Factors Limiting the Forecast Skill of the Boreal Summer Intraseasonal Oscillation in a Subseasonal-to-Seasonal Model

In this study,we evaluate the forecast skill of the subseasonal-to-seasonal(S2S)prediction model of the Beijing Climate Center(BCC)for the boreal summer intraseasonal oscillation(BSISO).We also discuss the key factors that inhibit the BSISO forecast skill in this *** on the bivariate anomaly correlation coefficient(ACC)of the BSISO index,defined by the first two EOF modes of outgoing longwave radiation and 850-hPa zonal wind anomalies over the Asian monsoon region,we found that the hindcast skill degraded as the lead time *** ACC dropped to below 0.5for lead times of 11 days and longer when the predicted BSISO showed weakened strength and insignificant northward *** identify what causes the weakened forecast skill of BSISO at the forecast lead time of 11 days,we diagnosed the main mechanisms responsible for the BSISO northward *** same analysis was also carried out using the observations and the outputs of the four-day forecast lead that successfully predicted the observed northward-propagating *** found that the lack of northward propagation at the 11-day forecast lead was due to insufficient increases in low-level cyclonic vorticity,moistening and warm temperature anomalies to the north of the convection,which were induced by the interaction between background mean flows and BSISO-related anomalous *** BCC S2S model can predict the background monsoon circulations,such as the low-level southerly and the northerly and easterly vertical shears,but has limited capability in forecasting the distributions of circulation and moisture anomalies.

A conference key establishment protocol allows a group of conferees to agree on a secret key shared among them for secure group *** paper proposes a three-level conference key establishment protocol based on the Universal Mobile Telecommunications System (UMTS) framework to establish a group-level key,home location register (HLR) level keys,and visitor location register (VLR) level keys simultaneously for a group of *** group-level key is used to secure the communications for all conferees,the HLR-level key is for those within the same HLR domain,and the VLR-level key is for those within the same VLR *** group-level key can be used for securing inter-domain group-oriented applications such as commercial remote conferencing *** HLRand VLR-level keys can be used for securing intra-domain subgroup applications (e.g.,location-based or context-aware services) and dynamic key *** our proposed protocol exploits existing UMTS security functions and the exclusive-or operation,it is compatible with UMTS *** means that it is fast and easy to implement on the existing UMTS ***,the proposed protocol has low computational complexities and can provide cost effectiveness,load-amortization,scalability,user authentication,key establishment,key confirmation,key updating,and lawful interception.

Supercapacitors have been recognized as one of the promising energy storage devices in the future energy *** this perspective,rapid progress is made in the development of fundamental and applied aspects of *** techniques have been developed specifically to estimate the specific *** efforts have been made in the literature to increase the specific capacitance of electrode ***,researchers pay more attention on designing supercapacitors of asymmetric type with extending cell voltage and dissimilar materials with complementary working *** try to increase the specific energy of asymmetric supercapacitors(ASCs).Conversely,it is still a challenge to find a suitable operation conditions for ASCs in various designs,especially for the one with battery type *** this review,we describe our recent research works and other reports on the preparation of various nanostructured electrode materials and the performances of both symmetric and asymmetric ***,we demonstrate effects of charge balance on the capacitive performances of ASCs which consist of one electrode material of the battery type and one capacitive *** also demonstrate how to evaluate the charge capacities of both positive and negative electrode materials for this ASC application.

AIM： To evaluate the efficacy of thalidomide in com- bination with other therapies to treat patients with ad- vanced hepatocellular carcinoma （HCC）. METHODS： We performed a retrospective analysis of all patients with HCC who were treated with thalido- mide for at least two months. The medical records of patients with HCC who were treated at our institution between April 2003 and March 2008 were reviewed. Image studies performed before and after treatment, tumor response, overall survival, and the decrease in o-fetoprotein （AFP） levels were evaluated. RESULTS： A total of 53 patients with HCC received either 100 or 200 mg/d of thalidomide. The patient population consisted of 9 women and 44 men with a median age of 61 years. Thirty patients （56.6%） were classified as Child-Pugh A, and 12 patients （22.6%） were classified as Child-Pugh B. Twenty-six patients had portal vein thrombosis （49.1%）, and 25 patients had extrahepatic metastasis （47.1%）. The median duration of thalidomide treatment was 6.0 mo. Six of the 53 patients achieved a confirmed response （11.3%）, one achieved a complete response （1.9%） and 5 achieved a partial response （9.4%）. The disease control rate （CR ＋ PR ＋ SD） was 28.3% （95% CI： 17.8-42.4）, and the median overall survival rate was 10.5 too. The 1- and 2-year survival rates were 45% and 20%, respectively. Only one complete response patient showed an im- proved overall survival rate of 66.8 mo. Sixteen patients （30.2%） showed more than a 50% decrease in their serum AFP levels from baseline, indicating a better re- sponse rate （31.3%）, disease control rate （43.8%）, and overall survival time （20.7 mo）. The therapy was well tolerated, and no significant toxicities were observed. CONCLUSION： Thalidomide was found to be safe for advanced HCC patients, demonstrating anti-tumor ac- tivity including response, survival, and AFP decreases of greater than 50% from baseline.

Intestinal obstruction is a common complication associated with Meckel's diverticulum in adults. The diverticulum itself or its fibrous band can lead to an intestinal volvulus, intussusceptions, or closed-loop obstructions,which require surgery. The incarceration of Meckel's diverticulum in either inguinal or femoral hernia sacs(Littre's hernia) is another, less common, etiology underlying intestinal obstruction. This case report describes a 45-year-old man who had an obstruction associated with a Meckel's diverticulum that passed through a congenital defect in the mesocolon into the right subphrenic space. The patient, who had not undergone abdominal surgery previously, came to the emergency room with acute onset of intermittent epigastric pain and abdominal distention. Computed tomography images showed the presence of a segment of the small bowel and a diverticulum in the right subphrenic space and paracolic gutter. The twisted mesentery and the dilated loops of the proximal small bowel were indicative of an intestinal volvulus and obstruction. Meckel's diverticulum complicated by a transmesocolic internal hernia was diagnosed, and this condition was confirmed during emergency surgery. The patient's postoperativerecovery was uneventful. This case report highlights another presentation of Meckel's diverticulum, that is, in combination with a transmesocolic internal hernia. This etiology may lead to an intestinal volvulus and necessitate early surgery.

Hepatitis B virus （HBV） reactivation and recurrence are common in patients undergoing immunosuppression therapy. Tumor necrosis factor （TNF） blockage therapy is effective for the treatment of many autoimmune inflammatory diseases. However, the role of TNF-a blockage therapy in the innate and adaptive immune responses against HBV is still not clear. A detailed analysis of HBV infection under TNF-a blockage therapy is essential for the prophylaxis and therapy for HBV reactivation and recurrence. In this study, HBV clearance and T-cell responses were analyzed in a HBV-transfected mouse model under anti-TNF blockage therapy. Our results demonstrated that under TNF-a blockage therapy, HBV viral clearance was impaired with persistent elevated HBV viral load in a dose- and temporal-dependent manner. The impairment of HBV clearance under anti-TNF-a blockage therapy occurred at early time points after HBV infection. In addition, TNF-a blockade maintained a higher serum HBV viral load and increased the number of intrahepatic programmed cell death （PD）- 1highCD127low exhausted T cells. Furthermore, TNF-a blockade abolished Toll-like receptor 9 （TLR9） ligand-induced facilitation of HBV viral clearance. Taken together, TNF-a blockade impairs HBV clearance and enhances viral load, and these effects depend on early administration after HBV infection. Our results here demonstrate that early TNF-a blockade reduces viral clearance and persistently maintains elevated HBV viral load in a mouse model, suggesting that HBV may reactivate during theraoy with TNF-a-blocking agents.

Optimal design theory for linear tuned mass dampers （TMD） has been thoroughly investigated, but is still under development for nonlinear TMDs. In this paper, optimization procedures in the time domain are proposed for design of a TMD with nonlinear viscous damping. A dynamic analysis of a structure implemented with a nonlinear TMD is conducted first. Optimum design parameters for the nonlinear TMD are searched using an optimization method to minimize the performance index. The feasibility of the proposed optimization method is illustrated numerically by using the Taipei 101 structure implemented with TMD. The sensitivity analysis shows that the performance index is less sensitive to the damping coefficient than to the frequency ratio. Time history analysis is conducted using the Taipei 101 structure implemented with different TMDs under wind excitation. For both linear and nonlinear TMDs, the comfort requirements for building occupants are satisfied as long as the TMD is properly designed. It was found that as the damping exponent increases, the relative displacement of the TMD decreases but the damping force increases.

There have been some breakthroughs in the diagnosis and treatment of esophageal achalasia in the past few ***,the introduction of high-resolution manometry with pressure topography plotting as a new diagnostic tool has made it possible to classify achalasia into three *** most favorable outcome is predicted for patients receiving treatment for type Ⅱ achalasia (achalasia with compression).Patients with typeⅠ(classic achalasia) and type Ⅲ achalasia (spastic achalasia) experience a less favorable ***,the first multicenter randomized controlled trial published by the European Achalasia Trial group reported 2-year follow-up results indicating that laparoscopic Heller myotomy was not superior to endoscopic pneumatic dilation (PD).Although the follow-up period was not long enough to reach a convincing conclusion,it merits the continued use of PD as a generally available technique in ***,the novelendoscopic technique peroral endoscopic myotomy is a promising option for treating achalasia,but it requires increased experience and cautious *** all this good news,the bottom line is a real break-through from the basic studies to identify the actual cause of achalasia that may impede treatment success is still anticipated.

AIM:To study potential predictive factors for early radical resection in two-stage resection for left malignant colonic ***:Thirty-eight cases of left-sided obstructive colon cancer undergoing two-stage operations were reviewed between January 1998 and August *** were classified into two groups(n = 19 each):early radical resection(interval ≤ 10 d) and late radical resection(interval > 10 d).Baseline demographics,post-diversion outcome,perioperative data,tumor characteristics,outcome and complications were ***:The baseline demographics revealed no differences except for less pre-diversion sepsis in the early group(P < 0.001) and more obstruction days in the late group(P = 0.009).The mean intervals of early and late radical resections were 7.9 ± 1.3 d and 17.8 ± 5.5 d,respectively(P < 0.001).After diversion,the presence of bowel sounds,flatus,removal of the nasogastric tube and the resumption of oral feeding occurred earlier in the early *** operation time and duration of hospital stay were both significant reduced in the early *** rates did not differ between ***:The earlier recovery of bowel function seems to be predictive of early radical *** contrast,pre-diversion sepsis and more obstruction days were predictive of delayed radical resection.
