AIM:to analyze the change of dimethylarginine plasma levels in cirrhotic patients receiving transjugular intrahepatic portosystemic shunt(tIPS).MEtHODS:to determine arginine,asymmetric dimethylarginine(ADMA),symmetric dimethylarginine(SDMA),and nitric oxide(NO) plasma levels,blood samples were collected from the superior cava,hepatic,and portal vein just before,directly after,and 3 mo after ***:A significant increase in the arginine/ADMA ratio after tIPS placement was ***,tIPS placement enhanced renal function and thereby decreased systemic SDMA *** patients with renal dysfunction before tIPS placement,both the arginine/ADMA ratio and creatinine clearance rate increased significantly,while this was not the case in patients with normal renal function before tIPS *** function did not change significantly after tIPS placement and no significant decline in ADMA plasma levels was ***:the increase of the arginine/ADMA ratio after tIPS placement suggests an increase in intracellular NO *** addition,this study suggests that tIPS placement does not alter dimethylarginine dimethylaminohydrolase(DDAH) activity and confirms the major role of the liver as an ADMA clearing organ.

Aim: to investigate whether unilateral vision loss reduced any aspects of qual ity of life in comparison with normal vision and to compare its impact with that of bilateral vision loss. Methods: this study used cluster stratified random sa mple of 3271 urban participants recruited between 1992 and 1994 for the Melbourn e Visual Impairment Project. All predictors and outcomes were from the 5 year fo llow up examinations conducted in 1997-9. Results: there were 2530 participants who attended the follow up survey and had measurement of presenting visual acui ty. Both unilateral and bilateral vision loss were significantly associated with increased odds of having problems in visual functions including reading the tel ephone book, newspaper, watching television, and seeing faces. Non-correctable by refraction unilateral vision loss increased the odds of falling when away fro m home (OR=2.86, 95%CI 1.16 to 7.08), getting help with chores (OR=3.09, 95%CI 1.40 to 6.83), and becoming dependent (getting help with meals and chores) (OR =7.50, 95%CI 1.97 to 28.6). Non-correctable bilateral visual loss was associat ed with many activities of daily living except falling. Conclusions: Non-correc table unilateral vision loss was associated with issues of safety and independen t living while non-correctable bilateral vision loss was associated with nursin g home placement, emotional wellbeing, use of community services, and activities of daily living. Correctable or treatable vision loss should be detected and at tended to.

Perception of the Psychosocial Aspects of Subfertility by Parents Following Successful Medically Assisted Conception: A Qualitative Study

Nearly 5% of new-borns in the Netherlands are now born following Medically Assisted Conception (MAC), a steadily increasing number. Our qualitative study explored the subjective impact of subfertility and fertility treatment for parents who successfully achieve pregnancy following MAC and make health professionals more aware of the psychological needs of those clients. the study was based on the constructive/interpretative paradigm using a constant comparison/grounded theory design. two men and nine women in the Netherlands who conceived as a result of MAC were interviewed in 2011. Our findings show that MAC-parents were exposed to a range of emotions such as disappointment, hope and uncertainty during the subfertile period, and these feelings were even felt after successful fertility treatment. the relationship and social support from friends and others in the same position were very important. Regarding the relationship with the partner during the subfertile period, the main thing the couple have to do is to keep talking with one another. Although they did not always agree on all issues, it was found that maintaining the communication strengthened the links between them by the end of the process. We concluded that the feelings and emotions during fertility treatment play a big role during the subfertile period and remain important during pregnancy and even after childbirth. Subfertile clients want maternity care providers to ask those who successfully achieve pregnancy following MAC about their experiences during fertility treatment, check that they still are receiving adequate social support, and involve the partner in prenatal checks.

Human exposure to consumer chemicals has become a worldwide *** this work,a comprehensive sampling strategy is presented,to our knowledge being the first to study all relevant exposure pathways in a single cohort using multiple methods for assessment of exposure from each exposure *** selected groups of chemicals to be studied are consumer chemicals whose production and use are currently in a state of transition and are;per-and polyfluorinated alkyl substances(PFASs),traditional and“emerging”brominated flame retardants(BFRs and EBFRs),organophosphate esters(OPEs)and phthalate esters(PEs).Information about human exposure to these contaminants is needed due to existing data gaps on human exposure intakes from multiple exposure pathways and relationships between internal and external *** environment,food and biological samples were collected from 61 participants and their households in the Oslo area(Norway)on two consecutive days,during winter ***,dust,hand wipes,and duplicate diet(food and drink)samples were collected as indicators of external exposure,and blood,urine,blood spots,hair,nails and saliva as indicators of internal exposure.A food diary,food frequency questionnaire(FFQ)and indoor environment questionnaire were also *** 2000 samples were collected in total and participant views on their experiences of this campaign were collected via *** 91%of our participants were positive about future participation in a similar project,some tasks were viewed as *** the food diary and collection of duplicate food/drink portions were the tasksmost frequent reported as“hard”/“very hard”.Nevertheless,a strong positive correlation between the reported total mass of food/drinks in the food record and the total weight of the food/drinks in the collection bottles was observed,being an indication of accurate performance of the participants despite the challenges of the sampling campaign.

Patient involvement in lung foundation research: A seven year longitudinal case study

Patient involvement in health research is getting more accepted over the years. Until recently scientists and medical professionals were the sole assessors of quality and relevance of research proposals. In the Netherlands, as in other European and North American countries, emancipatory, political and democratic developments stimulated the emergence of patient involvement as a new “voice” in the appraisal of research. A time-series cross sectional longi-tudinal case study was used to describe and analyse a seven year period since the introduction of the patients’ perspective in the Long-fonds research cycle. Longfonds, the Lung Foundation in the Netherlands (LFN) was formerly called “Astma Fonds”. the study was conducted using an actors-interaction model against the background of the dynamics in society. the introduction of patient involvement resulted in a paradigm shift. the scientific and societal relevance of research proposals are now being reviewed by all parties in a more ef- fective and efficient way. Patients, now involved in the review procedure of research funding, are trained and equipped with an appraisal tool for societal relevance from a patients perspective. Scientific relevance and societal relevance are evaluated separately and balanced in the re- search funding application approval process. Societal relevance is being evaluated by a pa- tient advocates group. the results show how a government initiative and an approach by a patient organisation have led to more patient involvement in lung research. It requires “believers” both to initiate and continue the work and to promote the lessons learned inside and outside the patient organisation. As this depends on devoted individuals, the continuity of patient involvement remains vulnerable. this seven year study offers valuable insight in patient involvement against the background of the changeing health discourse.

the first search for the doubly heavy Ωbc0baryon and a search for the Ξbc0baryon are performed using pp collision data collected via the LHCb experiment from 2016 to 2018 at a centre-of-mass energy of 13 teV,corresponding to an integrated luminosity of 5.2 *** baryons are reconstructed via their decays to ∧c+π-andΞc+π-.No significant excess is found for invariant masses between 6700 and 7300 MeV/c2,in a rapidity range from2.0 to 4.5 and a transverse momentum range from 2 to 20 MeV/*** limits are set on the ratio of the Ωbc0andΞbc0production cross-section times the branching fraction to ∧c+π-（Ξc+π-） relative to that of the ∧b0（Ξb0） baryon,for different lifetime hypotheses,at 95% confidence *** upper limits range from 0.5×10-4to 2.5×10-4for theΩbc0→∧c+π-(Ξbc0→∧c+π-) decay,and from 1.4×10-3to 6.9×10-3for the Ωbc0→Ξc+π-(Ξbc0→Ξc+π-) decay,depending on the considered mass and lifetime of the Ωbc0(Ξbc0) baryon.

Objective: We examined the hypothesis that cognitive dy-sfunction in Alzheimer’s disease is associated with abnormal spontaneous fluc tuations of EEG synchronization levels during an eyes-closed resting state. Met hods: EEGs were recorded during an eyes-closed resting state in Alzheimer patie nts (N=24;9 males; mean age 76.3 years; SD 7.8; range 59-86) and nondemented su bjects with subjective memory complaints (N=19;9males;mean age 76.1 years; SD 6. 7; range: 67-89). the mean level of synchronization was determined in different frequency bands with the synchronization likelihood and fluctuations of the syn chronization level were analysed with detrended fluctuation analysis (DFA). Resu lts: the mean level of EEG synchronization was lower in Alzheimer patients in th e upper alpha (10-13 Hz) and beta (13-30 Hz) band. Spontaneous fluctuations of synchronization were diminished in Alzheimer patients in the lower alpha (8-10 Hz) and beta bands. In patients as well as controls the synchronization fluctuat ions showed a scale-free pattern. Conclusions: Alzheimer’s disease is characte rized both by a lower mean level of functional connectivity as well as by dimini shed fluctuations in the level of *** dynamics of these fluctuat ions in patients and controls was scale-free which might point to self-organiz ed criticality of neural networks in the brain. Significance: Impaired functiona l connectivity can manifest itself not only in decreased levels of synchronizati on but also in disturbed fluctuations of synchronization levels.

AIM: to improve the interpretation of fecal immunochemical test (FIt) results in colorectal cancer (CRC) cases from screening and referral cohorts. MEtHODS: In this comparative observational study, two prospective cohorts of CRC cases were compared. the first cohort was obtained from 10 322 average risk subjects invited for CRC screening with FIt, of which, only subjects with a positive FIt were referred for colonoscopy. the second cohort was obtained from 3637 subjects scheduled for elective colonoscopy with a positive FIt result. the same FIt and positivity threshold (OC sensor; ≥ 50 ng/mL) was used in both cohorts. Colonoscopy was performed in all referral subjects and in FIt positive screening subjects. All CRC cases were selected from both cohorts. Outcome measurements were mean FIt results and FIt scores per tissue tumor stage (t stage). RESULtS: One hundred and eighteen patients with CRC were included in the present study: 28 cases obtained from the screening cohort (64% male; mean age 65 years, SD 6.5) and 90 cases obtained from the referral cohort (58% male; mean age 69 years, SD 9.8). the mean FIt results found were higher in the referral cohort (829 ± 302 ng/mLvs 613 ± 368 ng/mL,P = 0.02). tissue tumor stage (t stage) distribution was dif-ferent between both populations [screening population: 13 (46%) t1, eight (29%) t2, six (21%) t3, one (4%) t4 carcinoma; referral population: 12 (13%) t1, 22 (24%) t2, 52 (58%) t3, four (4%) t4 carcinoma], and higher t stage was significantly associated with higher FIt results (P tumor stage, no significant difference in mean FIt results was observed (screening vs referral: t1 498 ± 382 ng/mL vs 725 ± 374 ng/mL, P = 0.22; t2 787 ± 303 ng/mL vs 794 ± 341 ng/mL, P = 0.79; t3 563 ± 368 ng/mLvs 870 ± 258 ng/mL,P = 0.13; t4 not available). After correction for t stage in logistic regression analysis, no significant differences in mean FIt results were observed between both types of cohorts (P = 0.10). CONCLUSION
