目的探讨中医调神补肺针刺法联合体外膈肌起搏器治疗慢性阻塞性肺疾病急性发作期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)并发肺性脑病的效果。方法选取该院收治的110例AECOPD并发肺性脑病患者作为研究对象,采用随机数字表分为针刺组、对照组各55例,两组均给予无创正压通气(non-invasive positive pressure ventilation,NIPPV)等基础疗法+体外膈肌起搏器治疗,针刺组患者同时接受中医调神补肺针刺法;比较两组患者治疗前后的血气参数、格拉斯哥昏迷评分(Glasgow coma scale,GCS)、中医相关证候评分、治疗结局。结果治疗前,针刺组和对照组的动脉血氧分压(arterial oxygen partial pressure,pao_(2))、二氧化碳分压(partial pressure of carbon dioxide,PCO_(2))、pH值、血氧饱和度(blood oxygen saturation,SaO_(2))测定值比较,两组差异均无统计学意义(P>0.05);针刺组在治疗2 d、3 d的pao_(2)测定值高于对照组(P0.05);针刺组在治疗2 d、治疗3 d的GCS评分高于对照组,差异具有统计学意义(P0.05);针刺组在治疗5 d的中医证候积分低于对照组,差异具有统计学意义(P0.05);针刺组的NIPPV时间、住院时间均低于对照组(P<0.05)。结论中医调神补肺针刺法联合体外膈肌起搏器治疗AECOPD并发肺性脑病有利于早期改善患者血气水平、意识状态,减少患者接受辅助呼吸及住院的时间。

目的:探讨支气管肺泡灌洗联合比阿培南降阶梯疗法对慢性阻塞性肺疾病(COPD)伴重症肺炎患者的治疗效果。方法:选取2020年2月~2023年2月期间某院收治的80例COPD伴重症肺炎患者作为研究对象,采用随机数字表法分为对照组和观察组,每组40例。对照组患者给予比阿培南降阶梯疗法治疗,观察组患者在对照组治疗基础上给予支气管肺泡灌洗。比较两组患者血气分析指标[血氧饱和度(SO_(2))、动脉血氧分压(pao_(2))、动脉血二氧化碳分压(PaCO_(2))]、炎症指标[白细胞计数(WBC)、C反应蛋白(CRP)、白介素-6(IL-6)、降钙素原(PCT)]、肺功能指标[第1秒用力呼气容积(FEV_(1))、呼气流量峰值(PEF)、第1秒用力呼气容积/用力肺活量(FEV_(1)/FVC)]、临床疗效及不良反应发生情况。结果:治疗后,两组患者SO_(2)、pao_(2)均升高(P0.05)。结论:支气管肺泡灌洗联合比阿培南降阶梯疗法治疗COPD伴重症肺炎患者临床疗效确切,可有效改善患者肺功能,减轻炎症反应,且未增加不良反应的发生风险。

Tribological and rheological performance of lithium grease with poly-a-olefin and alkyl-tetralin as base oils

A new category of lithium greases was synthesized by using poly-a-olefin(pao8) and alkyl-tetralin as base oil, where the alkyl-tetralins were synthesized by the alkylation of tetralin and olefins. The influence of thickener concentration, alkyl-tetralin content and type of blend oils on the rheological and tribological performance of lithium grease was investigated. The microstructures of soap fibers were measured to reveal the structure-property correlations. The concentration of thickener and alkyl-tetralin content obviously affect the lubricating performance of lithium grease, while the molecular structure of alkyltetralin has no obvious impact on their properties. It was found that alkyl-tetralin could significantly enhance the thickening ability of pao8 base oils, and decrease the amount of thickeners by 1.5%(mass).Lithium greases prepared using 20%(mass) alkyl-tetralin as co-base oil exhibited high colloidal stability,excellent rheological behaviors and tribological properties.

Background Long-term maintenance of sinus rhythm after successful conversion of chronic atrial fibrillation （CAF）, often ameliorates patients＇ symptoms, reduces the risk of ischemic stroke and improves cardiovascular hemodynamics. This prospective study aims to evaluate the long-term efficacy and safety of very low-dose amiodarone （100 mg daily） for the maintenance of sinus rhythm after successful direct-current （DC） cardioversion in patients with CAF and rheumatic heart disease （RHD） post intervention. Methods This study was a randomized prospective trial. One day after successful DC cardioversion （remained normal sinus rhythm） in patients with CAF and RHD post intervention for more than six months and adequate anticoagulation, all were randomly administered either amiodarone 200 mg daily in group A or amiodarone 100 mg dally in group B. Results A total of 76 patients （40 men and 36 women） were examined from February 1998 to December 1999. The mean age of the patients was （664- 10） years, and the mean follow-up was （674-8） months （range 61 to 84 months）. Actuarial rates of the maintenance of sinus rhythm were similar in the two groups after 5 years of follow-up. Four patients （11%） in group A but none in group B experienced significant adverse effects that necessitated withdrawal of amiodarone. No death occurred during the study period. Conclusion A very low dose of amiodarone results in adequate long-term efficacy and is safe for maintaining sinus rhythm in patients with CAF and RHD post intervention after successful DC cardioversion.

The production of poly-α-olefins(paos)has attracted attention due to their excellent viscosity-temperature dependence,wear characteristics,oxidative properties,and high thermal *** this study,indene extracted during coal tar refining was used as a raw material to synthesize a bis(indenyl)zirconium dichloride metallocene catalyst.A pao with low viscosity and a high viscosity index was produced via the oligomerization of 1-decene in the presence of both the prepared metallocene and a methylaluminoxane(MAO)***,the effects of different synthesis reaction parameters,such as Al:Zr ratio,amount of catalyst,and reaction temperature,on the conversion ratio and product selectivity were investigated in *** produced pao was thoroughly characterized using Fourier-transform infrared,^(13)C,and^(1)H nuclear magnetic resonance spectroscopies;gas chromatography;and viscosity *** 70℃,the metallocene catalyst created more stable active *** addition,the alkylation effect of MAO was ***,the obtained catalysis results demonstrated that a high conversion ratio of~93%was achieved at a low reaction temperature of 70℃,with a catalyst dosage of 0.0848 mmol and Al:Zr ratio of 8.48mmol:***,under these optimal conditions,the kinematic viscosity of pao was 4.25 mm2/s at 100℃,and the viscosity index was 139,indicating good viscosity-temperature properties.

目的探讨无创通气治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭患者的临床效果,为临床提供可借鉴经验。方法将2021年6月—2022年9月聊城市茌平区人民医院收治的COPD合并呼吸衰竭患者75例作为研究对象,按照随机数字表法分为对照组37例和观察组38例,对照组实施常规治疗,观察组实施无创通气治疗。比较两组临床疗效和治疗前后血气指标改善情况。结果观察组治疗总有效率为94.74%,高于对照组的78.38%,差异有统计学意义(χ^(2)=4.341,Ppao_(2)和PaCO_(2))比较差异无统计学意义(P>0.05);治疗后,两组患者pao_(2)均升高,PaCO_(2)均降低,且观察组pao_(2)高于对照组,PaCO_(2)低于对照组,差异均有统计学意义(P<0.05)。结论应用无创通气治疗COPD合并呼吸衰竭患者可提高临床效果,有利于改善患者血气状态和病情康复,值得临床推广应用。

The Dormancy-associated MADS-box(DAM)gene cluster in peach serves as a key regulatory hub on which the seasonal temperatures act and orchestrate dormancy onset and exit,chilling response and floral bud developmental ***,how different temperature regimes interact with and regulate the six linked DAM genes remains ***,we demonstrate that chilling downregulates DAM1 and DAM3–6 in dormant floral buds with distinct patterns and identify DAM4 as the most abundantly expressed *** reveal multiple epigenetic events,with tri-methyl histone H3 lysine 27(H3K27me3)induced by chilling specifically in DAM1 and DAM5,a 21-nt sRNA in DAM3 and a ncRNA induced in *** induction is inversely correlated with downregulation of their cognate *** also show that the six DAMs were hypermethylated,associating with the production of 24-nt ***,the chilling-responsive dynamic of the different epigenetic elements and their interactions likely define distinct expression abundance and downregulation pattern of each *** further show that the expression of the five DAMs remains steadily unchanged or continuously downregulated at the ensuing warm temperature after chilling,and this state of regulation correlates with robust increase of sRNA expression,H3K27me3 and CHH methylation,which is particularly pronounced in *** robust increase of repressive epigenetic marks may irreversibly reinforce the chillingimposed repression of DAMs to ensure flower-developmental programming free from any residual DAM *** together,we reveal novel information about genetic and epigenetic regulation of the DAM cluster in peach,which will be of fundamental significance in understanding of the regulatory mechanisms underlying chilling requirement and dormancy release,and of practical application for improvement of plasticity of flower time and bud break in fruit trees to adapt changing climates.
