Stroke as a Presenting Feature of COVID-19

Objective: To analyze the coronavirus disease-2019 (COVID-19) patients presenting with acute stroke by determining their clinical characteristics, hospitalization course and prognosis. The common and conventional stroke risk factors in these patients were assessed with the aim of determining the role and contribution of the COVID-19 infection to stroke pathogenesis. Methods: Retrospective observational study involving 24 patients from a single tertiary care center over a time period of three months. Risk factors such as Age, Hypertension, Diabetes Mellitus, smoking status and underlying cardiac history were analyzed. COVID-19 relevant laboratory and radiological data were documented. Results: 87.5% of patients had ischemic stroke, with 58.3% of total patients being younger than 55 years. An equal incidence of both Diabetes Mellitus and Hypertension (37.5%) was identified. 29.2% were completely asymptomatic for COVID-19, of which 85.7% had no chest X-ray changes on admission. Eight patients (61.5%) developed pneumonia during admission despite an initially normal chest X-ray. Conclusion: Patients without COVID-19 symptoms and with normal chest radiography presenting with stroke does not rule out a possible underlying COVID-19 infection. Such patients may be positive for the virus and may go on to develop pneumonia shortly after suffering from strokes. This could suggest that stroke in COVID-19 patients is a possible initial presenting feature and consequence of the inflammatory state triggered by the Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) infection. It is imperative to analyze the association of COVID-19 and stroke, and to maintain a high index of suspicion of COVID-19 infection in stroke patients, to enhance early detection and reduce transmission.

Altered Pulmonary Function among the Transport Workers in Dhaka City

Motor vehicular exhaust is threatening to human health in Dhaka city. To find out the extent of respiratory problem among a particular cadre of transport workers, the human hauler helpers (HHH) of Dhaka city, we conducted a cross sectional study from March 2006 to June 2006. We enrolled 108 HHH from Mirpur to Mohakhali and Gabtoli to Mohakhali routes and collected data using a pre tested questionnaire including interview and physical examination. The pulmonary functions of the participants were measured with electronic spirometer. Among the participants with current respiratory problems 69% complained of gradual increase in respiratory problems and 38% gave history of feeling better on weekly holidays. Occurrence of hoarseness of voice (90%) and wheeze (71%) was extremely high among the helpers followed by cough (50%) and nasal irritation (50%). Spirometry showed 41.7% had obstructive and 4.6% had combined features of pulmonary impairment. Logistic regression analysis showed that respondents who worked 24 months or more were 6.9 times more likely to develop respiratory problem than the less exposed group (OR 6.9;95% CI 3.42, 8.41). This study provides new information on respiratory health of a specific group of transport workers in Dhaka city which warrants further evaluation with a representing sample covering other areas of Dhaka city among all transport workers.

MDCT imaging following nephrectomy for renal cell carcinoma: Protocol optimization and patterns of tumor recurrence

The purpose of this pictorial essay is to review the common and uncommon sites of renal cell carcinoma recurrence throughout the body by examining their appearances on computerized tomography(CT). CT imaging protocols will be discussed. The sites of recurrence have been categorized into 4 groups: chest and mediastinum, abdomen and pelvis, musculoskeletal, and neurological. For each site of recurrence, a representative CT image correlate with discussion is provided. The unique CT appearance of renal cell carcinoma recurrence and how it can be used in lesion detection will be discussed. Renal cell carcinoma recurrences are hypervascular like the primary tumor, which can aid in not only lesion detection but also in some cases, differentiation from other primary tumors. Through CT case review of various sites of recurrence, lesions are shown to be easily seen on arterial phase while sometimes being nearly inconspicuous on venous or delayed phases. Coronal and sagittal reconstructions can also improve diagnostic sensitivity. CT is the most commonly used imaging tool for surveillance of renal cell carcinoma recurrence after nephrectomy. Knowledge of sites of recurrence as well as the utility of arterial phase imaging and multiplanar reconstructions will aid in optimizing detection of disease recurrence.

Design Optimization for Generating a High Static Magnetic Field

This paper details the creation of a device capable of generating a powerful and consistent static magnetic field. This apparatus serves the purpose of quantifying the magnetostrictive strain found in materials like annealed cobalt ferrite and Terfenol-D, specifically those shaped as cylindrical rods. In our investigation, the use of static magnetic fields proves most advantageous. This choice is made to mitigate the generation of eddy currents, which would inevitably occur if the magnetic field intensity were varied. The fundamental idea behind this design involves employing a C-shaped iron core constructed from low-carbon mild steel. On this core, three coils are mounted, each capable of producing one-third of the required 9000 Oersted (Oe) magnetic field strength. The test specimen is situated within the “jaws” of the C-shaped core, thus completing the magnetic circuit. To manage the heat generated by each coil, a cooling system consisting of copper tubes is employed. These tubes facilitate the flow of air to dissipate the heat. To model and predict the magnetic field strength produced by the coils, finite element analysis (FEMM) software is utilized, and the results align closely with the anticipated outcomes. This design effectively generates a robust and unchanging magnetic field measuring a stable 9000 Oe in total. Consequently, this equipment finds utility in characterizing the magnetic properties of specific materials.

Chronic Kidney Disease in Sub-Saharan Africans: A Study of 462 Patients

Chronic kidney disease is a global public health problem due to its increasing prevalence as well as its main risk factors such as hypertension and diabetes. However, in Africa, few studies have been done on chronic kidney disease. The aim of our study is to describe the epidemiological, clinical, paraclinical and therapeutic aspects of chronic kidney disease. It was a retrospective and descriptive study carried out from the first of January 2004 to the 31st of December 2013 at Principal hospital in Dakar. Records of any patient aged 18 and over with chronic kidney disease were included. Chronic kidney disease was defined according to the KDIGO 2012 recommendations. Among the 8873 patient records used during our study, 462 presented with chronic kidney disease, which was a hospital prevalence of 5.2%. The sex ratio was 1.61. The mean age of the patients was 58.37 ± 19.97 years. There were 75.32% of the patients who were aged 50 and over. The mean serum creatinine was 49.14 ± 56.83. The mean glomerular filtration rate was 27.47 ± 19.86 ml/min/1.73 m2. Chronic renal failure was diagnosed in 92% of patients, including 34.9% in the end-stage of the renal disease. The mean proteinuria was 3.07 ± 4.92 g/24 h. Leukocyturia was present in 34.17% of patients. Hematuria was present in 25% of patients. Hypertension and diabetes were the most common causes, found in 61.25% and 35.93% of patients, respectively. Hemodialysis was performed in 49 patients. Peritoneal dialysis was performed in 2 patients. One patient had undergone a kidney transplant. This study establishes the relatively high prevalence of chronic kidney disease and its risk factors including hypertension and diabetes. Misoprostol: An Effective Agent for Cervical Ripening and Labor Induction: A 2-Year Review in a Tertiary Center

Background: Misoprostol a PGE1?analogue has increasingly been used for cervical ripening in preparation for induction of labour with variable outcome for both mother and baby. Objectives: To determine the effectiveness of Misoprostol in cervical ripening and labour outcome in Aminu Kano Teaching Hospital Kano. Study Design: A study of all patients who had cervical ripening for induction of labour using Misoprostol at AKTH Kano, Nigeria, between 1st Jan 2012-31st Dec 2013. Socio-demographic data of these patients including number of doses inserted before ripening is achieved, duration, course and outcome of labour were documented. Result: Four hundred and two women were admitted for cervical ripening. 365 women met the inclusion criteria out of the 358 folders retrieved, giving a retrieval rate of 98%. Despite this six folders did not contain enough information for the study and were excluded leaving us with a sample size of 352. They had a successful cervical ripening with an average of 2 insertions. Spontaneous onset of labour without further intervention was observed in 266 women (75.5%). However, 86 women (24.5%) were induced with oxytocin following cervical ripening. The mean duration of labour among those that had spontaneous onset of labour following Misoprostol insertion was 9.8 hours. 96% of the patients had vaginal delivery while 4% had Caesarean section. Conclusion: Misoprostol was associated with favorable outcome following cervical ripening with a low caesarean section rate.

Despite the progress of international accounting harmonization, there remain a number of countries which have not adopted International Financial Reporting Standards （IFRSs） but continue to adhere to their own accounting laws or standards, including Libya and some surrounding countries. This paper examines the arguments surrounding the appropriateness of accounting harmonization and the obstacles to achieve it and seeks to apply these arguments in the case of Libya. The conclusion is that although harmonization with IFRSs is not precluded by any cultural considerations, historical factors and accounting education deficiencies may make the adoption of IFRSs more difficult, while the absence of an active stock market may make it less desirable.

The Impact of Severity of Periodontal Bone Loss and the Levels of Glycated Hemoglobin (HbA1c) on the Periodontal Clinical Parameters of the 2017 World Workshop among Type 2 Diabetic Patients in Saudi Arabia

Background: Type-2 diabetic patients (uncontrolled levels of glucose blood) usually have periodontal diseases and alveolar bone loss. Objectives: The present study was designed to clarify the impact of severity of periodontal bone loss and the levels of glycated hemoglobin (HbA1c) on the periodontal clinical parameters of the 2017 World Workshop among type 2 diabetic patients in Saudi Arabia (Saudi and non-Saudi). Material and Methods: This study was done on 298 type 2 diabetic patients, selected from the internship clinics, College of Dentistry, King Khalid University, Abha, Saudi Arabia. The selection of patients was dependent on the levels of glycated hemoglobin (HbA1c), and they were categorized into controlled (7% HbA1c). All patients were divided according to the severity of periodontal bone loss into three groups, group I: mild periodontal bone loss, group II: moderate periodontal bone loss, and group III: severe periodontal bone loss. Clinical evaluation of periodontal diseases was carried out by clinical parameters according to the 2017 World Workshop. All data were collected and analyzed. A p-value of Results: The severity of periodontal bone loss were determined in controlled type 2 diabetics (7% HbA1c). An increased percentage of patients with severe periodontal bone loss was observed in uncontrolled type 2 diabetics (>7% HbA1c) (42.9%), as compared to controlled type 2 diabetics (7% HbA1c). Gastro-Intestinal Parasites of Camels (Camelus dromedarius) from Mogadishu, Somalia

Somalia has the largest dromedary (Camelus dromedarius) population in the world. However, scientific research and camel diseases investigation in the country were lacking since 1980s. A total of 167 camels (131 semi-intensive dairy camels system and 36 free grazing systems) were sampled from three districts in Mogadishu citybetween December 2015 and March 2016 to investigate the prevalence rate of gastro-intestinal parasites in camels from Mogadishu city using different coprological techniques. The overall prevalence of camel gastro-intestinal parasites was 50.3%. The prevalence rate were significantly varies between the extensive and semi-intensive production systems (p = 0.000) and between the different districts (p = 0.000). Nematodes revealed higher prevalence rate (47.9%) than Cestodes (6.0%), Trematodes (4.2%) and Coccidia (0.65%). Eggs of eight genera of nematodes, two genera of trematodes and two genera of cestodes as well as Eimeria sp. Oocysts were identified in this study. Dictyocaulus sp. and Trichostrongylus sp. were the most prevalent followed by Parascaris equorum and Strongyloides sp. About 56.0% of the infected camels revealed mix-infection of up to five different parasite species. Moreover, 35.7% of these animals revealed heavy infection. The important role of the camel has inspired investigators and researchers to look for diseases that may threaten camel health and production. However, in Somalia in spite of having the largest counts of camels but in depth scientific data are not available. Therefore, the present paper was to be the first detailed data on camel gastro-intestinal parasites in Mogadishu area and may be in whole Somalia. We came to conclude that gastrointes-tinal parasites were highly prevalent in camels from Mogadishu particularly in nomadic herds of camels. Adenoma of the Nipple, Mimicking Paget’s Disease of the Breast: Report of a Case

Nipple adenoma is a rare benign condition that simulates malignancy. A 37-year-old woman presented with unilateral bloody nipple discharge for 1-year duration followed by severe nipple erosion. As biopsy revealed nipple adenoma and therefore, complete local excision was done. The final histopathology showed florid papillomatosis which was adequately excised. Nipple adenoma although rare entity this should be included in the differential diagnosis of any nipple erosion such as carcinoma and Paget’s disease of the breast specially when associated with bloody discharge in premenopausal women.
