AIM: To investigate the prognostic value of the expression of connexin (Cx) 26, 32 and 43 messenger RNA (mRNA) in hepatocellular carcinoma (HCC) tissues. METHODS: Using a reverse-transcriptase polymerase chain reaction (RT-PCR), Cx 26, Cx 32 and Cx 43 mRNAs were determined in the liver tissues of 15 controls and in HCC tissues of 25 patients undergoing curative hepatic resection. The patients were followed up ***: Cx 26 and Cx 32 mRNAs were significantly lower in HCC tissues compared with controls (both P<0.01). By multivariate analysis, a lower level of Cx 26 and Cx 32 mRNA correlated significantly with a risk of HCC recurrence (P = 0.033) and recurrence-related mortality (P = 0.031,P= 0.031). Cx 43 mRNA was higher in HCC tissues compared with controls but did not correlate with postoperative recurrence or recurrence-related mortality. Other significant predictors of HCC recurrence included cellular dedifferentiation (P = 0.033), less encapsulation (P = 0.050), vascular permeation (P= 0.046), and daughter nodules (P= 0.046). Significant variables related to recurrence-related mortality consisted of cell dedifferentiation (P = 0.031), vascular permeation (P = 0.048), and daughter nodules (P = 0.048).The levels of Cx 26 and Cx 32 mRNAs correlated significantly with cell differentiation (P= 0.031). CONCLUSION: A low expression of Cx 26 and Cx 32 mRNAs in HCC tissues is predictive of postoperative recurrence of HCCs.

AIM To evaluate the clinical and radiographic results of patients with complicated infectious spondylitis treated with single-stage anterior debridement and reconstruction using tantalum mesh cage(TaMC) followed by immediate *** Single-stage radical debridement and subsequent reconstruction with TaMC instead of autograft or allograft were performed to treat 20 patients with spinal deformity or instability due to complicated infectious spondylitis. Clinical outcomes were assessed by careful physical examination and regular serological tests to determine the infection control. In addition, the visual analog score(VAS), neurologic status, length of vertebral body reconstruction, and the correction of sagittal Cobb angle on radiography were recorded and compared before and after surgery. The conditions of the patients were evaluated based on the modified Brodsky's *** The average VAS score significantly decreased after the surgery(from 7.4 ± 0.8 to 3.3 ± 0.8, P < 0.001). The average Cobb angle correction was 14.9 degrees. The neurologic status was significantly improved after the surgery(P = 0.003). One patient experienced refractory infection and underwent additional debridement. Eighteen patients achieved good outcome based on the modified Brodsky's criteria and significant improvement after the surgery(P < 0.001). No implant breakage orTaMC dislodgement was found during at least 24 mo of *** Single-stage anterior debridement and reconstruction with TaMC followed by immediate instrumentation could be an alternative method to manage the patients with spinal deformity or instability due to complicated infectious spondylitis.

AIM: To evaluate how long patients with small bowel obstruction caused by postoperative adhesions can tolerate conservative ***: The records of patients with small bowel obstruction due to postoperative adhesions were retrospectively reviewed. Data collected included the number of admissions, type of management for each admission,duration of conservative treatment, number of repeat laparotomies, and operative ***: One hundred fifty-five patients with this condition from January 1999 to December 2001, for a total of 293 admissions were enrolled in this study. Medical treatment alone was given in 220 admissions, and repeat laparotomy was performed in 73 admissions. The period of observation in patients managed medically ranged from 2 to 12 days (average: 6.9 days), while for those who underwent surgery,the range was 1 to 14 days (average 5.4 days). At surgery,adhesions were the only finding in 46 cases, while there were intestinal complications in 27, or 9.2 % of all 293admissions. Fever and leukocytosis greater than 15 000/mm3were prediction of intestinal ***: With closely monitoring, most patients with small bowel obstruction due to postoperative adhesions could tolerate supportive treatment and recover well averagely within 1 week, although some patients require more than 10 days of observation.

Introduction: There is a need for the development of a new approach to establish an oral and maxillofacial cancer model to study the tumorgenesis and its route for metastasis. The purpose of this study is to use Vx2-induced squamous cell carcinoma directly transplanted into both facial (parotid area) and submandibular of rabbits to examine the cytology and histology of the induced tumors and their surrounding oral and maxillofacial area including cervical lymph nodes and distant metastasis. Materials& Methods Five adult male New Zealand White rabbits (weight: 2.5-3.5 kg) received Vx2 solid pieces implantation into both left facial (parotid area) and right submandibular areas. The size, shape of tumors and body weight of the rabbits were recorded weekly. Aspiration cytology and autopsy were performed after 6 weeks of inoculation. The primary tumor mass, cervical lymph nodes, lung, and other sites of metastasis were assessed and examined histologically. Results Grossly palpable tumor masses (up to 5 cm×5 cm and 3 cm×3 cm) were found on both facial and submandibular areas after 4 weeks of tumor implantation. The size of tumor mass increased up to 10 cm×10 cm with ulcerated & bleeding surface on submandibular area; also a tumor mass (6 cm×6 cm) were found on maxillofacial (parotid area) and buccal tumor with ulcerations after 6 weeks inoculation. In addition, multiple cervical lymph nodes and pulmonary metastases were detected grossly and microscopically on all the experimental specimens. The cytology and histopathology in primary tumors and metastasis cervical lymph nodes and lung were poor differentiated squamous cell carcinoma & anaplastic in nature. Discussion In the past 25 years, our laboratory applied DMBA-induced hamster buccal pouch cancer (SCC) model in many research projects. 目的:探讨台湾医院门诊应用电子处方警示系统的效果。方法:采集1998-2009年的医保数据库,纳入各年均有数据库数据的259家医院进行分析。根据台湾医院应用门诊电子处方警示系统的发展,12年分为4个阶段:1998—2000年(T1)、2001-2003年(T2)、2004—2006年(T3)、2007—2009年(T4)。重复用药率是计算每个时间段医院门诊重复用药处方数占全部处方数的比例。应用广义估计方程(Generalized Estimating Equation,GEE)分析4个阶段的重复用药率改变。结果 :T1总体门诊重复用药率11.7%,T2减少到10.4%,但T2-T4维持在10. 5%上下。GEE分析发现重复用药率如期呈现下降,但降幅逐渐减小(T2:b=-2.44; T3:b-3.20;T4:b=-3.30;P<0.001)。结论:医院门诊重复用药率随电子处方警示系统应用趋势呈现递减后维持平稳,表示系统虽然有效,但仍有需要持续改善之处。

The foothill of western Taiwan Island is mainly covered by young sedimentary rock formations, such as the Pliocene Cholan formation, Kueichulin formation, and the Plio-Pleistocene Toukoshan formation Hsiangshan facies. Due to their low strength, those formations are categorized to weak rocks. As the rapid development, in recent years, there are more and more tunnel constructions in this soft rock area. Various difficulties were encountered during constructions;however, most of the countermeasures are still on the empirical side. This study first developed a physical model system to qualitative and quantitative simulate tunneling in soft rock. Then, the finite element software ABAQUS was applied to numerically model the physical model and a real soft rock tunneling case.

AIM: To examine the methylation levels of interleukin-1 receptor-associated kinase 3(IRAK3) and GLOXD1 and their potential clinical applications in hepatocellular carcinoma(HCC).METHODS: m RNA expression and promoter methylation of IRAK3 and GLOXD1 in HCC cells were analyzed by reverse transcription-polymerase chain reaction(RTPCR) and methylation-specific PCR(MSP), respectively. Using pyrosequencing results, we further established a quantitative MSP(Q-MSP) system for the evaluation of IRAK3 and GLOXD1 methylation in 29 normal controls and 160 paired HCC tissues and their adjacent nontumor tissues. We also calculated Kaplan-Meier survival curves to determine the applications of gene methylation in the prognosis of ***: IRAK3 and GLOXD1 expression was partially restored in several HCC cell lines after treatment with 5-aza-2′-deoxycytidine(DNA methyltransferase inhibitor; 5DAC). A partial decrease in the methylated band was also observed in the HCC cell lines after 5DAC treatment. Using GLOXD1 as an example, we found a significant correlation between the data obtained from the methylation array and from pyrosequencing. The methylation frequency of IRAK3 and GLOXD1 in HCC tissues was 46.9% and 63.8%, respectively. Methylation of IRAK3 was statistically associated with tumor stage. Moreover, HCC patients with IRAK3 methylation had a trend toward poor 3-year disease-free survival(P < 0.05). CONCLUSION: IRAK3 and GLOXD1 were frequently methylated in HCC tissues compared to normal controls and nontumor tissues. IRAK3 methylation was associated with tumor stage and poor prognosis of patients. These data suggest that IRAK3 methylation is a novel prognostic marker in HCC.
