Cyclophosphamide-associated enteritis presenting with severe protein-losing enteropathy in granulomatosis with polyangiitis:A case report

BACKGROUND Although cyclophosphamide(CPA)is the key drug for the treatment of autoimmune diseases including vasculitides,it has some well-known adverse effects,such as myelosuppression,hemorrhagic cystitis,infertility,and ***,CPA-associated severe enteritis is a rare adverse effect,and only one case with a lethal clinical course has been ***,the appropriate management of patients with CPA-associated severe enteritis is *** SUMMARY We present the case of a 61-year-old woman diagnosed with granulomatosis with polyangiitis based on the presence of symptoms in ear,lung,and,kidney with positive myeloperoxidase-antineutrophil cytoplasmic *** received pulsed methylprednisolone followed by prednisolone 55 mg/d and intravenous CPA at a dose of 500 mg/*** days after the second course of intravenous CPA,she developed nausea,vomiting,and diarrhea,and was admitted to the *** testing revealed hypoalbuminemia,suggesting proteinlosing *** tomography revealed wall thickening of the stomach,small intestine,and colon with contrast enhancement on the lumen *** and immunosuppressive therapy were not effective,and the patient’s enteritis did not improve for>4 *** her condition became seriously exhausted,corticosteroids were tapered and supportive therapies including intravenous hyperalimentation,replenishment of albumin and gamma globulin,plasma exchange,and infection control were *** supportive therapies improved her condition,and her enteritis gradually *** was finally discharged 7 mo *** Immediate discontinuation of CPA and intensive supportive therapy are crucial for the survival of patients with CPA-associated severe enteritis.

An advanced partitioning process for key elements separation from high level liquid waste

To separate MA （Am, Cm） and some fission product elements （FPs） such as Tc, Pd, Cs and Sr from high level liquid waste （HLLW） systematically, we have been studying an advanced aqueous partitioning process, which uses selective adsorption as the separation method. For this process, we prepared several novel adsorbents which were immobilized in a porous sili- c^polymer composite support （SiO2-P）. Adsorption and separation behavior of various elements was studied experimentally in detail. Small scale separation tests using simulated HLLW solutions were carried out. Pd（II） was strongly adsorbed by the AR-01 anion exchanger and effectively eluted off by using thiourea. Successful separation of Pd（ll） from simulated HLLW was achieved. Te（VII） also exhibited strong adsorption on AR-01 and could be eluted off by using U（IV） as a reductive eluent. Am（Ⅲ） presented significantly high adsorbability and selectivity onto R-BTP/SiOz-P adsorbents over various FPs including Ln（Ⅲ）. The R-BTP adsorbents were fairly stable in 3 M HNO3, but instable against 7-irradiation-3M HNO3. An advanced par- titioning process consisting of three separation columns for the target elements separation from HLLW was proposed and the obtained experiment results indicated that the proposed process is essentially feasible.

We compared resistance to decay, mold fungi, termites and insect larvae of particleboards incorporated with the raw boron minerals ulexite and colemanite to that of particleboards impregnated with the commercial boron preservative zinc borate, or boric acid plus a borax mixture. We also quantified water absorption, thickness swell and boron release of particleboards. Ulexite had the best decay resistance, and colemanite had the best termite resistance. However, ulexite and colemanite were not as effective as zinc borate or the boric acid/borax mixture in preventing mold growth. In general, the boric acid/borax mixture combination was more effective against Anobium larvae than the other treatments. Less boron was released by specimens containing zinc borate and colemanite than by those containing ulexite or the boric acid/borax mixture. In general, water absorption and thickness swell were similar among the different treatments, but both were slightly higher in the ulexite-incorporated specimens. Further mechanical tests will be needed to evaluate the particle- board properties and thereby the compatibility of these boron minerals with various manufacturing processes.

Recently emerging disadvantages in conventional disinfection have heightened the need for finding a new solution. Developments in the use of high pressure carbon dioxide for food preservation and sterilization have led to a renewed interest in its applicability in wastewater treatment and water disinfection. Pressurized CO2 is one of the most investigated methods of antibacterial treatment and has been used extensively for decades to inhibit pathogens in dried food and liquid products. This study reviews the literature concerning the utility of CO2 as a disinfecting agent, and the pathogen inactivation mechanism of CO2 treatment is evaluated based on all available research. In this paper, it will be argued that the successful application and high effectiveness of CO2 treatment in liquid foods open a potential opportunity for its use in wastewater treatment and water disinfection. The findings from models with different operating conditions（pressure, temperature, microorganism, water content, media …） suggest that most microorganisms are successfully inhibited under CO2 treatment. It will also be shown that the bacterial deaths under CO2 treatment can be explained by many different ***, the findings in this study can help to address the recently emerging problems in water disinfection, such as disinfection by-products（resulting from chlorination or ozone treatment）.

Pentacene thin film transistor with low threshold voltage and high mobility by inserting a thin metal phthalocyanines interlayer

We herein report the effective performance enhancement of the pentacene-based organic thin film transistors with silicon di- oxide dielectric by inserting a thin metal phthalocyanines interlayer between Au source/drain electrodes and the pentacene active layer. The threshold voltage decreased remarkably from ca. -20 V to a few volts （below -7.6 V） while the mobility in- creased 1.5-3 times after the insertion of the interlayer of only ca. 2 nm, which could be attributed to the reduction of the car- tier injection barrier. The results suggest a simple and effective way to achieve low-threshold-voltage pentacene-based organic thin film transistors with high mobility on silicon dioxide dielectric.

AIM: To utilize transcatheter arterial steroid injection therapy (TASIT) via the hepatic artery to reduce hepatic macrophage activity in patients with severe acute hepatic ***: Thirty-four patients with severe acute hepatic failure were admitted to our hospital between June 2002 to June 2006 providing for the possibility of liver transplantation (LT). Seventeen patients were treated using traditional liver supportive procedures, and the other 17 patients additionally underwent TASIT with 1000 mg methylprednisolone per day for 3 continuous days. RESULTS: Of the 17 patients who received TASIT, 13 were cured without any complications, 2 died, and 2 underwent LT. Of the 17 patients who did not receive TASIT, 4 were self-limiting, 7 died, and 6 underwent LT. Univariate logistic analysis revealed that ascites, serum albumin, prothrombin time, platelet count, and TASIT were significant variables for predicating the prognosis. Multivariate logistic regression analysis using stepwise variable selection showed that prothrombin time, platelet count, and TASIT were independent predictive factors. CONCLUSION: TASIT might effectively prevent the progression of severe acute hepatic failure to a fatal stage of fulminant liver failure.

Screening of agonistic activities against four nuclear receptors in wastewater treatment plants in Japan using a yeast two-hybrid assay

To assess the potential endocrine disruptive effects through multiple nuclear receptors （NRs）, especially non-steroidal NRs, in municipal wastewater, we examined the agonistic activities on four NRs （estrogen receptor α, thyroid hormone receptor α, retinoic acid receptor ct and retinoid X receptor α） of untreated and treated wastewater from municipal wastewater treatment plants （WWTPs） in Japan using a yeast two-hybrid assay. Investigation of the influent and effluent of seven WWTPs revealed that agonistic activities against steroidal and non-steroidal NRs were always detected in the influents and partially remained in the effluents. Further investigation of four WWTPs employing conventional activated sludge, pseudo-anoxic-oxic, anoxic-oxic and anaerobic-anoxic-oxic processes revealed that the ability to reduce the agonistic activity against each of the four NRs varies depending on the treatment process. These results indicated that municipal wastewater in Japan commonly contains endocrine disrupting chemicals that exert agonistic activities on steroidal and non-steroidal NRs, and that some of these chemicals are released into the natural aquatic environment. Although the results obtained in yeast assays suggested that measured levels of non-steroidal NR agonists in the effluent of WWTPs were not likely to cause any biological effect, further study is required to assess their possible risks in detail.
