Previously.we reported combined chemoimmunotherapy. using interferon (IFN) αand 5-fluorouracil (5-FU) for patients with advanced hepatocellular carcinoma (HCC), and this regimen improved the prognosis. Recently. we experienced an HCC patient who died of severe interstitial pneumonia during the combined IFN αand 5-FU therapy. This is the first report of the occurrence of interstitial pneumonia during combined IFN αand 5-FU treatment. A 60-y.ar old man was admitted to Osaka University.Hospital to receive sy.temic chemo immunotherapy.for recurrent HCC. In the second week of the chemo immunotherapy. he showed a decreased level of consciousness, and respiratory.insufficiency. Emergency.roentgenogram revealed diffuse infiltration in both lungs. Respiratory.dy.function due to interstitial pneumonia was suspected, and steroid pulse therapy.was started. However, the patient showed respiratory.failure, and he died 32 day. after the start of the therapy. Autopsy.findings showed atelectasis in the bilateral lungs, which showed elastic hard solidity.and a dark red color; esophageal varices were also shown, and there was cirrhosis with a large tumor in the liver. Microscopically. the alveolar wall showed marked fibrous thickness and moderate inflammatory.change, which is consistent with acute interstitial pneumonia, and the acute pulmonary.change was suspected to have been the cause of death. The association of IFN with the development of interstitial pneumonia has been reported. However, the prognosis of IFN induced interstitial pneumonia has mostly.been favorable when the medication was discontinued. It has been postulated that interstitial pneumonia induced by.the combination of IFN and 5-FU may.be therapy.resistant. The combination of IFN αand 5-FU is a useful therapy.for patients with advanced HCC, such as that with portal vein invasion or multiple metastatic foci. Thus, interstitial pneumonia in these patients should be carefully.managed.

Molecular prognostic prediction in liver cirrhosis

The natural history.of cirrhosis varies and therefore prognostic prediction is critical given the sizable patient population. A variety.of clinical prognostic indicators have been developed and enable patient risk stratification although their performance is somewhat limited especially.within relatively.earlier stage of disease. Molecular prognostic indicators are expected to refine the prediction,and potentially.link a subset of patients with molecular targeted interventions that counteract poor prognosis. Here we overview clinical and molecular prognostic indicators in the literature,and discuss critical issues to successfully.define,evaluate,and deploy.prognostic indicators as clinical scores or tests. The use of liver biopsy.has been diminishing due to sampling variability.on fibrosis assessment and emergence of imaging- or lab testbased fibrosis assessment methods. However,recent rapid developments of genomics technologies and selective molecular targeted agents has highlighted the need for biopsy.tissue specimen to explore and establish molecular information-guided personalized/stratified clinical care,and eventually.achieve "precision medicine".

Determining factors for carotid mean/max intima-media thickness and brachial flow-mediated dilation in healthy.y.ung women

Background: Many.factors can contribute to atherosclerotic-ty.e vascular changes in older individuals or men. Thus, confining the investigation to y.ung women with no clinical evidence of the condition could enhance understanding of the early.stages of cardiovascular disease. The aim of this study.was to determine whether carotid mean/max intima-media thickness (IMT) and brachial flow-mediated dilation (FMD) values, which are well-known event-related indices, are associated with laboratory.data and the other vascular indices of atherosclerosis in healthy.y.ung women. Methods: Carotid mean/max IMT and brachial FMD were measured in y.ung women with no clinical evidence of atherosclerosis (n = 110;mean age, 39 y.ars) who were instructed not to eat, drink or smoke after 9 PM the evening before testing. All participants also underwent laboratory.assessment, including simultaneous measurements of arterial stiffness such as augmentation index (AI), cardioankle vascular index (CAVI) and brachial-ankle pulse wave velocity.(baPWV). Results: Mean IMT was signifi-cantly.and positively.associated with age (p = 0.002), CAVI (p = 0.044), low-density.lipoprotein-cholesterol (LDL-C, p = 0.047) and high-sensitive C-reactive protein (hs-CRP, p = 0.002) values but was not related to FMD, AI, baPWV or trigly.erides (TG) in the multivariate regression analy.is. Similarly. max IMT was positively.associated with age (p p = 0.003) and hs-CRP (p = 0.005) values but was not related to FMD, AI, CAVI, baPWV, TG or blood pressure level in the multivariate regression analy.is. The association between LDL-C and max IMT was much stronger than that between LDL-C and mean IMT. Brachial FMD was positively.associated only.with heart rate in the multivariate regression analy.is. Conclusions: These results suggest that mean IMT more closely.represents the sclerotic aspect of vascular change, whereas max IMT represents the atherotic aspect in healthy.y.ung women. Hepatic gene expression profiles associated with fibrosis progression and hepatocarcinogenesis in hepatitis C patients

AIM: To determine fibrosis progression and hepatocellular carcinoma (HCC), using simultaneous gene expression ***: Total RNA samples were extracted from liver biopsies from 19 patients with hepatitis C virus (HCV)infection and 3 patients without HCV infection. Among the 19 HCV-infected patients, 7 and 12 patients had grade F1-2 and F3-4 fibrosis, respectively. Of the 12 patients with F3-4 fibrosis, 8 had HCC. Gene expression in the liver samples was determined using an oligonucleotide microarray. The following comparisons were performed:normal livers vs HCV-infected livers; F1-2 vs F3-4; and F3-4 with HCC vs F3-4 without HCC. Genes that were differentially.expressed between these groups were identified based on signal-to-noise ***: In the HCV-infected livers, genes involved in immune responses were highly.expressed. Expression levels of genes for plasma proteins and drug-metabolizing enzy.es were decreased and those of genes involved in the cell cy.le and oncogenesis were increased in the F3-4 cases as compared to the F1-2 cases. Among the F3-4 cases, genes involved in carbohy.rate metabolism tended to be more highly.expressed in patients with HCC than in patients without ***: We identified genes that are associated with fibrosis progression and hepatocarcinogenesis. This information may.be used to detect increased carcinogenic potential in the livers of patients with HCV infection.

NF-κB and ERK-signaling pathway. contribute to the gene expression induced by.cag PAI-positive-Helicobacter py.ori infection

AIM: To elucidate the sequential gene expression profile in AGS cells co-cultured with wild-ty.e Hellcobacter py.ori (Hpy.on) as a model of H py.ori-infected gastric epithelium, and to further examine the contribution of cag-pathogenicity.islands (cagPAI)-coding ty.e Ⅳ secretion sy.tem and the two pathway., nuclear factor kappa B (NF-κB) and extracellular signal-regulated kinases (ERK) on wild-ty.e H py.ori-induced gene expression. METHODS: Gene expression profiles induced by.H py.ori were evaluated in AGS gastric epithelial cells using cDNA microarray. which were present in the 4 600 independent clones picked up from the human gastric tissue. We also analy.ed the contribution of NF-κB and ERK signaling on H py.ori-induced gene expression by.using inhibitors of specific signal pathway.. The isogenic mutant with disrupted capE (△cagE) was used to elucidate the role of capPAI-encoding ty.e Ⅳ secretion sy.tem in the gene expression profile. RESULTS: According to the expression profile, the genes were classified into four clusters. Among them, the clusters characterized by.continuous upregulation were most conspicuous, and it contained many.signal transducer activity.associated genes. The role of cagPAI on cultured cells was also investigated using isogenic mutant cagE, which carries non-functional cagPAI. Then the upregulation of more than 80% of the induced genes (476/566) was found to depend on cagPAI. Signal transducer pathway.through NF-κB or ERK are the major pathway. which are known to be activated by.cagPAI-positive H py.ori. The role of these pathway. in the whole signal activation by.cagPAI- positive H py.ori was analy.ed. The specific inhibitors against NF-κB or ERK pathway.blocked the activation of gene expression in 65% (367/566) or 76% (429/566) of the genes whose activation appealed to depend on cagPAI. CONCLUSION: These results suggest that more than half of the genes induced by.cagPAI-positive H py.ori depend on NF-κB and ERK signaling activation, and t

鼻NK／T细胞淋巴瘤多发生于亚洲和拉丁美洲，而西方国家发生率很低，该文旨在研究生活方式及环境因素与鼻NK／T细胞淋巴瘤发生的关系。2000—2005年间，日本5所、韩国和中国各1所大学医院参与此项研究，试验组88例。对照组305例。对年龄、性别和国家因素调整后。鼻NK／T细胞淋巴瘤的比值比（OR值）为：农场主4．15，农民2．81，杀虫剂使用者4．01，垃圾焚烧场附近居民11．65，先前饮酒者2．95，当前吸烟者0．49。工作期间对杀虫剂采取戴手套、口罩、顺风喷洒防护措施的农民OR值分别为3．30、1．18、2．20，远低于未采取防护措施者。结论：杀虫剂和化学溶剂是鼻NK/T细胞淋巴瘤的病因，生活方式及环境是鼻NK／T细胞淋巴瘤发病的危险因素。
