Background:The initial randomized,double-blinded,actively controlled,phase III ANEAS study(NCT03849768)demonstrated that aumolertinib showed superior efficacy relative to gefitinib as first-line therapy in epidermal growth factor receptor(EGFR)-mutated advanced non-small cell lung cancer(NSCLC).Metastatic disease in the central nervous system(CNS)remains a challenge in the management of *** study aimed to compare the efficacy of aumolertinib versus gefitinib among patients with baseline CNS metastases in the ANEAS ***:Eligible patients were enrolled and randomly assigned in a 1:1 ratio to orally receive either aumolertinib or gefitinib in a double-blinded *** with asymptomatic,stable CNS metastases were ***-up imaging of the same modality as the initial CNS imaging was performed every 6 weeks for 15 months,then every *** responsewas assessed by a neuroradiological blinded,independent central review(neuroradiological-BICR).The primary endpoint for this subgroup analysis was CNS progression-free survival(PFS).Results:Of the 429 patients enrolled and randomized in the ANEAS study,106 patients were found to have CNS metastases(CNS Full Analysis Set,cFAS)at baseline by neuroradiological-BICR,and 60 of them had CNS target lesions(CNS Evaluable for Response,cEFR).Treatment with aumolertinib significantly prolonged median CNS PFS compared with gefitinib in both cFAS(29.0 vs.8.3 months;hazard ratio[HR]=0.31;95%confidence interval[CI],0.17-0.56;P29.0 vs.8.3 months;HR=0.26;95%CI,0.11-0.57;Plity of CNS progression without prior non-CNS progression or death was consistently lower with aumolertinib than with gefitinib in patients with and without CNSmetastases at *** new safety findings were ***:The

目的系统评价泡沫敷料与纱布敷料对气管切开患者的伤口换药效果。方法全面检索自建库至2023年6月14日在CBM、PubMed、EMBASE等国内外权威数据库的相关临床研究,按纳排标准筛选文献并提取数据,采用RevMan软件进行Meta分析。结果纳入50项研究,共计2496例患者。Meta分析结果显示:气管切开患者泡沫敷料组的切口感染发生率(OR=0.17,95%CI:0.13~0.22,P1)、肺部感染发生率(OR=0.25,95%CI:0.14~0.45,P1)、切口周围皮肤并发症发生率(OR=0.23,95%CI:0.19~0.28,P1)、更换敷料频率(MD=-2.61次/d,95%CI:-3.33~-1.88,P1)、拔管时间(MD=-4.90 d,95%CI:-5.99~-3.80,P1)和切口愈合时间(MD=-4.16 d,95%CI:-5.62~-2.71,P1)均显著低于纱布敷料组,但换药间隔时间(MD=13.17 h,95%CI:10.69~15.65,P1)明显长于纱布敷料组。结论与传统无菌纱布敷料相比,泡沫敷料可显著降低气管切开患者的切口感染和肺部感染、减少切口周围皮肤并发症发生率,延长换药间隔时间,缩短更换敷料频率、缩短拔管时间,促进切口愈合。

Axial length/corneal radius of curvature ratio and refractive development evaluation in 3-to 4-year-old children: the Shanghai Pudong Eye Study

AIM: To measure and analysis axial length(AL)/corneal radius of curvature(CRC) ratio and other refractive parameters, provide a medical reference range for refractive development evaluation and earlier visual impairment screening of 3 to 4 y kindergarten ***: Between April and June 2017, a total of 4350 participants aged 3-to 4-year-old(8700 eyes) from 10 cluster random sampling kindergartens in Shanghai, Pudong District were involved. According to the measurement and analysis of the unaided visual acuity(VA), AL, CRC, AL/CRC ratio, astigmatism and other refractive parameters, the data distribution and reference range were ***: Uncorrected VA of examined children was 0.23±0.08(logMAR, mean±SD) [95% confidence interval(CI) range ≤0.36];AL was 22.10±0.79 mm(95%CI 20.55-23.65);CRC was 7.86±0.26 mm(95%CI, 7.35-8.37);AL/CRC ratio was 2.81±0.12(95%CI, 2.57-3.05). The median of astigmatism was-0.5 D, a total of 56.3% had astigmatism -1.50 D;71% were astigmatism with the rule. Eye-specific analyses were conducted. Statistical difference of VA was in right and left eyes. There were no significant differences between two eyes of AL, CRC, AL/CRC ratio and astigmatism(P>0.05).CONCLUSION: VA and AL/CRC ratio reference could be used to assess refractive development in children and screening uncorrected refractive errors or amblyopia. Astigmatism needs to be considered in the diagnosis.

三叉神经痛个体化微血管减压手术策略分析

目的:探讨个体化微血管减压术(MVD)治疗三叉神经痛(TN)的临床疗效,以期为患者提供个体化治疗策略及新的治疗思路。方法:回顾性分析2021年1月至2023年9月安徽医科大学第一附属医院神经外科收治的46例TN患者的临床资料,男19例,女27例;发病年龄为(58.3 ± 9.0)岁。根据巴罗神经学研究所(BNI)提出的疼痛分级评估患者术前疼痛分级及手术疗效。其中术前BNI Ⅳ级27例,Ⅴ级19例。46例患者手术均采用经乙状窦后入路,结合术中血管情况采取显微镜手术完成10例,内镜完成手术12例,内镜和显微镜双镜联合24例。探明责任血管后,予以涤纶垫棉减压。术后通过电话或者门诊跟踪随访患者。结果:46例TN MVD患者,43例(93.5%)术后疼痛即刻完全缓解(BNI分级Ⅰ级),3例(6.5%)术后疼痛部分缓解(BNI分级Ⅱ级)。4例患者术后出现面部麻木、感觉减退,2例出现面瘫(House-Brackmann分级Ⅱ级1例、Ⅲ级1例),8例出现颅内积气,4例出现术后发热,2例皮下积液。经过治疗,颅内积气、发热均治愈,皮下积液1例消失,1例持续存在。46例患者随访中位时间为16.2(1~33)个月,3例术后BNI Ⅱ级患者2例完全缓解(BNI分级Ⅰ级),1例复发加重(BNI分级Ⅳ级)。结论:责任血管的复杂程度是制定TN MVD策略的重要考量因素之一,结合术中具体血管情况制定个体化的手术方案,是TN治疗值得借鉴的手术策略。

BACKGROUND: Cognitive impairment after stroke associates with various factors, such as age, educational years, etc. Besides concerning about the recovery of limb function after stroke, we should also focus on the rehabilitation of cognition. Moreover, we’d better pay attention to the control of all the risk factors of stroke, and improve the quality of life in stroke patients. OBJECTIVE: To analyze the factors that affect cognitive impairment after stroke. deSIGN: A related factors analysis. SETTINGS: department of Neurology of Dalian Port Hospital and Dalian Second People’s Hospital. PARTICIPANTS: Totally 148 stroke inpatients were selected from department of Neurology, Dalian Port Hospital and Dalian Second People’s Hospital from April 2004 to december 2005, including 100 males and 48 females, aging 45-75 years with an average age of (67±8) years; Their educational years ranged 2-10 years with an average of (6.1±3.7) years; The disease course ranged 15-30 days; All were right-handed. Inclusive criteria: All were accorded with the diagnostic standard set by the Fourth National Academic Meeting for Cerebrovascular Disease; Confirmed by CT or MRI; Informed consents were obtained from all the subjects. METHODS: After the disease conditions were stable, the patients were assessed with Wechsler memory scale (WMS) and Wisconsin card sorting test (WCST). WMS included forward and backward recitation of numbers and short-term memory (verbal memory, visual recognition). The number of times for correct and wrong classifications in WCST and the time to complete the trail making tests A and B were recorded. The focal volume, area and layer were recorded at 24 hours after admission. The general data of the patients were recorded, including name, sex, age, educational years, history of hypertension, history of diabetes mellitus. 目的探讨不同氧(O2)浓度环境对肺动脉平滑肌细胞(PASMC)表型的影响及肺动脉高压的形成机制。方法采用酶解法分离培养大鼠原代PASMC,经鉴定后将稳定传代的PASMC分别置于正常O2含量(C组)及55%O2(H55组)、75%O2(H75组)、95%O2(H95组)的密闭容器培养6 h。分别采用实时荧光定量PCR法和Western blot法检测α-平滑肌肌动蛋白(α-SMA)、平滑肌22α蛋白(SM22α)、骨桥蛋白(OPN)、基质金属蛋白酶2(MMP-2)的mRNA和蛋白表达。结果H55组的α-SMA、SM22α、OPN、MMP-2的mRNA和蛋白相对表达量与C组差异均无统计学意义(P均>0.05);与C组和H55组比较,H75组和H95组α-SMA、SM22α的mRNA和蛋白相对表达量均更低,OPN、MMP-2的mRNA和蛋白相对表达量均更高(P均2的mRNA相对表达量更高(P2浓度75%以上环境可通过诱导PASMC表型转化,使PASMC获得较强分泌能力,形成肺动脉高压病理基础,并且这种诱导作用呈现O2浓度依赖性。

The Circular Electron Positron Collider(CEPC)is a large scientific project initiated and hosted by China,fostered through extensive collaboration with international *** complex comprises four accelerators:a 30 GeV linac,a 1.1 GeV Damping Ring,a Booster capable of achieving energies up to 180 GeV,and a Collider operating at varying energy modes(Z,W,H,and tt).The linac and Damping Ring are situated on the surface,while the subterranean Booster and Collider are housed in a 100 km circumference underground tunnel,strategically accommodating future expansion with provisions for a potential Super Proton Proton Collider(SPPC).The CEPC primarily serves as a Higgs *** its baseline design with synchrotron radiation(SR)power of 30 MW per beam,it can achieve a luminosity of 5×10^(34)cm^(-2)s^(-1)per interaction point(IP),resulting in an integrated luminosity of 13 ab^(-1)for two IPs over a decade,producing 2.6 million Higgs *** the SR power to 50 MW per beam expands the CEPC's capability to generate 4.3 million Higgs bosons,facilitating precise measurements of Higgs coupling at sub-percent levels,exceeding the precision expected from the HL-LHC by an order of *** Technical design Report(TDR)follows the Preliminary Conceptual design Report(Pre-CDR,2015)and the Conceptual design Report(CDR,2018),comprehensively detailing the machine's layout,performance metrics,physical design and analysis,technical systems design,R&D and prototyping efforts,and associated civil engineering ***,it includes a cost estimate and a preliminary construction timeline,establishing a framework for forthcoming engineering design phase and site selection *** is anticipated to begin around 2027-2028,pending government approval,with an estimated duration of 8 *** commencement of experiments and data collection could potentially be initiated in the mid-2030s.

Objective:To exploit- the effects of gentiana scabra bage on the expression of hepatic collagen proteins in Paragonimus skrjabini rats with liver ***:Immunohistochemical technique was used to observe the changes of content of hepatic type Ⅰ,Ⅲ collagen proteins in Paragonimus skrjabini rats with liver fibrosis before and after the gentiana scabra bage ***:Comparing with the model group,changes of hepatic tvpe Ⅰ and type Ⅲ collagen proteins in gentiana scabra bage treated group were significantly ***:Gentiana scabra bage treatment can reduce the content of hepatic type Ⅲ and type Ⅰ collagen protein significantly in Paragonimus skrjabini rats with liver fibrosis,thereby,playing the role against hepatic fibrosis.
