over left flank for around 30 years after surgical removal of left renal stones. Fecal material spilled from the two small openings of the scar, bothered her all day long. During the course of the 30 years, she suffered from intermittent fever, diarrhea and wound pain and presented with malnourished condition. After serial examinations, tumor associated with iatrogenic colo-cutaneous fistula was impressed and she received en bloc resection. Pathology revealed squamous cell carcinoma arising from the fistula with colon and spleen invasion. To the best of our knowledge,no such case has been reported, as yet.

This theoretical paper presents the influence of learning styles and culture values on facilitating undergraduate students＇ learning. Learning styles describe the way people process information. Many learning style inventories have been developed and applied widely, for example, Kolb＇s Learning Style Inventory （LSI） and Honey and Mumford＇s Learning Styles Questionnaire （LSQ）. However, limited research has been conducted in the field of hospitality and tourism higher education, particularly in eastern culture. Past research in the hospitality and tourism higher education did not reveal if certain learning styles can contribute to better learning outcomes, however, there is a significant difference in preferred learning styles of students in terms of different cultural values. Therefore, it makes it pivotal to consider the influence of culture difference when examining how learning styles contribute to better learning outcomes in future research. Therefore, hospitality and tourism educators can properly plan useful strategies to accommodate different learning styles and enhance students＇ learning.

The known factors that have contributed to the decline of Helicobacter pylori (H. pylori) eradication rate include antibiotic resistance, poor compliance, high gastric acidity, high bacterial load, and cytochrome P450 2C19 (CYP2C19) polymorphism. Proton pump inhibitor (PPI) is important in the eradication regimen. The principal enzyme implicated in the metabolism of PPIs is CYP2C19. The effects of PPI depend on metabolic enzyme, cytochrome P450 enzymes, and CYP2C19 with genetic differences in the activity of this enzyme (the homozygous EM, heterozygous EM (HetEM), and poor metabolizer). The frequency of the CYP2C19 polymorphism is highly varied among different ethnic populations. The CYP2C19 genotype is a cardinal factor of H. pylori eradication in patients taking omeprazole- based or lansoprazole-based triple therapies. In contrast, the CYP2C19 polymorphism has no significant effect on the rabeprazole-based or esomeprazole-based triple therapies. The efficacy of levofloxacin-based rescue triple therapy might be also affected by the CYP2C19 polymorphism, but CYP2C19 genotypes did not show obvious impact on other levofloxacin-based rescue therapies. Choice of different PPIs and/or increasing doses of PPIs should be individualized based on the pharmacogenetics background of each patient and pharmacological profile of each drug. Other possible factors influencing gastric acid secretion (e.g., IL-1β- 511 polymorphism) would be also under consideration.

Background： Q fever endocarditis, a chronic illness caused by Coxiella burnetii, can be fatal ifmisdiagnosed or left untreated. Despite a relatively high positive rate of Q fever serology in healthy individuals in the mainland of China, very few cases of Q fever endocarditis have been reported. This study summarized cases of Q fever endocarditis among blood culture negative endocarditis （BCNE） patients and discussed factors attributing to the low diagnostic rate. Methods： We identified confirmed cases of Q fever endocarditis among 637 consecutive patients with infective endocarditis （IE） in the Peking Union Medical College Hospital between 2006 and 2016. The clinical findings for each confirmed case were recorded. BCNE patients were also examined and each BCNE patient＇s Q fever risk factors were identified. The risk factors and presence of Q fever serologic testing between BCNE patients suspected and unsuspected of Q fever were compared using the Chi-squared or Chi-squared with Yates＇ correction for continuity. Results： Among the IE patients examined, there were 147 BCNE patients, of whom only 11 patients （7.5%） were suspected of Q fever and undergone serological testing for C. burnetii. Six out of 11 suspected cases were diagnosed as Q fever endocarditis. For the remaining 136 BCNE patients, none of them was suspected of Q fever nor underwent relevant testing. Risk factors for Q fever endocarditis were comparable between suspected and unsuspected patients, with the most common risk factors being valvulopathy in both groups. However, significantly more patients had consulted the Infectious Diseases Division and undergone comprehensive diagnostic tests in the suspected group than the unsuspected group （100% vs. 63%, P = 0.03）. Conclusions： Q fever endocarditis is a serious yet treatable condition. Lacking awareness of the disease may prevent BCNE patients from being identified, despite having Q fever risk factors. BACKGROUND Colonic transendoscopic enteral tubing(TET)requires double cecal intubation,raising a common concern of how to save cecal intubation time and make the tube *** hypothesized that cap-assisted colonoscopy(CC)might reduce the second cecal intubation time and bring potential benefits during the TET *** To investigate if CC can decrease the second cecal intubation time compared with regular colonoscopy(RC).METHODS This prospective multicenter,randomized controlled trial was performed at four ***≥7 years needing colonic TET were recruited from August 2018 to January *** subjects were randomly assigned to two *** primary outcome was the second cecal intubation *** outcomes included success rate,insertion pain score,single clip fixation time,purpose and retention time of TET tube,length of TET tube inserted into the colon,and all procedurerelated(serious)adverse *** A total of 331 subjects were randomized to the RC(n=165)or CC(n=166)*** median time of the second cecal intubation was significantly shorter for CC than RC(2.2 min vs 2.8 min,P<0.001).In patients with constipation,the median time of second cecal intubation in the CC group(n=50)was shorter than that in the RC group(n=43)(2.6 min vs 3.8 min,P=0.004).However,no difference was observed in the CC(n=42)and RC(n=46)groups of ulcerative colitis patients(2.0 min vs 2.5 min,P=0.152).The insertion pain score during the procedure in CC(n=14)was lower than that in RC(n=19)in unsedated colonoscopy(3.8±1.7 vs 5.4±1.9;P=0.015).Multivariate analysis revealed that only CC(odds ratio[OR]:2.250,95%confidence interval[CI]:1.161-4.360;P=0.016)was an independent factor affecting the second cecal intubation time in difficult *** did not affect the colonic TET tube’s retention time and length of the tube inserted into the ***,multivariate analysis found that only endoscopic clip number(OR:2.201,95%CI:1.541-3.143;P<0.001)was an independe

The European Center for Medium-Range Weather Forecast （ECMWF） Re-Analysis （ERA-40） and the National Centers for Environmental Prediction-National Center for Atmospheric Research （NCEP-NCAR） ECMWF （ERA-40） and NCEP–NCAR reanalysis data were compared with Antarctic station observations, including surface-layer and upper-layer atmospheric observations, on intraseasonal and interannual timescales. At the interannual timescale, atmospheric pressure at different height levels in the ERA-40 data are in better agreement with observed pressure than that in the NCEP–NCAR reanalysis data. ERA-40 reanalysis also outperforms NCEP–NCAR reanalysis in atmospheric temperature, except in the surface layer where the biases are somewhat larger. The wind velocity fields in both datasets do not agree well with surface-and upper-layer atmospheric observations. At intraseasonal timescales, both datasets capture the observed intraseasonal variability in pressure and temperature during austral winter.
