背景与目的奥沙利铂、伊立替康、氟尿嘧啶和亚叶酸的联合化疗方案(FOLFIRINOX)极大提高了晚期胰腺癌患者的生存期。然而,FOLFIRINOX方案作为吉西他滨失败后二线治疗的疗效尚未进行前瞻性的检测。我们研究了减量FOLFIRINOX方案治疗吉西他滨抵抗的晚期胰腺癌患者的可行性和安全性。方法在14家医院进行了这一多中心II期前瞻、开放标签的单臂研究。经组织学确诊为胰腺浸润性导管腺癌、有可测量或可评价的病灶、东部肿瘤协作组体能状况评分0或1分、器官功能良好、年龄19岁或以上的患者符合入组条件。减量FOLFIRINOX方案的组成:奥沙利铂65 mg/m2、伊立替康135 mg/m2和亚叶酸400 mg/m2,第1天静脉注射;5?氟尿嘧啶2000 mg/m2,第1–2天持续静脉输注46 h;每2周重复。主要终点为FOLIFLIOX治疗起始后的无进展生存期。次要终点为客观缓解率、疾病控制率、总生存期、安全性和耐受性。采用Kaplan?Meier法评估了总生存期和无进展生存期。结果我们招募了来自14个中心的39例患者。客观缓解率为10.3%,疾病控制率为64.1%。6个月和1年总生存率分别为59.0%和15.4%。中位无进展生存期和总生存期分别为3.8个月[95%置信区间(confidence interval,CI)=1.5–6.0个月]和8.5个月(95%CI=5.6–11.4个月)。3或4级不良事件为中性粒细胞减少(41%)、恶心(10.3%)、食欲减退(10.3%)、贫血(7.7%)、黏膜炎(7.7%)、肺炎/胸膜融合(5.1%)和乏力(5.1%)。发生1例由感染性休克引起的治疗相关死亡。结论减量FOLFIRINOX方案有望作为二线方案治疗吉西他滨抵抗的胰腺癌。

目的评价卵巢恶性肿瘤患者治疗后;F-FDG PET/CT显像的诊断和预后价值。方法回顾性分析卵巢恶性肿瘤患者治疗后的FDG PET/CT显像结果,计算PET/CT诊断肿瘤复发或第二原发肿瘤的准确性,并评价其对患者诊疗过程的影响。用Cox回归分析PET/CT显像结果与患者总体生存率的关系。结果 268例卵巢恶性肿瘤患者[平均年龄（50±13）岁]共行416次PET/CT显像,其中87次（21%）结果阳性,329次结果阴性;灵敏度、特异性、准确性、阳性预测值和阴性预测值分别为98.8%、98.2%、98.3%、93.1%和99.7%。362次随访显像中,38次（11%）的显像结果使患者治疗决策发生了改变;54次因临床有可疑征象而进行的PET/CT显像中,9次（17%）显像排除了肿瘤复发。PET/CT显像后进行了中位时间26.7个月的随访,24例（9%）患者在随访期内死亡。单因素分析结果发现,患者年龄、国际妇产科联盟（FIGO）分期、复发病史和PET/CT显像结果均与患者的总体生存率相关。多因素分析结果显示,PET/CT显像结果与患者的总体生存率相关（P<0.05）。结论卵巢恶性肿瘤治疗后FDG PET/CT是诊断复发和第二原发肿瘤的有效方法,其便于对患者采用更合适的治疗手段,并能帮助预测预后。

The (Y,Gd)BO 3∶Eu phosphor was synthesized by solid state reaction. The UV spectra showed that in a certain range of Gd 3+ concentration, more Gd 3+ absorbed energy and transferred it to Eu 3+ with its increasing concentration. From the spectra in VUV region, it was observed that both the doping and the concentrations of Gd 3+ , Eu 3+ greatly affected the absorption of the host lattice. The absorbances at 147 nm and 170 nm increased when the Gd 3+ was doped which can be explained as that Gd 3+ transferred energy to BO 4. The optical properties of (Y,Gd)BO 3∶Eu were the best when the concentration of Eu 3+ was about 0.04.

Plasmodium vivax, a major agent of malaria in both temperate and tropical climates, has a limited capacity for infecting humans lacking the Duffy(Fy) blood group antigen as this receptor is critical for erythrocyte invasion. Recent surveys in various endemic regions, however, have reported P. vivax infections in Duffy-negative individuals, suggesting that the parasite may utilize alternative receptor-ligand pairs to complete erythrocyte invasion. Here, we identified and characterized a novel parasite ligand, GPI-anchored micronemal antigen(Pv GAMA), that can bind human erythrocytes regardless of Duffy antigen status. Specific antibodies raised against PvG AMA localize the protein to the parasite microneme, consistent with a role in invasion. These antibodies also inhibited Pv GAMA binding to erythrocytes in a dose-dependent manner. Pv GAMA appears to play a role in a sialic acid-independent, chymotrypsin-sensitive invasion pathway. Thus, Pv GAMA may be an adhesion molecule for invasion of Duffy positive and negative human erythrocytes and is an important P. vivax vaccine candidate.

The quasioptical system of the dual channel receiver is designed and developed for the simultaneous observation with two bands. The frequency independent matching of Gaussian beam is applied to design the quasioptical system with two focusing elements for each band, respectively. The emphasis is on the design to be satisfied with two bands simultaneously. The beam measurement system is also developed to measure the quasioptical system and their components as well as to align the quasioptical elements. We have measured the intensity distributions of the Gaussian beam in the transverse plane and the variations of the beam widths along the propagation axis near the Cassegrain focus. The measured results are also compared the theory.

AIM: To determine the etiology and prognostic factors for neonatal gastric perforation(NGP), a rare but life-threatening ***: Between 1980 and 2011, nine patients un-derwent surgical intervention for NGP at Seoul National University Children's Hospital. The characteristics and prognosis of the patients were retrospectively ***: Among the nine patients, three(33.3%) were preterm babies and five(55.5%) had associated anomalies, which included diaphragmatic eventration(n = 2), congenital diaphragmatic hernia, esophageal atresia with tracheoesophageal fistula, and antral web. Three(33.3%) patients were born before 1990 and three(33.3%) had a birth weight < 2500 g. Pneumo-peritoneum was found on preoperative images in six(66.7%) patients, and incidentally in the other three(33.3%) patients. Surgery was performed within 24 h after the onset of symptoms in seven(77.8%) patients. The overall mortality rate was 22.2%(2/9). The time between symptoms and surgical intervention was the only prognostic factor for survival, whereas premature birth and birth weight were ***: Early detection and advances in neo-natal intensive care may improve the prognosis of NGP.

AIM: To expand the living donor liver transplantation(LT) pool of eligible patients with hepatocellular carcinoma(HCC) using new morphological and biological ***: Patients with HCC who underwent living donor LT(LDLT) from March 2005 to May 2013 at the National Cancer Center Korea(NCCK) were enrolled. We performed the 18F-fluorodeoxyglucose positron emission tomography/computed tomography(PET/CT)before LDLT. Overall and disease-free survival analysis was done in patients to evaluate the usefulness of new NCCK criteria using PET/CT and total tumor size(10 cm).RESULTS: We enrolled a total of 280 patients who pathologically confirmed to have HCC and performed the PET/CT before transplantation. Among them, 164(58.6%) patients fulfilled the NCCK criteria and 132 patients(47.1%) met the Milan criteria. Five-year overall and disease-free survival rates for patients who fulfilled the NCCK criteria showed 85.2% and 84.0%, respectively, and were significantly higher than those beyond the NCCK criteria(60.2% and 44.4%, respectively; P < 0.001). The correlation analysis between preoperative imaging tests and pathologic reports using Cohen's Kappa demonstrated the better results in the NCCK criteria than those in the Milan criteria(0.850 vs 0.583). The comparison of diseasefree analysis among the NCCK, Milan, and University of California, San Francisco(UCSF) criteria using the receiver operating characteristics curves revealed the similar area under the curve value criteria(NCCK vs Milan, P = 0.484; NCCK vs UCSF, P = 0.189 at 5-years).CONCLUSION: The NCCK criteria using hybrid concept of both morphological and biological parameters showed an excellent agreement between preoperative imaging and pathological results, and favorable survival outcomes. These new criteria might select the optimal patients with HCC waiting LDLT and expand the selection pool.
