Clinical analysis of patients with deep sternal wound infection-induced sepsis:a retrospective cohort study

Background:This study aimed to summarize the clinical characteristics of patients with deep sternal wound infection-induced sepsis after median sternotomy and improve the treatment outcomes of infection-related sepsis.Methods:A retrospective cohort study was conducted on 21 patients with deep sternal wound infection-induced sepsis after median sternotomy who were admitted to the Department of Critical Care.The clinical manifestations,laboratory test results,infection control,and organ and nutritional support of the patients were summarized,and the follow-up data were obtained.Results:The primary symptoms of deep sternal wound infection-induced sepsis included dyspnea,high fever,chills,and altered state of consciousness.Laboratory test results revealed increased inflammatory markers and decreased oxygenation index.Renal and liver function injury were observed in 8 and 4 patients,respectively;18 and 12 patients demonstrated elevated D-dimer and N-terminal Pro B type natriuretic peptide levels,respectively.Of the 8 patients whose wound secretions tested positive for bacteria,Acinetobacter baumannii and Staphylococcus aureus infections were present in 6 and 2 patients,respectively.One of the 6 patients whose blood cultures tested positive for bacteria demonstrated Candida albicans infection.Fifteen patients received ventilator-assisted ventilation and 2 patients received renal replacement therapy.Of all the 21 patients,17 were cured,2 died,and 2 were discharged.Conclusion:Postmedian sternotomy sepsis attributed to a deep sternal wound infection usually results from a preexisting condition.The most prominent clinical manifestation is dyspnea,which is sometimes accompanied by the impairment of organ function.Infection prevention,proper nutrition support,and maintenance of healthy organ function are the cornerstones for successful treatment outcomes.

As important atmospheric circulation patterns in Northern Hemisphere(NH),the North Atlantic Oscillation(NAO)and the Western Pacific teleconnection(WP)affect the winter climate in Eurasia.In order to explore the combined effects of NAO and WP on East Asian(EA)temperature,the NAO and WP indices are divided into four phases from 1980−2021:the positive NAO and WP phase(NAO+/WP+),the negative NAO and WP phase(NAO−/WP−),the positive NAO and negative WP phase(NAO+/WP−),the negative NAO and positive WP phase(NAO−/WP+).In the phase of NAO+/WP+,the low geopotential height(GH)stays in north of EA at 50°−80°N;the surface air temperature anomaly(SATA)is 0.8−1℃lower than Southern Asian.In the phase of NAO−/WP−,the center of high temperature and GH locate in the northeast of EA;the cold air spreads to Southern Asia,causing the SATA decreases 1−1.5℃.In the phase of NAO+/WP−,the high GH belt is formed at 55°−80°N.Meanwhile,the center of high SATA locates in the north of Asia that increases 0.8−1.1℃.The cold airflow causes temperature dropping 0.5−1℃in the south of EA.The SATA improves 0.5−1.5℃in south of EA in the phase of NAO−/WP+.The belt of high GH is formed at 25°−50°N,and blocks the cold air which from Siberia.The NAO and WP generate two warped plate pressure structures in NH,and affect the temperature by different pressure configurations.NAO and WP form different GH,and GH acts to block and push airflow by affecting the air pressure,then causes the temperature to be different from the north and south of EA.Finally,the multiple linear regression result shows that NAO and WP are weakened by each other such as the phase of NAO+/WP+and NAO−/WP−.

Pyroptosis,a unique lytic programmed cell death,inspired tempting implications as potent anti-tumor strategy in pertinent to its potentials in stimulating anti-tumor immunity for eradication of primary tumors and metastasis.Nonetheless,rare therapeutics have been reported to successfully stimulate pyroptosis.In view of the intimate participation of reactive oxygen species(ROS)in stimulating pyroptosis,we attempted to devise a spectrum of well-defined subcellular organelle(including mitochondria,lysosomes and endoplasmic reticulum)-targeting photosensitizers with the aim of precisely localizing ROS(produced from photosensitizers)at the subcellular compartments and explore their potentials in urging pyroptosis and immunogenic cell death(ICD).The subsequent investigations revealed varied degrees of pyroptosis upon photodynamic therapy(PDT)towards cancerous cells,as supported by not only observation of the distinctive morphological and mechanistic characteristics of pyroptosis,but for the first-time explicit validation from comprehensive RNA-Seq analysis.Furthermore,in vivo anti-tumor PDT could exert eradication of the primary tumors,more importantly suppressed the distant tumor and metastatic tumor growth through an abscopal effect,approving the acquirement of specific anti-tumor immunity as a consequence of pyroptosis.Hence,pyroptosis was concluded unprecedently by our proposed organelles-targeting PDT strategy and explicitly delineated with molecular insights into its occurrence and the consequent ICD.

Multisensory enhancement,as a facilitation phenomenon,is responsible for superior behavioral performance when an individual is responding to cross-modal versus modality-specific stimuli.However,the event-related potential（ERP） counterparts of behavioral multisensory enhancement are not well known.We recorded ERPs and behavioral data from 14 healthy volunteers with three types of target stimuli（modality-specific,bimodal,and trimodal） to examine the spatio-temporal electrophysiological characteristics of multisensory enhancement by comparing behavioral data with ERPs.We found a strong correlation between P3 latency and behavioral performance in terms of reaction time（RT）（R = 0.98,P ＆lt;0.001）,suggesting that P3 latency constitutes a temporal measure of behavioral multisensory enhancement.In addition,a fast RT and short P3 latency were found when comparing the modality-specific visual target with the modality-specific auditory and somatosensory targets.Our results indicate that behavioral multisensory enhancement can be identified by the latency and source distribution of the P3 component.These findings may advance our understanding of the neuronal mechanisms of multisensory enhancement.

Routing Loop Audit Mechanism Based on SDN

Routing loops can cause packet loss and long delay problems in the traditional network. Even in future networks – Software-Defined Networks, with loop-free implementation mechanism, it still suffers loop problems. In this paper, we propose an architecture to solve the loop problem in SDN. Unlike the existing passive routing loop detection algorithm, this algorithm processes based on the SDN loop-free characteristic, by auditing third-party network forwarding rules’ modification requests to avoids loop generation, thus avoid the network loop problems. Testbed is built to simulate our proposed algorithm. The evaluation shows that the loop audit algorithm proposed in this paper has better performance in extra spatial utilization and smaller number of extra interactions between SDN controller and switches.

Surface modification by poly(ethylene glycol)(PEGylation)has been acknowledged as a powerful strategy in minimizing non-specific reactions for biomedical devices.Once applied into manufacture of drug/gene delivery systems,PEGylation has demonstrated to significantly improve their biocompatibility and stealthiness in physiological environment.Nonetheless,reluctant cell membrane affinities thus cellular uptake efficiencies owing to PEGylation brought up further issues that are imperative to be resolved.Pertain to this PEGylation dilemma,we attempted to introduce peptide(GPLGVRG)linkage between block copolymer of PEG-poly{N’-[N-(2-aminoethyl)-2-aminoethyl]aspartamide}PAsp(DET),wherein the cationic PAsp(DET)could self-assemble with pDNA into nanoscaled complex core.Noteworthy was the peptide linkage whose amino acids sequence could be specifically recognized and degraded by matrix metalloproteinases(MMPs)(overexpressed in extracellular milieu of tumors).Therefore,our subsequent studies validated facile detachment of PEGylation from the aforementioned polyplex micelles upon treatment of MMPs,which elicited improved cytomembrane affinities and cellular uptake efficiencies.In addition,promoted escape from endosome entrapment was also confirmed through direct endosome membrane destabilization by PAsp(DET),which was further elucidated to be attributable to dePEGylation as well as elevated charged density of PAsp(DET)in acidic endosomes.These benefits from dePEGylation eventually contributed to promoted gene expression at the affected cells and potent tumor growth suppression based on anti-angiogenic approach.Therefore,our developed strategy has provided a facile approach in overcoming the dilemma of PEGylation,which could be informative in design of drug/gene delivery systems.

An SDN Oriented Loop Detection Mechanism Based on TTL Statistics

The rise of the network has injected new impetus into the development of traditional networks.Due to the complexity of the network itself and its network programmability,there is a risk of routing loops occurring in the SDN network.This paper proposes a loop detection mechanism.According to the Time To Live(TTL)value of the loop packet,there is approximately periodicity in the same loop.We use sFlow to count the number of packets corresponding to each TTL value of a switch in the loop over a period of time,and perform discrete Fourier transform on the obtained finite-length sequence to observe its frequency domain performance and determine whether there are periodic features.By doing so,it is determined whether there is a routing loop,and the purpose of passively detecting the routing loop is achieved.Compared to existing algorithms,it has advantages in real-time,scalability and false positive rate.The experimental results show that the routing loop detection algorithm based on TTL statistics in this paper still maintains high judgment accuracy under the scenarios of lower stream sampling rate and smaller detection period.

为了解微小膜壳绦虫感染仓鼠小肠组织的病理学特征及黏蛋白2(MUC2蛋白)的表达情况,试验从贵阳市宠物市场随机选取60只宠物仓鼠,采用饱和盐水浮聚法检测仓鼠粪便中的虫卵并进行鉴定,根据仓鼠感染微小膜壳绦虫的情况将仓鼠分为未感染组、无症状感染组、普通型感染组和重型感染组,解剖所有仓鼠获取肠道内寄生虫及小肠组织,制备小肠组织切片并进行苏木精-伊红(H.E.)染色、阿利新蓝-过碘酸-雪夫(AB-PAS)染色及免疫组织化学染色,通过实时荧光定量PCR(qPCR)扩增分析仓鼠小肠MUC2基因相对表达情况。结果表明:微小膜壳绦虫在宠物仓鼠中的感染率为74.5%,其感染可引起仓鼠小肠绒毛损伤、上皮细胞变性坏死、杯状细胞异常、白细胞浸润等病理改变;与未感染组相比,无症状组杯状细胞数量及MUC2基因相对表达量差异不显著(P>0.05),普通型感染组的杯状细胞数量极显著增加(P<0.01),重型感染组的杯状细胞极显著减少(P<0.01),同时重型感染组的MUC2基因及MUC2蛋白表达水平极显著增高(P<0.01)。说明微小膜壳绦虫感染会导致仓鼠小肠绒毛结构破坏,杯状细胞数量及形态异常,并引起杯状细胞分泌MUC2蛋白增多,且重型感染组杯状细胞数量减少,但其分泌的MUC2蛋白量增加。

在跨孤岛联邦学习中,各参与者对最终训练出的模型贡献各异,如何评估他们的贡献并给予适当激励,成为联邦学习研究中一项关键问题。目前的激励方法主要着眼于奖励有效模型更新的参与者,同时惩罚不诚实者,侧重于激励计算行为。然而,参与者所提供的数据质量同样影响学习效果,但现有方法未充分考虑数据质量,并缺乏鉴定数据真实性的手段。为提升激励的准确性,需对参与者数据质量进行评估。通过融合零知识证明与区块链技术,文章提出一种评估参与者数据质量的协议,构建了全新联邦学习激励方案。该方案可在不泄露明文数据的前提下,评估参与者所用数据集质量,通过区块链系统向合格参与者发放激励,拒绝不合格者。实验证实,在部分用户提供虚假数据的情况下,该方案仍能准确给出激励结果,同时提升联邦学习模型的准确率。
