National trends in resection of the distal pancreas

AIM:To investigate national trends in distal pancreatectomy(DP) through query of three national patient care ***:From the Nationwide Inpatient Sample(NIS,2003-2009),the National Surgical Quality Improvement Project(NSQIP,2005-2010),and the Surveillance Epidemiology and End Results(SEER,2003-2009) databases using appropriate diagnostic and procedural codes we identified all patients with a diagnosis of a benign or malignant lesion of the body and/or tail of the pancreas that had undergone a partial or distal *** of laparoscopy was defined in NIS by the International Classification of Diseases,Ninth Revision correspondent procedure code;and in NSQIP by the exploratory laparoscopy or unlisted procedure current procedural terminology *** SEER,patients were identified by the International Classification of Diseases for Oncology,Third Edition diagnosis codes and the SEER Program Code Manual,third edition procedure *** analyzed the databases with respect to trends of inpatient outcome metrics,oncologic outcomes,and hospital volumes in patients with lesions of the neck and body of the pancreas that underwent operative ***:NIS,NSQIP and SEER identified 4242,2681 and 11 082 DP resections,***,laparoscopy was utilized in 15%(NIS) and 27%(NSQIP).No significant increase was seen over the course of the *** was performed for malignancy in 59%(NIS) and 66%(NSQIP).Neither patient Body mass index nor comorbidities were associated with operative approach(P = 0.95 and P = 0.96,respectively).Mortality(3% vs 2%,P = 0.05) and reoperation(4% vs 4%,P = 1.0) was not different between laparoscopy and open *** complications(10% vs 15%,P < 0.001),hospital costs [44 741 dollars,interquartile range(IQR) 28 347-74 114 dollars vs 49 792 dollars,IQR 13 299-73 463,P = 0.02] and hospital length of stay(7 d,IQR 4-11 d vs 7 d,IQR 6-10,P < 0.001) were less when laparoscopy was *** and two year sur

Background： Pancreatectomies have been identified as procedures with an increased risk of *** surgical patients, readmissions within 30 days of discharge are usually procedure-related. We sought to determine predictors of 30-day readmission following pancreatic resections in a large healthcare ***： We retrospectively collected information from the records of 383 patients who underwent pancreaticresections from 2004–2013. To find the predictors of readmission in the 30 days after discharge,we performed a univariate screen of possible variables using the Fisher’s exact test for categorical variables and the Mann–Whitney U test for continuous variables. Multivariate analysis was used to determinethe independent factors.

Antimicrobial susceptibility testing before first-line treatment for Helicobacter pylori infection in patients with dual or triple antibiotic resistance

AIM To evaluate the efficacy of antimicrobial susceptibilityguided therapy before first-line treatment for infection in patients with dual or triple antibiotic *** A total of 1034 patients infected by Helicobacter pylori(H. pylori) during 2013-2014 were tested for antimicrobial susceptibility. 157 of 1034(15%) patients showed resistance to two(127/1034; 12%) and to three(30/1034; 3%) antibiotics. Sixty-eight patients with dual H. pylori-resistance(clarithromycin, metronidazole or levofloxacin) were treated for 10 d with triple therapies: OAL(omeprazole 20 mg b.i.d., amoxicillin 1 g b.i.d., and levofloxacin 500 mg b.i.d.) 43cases, OAM(omeprazole 20 mg b.i.d., amoxicillin 1 g b.i.d., and metronidazole 500 mg b.i.d.) 12 cases and OAC(omeprazole 20 mg ***., amoxicillin 1 g b.i.d., and clarithromycin 500 mg b.i.d.) 13 cases based on the antimicrobial susceptibility testing. Twelve patients showed triple H. pylori-resistance(clarithromycin, metronidazole and levofloxacin) and received for 10 d triple therapy with OAR(omeprazole 20 mg ***., amoxicillin 1 g b.i.d., and rifabutin 150 mg b.i.d.). Eradication was confirmed by 13C-urea breath test. Adverse effects and compliance were assessed by a questionnaire. RESULTS Intention-to-treat eradication rates were: OAL(97.6%), OAM(91.6%), OAC(92.3%) and OAR(58.3%). Cure rate was significantly higher in na?ve patients treated with OAR-10 compared to patients who had two or three previous treatment failures(83% vs 33%). Adverse events rates for OAL, OAM, OAC and OAR were 22%, 25%, 23% and 17%, respectively, all of them mild-moderate. CONCLUSION Antimicrobial susceptibility-guided triple therapies during 10 d for first-line treatment leads to an eradication rate superior to 90% in patients with dual antibiotic H. pylori resistance.

Robust H-Filtering for Lipschitz Nonlinear Systems via Multiobjective Optimization

In this paper, a new method of filtering for Lipschitz nonlinear systems is proposed in the form of an LMI optimization problem. The proposed filter has guaranteed decay rate (exponential convergence) and is robust against unknown exogenous disturbance. In addition, thanks to the linearity of the proposed LMIs in the admissible Lipschitz constant, it can be maximized via LMI optimization. This adds an extra important feature to the observer, robustness against nonlinear uncertainty. Explicit bound on the tolerable nonlinear uncertainty is derived. The new LMI formulation also allows optimizations over the disturbance attenuation level ( cost). Then, the admissible Lipschitz constant and the disturbance attenuation level of the filter are simultaneously optimized through LMI multiobjective optimization.

Kinematic Decoupled Core Paradigm: Counterrotation or Just Warping

Several mechanisms have been proposed in recent years to explain kinematic decoupled cores (KDCs) in early type galaxies as well as the large differences in angular momentum between KDCs and host galaxy. Most of the proposed scenarios involve large fractions of merging events, high speed interactions with dwarf spheroidal galaxies, cusp effect of the dark matter density profiles, etc. We here argue that counterrotation as well as fast and slow rotation of disks or spheroids at the center of galaxies can also be explained by a misalignment of the central spheroid equatorial plane with regard to that defined by the observed external stellar rotation. Contrary to what happens at the outer region of disk galaxies, once instability has led to the inner warped core, the perturbed orbits can maintain a common orientation due to the rigid body like rotation at the central region of the galaxy. The spatial configuration that furnishes the smallest angular momentum difference between the KDC and the host galaxy is completely defined by observed parameters in the plane of the sky, namely, the inclination of the inner and outer disks and the angle between the two lines of nodes. As an example we modeled the paradigmatic and extreme case of the 2D radial velocity field of NGC 4382 nucleus. Tilt angles of the KDC not larger than 30 degrees also allow explaining fast and low rotators of the called “Sauron paradigm” in a unified scenario. The maximum for the three parameters, namely, velocity of the inner rotator, difference of position angle and difference with the outer rotation velocity of the whole Sauron sample, are consistently correlated in agreement with the proposed scenario. These quantities do not correlate with the galaxies magnitude, mass (since large and dwarf spheroidals show apparent counterrotation as well) or environment, also suggesting that an internal phenomenon like the central spheroid warping, that we are here proposing, may be at work.

For a long time and until now, rubber is the most used material for the manufacture of tires for motor vehicles. Unfortunately, once the tire meets its life cycle, the remaining rubber cannot be recycled, so the tires are discarded in collection centers and often in clandestine dumps. This represents a serious environmental problem because, in one case, these waste tires become breeding grounds for insects and wildlife that is harmful to humans. In the second case, the tires are burned, releasing highly damaging gases into the atmosphere. On the other hand, concrete is worldwide the construction material par excellence. It is basically composed of cement, gravel and sand. Mixing these three components in different proportions, their mechanical strength in compression can be increased. However, due to its fragile nature, concrete, once a crack is formed, it rapidly advances by fragmenting the material and producing its rapid collapse. In the present work, in order contribute to the care of the environment as well as to modify the fracture mode of the concrete, rubber particles obtained from waste tires were used as sand substitute in hydraulic concrete. In addition, rubber modified samples concrete were lately exposed to 70 kGy of gamma radiation in order to study the effects of this radiation on the mechanical deformation of concrete. The results showed a decrease in the mechanical properties of the concrete with rubber particles with respect to the traditional concrete itself. However, such decreases were offset by the fact that samples with rubber addition do not collapses as fast as the free rubber samples. The acquired data pave the way for research with great benefits, such as the use of recycled tires in concrete for its fracture mode modification in a beneficial way, as well as a possible decrease in the cost of concrete.
