Pathological diagnosis and immunohistochemical analysis of giant retrosternal goiter in the elderly: A case report

BACKGROUND Elderly giant retrosternal thyroid goiter is a rare yet significant medical condition,often presenting clinical symptoms that can be confused with other diseases,posing diagnostic and therapeutic challenges.This study aims to delve into the characteristics and potential mechanisms of this ailment through pathological diagnosis and immunohistochemical analysis,providing clinicians with more precise diagnostic and treatment strategies.A 77-year-old male,was admitted to hospital with the chief complaint of finding a goiter in the semilunar month during physical examination,accompanied by dyspnea.Locally protruding into the superior mediastinum,the adjacent structure was compressed,the trachea was compressed to the right,and the local lumen was slightly narrowed.The patient was diagnosed with giant retrosternal goiter.Considering dyspnea caused by trachea compression,our department planned to perform giant retrosternal thyroidectomy.Immunohistochemical results:Tg(+),TTF-1(+),Calcitonin(CT)(I),Ki-67(+,about 20%),CD34(-).Retrosternal goiter means that more than 50%of the volume of the thyroid gland is below the upper margin of the sternum.As retrosternal goiter disease is a relatively rare disease,once the disease is diagnosed,it should be timely surgical treatment,and the treatment is more difficult,the need for professional medical team for comprehensive treatment.CONCLUSION The imaging manifestations of giant retrosternal goiter are atypical,histomorphology and immunohistochemistry can assist in its diagnosis.This article reviews the relevant literature of giant retrosternal goiter immunohisto-chemistry and shows that giant retrosternal goiter is positive for Tg,TTF-1,and Ki-67.

In a perfect quantum key distribution(QKD)protocol,quantum states should be prepared and measured with mutually unbiased bases(MUBs).However,in a practical QKD system,quantum states are generally prepared and measured with imperfect MUBs using imperfect devices,possibly reducing the secret key rate and transmission distance.To analyze the security of a QKD system with imperfect MUBs,we propose virtual MUBs to characterize the quantum channel against collective attack,and analyze the corresponding secret key rate under imperfect state preparation and measurement conditions.More generally,we apply the advantage distillation method for analyzing the security of QKD with imperfect MUBs,where the error tolerance and transmission distance can be sharply improved.Our analysis method can be applied to benchmark and standardize a practical QKD system,elucidating the security analysis of different QKD protocols with imperfect devices.

Layered oxide cathodes for sodium-ion batteries:microstructure design,local chemistry and structural unit

Because of the low price and abundant reserves of sodium compared with lithium,the research of sodium-ion batteries(SIBs)in the field of large-scale energy storage has returned to the research spotlight.Layered oxides distinguish themselves from the mains cathode materials of SIBs owing to their advantages such as high specific capacity,simple synthesis route,and environmental benignity.However,the commercial development of the layered oxides is limited by sluggish kinetics,complex phase transition and poor air stability.Based on the research ideas from macro-to micro-scale,this review systematically summarizes the current optimization strategies of sodium-ion layered oxide cathodes(SLOC)from different dimensions:microstructure design,local chemistry regulation and structural unit construction.In the dimension of microstructure design,the various structures such as the microspheres,nanoplates,nanowires and exposed active facets are prepared to improve the slow kinetics and electrochemical performance.Besides,from the view of local chemistry regulation by chemical element substitution,the intrinsic electron/ion properties of SLOC have been enhanced to strengthen the structural stability.Furthermore,the optimization idea of endeavors to regulate the physical and chemical properties of cathode materials essentially is put forward from the dimension of structural unit construction.The opinions and strategies proposed in this review will provide some inspirations for the design of new SLOC in the future.

Fast charge transfer kinetics in Sv-ZnIn_(2)S_(4)/Sb_(2)S_(3)S-scheme heterojunction photocatalyst for enhanced photocatalytic hydrogen evolution

Constructing a S-scheme heterojunction with tight interface contact and fast charge transfer is beneficial to improving the photocatalytic hydrogen evolution performance.Herein,a unique one-dimensional(1D)/two-dimensional(2D)S-scheme heterojunction containing 1D Sb_(2)S_(3) nanorods and 2D ZnIn_(2)S_(4) with affluent sulfur vacancies(denoted as Sv-ZnIn_(2)S_(4)@Sb_(2)S_(3)) was designed.The introduced sulfur vacancy can promote the effective adsorption of H+for the following interfacial hydrogen-evolution reaction.Furthermore,the larger contact area and stronger electron interaction between Sb_(2)S_(3) and ZnIn_(2)S_(4) effectively inhibits the recombination of photo-generated electron–hole pairs and abridges the migration distance of charges.As a result,the optimal Sv-ZnIn_(2)S_(4)@Sb_(2)S_(3) sample achieves H_(2) evolution activity of 2741.3 mol·h^(−1)·g^(−1),which is 8.6 times that of pristine ZnIn_(2)S_(4) and 3.0 times that of the Sv-ZnIn_(2)S_(4) samples.Based on the experimental result,the photo-reactivity S-scheme mechanism of hydrogen evolution from water splitting with Sv-ZnIn_(2)S_(4)@Sb_(2)S_(3) is proposed.This work provides an effective method for developing S-scheme heterojunction composites of transition metal sulfide with high hydrogen evolution performance.

Nanoelectrodes have been shown to be powerful tools for nanoscopic research on single entities,deepening the understanding of heterogeneity and stochastics for single molecules,nanoparticles and cells[1–3].However,despite their importance,current nanoelectrode preparations are highly restrained to flame/electrochemical etching,metal sputtering,electrochemical deposition,etc.[3],resulting in a limited range of materials(mainly gold,platinum and carbon)for potential-resolved detection of redox-active species(Table S1 online).The enrichment of preparation methods to introduce novel materials and mechanisms should prove significant for the development of nanoelectrodes.

Identification of an immune-related gene signature for predicting prognosis and immunotherapy efficacy in liver cancer via cell-cell communication

BACKGROUND liver cancer is one of the deadliest malignant tumors worldwide.Immunotherapy has provided hope to patients with advanced liver cancer,but only a small fraction of patients benefit from this treatment due to individual differences.Identifying immune-related gene signatures in liver cancer patients not only aids physicians in cancer diagnosis but also offers personalized treatment strategies,thereby improving patient survival rates.Although several methods have been developed to predict the prognosis and immunotherapeutic efficacy in patients with liver cancer,the impact of cell-cell interactions in the tumor microenvir-onment has not been adequately considered.AIM To identify immune-related gene signals for predicting liver cancer prognosis and immunotherapy efficacy.METHODS Cell grouping and cell-cell communication analysis were performed on single-cell RNA-sequencing data to identify highly active cell groups in immune-related pathways.Highly active immune cells were identified by intersecting the highly active cell groups with B cells and T cells.The significantly differentially expressed genes between highly active immune cells and other cells were subsequently selected as features,and a least absolute shrinkage and selection operator(LASSO)regression model was constructed to screen for diagnostic-related features.Fourteen genes that were selected more than 5 times in 10 LASSO regression experiments were included in a multivariable Cox regression model.Finally,3 genes(stathmin 1,cofilin 1,and C-C chemokine ligand 5)significantly associated with survival were identified and used to construct an immune-related gene signature.RESULTS The immune-related gene signature composed of stathmin 1,cofilin 1,and C-C chemokine ligand 5 was identified through cell-cell communication.The effectiveness of the identified gene signature was validated based on experi-mental results of predictive immunotherapy response,tumor mutation burden analysis,immune cell infiltration analysis,survival analysis,and expression an

Maternal weight and its association with risk of overweight in offspring:a trajectory analysis from a birth cohort in China

Background Most studies on the association of maternal pregnancy weight with offspring weight trajectory have a short follow-up time.This study aimed to explore the associations of maternal prepregnancy body mass index(BMI)and gestational weight gain(GWG)with childhood weight trajectories in a 7-year birth cohort.Methods A total of 946 mother–child pairs(467 boys and 479 girls)from a longitudinal birth cohort in Tianjin City,China,were included in this study,ranging from pregnancy to offspring at 7 years.The outcome variable was defined as overweight or not overweight in offspring at the last round.A group-based trajectory model was applied to identify childhood BMI trajectory groups.Results Five discrete BMI trajectory groups were identified and characterized as constant underweight(25.2%),constant normal weight(42.8%),and high or increasing trajectory[at risk of overweight(16.9%),progressive overweight(11.0%)and progressive obesity(4.1%)].Maternal prepregnancy overweight was associated with 1.72(95%CI 1.14–2.60,P=0.01)to 4.02(95%CI 1.94–8.36,Pweight[relative risk ratio(RRR)2.09,95%CI 1.27–3.46,P=0.004]and progressive obesity(RRR 3.33,95%CI 1.13–9.79,P=0.029).Children in all high or increasing trajectory groups were associated with greater overweight risk at the last round[risk ratios(RRs)ranged from 3.54(95%CI 2.53–4.95,Pweight and excessive gestational weight gain were associated with increasing or high-level childhood body mass index trajectories as well as a greater risk of overweight at 7 years.

Erosion Characteristics of Oil-immersed On-load Tap Changer Contacts Under Varying Contact Speeds and Pressures

With the growing demand for precise voltage adjustment and reactive regulation,the frequent operation of on-load tap changers(OLTCs)in oil-immersed systems has led to increased erosion of switch contacts by arcs during the switching process.This erosion causes significant wear on the contacts,thereby reducing their lifespan.Therefore,the present study aims to investigate the behavior and mechanism of arc erosion on contact surfaces in oil-immersed OLTCs.To achieve this,a self-designed friction and wear test device for OLTC contacts was utilized to conduct experiments at various sliding speeds and contact pressures.Additionally,finite element analysis was employed to validate the experimental results regarding the influence of sliding speed on arc energy.The surface morphology of the contacts was observed using an optical microscope.The findings revealed that as the sliding speed increased,the arc energy,arc initiation rate,and contact resistance initially exhibited an upward trend,then decreased,and eventually increased again.The minimum values were observed at a sliding speed of 90 mm/s.Moreover,the arc energy,arc initiation rate,and contact resistance decreased gradually as the contact pressure increased.After reaching a contact pressure of 1.5 N,the variation in the arc energy stabilized.At lower contact pressures,arc erosion dominated the wear on the contact surface.However,at higher contact pressures,the wear transitioned from predominantly arc erosion to a combination of mechanical wear and arc erosion.In summary,experimental and analytical investigations provided insights into the effects of sliding speed and contact pressure on the behavior of arc erosion,contact resistance,and surface damage of OLTC contacts in oil-immersed systems.

Effect of oxide inclusions on MnS precipitates and tensile mechanical property of high-strength low-alloy steel

The key role of oxide inclusions on the microstructure and mechanical property of a high-strength low-alloy steel was investigated.The field emission scanning electron microscope equipped with energy-dispersive spectrometry was used to characterize MnS precipitates.Oxide inclusions play an important role in the shape control of MnS precipitates.More oxides fovored to decrease the size and the aspect ratio of MnS precipitates.With less oxide inclusions in the steel,approximately over 16.7%MnS precipitates were with aspect ratio a>5 and pure MnS precipitates accounted for 75.9%in number.However,with more oxide inclusions in the steel,only 7.4%MnS precipitates were with a>5 and pure MnS precipitates accounted for 60.1%in number.Refinement of MnS by oxide inclusions improved the strength and inhibited the anisotropy.More oxide inclusions in the steel increased the yield strength and tensile strength of the steel in both longitudinal and transverse directions,and lowered the anisotropy of the mechanical property.

Combined Promoting Effects of Specific Organic Functional Groups and Alumina Surface Characteristics for the Design of a Highly Efficient NiMo/Al_(2)O_(3) Hydrodesulfurization Catalyst

To prepare a highly efficient NiMo/Al_(2)O_(3) hydrodesulfurization catalyst,the combined effects of specific organic functional groups and alumina surface characteristics were investigated.First,the correlation between the surface characteristics of four different alumina and the existing Mo species states was established.It was found that the Mo equilibrium adsorption capacity can be used as a specific descriptor to quantitatively evaluate the changes in surface characteristics of different alumina.A lower Mo equilibrium adsorption capacity for alumina means weaker metal-support interaction and the loaded Mo species are easier to transform into MoS2.However,the Mo-O-Al bonds still exist at the metal-support interface.The introduction of cationic surfactant hecadecyl trimethyl ammonium bromide(CTAB)can further improve Mo species dispersion through electrostatic attraction with Mo anions and interaction of its alkyl chain with the alumina surface;meanwhile,the introduction of ethylenediamine tetraacetic acid(EDTA)can complex with Ni ions to enhance the Ni-promoting effect on Mo.Therefore,the NiMo catalyst designed using alumina with lower Mo equilibrium adsorption capacity and the simultaneous addition of EDTA and CTAB exhibits the highest hydrodesulfurization activity for 4,6-dimethyl dibenzothiophene because of its proper metal-support interaction and more well-dispersed Ni-Mo-S active phases.
