目的评估高龄患者行腹腔镜膀胱根治W形回肠新膀胱术的临床价值与风险。方法 2018年12月至2023年5月于苏州大学附属第一医院住院的连续性的34例膀胱癌患者被纳入研究。高龄组12例，年龄（72.3±1.9）岁；非高龄组22例，年龄（59.9±6.1）岁。均行机器人辅助或常规腹腔镜膀胱根治W形新膀胱术。回顾性分析两组患者的一般资料、手术情况、术后90 d内并发症发生情况、术后新膀胱功能和肿瘤学预后。结果高龄组患者和非高龄组患者手术均顺利，在手术时间、手术估计出血量、术后排气/恢复半流质饮食/留置引流时间、术后住院时间、肿瘤分期、淋巴结阳性率方面的差异无统计学意义（P>0.05）。高龄组与非高龄组术后90 d内并发症发生率分别为58.3%和50.0%，差异无统计学意义（P>0.05），均为轻度并发症，且各类并发症差异无统计学意义（P>0.05）；高龄组与非高龄组相比，日间/夜间尿控率、尿量等新膀胱功能指标的差异无统计学意义（P>0.05）。高龄组术后随访7～20个月，中位时间14(10,16)个月，未见肿瘤复发或转移。结论高龄不是原位新膀胱的禁忌；对于合适的高龄膀胱癌患者，可考虑行腹腔镜膀胱根治W形回肠新膀胱术，并对其加强围术期监测和术后随访；其围术期并发症的发生率和分级是可接受的，且高龄患者术后能够获得良好的尿控和肿瘤学预后。

Background Rat renal transplantation is an essential experimental model and requires greater microsurgical ***,training novices to perform quick and reliable microvascular anastomosis is of vital importance for rat renal *** this study,we developed and evaluated a staged microvascular anastomosis training program for novices,harvesting and transplanting both kidneys from one rat *** Five trainees without any prior microsurgical experience underwent a training program in which the goals were staged according to *** trainee had to achieve satisfactory results as evaluated by a mentor before entering the next *** renal transplantation was accomplished by end-to-end technique with a bladder *** the intensive rat renal transplantation stage,the trainees required an average of 20 independent attempts at isotransplantation as final training *** After 2 months of intensive practice,all trainees had achieved stable and reproducible rat renal transplantation,with a satisfactory survival rate of 85.9% at postoperative Day *** total mean operative time was 78.0 minutes and the mean hot ischemia time was 26.2 *** experience increasing,the operative time for each trainee showed a decreasing trend,from 90-100 minutes to 60-70 *** 20 cases,the mean operative time of the trainees was not statistically significantly different from that of the *** Harvesting and transplanting both kidneys from one rat donor after a staged microvascular anastomosis training program is feasible for novices without any prior microsurgical skills.

Establishment of orthotopic transplantation model of human bladder cancer and detection by MRI

Objective: To establish an orthotopic bladder cancer model bearing human bladder cancer for experimental research, and monitor tumor progression by magnetic resonance imaging (MRI). Methods: The mucosa was mechanically damaged transurethrally under direct vision, and then human bladder cancer cell line T24 was inoculated into the bladders of BALB/c nude mice to establish orthotopic bladder cancer model. To find a suitable concentration of Gd-DTPA for this re- search. MRI was performed weekly to assess tumor growth, using Gd-DTPA as contrast agent. The pathologic morphology of the bladders and other specimens were observed with HE stain. Results: All the 25 mice developed bladder cancer after inoculation. The best concentration of Gd-DTPA was 1.408 mg/mL. On MRI, no change in the bladders was observed on day 7 after inoculation, filling defect in the bladders, accordant to actual tumor size, was detected on days 14, 21 and 28. Pathologic examination showed that tumor grew in the mucosa or superficial muscle of bladder on day 7, confined in muscle layer on days 14–28, and invaded serosa on day 35. Conclusion: Transurethrally damaged bladder mucosa under direct vision and instilled bladder cancer cell T24, we successfully established an orthotopic bladder cancer model. Tumor growth simulated the progression of human bladder cancer approximately. MRI was a reliable way for dynamic detection of murine orthotopic bladder tumor.

This paper considers the problem of approximating the infinite-horizon value function of the discrete-time switched LQR *** particular,the authors propose a new value iteration method to generate a sequence of monotonically decreasing functions that converges exponentially to the value *** method facilitates us to use coarse approximations resulting from faster but less accurate algorithms for further value iteration,and thus,the proposed approach is capable of achieving a better approximation for a given computation time compared with the existing *** numerical examples are presented in this paper to illustrate the effectiveness of the proposed method.
