Causes of Surgical Delay and Demographic Characteristics in Patients with Hip Fracture

Background: Several studies analyze how surgical delay affects patients with hip fracture. The aim of this study was to identify the causes of surgical delay and demographic characteristics in patients with hip fracture who had delays longer than 24 hours from admission to hospital. Methods: Quantitative retrospective register study of 484 patients was consecutively included during the period November 1, 2010 and October 31, 2011 in the University Hospital in Lund (Sweden). Results: A frequency of 29.4% had a surgical delay longer than 24 hours. The main reasons for delays to surgery were lack of theatre facilities (54%), medical unstable patient (16%) and anticoagulant treatment (10%). Of all patients, 69% (n = 332) were women and 31% (n = 151) were men. The mean age for women were 83.6 (CI 83-85) vs. 79 (CI 77-81) for men, respectively. The most common type of hip fracture was displaced cervical hip fracture (39%, n = 188) with a majority of fractures in male patients. In total, women suffered hip fractures to a greater extent than men (69% vs. 31%, p = 0.016), but no relationship was found with respect to the fracture type and age (p = 0.358). Conclusion: The main result demonstrated that delays longer than 24 hours were due to lack of theatre facilities. Further researches have to be done in order to investigate whether lack of theatre facilities depends on improper operation planning and/or on lack of medical staff.

Background: Teamcare should, like all patient care, also contribute to evidence-based practice (EBP). Randomized controlled trials (RCTs) focusing on teamcare have been performed but no study has addressed its essentials. How far this EBP has progressed in different health aspects is generally established in systematic reviews of RCTs. Aim: The aim is to determine the essentials of teamcare including the nurse profession in RCTs of multi- or interdisciplinary interventions in somatic care focusing on the stated context, goals, strategies, content as well as effectiveness of quality of care. Methods: A systematic review was performed according to Cochrane review assumptions to identify, appraise and synthesize all empirical evidence meeting pre-specified eligibility criteria. The PRISMA statement guided the data selection process of 27 articles from PubMed and CINAHL. Results: Eighty-five percent of RCTs in somatic care showed a positive effectiveness of teamcare interventions, of which interdisciplinary ones showed a greater effectiveness compared with the multidisciplinary approach (100% vs 76%). Also theory-based RCTs presented higher positive effectiveness (85%) compared with non-theory-based RCTs (79%). The RCTs with positive effectiveness showed greater levels for professional-centered ambition in terms of goals and for team-directed initiatives in terms of strategy, and a significantly higher level for patient-team interaction plans in terms of content was shown. Conclusions: Teamcare RCTs are still grounded in the multidisciplinary approach having a professional-centered ambition while interdisciplinary approaches especially those that are theory-based appear to be essential with regard to positive effectiveness and preferable when person-centered careis applied.

Establishing and Sustaining Academic and Local Health Collaborations: A Case Study of Practice-Based Teaching and Workforce Development

The pandemic highlighted significant gaps in the public health infrastructure impacted by shortages of public health workers, an undertrained workforce, and years of disinvestment. These gaps required innovative problem-solving by public health agencies (PHAs), including local health departments (LHDs), to respond to rapidly changing community conditions during and after the pandemic. Many schools and programs of public health (SPPH) worked with PHAs to mobilize public health (PH) students through practice-based teaching (PBT). Current research indicates PBT benefits all stakeholders—PHAs, students, faculty, SPPH, and ultimately the community served. However, more research is needed on the utility of PBT in addressing a community’s systemic public health issues, the extent to which the academic-community collaboration enhances a PHA’s capacity, and the impact of the pedagogy on preparing the workforce for an evolving PH landscape. This paper examines the process of a semester-long PBT course, guided by the PBT STEPS framework, which includes five steps from collaboration to implementation to evaluation of a PBT course. The collaborating PHA and its student group addressed community trauma and resilience issues during the semester. Additionally, it examines the longer-term impacts after the semester for the PHA, community, and the workforce by 1) conducting a formative evaluation to understand needs and gaps in the community;2) redesigning an intervention that merged the results of the formative evaluation with the intervention developed during the semester;and 3) securing funding and resources for intervention sustainability. Through the documentation of a post-course partnership between an LHD and faculty at a large school of public health, this case study illustrates the potential for PBT to lay the foundation for ongoing research that supports more impactful interventions for PHAs while bolstering the workforce abilities of students as future practitioners.

Effect of nonionic surfactants in release media on accelerated in-vitro release profile of sirolimus eluting stents with biodegradable polymeric coating

It is a well-known fact that sirolimus(SRL) undergoes degradation process via hydrolysis in aqueous media, leading to incorrect assessment of drug amount and thus release characteristics of *** main objective of the present study was to evaluate the effect of nonionic surfactants in media on invitro release profiles for sirolimus eluting stents(SES) coated with biodegradable polymeric *** buffer and acetate buffer incorporating nonionic surfactants with varying concentrations were examined for adequate solubility and stability(by RP-HPLC). Good sink condition was achieved in phosphate buffer(at pH 4.0) with 1.0% Tween 20, 1.0% Brij 35% and 0.5% Brij 58. Hydrodynamic size(by DLS) and the micelle-water partition coefficient(P) with standard free energy of solubilization(ΔGs°) of drug were evaluated to get some understanding about the solubilization phenomena. About 80% of drug release during the period of 48 h was achieved in optimized drug release media which was 1.0% Tween20 in phosphate buffer pH 4.0. The obtained accelerated SRL release profile in optimized medium correlated well with the real time in-vitro release in phosphate buffer(pH 7.4). Surface morphology changes(by SEM), changes in gravimetric weights and molecular weight change(by GPC) were examined before and after drug release to understand the drug release mechanism which explains that the polymer did not undergo degradation during the drug release.

Many early Japanese books record a large amount of information,including historical politics,economics,culture,and so on,which are all valuable *** books are waiting to be reorganized at the ***,a large amount of the books are described by Kuzushiji,a type of handwriting cursive script that is no longer in use today and only readable by a few ***,researchers are trying to detect and recognise the characters from these books through modern ***,the characteristics of the Kuzushiji,such as Connect-Separate-characters and Manyvariation,hinder the modern technique assisted ***-Separatecharacters refer to the case of some characters connecting each other or one character being separated into unconnected parts,which makes character detection *** is one of the typical characteristics of Kuzushiji,defined as the case that the same character has several variations even if they are written by the same person in the same book at the same time,which increases the difficulty of character *** this sense,this paper aims to construct an early Japanese book reorganisation system by combining image processing and deep learning *** experimentation has been done by testing two early Japanese *** terms of character detection,the final Recall,Precision and F-value reaches 79.8%,80.3%,and 80.0%,*** deep learning based character recognition accuracy of Top3 reaches 69.52%,and the highest recognition rate reaches 82.57%,which verifies the effectiveness of our proposal.

Development of an Autonomous Flight Control System for Small Size Unmanned Helicopter Based on Dynamical Model

It is devoted to the development of an autonomous flight control system for small size unmanned helicopter based on dynamical model. At first, the mathematical model of a small size helicopter is described. After that simple but effective MTCV control algorithm was proposed. The whole flight control algorithm is composed of two parts: orientation controller based on the model for rotation dynamics and a robust position controller for a double integrator. The MTCV block is also used to achieve translation velocity control. To demonstrate the performance of the presented algorithm, simulation results and results achieved in real flight experiments were presented.

Employing LAB and/or their metabolites to reduce aflatoxins from food products is one of the fascinating areas of ***,in the present work an attempt was made to study the effect of isolated lactic acid bacteria(LAB)on the reduction of aflatoxin M1(AFM1)from phosphate buffer saline(PBS)and reconstituted skim milk(RSM).Out of 68 isolates,nine strains of LAB isolated from food products were examined to bind and reduce *** isolates were incubated with AFM1 at different time and temperature(24 h,48 h,and 72 h at 37℃ and 4℃)and later the residual toxin in the supernatant was *** preliminary results through thin layer chromatography(TLC)revealed that all isolates were able to bind AFM1 in ***,based on the intensity of spots observed in TLC sheets,it was indicated that two isolates,H1 and S2 genotypically identified as *** and *** shown better AFM1 binding ability as compared to other ***,the heat treated cells of both the selected strains did not show significant difference to AFM1 binding ability as compared to live *** of high performance liquid chromatography(HPLC)observed that *** H1 and *** had shown 78% and 72%AFM1 binding in PBS at 37℃ after 72 *** was a significant difference(P<0.05)in AFM1 binding ability of *** S2 at 37℃&4℃,but such difference was not observed for *** ***,these isolates also observed reduction of naturally present AFM1 in 10%*** is the first study reporting the AFM1 binding capability of the genus Weissella(strain *** H1)and Enterococcus *** results showed that indigenous strains of LAB are able to bind AFM1 effectively.

We supercooled fresh-cut onion at−5℃ for 12 *** supercooling,the electric impedance properties of the samples were evaluated by electrical impedance spectroscopy over the frequency range of 42 Hz−5 *** time-temperature profiles of samples indicated that the freezing point and supercooling point were−2.3℃±0.7℃ and−6.9℃±1.0℃,*** results indicated that 34 of the 36 supercooled samples exhibited a definite circular arc in the Cole-Cole plot,which suggested that the cell membrane remained intact during *** the other two samples which did not exhibit a definite circular arc,the cell membrane had sustained serious damage during ***,there was large difference in drip loss percentage between supercooled samples exhibited a definite circular arc in the Cole-Cole plot and samples not exhibiting a definite circular *** results suggest that fresh-cut onions can be supercooled at−5℃.

Background and Aims:The serine proteinase inhibitor alpha-1 anti-trypsin(AAT)protects the body against protease *** functions of AAT beyond those attributed to its antiprotease activity have been described,among them immunomodulatory and anti-inflammatory *** present study aimed to determine the efficacy of AAT for the treatment of immune-mediated liver injury using the models of concanavalin A-induced immune-mediated hepatitis and acetaminophen-induced liver ***:AATwas administered to mice subjected to concanavalin A-induced immune-mediated hepatitis or 2 h after acetaminophen-induced liver *** were followed for changes in serum levels of liver enzymes,liver histology,and for interferon gamma serum ***:Treatment with AATalleviated concanavalin A-induced immunemediated liver damage,as demonstrated by a reduction in the serum levels of liver enzymes and interferon gamma,and an improved lymphocyte infiltration into the liver on liver ***,treatment with AAT was associated with alleviation of the acetaminophen-induced liver ***:AAT exerts an hepatoprotective effect on immune-mediated and drug-induced liver *** data support its potential use in patients with immune-associated liver disorders.
