Background: From May 2018 to September 2022, the Democratic Republic of Congo (DRC) experienced seven Ebola virus disease (EVD) outbreaks within its borders. During the 10th EVD outbreak (2018–2020), the largest experienced in the DRC and the second largest and most prolonged EVD outbreak recorded globally, a WHO risk assessment identified nine countries bordering the DRC as moderate to high risk from cross border importation. These countries implemented varying levels of Ebola virus disease preparedness interventions. This case study highlights the gains and shortfalls with the Ebola virus disease preparedness interventions within the various contexts of these countries against the background of a renewed and growing commitment for global epidemic preparedness highlighted during recent World Health Assembly *** text: Several positive impacts from preparedness support to countries bordering the affected provinces in the DRC were identified, including development of sustained capacities which were leveraged upon to respond to the subsequent coronavirus disease 2019 (COVID-19) pandemic. Shortfalls such as lost opportunities for operationalizing cross-border regional preparedness collaboration and better integration of multidisciplinary perspectives, vertical approaches to response pillars such as surveillance, over dependence on external support and duplication of efforts especially in areas of capacity building were also identified. A recurrent theme that emerged from this case study is the propensity towards implementing short-term interventions during active Ebola virus disease outbreaks for preparedness rather than sustainable investment into strengthening systems for improved health security in alignment with IHR obligations, the Sustainable Development Goals and advocating global policy for addressing the larger structural determinants underscoring these ***: Despite several international frameworks established at the global level for e

目的:精神分裂症目前最有效的治疗即应用药物控制症状。以奥氮平（olanzapine,OLZ）为代表的非典型抗精神病药物（atypical antipsychotics,AAPs）是当今临床主要用药之一,然而,其所致的以胰岛素抵抗（insulin resistance,IR）为核心的代谢紊乱不良反应严重限制了其临床应用,且机制尚未明确。巨噬细胞迁移抑制因子（macrophage migration inhibitory factor,MIF）是重要的促炎因子,基础及临床研究都显示其在肥胖、II型糖尿病等代谢紊乱中起到关键作用,然而,MIF是否参与了AAPs诱导的IR尚无研究报道。本研究旨在通过临床、动物、细胞三个层面全面探讨MIF在OLZ诱导的IR中的作用及机制。临床收集单一服用奥氮平的精神分裂症患者血液样本,探讨MIF与OLZ诱导的胰岛素抵抗的相关性。利用野生鼠及MIF敲除鼠建立OLZ诱导的IR小鼠模型,探讨MIF与代谢紊乱的相关性;建立OLZ诱导IR的细胞模型,初步探讨MIF参与OLZ诱导代谢紊乱的机制。材料和方法:（1）临床收集单一服用OLZ的精神分裂症患者血液样本,检测MIF水平及糖脂代谢指标,提取DNA,将病人分为CATT 5/5和CATT non-5/5两组,CATT是一种MIF的启动子区常见的基因多态性,CATT5等位基因具有比较低的启动子活性,因而伴随血液中较低的MIF水平。（2）野生与MIF敲除雌性小鼠自主摄入OLZ或溶剂对照,通过ITT、GTT、血糖、血脂等评估模型有效性。检测并比较各组小鼠代谢指标及血浆MIF水平;检测胰岛素敏感器官MIF表达及胰岛素信号相关蛋白（JNK、Akt等）的活性。检测下丘脑中食欲相关基因表达。（3）培养3T3-L1脂肪细胞,用不同浓度（0-100u M）和不同处理时间（1、3、5、7天）的OLZ处理细胞后检测胰岛素信号相关蛋白的活性;在培养细胞中加入MIF抑制剂或JNK抑制剂后观察OLZ对胰岛素信号的影响。（4）培养原代下丘脑神经元,OLZ处理细胞后检测食欲相关基因表达,MIF基因表达;结果:（1）在精神分裂症患者奥氮平治疗中,随着疗程的进行,血浆MIF含量有升高趋势,HOMA-IR指数明显升高。CATT 5/5精神分裂患者血液中MIF水平和HOMA-IR指数都没有变化,血液中其它糖尿病相关指数也没有变化。相反地,CATT non-5/5精神分裂患者血液中MIF水平和HOMA-IR指数都有明显升高,血液中胰岛素、TG含量均增加,BMI指数升高,HDL下降。以上结果表明,给予奥氮平治疗的精神分裂患者的糖尿病发病程度与CATT5等位基因相关,即与MIF水平正相关。（2）OLZ诱导了野生小鼠IR发生,同时存在血浆MIF水平显著升高、脂肪组织MIF表达显著升高、胰岛素信号传导受阻及JNK激活;而MIF敲除鼠的OLZ给药组的IR程度较之显著减轻,也未发现胰岛素信号传导受阻及JNK激活。（3）OLZ处理3T3-L1脂肪细胞后JNK活性增高,Akt S473活性降低,此变化呈剂量和时间依赖模式。在细胞培养基中加入SP600125后可部分抵消OLZ引起的Akt S473活性减低。（4）OLZ处理原代下丘脑神经元后,Ag RP表达升高,AMPK磷酸化水平升高。结论:临床结果证明MIF与OLZ诱导的胰岛素抵抗相关;在OLZ诱导的IR小鼠模型中证明MIF在脂肪组织中参与OLZ诱导IR,其机制可能是干扰了胰岛素信号通路,并可能与JNK的激活有关。在下丘脑中,OLZ可能通过AMPK参与了食欲控制过程。OLZ可能通过以上两个方面通过MIF影响整体代谢紊乱。

Trachypithecus,which currently contains 20 species divided into four groups,is the most speciose and geographically dispersed genus among Asian *** several morphological and molecular studies,however,its evolutionary history and phylogeography remain poorly ***’s langur(Trachypithecus phayrei) is one of the most widespread members of the genus,but details on its actual distribution and intraspecific taxonomy are limited and ***,to elucidate the evolutionary history of Trachypithecus and to clarify the intraspecific taxonomy and distribution of ***,we sequenced 41 mitochondrial genomes from georeferenced fecal samples and museum specimens,including two *** analyses revealed a robustly supported phylogeny of Trachypithecus,suggesting that the *** group branched first,followed by the *** group,and the *** and *** groups most *** four species groups diverged from each other 4.5-3.1 million years ago(Ma),while speciation events within these groups occurred much more recently(1.6-0.3 Ma).Within ***,we found three clades that diverged 1.0-0.9 Ma,indicating the existence of three rather than two *** the phylogenetic species concept and based on genetic,morphological,and ecological differences,we elevate the *** subspecies to species level,describe a new species from central Myanmar,and refine the distribution of the three ***,our study highlights the importance of museum specimens and provides new insights not only into the evolutionary history of *** but the entire Trachypithecus genus as well.

多民族种族群体是指按照生活经验和传统民族分类,认同两种及以上民族遗产的群体。在美国,多民族人群增长速度是一般群体的三倍以上。以往研究表明,多民族种族个体的心理和行为问题比单一民族和少数民族群体更为严重。针对这一现状,研究者们找到了多个保护性因素,但目前还没有人考虑过"希望"这一重要变量。希望在个体对消极心理问题的认知中起着重要作用,有助于缓解一系列消极心理适应问题。本研究采用了一般种族歧视量表(GEDS),两项来自于社交网络列表的问题(SNL:测量家庭和朋友支持),希望量表(HS:包含两个维度:希望效能(hope agency)和希望手段(hope pathways)),宾夕法尼亚州立大学忧虑问卷(PSWQ),贝克抑郁量表(BDI)对203名(164名女性,39名男性)多民族成人群体进行调查,以检验种族歧视,社会支持(即家庭支持和朋友支持)和希望(即希望效能和希望手段)在消极心理适应问题(如忧虑和抑郁症状)中的作用。对忧虑(PSWQ)和抑郁(BDI)的评分进行了一系列的层次回归分析,以确定控制了种族歧视和社会支持后,希望是否对消极心理适应问题起到缓解作用。结果表明:1)控制了社会人口统计学变量后,种族歧视可以显著预测多民族种族个体忧虑和抑郁。2)控制社会人口变量和民族歧视后,家庭支持可以显著负向预测多民族种族群体的忧虑和抑郁,朋友支持仅能负向预测多民族种族群体中的抑郁。3)在控制了社会人口因素、种族歧视和社会支持后,希望效能仍可以显著预测多民族种族成人的忧虑和抑郁,但希望手段不可以。综上,本研究首次强调了在多民族种族群体中,在控制民族歧视,社会支持的情况下,希望仍可以缓解多民族种族群体的消极心理适应问题,这对缓解数量激增的多民族种族群体日益严重的心理问题有实际意义。

美国法律体系中出现了新的参与者:诉讼融资(ALF)实体。近年来,ALF实体已开始为小规模的案件和涉及数百万美元的复杂商事案件提供资助。实际上,助讼图利、高利贷和伦理戒律这些历史上的障碍已不再阻碍ALF(至少在一些司法辖区是这样),这使得其在近些年取得了一定的发展。但随着ALF方式变得越来越普遍,人们也逐渐开始关注到以下议题:即ALF实体融入律师-委托人双边关系给律师-委托人特免权带来的影响。本文探讨了传统的律师-委托人特免权学说,以确定特免权是否保护涉及ALF实体的交流。本文的结论是,ALF实体不能从共同委托人学说中受益,因为其实际上不是共同委托人。即使ALF实体寻求成为共同委托人,伦理原则也可能不会允许其在关系的投资决策阶段获得共同委托人地位,在关系的监管阶段也同样如此。另外,ALF实体在投资决策阶段的交流可能无法从同盟当事人-共同利益学说中受益,但如果所涉及的特定法院使用的是共同利益的宽泛定义,则其在监管阶段的交流或许能适用这一学说。此外,ALF实体也不太可能被认定为委托人或律师的代理人,故特免权保护一般不应囊括它。本文的最终结论是,特免权学说不应扩展到ALF存在的情境,概因如此会损害特免权本身。相反,如果ALF实体在法律市场中的参与足够有益且具备充分的特免权保护需求,那么一项独立的ALF特免权可能是最好的解决方案。

神经炎症的一个主要标志是小胶质细胞和星形胶质细胞的激活以及炎症介质(如IL-1β、TNF-α、iNOS和IL-6)的诱导。神经炎症会导致从急性中枢神经系统创伤到慢性神经退行性疾病等多种神经系统疾病的进展。mRNA稳定性的转录后调控途径和翻译效率是这些炎症介质表达的主要驱动因素。这一调节水平的一个共同元素以富含腺嘌呤和尿苷的元素(ARE)为中心,该元素存在于编码这些炎症介质的mRNA的3’非翻译区(UTR)中。(ARE)-结合蛋白(AUBPs)——如人类抗原R(HuR)、三四脯氨酸(TTP)和KH型剪接调节蛋白(KSRP)——是指导这些转录后通路的关键节点,可以促进(HuR)或抑制(TTP和KSRP)胶质细胞产生炎症介质。本综述将讨论ARE介导的RNA调节的基本概念及其对胶质细胞驱动的神经炎症性疾病的影响。还将讨论针对这种新的基因调控水平以达到治疗效果的策略,并对其治疗神经系统疾病潜力的初步研究进行回顾性分析。
