much attention has been given to the Internet of Things (IoT) by citizens, industries, governments, and universities for applications like smart buildings, environmental monitoring, health care and so on. With IoT, networkconnectivity is facilitated between smart devices from anyplace and ***-based health monitoring systems are gaining popularity and acceptance forcontinuous monitoring and detect health abnormalities from the data *** (ECG) signals are widely used for heart diseases detection.A novel method has been proposed in this work for ECG monitoring using IoTtechniques. In this work, a two-stage approach is employed. In the first stage, arouting protocol based on Dynamic Source Routing (DSR) and Routing byEnergy and Link quality (REL) for IoT healthcare platform is proposed for effi-cient data collection, and in the second stage, classification of ECG for Arrhythmia. Furthermore, this work has evaluated Support Vector machine (SVm),Artificial Neural Network (ANN), and Convolution Neural Networks (CNNs)-based approach for ECG signals classification. Deep-ECG will use a deep CNNto extract critical features and then compare through evaluation of simple and fastdistance functions in order to obtain an efficient classification of heart abnormalities. For the identification of abnormal data, this work has proposed techniquesfor the classification of ECG data, which has been obtained from mobile watchusers. For experimental verification of the proposed methods, the Beth Israel hospital (mIT/BIH) Arrhythmia and massachusetts Institute of Technology (mIT)Database was used for evaluation. Results confirm the presented method’s superior performance with regards to the accuracy of classification. The CNN achievedan accuracy of 91.92% and has a higher accuracy of 4.98% for the SVm and2.68% for the ANN.

Aim: To determine the predictive value of the hypo-osmotic swelling (hos) test to identify viable, non-motile sperm. methods: Semen samples from 20 men with severe asthenozoospermia underwent traditional seminal analysis, eosin-nigrosin (EN) staining and the hos test. A further EN stain was then performed on a hos pre-treated aliquot and a total of 2000 further sperm examined. Results: The median sperm density was 5.1 million/mL (IQR 4.3-13.1) and the median motility was 3.0 % (IQR 0-7). Seven samples showed complete asthenozoospermia. Initial EN staining showed 59 % viability (range 48-69) despite the poor standard parameters and 47 % (range 33-61) in the complete asthenozoospermia subgroup. The hos test showed 49.9 % reacted overall (range 40-59) and 41.7 % (range 22-61) in the complete asthenozoospermia subgroup. The combined hos/EN stain showed the positive predictive value of the hos test to identify viable sperm was 84.2 % overall and 79.7 % in the complete asthenozoospermia subgroup. Conclusion: The hos test can effectively predict sperm viability in patients with severe and complete asthenozoospermia.

目的 :探讨在周围神经选切治疗痉挛性脑瘫中阈值的意义。方法 :41例痉挛性脑瘫 ,16例SPR和 8例SAR术中电刺激脊神经前后根神经小束 ,2 8例经皮刺激坐骨神经 ,观察相应肌肉运动诱发反应 ,2 5例取周围神经标本 40个 ,进行透射电镜观察。结果 :电刺激神经小束或坐骨神经诱发的肌肉收缩反应或m波幅 ,存在着最大、最小现象。当达到最大后 ,保持稳定 ,不再随刺激强度增大而增加。周围神经呈原发性脱髓鞘病变 ,有髓纤维病变明显于无髓纤维 ,粗纤维重于细纤维。结论 :在周围神经选切治疗痉挛性脑瘫中 ,阈值作为判断所切神经小束中能够基本正常发挥有髓纤维传导功能的纤维含量 ,存在着一定的可行性。

A hybrid optical switch （hos） with physical layer of wavelength division multiplexing and optical code division multiplexing （WDm/OCDm） scheme is proposed. An additional feature to the hos than optical cross connect （OXC） is that the controller can process requests for both circuit establishment and burst scheduling. In our study, the measurement criteria of hos are the blocking probability, probability of error, and probability of outage. To simplify the analysis, no distinction is made between a circuit in progress and a burst in progress. moreover, a minimum fit （minF） resource allocation strategy is applied in order to increase the bandwidth efficiency and control the multiplexing interference of the OCDm. A 2D markov model for the hos is presented using the minF strategy. Numerical results reveal that the code parameters and the resource allocation strategy greatly affect the performance. Certain periority can be achieved by assigning shorter codes to high periority users and longer codes to low periority users. also, the probability of error and outage are reduced bv aonling the minF strategy.

Introduction: Cataract surgery is one of the procedures most performed worldwide. Those are several options for anesthetic management, with many factors that affect the choice of any given technique, depending on patients’ needs, surgeons’ preferences and institutional facilities and processes. although being more invasive than topic anesthesia, and with possible hazardous side effects, peribulbar block is still an effective and successful technique. The utilization of blunt canula and ultrasound guidance might enhance the safety pattern for this technique. methods: This article presents a revision of 1089 consecutive cases from February 2016 to January 2022, of patients submitted to cataract surgery under peribulbar anesthesia with blunt canula and ultrasound guidance. Results: 1318 surgical records were selected, with 1089 filling the inclusion criteria. There was a higher prevalence of the feminine gender (54%), with a median age of 64.3 (28 - 102) years. mean IAV was 2.9 mL (sd 0.16 mL, 2.5 - 3.0 mL), with total hos 91.1%;OR to reach G2 or above for each (ISB/ImB) was 11.0;CA was 47.9%, with mean FAV of 4.29 mL (sd 2.17mL, 2.5 - 15 mL). In 8.9% patients, hos could not be fully observed. ISB, ImB, CA and FAV were calculated for both groups (non-hos and hos-only). OR for CA (non-hos/hos-only) was 126.21. The main adverse effects were chemosis (2.9%), hyposphagma (5.7%) and high IOP (7.5%). No procedure was postponed due to anesthesia-associated adverse events. Discussion: This study points towards the feasibility of the described technique in an outpatient fashion, with low, mild and tolerable side effects associated.

Heart failure(HF)is a leading cause of morbidity and mortality in the United States.[1]A chronically,disabling and debilitating condition,HF often results in hospital admission and subsequent readmission.[2]After hos-pitalization,nearly 25%of older adults with HF are discharged to skilled nursing facilities(SNFs)for re-habilitation due to difficulties with functioning in-dependently.[3]Critically,patients with HF dis-charged to SNFs also show significantly higher rates of mortality.[2]Identifying factors that predict mortality among this important patient population,especially factors that could be addressed in the hospital setting,is an important step toward im-proving survivability among these patients.

Sepsis and septic shock have continued to produce significant morbidity and mortality, in recent times, in acutely injured patients as well as patients in the intensive care units despite advances in antibiotic therapy and in the cardi ovascular and pulmonary support for these patients. This emphasizes the need to gain a better understanding of the fundamental mechanisms of the pathogenesis of sepsis syndrome in the critically ill or injured patients. The present knowled ge of the mechanisms of septic pathogenesis clearly indicates involvement of mod ulations in the functions of the cells of body's immune defense system, namely, monocytes/macrophages, polymorphonuclear leukocytes, and lymphocytes. most of su ch knowledge has been derived from laboratory experiments in animal models of se ptic injury, or from studies of immune-system cells, in vitro. Clinical stud ies have also supported the role of immune perturbations. Yet, to date, very few the rapeutic approaches are available to effectively counter the sepsis-related immu ne disturbances. Functional modulations in the immune system cells, in the injured/septic hos ts, can exert not only adaptive/beneficial effects but also profoundly adverse effects on the non-immune-system cells such as endothelial, epithelial, neurona l, endocrine, neuro-endocrine, smooth muscle, skeletal muscle, and cardiac muscl e cells. The primary functional modulation in the immune-system cells after inju ry or with critical illness is activation of such cells via molecules from pat hogens, for example, lipopolysaccharide from gram negative organisms, lipoteic hoic acid/peptidoglycan from gram positive organisms, or zymosan from fungi. Suc h pathogenic molecules activate monocytes and tissue macrophages to result in th e expression and release of cytokine mediators (TNFα, IL-1, IL-6, IL-8, and IL- 10), as well as certain lipid mediators (PGE2, LTB4, PAF).
