Expression of gastric cancer-associated MG7 antigen in gastric cancer,precancerous lesions and ***-associated gastric diseases

AIM: To investigate the relationship between the antigen MG7 antigen expression and gastric cancer as well as precancerous condition; to study the relationship between the MG7 antigen expression and H. pyloriinfection in benign gastric lesions in order to find out the effect of H. pylori infection on the process of gastric cancer ***: The level of MG7 antigen expression was determined by immunohistochemical method in 383 gastric biopsied materials. The intestinal metaplasia was determined by histochemistry method. The H. pyloriinfection was determined by HE stain, PCR and ELISA in 291 specimens, among which only 34 cases of H. pylori-associated gastric lesions were followed ***: The positive rate of MG7 expression in normal gastric mucosa, intestinal metaplasia, dysplasia and gastric cancer increased gradually in ascending order (P＜0.01). The positive rate of MG7 antigen expression in type Ⅲ intestinal metaplasia of gastric mucosa was higher than that of type Ⅰand Ⅱ intestinal metaplasia, being highly significant (P＜0.05).The positive rate of MG7 antigen expression in superficial gastritis, atrophic gastritis and gastric cancer increased gradually (11.9 %, 64.8 %, 91.2 %, P＜0.01). There was no significant difference between ***-negative and H. pyloripositive intestinal metaplasia, atrophic gastritis and dysplasia of gastric epithelium in the positive rate of MG7 antigen expression. There was no expression of MG7 antigen in H. pylori-negative superficial gastritis. The positive rate of MG7 expression in H. pylori-positive superficial gastritis was 20.5 %, and the difference between them was significant (P＜0.05). A Clinical Analysis of 1255 Children with Burn Injuries

In a twenty-five year period(1958-1983),there were altogether 4651burn cases admitted to this institute,of whom 1255 were children(26.95%).Themortality rate of this series of young victims was 4.7%(59 cases).The causes ofdeath were septicemia(30 cases),shock(17),cerebral edema and cerebral hernia(4),acute renal failure(4),severe burn wound sepsis(2),and pulmonarycomplications resulting from severe inhalation burns(2).Since the mortality and the incidence of shock and septicemia were closelyrelated to the extensiveness of the burn injuries,a new classification of burninjuries in children is suggested as follows:*** burns:The affected area is less than 10% *** burns:10-29% TBSA is *** burns:30-49% TBSA is *** severe burns:More than 50% TBSA is injured.

Relationship between antimicrobial resistance and aminoglycoside-modifying enzyme gene expressions in Acinetobacter baumannii

Background Acinetobacter baumannii is one of the main gramnegative bacilli in clinical practice Nosocomial infections caused by multidrug resistance Acinetobacter baumannii is very difficult to treat This study was designed to investigate the antimicrobial resistance characteristics and four resistant gene expressions of aminoglycosidemodifying enzymes including Nacetyltransferases and Ophosphotransferases in Acinetobacter baumannii Methods Bacterial identification and antimicrobial susceptibility test were performed by PhoenixTM system in 247 strains of Acinetobacter baumannii Minimal inhibitory concentrations (MICs) of seven aminoglycosides including gentamicin, amikacin, kanamycin, tobramycin, netilmicin, neomycin and streptomycin in 15 strains of multidrug resistant Acinetobacter baumannii were detected by agar dilution Four aminoglycosidemodifying enzyme genes were amplified by polymerase chain　reaction (PCR) and verified by DNA sequencerResults The resistance rates of 247 strains of Acinetobacter baumannii against cefotaxime, levofloxacin, piperacillin, aztreonam, tetracycline, ciprofloxacin and chloramphenicol were more than 50% Imipenem and meropenem showed high antibacterial activities with resistance rates of 32% and 41% MIC50 and MIC90 of gentamicin, amikacin, streptomycin and kanamycin in 15 strains of multidrug resistant Acinetobacter baumanii were all more than 1024 mg/L, and the resistance rates were 100%, 100%, 100% and 933%, respectively But their resistance rates to tobramycin, netilmicin and neomycin were 867%, 933% and 467%, respectively Three modifying enzyme genes, including aacC1, aacC2 and aacA4 genes, were found in 15 strains, but aphA6 had not been detected Their positive rates were 933%, 200% and 200%, respectively These three genes existed simultaneously in No19 strain Nucleotide sequences of aacC1, aacC2 and aacA4 genes shared 100%, 979% and 997% identities with GenBank genes (AY307

Outcomes of treatment of sudden deafness using different Protocols:a retrospective analysis of 104 cases

Objective To compare different treatment protocols for sudden deafness(SD), for the purpose of identifying an appropriate approach to SD. Methods A total of 104 patients with diagnosis of sudden hearing loss treated from Jan 2006 to December 2008 were included in this study, of which 31 received the typical pharmaceutical treatment (groupⅠ), 40 received the typical pharmaceutical treatment plus polarized liquid (GroupⅡ) and 33 received the hyperbaric oxygen in addition to the treatment included in Group Ⅱ(Group Ⅲ). Results The total improvement rate (67.74%, 62.50% and 75.76% for Groups Ⅰ, Ⅱ and Ⅲ respectively) was not statistically different between the three groups (P > 0.05). Conclusion The three treatment protocols are similar when judged by the treatment outcomes in SD, neither being superior to the others. The two important factors that appear to influence treatment outcomes are the audiogram pattern and duration of hearing loss before seeking treatment. Patients with upsloping or peak-type audiograms and treated within 7 days from the onset have better prognosis than others.

Individualized immunosuppression: new strategies from pharmacokinetics,pharmacodynamics and pharmacogenomics

The ultimate goal of transplantation is the donor-specific immune tolerance, but at least in the first 15 to 20 years of this century, immunosuppressive agents are still the determinant of clinical outcome of transplant recipients. Individualizing patient's immunosuppression to optimize the balance between therapeutic efficacy and the occurrence of adverse events poses a great challenge to physicians. DATA SOURCES:The data in this article were taken mostly from MEDLINE (2000-2004), part of which were from the research of the authors. RESULTS:Individualized immunosuppression remains a problem because of the narrow therapeutic index and wide inter- and intra-patient variation of commonly-used im- munosuppressants. Recent progress in study of pharmaco-kinetics and pharmacodynamics improved the clinical outcome of transplant recipients. More importantly, the emergence of pharmacogenomics might provide a promising and complementary tool for traditional therapeutic drug monitoring (TDM). CONCLUSIONS:Individualizing organ recipient's immunosuppression to balance the therapeutic efficacy and the adverse events represents a great challenge to transplant clinicians. Pharmacogenomics shows great promise for an interesting and hopefully better future.

Influence of acute hyperglycemia in human sepsis on inflammatory cytokine and counterregulatory hormone concentrations

AIM: In human sepsis, a prominent component of the hypermetabolite is impaired glucose tolerance (IGT) and hyperglycemia. Elevations in plasma glucose concentration impair immune function by altering cytokine production from macrophages. We assessed the role of glucose in the regulation of circulating levels of insulin, glucagon, cortisol,IL-6 and TNF-α in human sepsis with normal or impaired glucose ***: According to the results of intravenous glucose tolerance test, forty patients were classified into two groups: control group (n=20) and IGT group (n=20).Plasma glucose levels were acutely raised in two groups and maintained at 15 mmol/L for 3 hours. Plasma insulin,glucagon and cortisol levels were measured by radioimmunoassay, the levels of TNF-α and IL-6 were detected by ***: In IGT group, the fasting concentrations of plasma glucose, insulin, glucagon, cortisol, IL-6 and TNFα levels were significantly higher than those in control group (P＜0.05). During clamp, the control group had a higher average amount of dextrose infusion than the IGT group (P＜0.01). In control group, plasma insulin levels rose from a basal value to a peak at an hour (P＜0.05) and maintained at high levels. Plasma glucagon levels descended from a basal value to the lowest level within an hour (P＜0.01)and low levels were maintained throughout the clamp. In IGT group, plasma insulin was more significantly elevated (P＜0.01), and plasma glucagon levels were not significantly declined. Plasma cortisol levels were not significantly changed in two groups. In control group, plasma IL-6 and TNF-α levels rose (P＜0.01) within 2 hours of the clamp and returned to basal values at 3 hours. Pharmacokinetic Study on Hyperoside in Beagle’s Dogs

Objective To develop and validate a simple, rapid, sensitive, and reproducible HPLC method for determination of hyperoside in plasma of dogs and for the subsequent pharmacokinetic (PK) study. Methods An accurate and reproducible HPLC-UV method was developed and validated for the determination of hyperoside in plasma of dogs, using kaempferol as internal standard. The plasma samples of dogs following ig administration of hyperoside were analyzed for the detection of quercetin after enzymatic hydrolysis treatment with combined β-glucuronidase and sulphatase. The analytes were separated on a Diamonsil C 18 column (250 mm × 4.6 mm, 5 μm). The mobile phase consisted of methanol-buffer solution (0.1 mol/L NH 4 Ac + 0.3 mmol/L EDTA-Na 2 )-acetic acid (60:40:1) and was delivered at a flow rate of 1 mL/min. The UV detector was set at 370 nm and the column temperature was maintained at 35 ℃. The sample injection volume was 20 μL. Data were collected and analyzed using the ANASTAR software. PK parameters were calculated with DAS software (2.0). Results Linear relationships were validated over the range of 0.01-1 μg/mL for hyperoside (r = 0.9997). The intra- and inter-day precision values for all samples were within 10.0%, and the accuracies of intra- and inter-day assays were within the range of 92.4%-102.4%. The validated method was successfully used to determine the hyperoside concentration in plasma of dogs for up to 12 h, after a single ig administration (25 mg/kg). The mean PK parameters for male and female dogs were as follows: C max (0.18 ± 0.05) and (0.16 ± 0.05) μg/mL, AUC 0-∞ (0.79 ± 0.34) and (0.86 ± 0.27) μg/(mL·h), t 1/2(ka) (0.89 ± 0.41) and (0.88 ± 0.28) h, respectively. Statistical analysis on the PK of hyperoside in male and female groups showed that sex had no significant impact on the PK of hyperoside (P > 0.05). Conclusion The method is able and sufficient to be used in drug PK studies of hyperoside.

Effects of tanshinone on neuropathological changes induced by amyloid β-peptide(1-40)injection in rat hippocampus

AIM: To investigate the effect of tanshinone (Tan) on the neuropathological changes induced by amyloid β- peptide1-40(Aβ1-40) injection in hippocampus in rats. METHODS: Aβ1-4010 μg was injected bilaterally into the dorsal blade of the dentate gyrus in the hippocampus. The level of acetylcholinesterase (AChE) in hippocampus was evaluated by histochemistry. The expressions of neuronal nitric oxide synthase (nNOS) and inducible form of NOS (iNOS) were detected by immunohistochemistry and Western blot. Aβ1-40-injected rats were treated ig with Tan, the major active ingredient from Salvia miltiorrhiza of Chinese herb extract. RESULTS: The level of AChE positive fibers of each subfield in Aβ1-40-injected hippocampus decreased significantly compared with those of control (P< 0.01). The expression of nNOS was down-regulated whereas the iNOS was up-regulated. After treatment with Tan (50 mg/kg, ig), the changes mentioned above were significantly improved. Moreover, the correlation analysis revealed a significant negative correlation between the area percentage of AChE positive fibers and the number of iNOS positiveneural cellsin CA1,CA2 toCA3 (CA2-3),and dentategyrus (DG)subfields (P<0.01). CONCLUSION: Tan can protect the neuropathological changes induced by Aβ1-40 injection in hippocampus.
