Genitourinary tumors and liver transplantation: A comprehensive review

Liver transplantation is as a crucial therapeutic option for patients with end-stage liver disease,but the persistent organ shortage emphasizes a need to explore unconventional donor sources,including individuals with a history of *** review investigates the viability of liver donation from individuals with current or past genitourinary malignancies,focusing on renal,prostate and urinary bladder *** rising incidence of urogenital malignancies among potential donors is thought to result from increasing donor *** of transmission risks reveals low rates of donor-derived cancer transmission,partic-ularly for early-stage renal and prostate *** with a history of genitourinary malignancy pose complex challenges regarding post-transplant immunosuppression and cancer ***,the evidence suggests acceptable outcomes can be achieved with careful patient selection and tailored management *** for pre-transplant evaluation and post-transplant surveillance are discussed,highlighting the need for individualized approaches in this patient *** prospective studies are warranted to refine guidelines and optimize outcomes in liver transplantation for patients with genitourinary malignancies.

BACKGROUND Refractory ascites is a rare complication following orthotopic liver transplantation(OLT).The broad spectrum of differential diagnosis often leads to delay in *** depends on recognition and treatment of the underlying *** pericarditis is a condition characterized by clinical signs of right-sided heart *** the advanced stages of the disease,hepatic congestion leads to formation of *** patients after OLT,cardiac etiology of ascites is easily overlooked and it requires a high degree of clinical *** SUMMARY We report a case of a 55-year-old man who presented with a refractory ascites three months after liver transplantation for alcoholic *** to transplantation the patient had a minimal amount of *** transplant procedure and the early postoperative course were *** posttransplant work up failed to reveal any typical cause of refractory post-transplant *** function of the graft was *** from atrial fibrillation,cardiac status was *** months post transplantation the patient developed dyspnea and severe fatigue with peripheral *** was still *** presenting signs of right-sided heart failure were highly suggestive of cardiac *** paracentesis was suggestive of cardiac ascites,and further cardiac evaluation showed typical signs of constrictive *** was performed followed by complete resolution of *** the follow-up the patient remained symptom-free with no signs of recurrent ascites and with normal function of the liver *** Refractory ascites following liver transplantation is a rare complication with many possible *** differential diagnosis needs to be considered.

In recent years,evidence supporting the theory of obesity paradox has increased,showing that obese/overweight people with prevalent chronic diseases experience lower mortality compared with patients of normal *** far,evidence is most comprehensive in cardiovascular and chronic renal diseases;however,published studies are prone to many biases,enabling us to reach a definite *** data in chronic liver disease is scarce and *** is traditionally associated with nonalcoholic fatty liver disease and steatosis in viral hepatitis and as such one would not expect the obesity paradox to be a real possibility in liver ***,there seem to be new data indicating the opposite-the obesity paradox exists in severe and end-stage liver cirrhosis,which could be attributed to a better lean mass in patients with higher body mass index,meaning that sarcopenia,as one of the most important prognostic factors of survival,is less likely to be ***,the problem of various methodological problems addressing the association between body weight and mortality,which is present both in liver disease and other chronic diseases,are preventing us from attaining an unanimous ***,we should be aware that the obesity paradox might be true,especially in severe and end-stage *** suggests focusing our efforts toward preserving or building up fat-free mass and decreasing infiammatory activity responsible for catabolism and sarcopenia,and implying that the underlaying cause should be treated.

Prevention and treatment of osteonecrosis of the jaw

Purpose: to update recommendations for the prevention and treatment of osteonecrosis of the jaw in patients on bisphophonate therapy. Osteonecrosis of the jaw is a rare and serious complication of bisphosphonate therapy. Also, it is one of the important and growing clinical public health issues, because biphosphonates are now more commonly used than before. Bisphophonates are primarly used in the treat-ment of cancer-related conditions such as bone metastases, hypercalcemia, lytic skeletal lesions. More recently, bisphophonate has been approved for the management of osteoporosis. The etiology and pathogenesis remain unknown, however, two important risk factors have been identified, i.e. the potency and length of bisphos- phonate use, and recent dental intervention. Recommendations: prior to the introduction of bisphosphonate therapy, all patients should undergo complete dental examination, any active oral cavity infec-tion should be treated and the potential sites of new infection removed. During bisphosphonate therapy, patients should strictly comply with oral hygiene and avoid any invasive procedure of the oral cavity.

在配电网发生故障后,主动配电网能够实现用户侧参与功率的实时供需平衡,满足故障后的负荷恢复。用户参与配电网的调度也是提升故障后负荷恢复比例的途径之一,可平移负荷具有较强的可控性,有效提升了主动配电网的灵活性,智能软开关等新兴可控设备的接入,也使负荷的快速供电成为可能。本文建立了考虑可平移负荷和智能软开关的主动配电网故障恢复模型,并考虑了负荷的重要性系数。通过改进的IEEE 33节点算例进行仿真分析,构建单一故障、多点故障、短时故障的场景,验证了本文所提出方法的有效性以及可控装置对故障后负荷恢复量的提升。

Dear Sir, I am B. Spajic, the urologist from Clinical Department of Urology, Sestre Milosrdnice University Hospital, Zagreb, Croatia. Recently, we had a rare case of a cholesterol granuloma of the right epididymis at our department, showing clinical signs of acute scrotum. The case described here appears to be the second reporting cholesterol granuloma in the epididymis and the first one presenting with clinical signs of acute scrotum.

Electron transport in inorganic semiconductors and metals occurs through delocalized bands formed by overlapping electron *** strong correlation of electronic wavefunctions with the ionic cores couples the electron and lattice motions,leading to efficient interaction and scattering that degrades coherent charge *** contrast,electronic states at high-energy with diffuse molecular orbitals may form nearly free-electron(NFE)bands with density maxima in nonnuclear interstitial voids,which are subject to weaker electron-phonon *** superatom molecule orbital(SAMO)of C60,which is bound to the hollow molecular core,behave as an elegant example.1 Further investigations show SAMOs of C60 are derived from the IP states of graphene,which float above and below the molecular plane and undergo free motion parallel to *** or wrapping graphene sheet into a nanotube or a fullerene transforms the IP states into diffuse atom-like orbitals that are bound primarily to hollow molecular cores,rather than component atoms.2-5 Later investigations found that flat aromatic molecules like C6F6 and C6H6 also have similar SAMOs.6 For large aggregates formed by fullerenes or carbon nanotubes(CNTs),SAMOs form NFE bands,which potentially have superior properties for *** difficulty for the application of SAMOs is that they typically exist above the frontier orbitals and therefore do not contribute to *** attempts have been tried to stabilize SAMOs to below the Fermi level(EF).Through first principles calculations,we found the NFE bands in bi-layer Janus graphane could be partially occupied as well as tuned by transverse electric ***,for CNTs arrays sandwiched between insulators like BN sheet,the intertube NFE states could be stabilized below EF if the CNTs is charged.7-8

Oxy-combustion is a promising carbon-capture technology,but atmospheric-pressure oxy-combustion has a relatively low net efficiency,limiting its application in power *** pressurized oxycombustion(POC),the boiler,air separation unit,flue gas recirculation unit,and CO_(2)purification and compression unit are all operated at elevated pressure;this makes the process more efficient,with many advantages over atmospheric pressure,such as low NO_(x)emissions,a smaller boiler size,and *** is also more promising for industrial application and has attracted widespread research interest in recent *** can produce high-pressure CO_(2)with a purity of approximately 95%,which can be used directly for enhanced oil recovery or ***,the pollutant emissions must meet the standards for carbon capture,storage,and *** of the high oxygen and moisture concentrations in POC,the formation of acids via the oxidation and solution of SO_(x)and NO_(x)can be increased,causing the corrosion of pipelines and ***,particulate matter(PM)and mercury emissions can harm the environment and human *** main distinction between pressurized and atmospheric-pressure oxy-combustion is the former’s elevated pressure;thus,the effect of this pressure on the pollutants emitted from POC—including SO_(x),NO_(x),PM,and mercury—must be understood,and effective control methodologies must be incorporated to control the formation of these *** paper reviews recent advances in research on SO_(x),NO_(x),PM,and mercury formation and control in POC systems that can aid in pollutant control in such systems.

收集处理了中国国家数字地震台网(CNDSN)48个地震台记录的大量高质量的宽频带远震波形资料,用于计算中国大尺度的地壳厚度横向变化和VP/VS值(泊松比)变化。用一种统计方法来挑选每个台站的互相关接收函数,多数台站的记录在200条以上。由常用的H-к(地壳厚度和VP/VS值)方法可以发现H和к之间有明显的折衷关系。因此在H-к域内,经常出现多个极大值。为了减少这种折衷,这里引入一种权重函数,用它计算出每个H-к网格上PS转换震相与反射震相的相关性。用四次方根叠加方法进一步降低线性叠加带来的不相关噪声。在减少H-к折衷方面,这种修改后的方法非常有效,得出了可靠的地壳厚度值和VP/VS值。在中国东部的地壳厚度为31～33km,莫霍界面相对平坦和清晰。在中国西部,地壳相对较厚,地形变化很大。天山褶皱带的莫霍界面厚度为51km,青藏高原中部的莫霍界面厚度达84km。在东经100°到110°之间,所谓的南北带上发生了过渡转变,此地区表现为频繁的地震活动和明显的重力异常。中国大陆地壳的VP/VS平均值约为1.730(σ=0.249),比全球大陆地壳的平均值1.78(σ=0.27)低很多。较低的VP/VS值意味着在中国大陆的地壳最低层中缺少镁铁层。
