Epidemiological, Diagnostic and Therapeutic Aspects of Locally Advanced and/or Metastatic Breast Cancers at the Chu-Mel of Cotonou

Introduction: Breast cancer mortality remains high in most developing countries. Benin Republic does not yet have a technical platform required for the diagnosis and effective management of breast cancer “according to the recommendations”. Objective: To study the epidemiological, diagnostic and therapeutic aspects of locally advanced and/or metastatic breast cancers at the Centre Hospitalier Universitaire-de la Mère et de l’Enfant Lagune (CHU-MEL) of Cotonou from 2018 to 2021. Study Methods: This was a retrospective cross-sectional study with a descriptive purpose, carried out from January 1, 2018 to September 30, 2021 in the Gynaecology-Obstetrics Section of the CHU-MEL of Cotonou. The study population was patients diagnosed with locally advanced and/or metastatic breast cancers. Results: At the end of the study, 63 cases of locally advanced and/or metastatic breast cancer were recorded in 80 cases of breast cancer;a 74.11% prevalence of locally advanced and metastatic breast cancer. The mean age of the patients was 42.6 years. The main reason for consultation was the discovery of a mass in the breast (65.1%). Clinically, an orange peel was noted in 39.6%. Mammography (47.6%) and breast ultrasonography (42.9%) were the reference imaging workups for breast cancer diagnosis. Histopathologically, infiltrating ductal carcinoma (94.1%) was the predominant type with Scarff-Bloom and Richardson (SBR) grade II found in 35.3%. Progesterone and estrogen receptors were present in 42.9%. Mastectomy with axillary node dissection was the surgical treatment performed (80%) after neoadjuvant chemotherapy (39.6%). Only one patient had received radiotherapy. No patient received hormone therapy. Conclusion: Locally advanced and metastatic cancers remain a real public health problem. It is essential that major investments be made in order to improve both their diagnosis and management.

Maternal Hospital Mortality in Cotonou: Incidence, Care-Associated Adverse Events and Causes

Introduction: Despite the many efforts made to combat preventable maternal deaths, these still remain high in Benin. It was therefore important to revisit the causes but especially the adverse events associated with care (EIS) in maternity. Objective: Determine the incidence, adverse events associated with care and causes of intra hospital maternal mortality in cotonou. Methods: This was a cross-sectional maternal mortality review study with retrospective data collection. It covered all cases of maternal death recorded between 2017 and 2021 in two (2) reference university maternities in Cotonou. Adverse events associated with care and the patient were analyzed using the maternal death audit grid validated for Benin. SPSS.26 software was used for data analysis. Results: The in-hospital maternal mortality ratio in Cotonou was 2028 maternal deaths per 100,000 live births in 2021. Only 7.2% (n = 36) of deaths were audited. The deceased mothers were 29.8 ± 7.4 years old, with no fixed monthly income in 82.7% (n = 420). Serious adverse events associated with care were: delay in decision to refer in 37% (n = 188), non-medical referral in 85.8% (n = 436) of cases, inadequate pre-referral treatment in 25.7% of cases. In receiving maternities, delay in diagnosis and inappropriate treatment at the receiving maternity were noted respectively in 22.9% and 28.6% of cases of maternal death. The direct causes of maternal death were dominated by serious obstetric hemorrhage in 43.9% (n = 223). As for the indirect causes, they were dominated by anemia excluding obstetric hemorrhages in 21.5% (n = 109). Conclusion: The in-hospital maternal mortality ratio was very high in Cotonou. The main cause was severe obstetric hemorrhage. There were several serious healthcare-associated adverse events whose correct management would significantly reduce the incidence of maternal deaths.

Dear Editor,Monkeypox is an infectious disease that is endemic in a dozen of African *** imported cases have been also reported outside of Africa in the past[1].Since early May 2022,monkeypox infections including human-to-human transmission,were reported in a multi-country outbreak in non-endemic countries and declared Public Health Emergency of International Concern(PHEIC)by the World Health Organization(WHO)in July 2022[2].As of 20 September 2022,a total of at least 62,798 human cases of monkeypox with 20 deaths have been confirmed in 115 countries in five WHO regions[3].

Epidemiology and Clinical Signs of Gynecological Cancers in an African Country South of the Sahara: Case of the Republic of Benin in 2022

Introduction: Gynaecological cancers are the deadliest of the women’s cancers in the Republic of Benin. Late diagnosis is the most common reason. Objective: This paper aims to describe the epidemiological characteristics, and clinical and pathological signs of gynaecological cancers treated in the Republic of Benin between 2018 and 2022. Patients and Methods: This was a cross-sectional, descriptive, retrospectively collected study of patient data treated between 2018 and 2022 in two university gynaecology departments in Cotonou. All gynaecological cancers that have histological evidence were included. The epidemiological, clinical and pathological characteristics of the cancers were assessed. Results: Cervical, endometrial and ovarian cancers were the most common in the proportions of 62.0%, 24.1%, 12.0% and 1.8% respectively. The mean age at diagnosis was 54 years. The victims were uneducated and had low economic power in 81% and 85% of cases, respectively. The consultation was late in 82.1% of cases. Metrorrhagia, postmenopausal metrorrhagia and pelvic cluster headache were the common reasons for consultation for cervical, endometrial and ovarian cancer, respectively. Diagnosis was late in 66.7% (n = 71). The most common histological types were squamous cell carcinoma, endometrioid adenocarcinoma, and serous cystadenocarcinoma for cervical, endometrial, and ovarian cancers, respectively. Conclusion: Gynaecological cancers were common and their consultation time was delayed. The diagnosis was made at the advanced stage and there were several reasons for this.

Perceptions and Knowledge about Cervical Cancer among Women Living with HIV in Parakou in 2017

Background: Cervical cancer (CC) is the fourth most common cancer in women and those living with HIV have a six times higher risk of cervical cancer compared to those without. The objective of this study was to assess knowledge and perceptions about cervical cancer in women living with HIV/AIDS (WLWHIV) in the municipality of Parakou in Benin. Methods: This was an analytical cross-sectional study conducted among WLWHIV monitored on antiretrovira (ARV) therapy (ART) centers in Parakou. Study participants were selected by systematic random sampling technique. Data collection took place from September 5, 2017 to November 6, 2017. Log-binomial regression was used to identify associated factors. Results: A total of 167 women were recruited into the study. The median age was 35 years [Interquartile range: 41.50 - 30.00]. More than half of the women had no perception of the cause of CC. Only 12.6% of women believed in the preventive capacity of early screening of CC. Among women, 15.6% knew that it can be prevented, but only 9.6% had heard of HPV and 4.8% knew that HPV vaccination is an effective means of preventing CC. The factors associated with the low level of knowledge were the age at first sexual intercourse (≤24 years), the ART follow-up site (private health structure) and the fact of not having visited a maternity hospital in the last 12 months preceding the survey. Conclusion: The perceptions and knowledge about cervical cancer in the population of women living with HIV and monitored on ART in Parakou were inadequate and disproportionate to their vulnerability to this cancer. In order to avoid a double burden on these women, their ART centers must integrate cervical cancer prevention interventions into ART initiation services.

This survey had the objective to determine the frequency of the obesity at the parturient, to value the impact of the obesity on the childbirth by Caesarean and the complications postoperative immediate in the teaching hospitals of Cotonou. Patients and Method: We did a prospective, comparative and analytic survey from January 13 to April 13, 2012. Eight hundred ninety-one consecutive parturients having had a Caesarean were included. We studied the anesthetic techniques, the techniques of control of the aerial ways, the number of necessary tests for the tracheal intubation, the complications and aftercare of the first 48 hours. A comparison has been made according to the BMI. The obesity has been defined by a BMI ≥ 30 kg/m2. The parturient has been classified in two groups: Non-obese: BMI ≤ 30 kg/m2, obese: BMI ≥ 30. Results: There were 703 (78.90%) Caesareans in emergency and 188 (21.09%) programmed Caesareans. The obesity which has been recovered at 286 parturient is 32.10%. The obese parturient also required more frequently several lumbar punctures (p Conclusion: Many changes led by pregnancy are added to those of the obesity to drive to functional changes, a reduction of the physiological reserve and finally to an increased anesthetic and obstetric risk. The obesity is frequent at the parturient in Benin. The post-operative complications are more frequent after the Caesarean at the obese parturient.

Practice of the Caesarean Section in Four Maternities in Benin Using Robson Classification

Objective: To measure for each of the ten (10) groups of women defined in Robson’s classification, the cesarean section rate, group size and its contribution to the overall cesarean section rate in four maternities in Benin. Methods: This was a retrospective study over a period of 3 years, from January 1, 2014 to December 31, 2016. Two workshops were organized for training in data collection and interpretation of results. Results: A total of 5442 files were included for the four health facilities during the study period. The overall cesarean section rate was 43.9%. The most represented group (30.3%) was multiparous women without scarred uterus, with a single term fetus, in cephalic presentation, and spontaneous labour (Group 3). The largest contributor to the overall cesarean section rate was that of multiparous women with at least one uterine scar with a single fetus, eventually in cephalic presentation (Group 5) with 29.5%. Conclusion: Caesarean section rates are high in groups of women with a favourable prognosis for vaginal delivery. It is also high in women with scarred uterus (Group 5).

Evaluation of Agricultural Contamination Level by Heavy Metals and Pesticides in Sediments of Rivers and Water Bodies for Shrimp Fishing in the Lower Ouémé: Case of the Aguigadji, Ahlan and Sele Stations in Benin

This study assessed sediment contamination by heavy metals and pesticide active ingredients linked to chemical inputs used in agricultural activities in the lower Ouémé. Pesticide residues from the organochlorine, pyrethroid and organophosphorus families were investigated by gas chromatography, and heavy metals (Cd, Pb, As, Ni, Zn, Fe, Mg, Cr and Hg) by atomic absorption spectrophotometry. The metallic pollution indices, the contamination factor (CF) and the ecological risk index were calculated. The results revealed 8 active ingredients in the rainy season and 9 in the dry season. Glyphosate was the active ingredient with the highest concentration at all stations, 9.65 ± 0.84 mg/kg recorded in the dry season at the Aguigadji station. All glyphosate values were above the EQS. DDT, Atrazine and Endosulfan also showed high concentrations in the dry and rainy seasons. Emamectin, Abamectin and Lambda Cyhalothrin also showed high concentrations in the dry season at Aguigadji, Ahlan and Sele. Only glyphosate was recorded at the control station (Toho), but in very low concentrations. Lead showed the highest concentrations at all the stations, 265.96 ± 21.02 mg/Kg in the rainy season and 255.38 ± 79.09 mg/Kg in the dry season, all detected at the Aguigadji station and above the EQS. Zn, Ni, Fe, Cu and Cr were all representative in both the dry and rainy seasons at the contaminated stations. Manganese showed high concentrations in the rainy season. Pb showed very high contamination (FC ≥ 6) at the Aguigadji and Ahlan stations and significant contamination (3 ≤ FC 6) at the Sele station in both the rainy and dry seasons. Ni, Hg and Cd, showed either moderate or significant contamination at the contaminated stations. The risk values showed a considerable ecological Ri (190 ≤ Ri < 380) in the rainy season and a moderate ecological Ri (95 ≤ Ri < 190) in the dry season at these contaminated stations.

Impact of Dewatering on the Spatio-Temporal Distribution of Benthic Macroinvertebrate Communities in the Okpara River, Benin

Surface watercourses are areas of very high ecological and heritage value. Macroinvertebrates are bioindicators of the health of aquatic ecosystems. The aim of this study was to assess the effects of dewatering and re-watering cycles on benthic macroinvertebrate (BMI) communities. Two data collections were carried out at two stations (Okpara 1 and Okpara 2) on the Okpara river before and after dewatering. Thus, 8 samples of benthic macroinvertebrates and 12 physico-chemical parameters (T°C, pH, Transparency, Depth, Conductivity, Dissolved Oxygen that were measured in situ, and BOD5, COD, NH4+, NO3-, NO2- and PO43-) were assayed in the laboratory. Canonical Correspondence Analysis (CCA) was used to match physico-chemical data to MIB families. Shannon and Piélou diversity indices were used to determine the effects of dewatering on MIBs. The increase in temperature values of pH, BOD5, COD, NH4+, NO3-, NO2- and PO43-, after re-watering indicates the effect of dewatering on the quality of Okpara aquatic ecosystems. The benthic macrofauna collected consisted of 62.42% insects, 0.65% crustaceans, 6.48% molluscs, 0.72% worms and 0.14% arachnids. Whereas after re-watering, 21.67% insects, 0% crustaceans, 0.22% molluscs, 7.56% worms and 0.29% arachnids were recorded. Insects, crustaceans and molluscs were more abundant before dewatering than after. This was revealed by low abundances and taxonomic richness, as well as low Shannon index values of samples collected after re-watering.

Introduction Overweight and obesity are considered to be global public health concerns. The objective was to estimate the prevalence of overweight and obesity and the associated factors. Methods This was a cross-sectional study conducted in 2016 and involving 3968 adolescents of 10 to 19 years from 40 secondary schools in Benin. Participants were selected using a 2-level random sampling technique. Data were collected through a self-administered questionnaire and were analyzed using Epi Info and Stata softwares. Associated factors were investigated by a logistic regression. Results Mean age was 16.54 ± 0.04 years and sex ratio was 1.16. The prevalence of overweight was 8.1% and that of obesity was 1.6%. Associated risk factors were female gender, sedentary lifestyle, consumption of sugary and carbonated beverages. Consumption of fruits and vegetables, regular physical activity, age over than 15 years were protective factors. Conclusion Overweight and obesity among school adolescents are increasing in Benin. The establishment of a school nutrition program and the promotion of regular physical activity should contribute to reducing their prevalence.
