The world is experiencing aging of its population. Age-specific incidence rates of cancer are higher and cancer is now recognized as a part of aging. Treating older patients can be challenging. The clinical behavior of some tumors changes with age and the aging process itself brings physiological changes leading to decline in the function of organs. It is essential to identify those patients with longer life expectancy, potentially more likely to benefit from aggressive treatment vs. those that are more vulnerable to adverse outcomes. A primary determination when considering therapy for an older cancer patient is a patient's physiologic, rather than chronologic age. In order to differentiate amongst patients of the same age, it is useful to determine if a patient is fit or frail. Frail older adults have multiple chronic conditions and difficulties maintaining independence. They may be more vulnerable to therapy toxicities, and may not have substantial lasting benefits from therapy. Geriatric assessment(GA) may be used as a tool to determine reversible deficits and devise treatment strategies to mitigate such deficits. GA is also used in treatment decision making by clinicians, helping to risk stratify patients prior to potentially high-risk therapy. An important practical aspect of GA is the feasibility of incorporating it into a busy oncology practice. Key considerations in performing the GA include: available resources, patient population, GA tools to use, and who will be responsible for using the GA results and develop care plans. Challenges in implementing GA in clinical practice will be discussed.

Temporal trends in 6-minute walking distance for older Japanese adults between 1998 and 2017

Rates and risk factors for suicidal ideation,suicide attempts and suicide deaths in persons with HIV:a systematic review and meta-analysis

IL-6 prevents Th2 cell polarization by promoting SOCS3-dependent suppression of IL-2 signaling

Defective interleukin-6 (IL-6) signaling has been associated with Th2 bias and elevated IgE levels. However, the underlying mechanism by which IL-6 prevents the development of Th2-driven diseases remains unknown. Using a model of house dust mite (HDm)-induced Th2 cell differentiation and allergic airway inflammation, we showed that IL-6 signaling in allergen-specific T cells was required to prevent Th2 cell differentiation and the subsequent IgE response and allergic inflammation. Th2 cell lineage commitment required strong sustained IL-2 signaling. We found that IL-6 turned off IL-2 signaling during early T-cell activation and thus inhibited Th2 priming. mechanistically, IL-6-driven inhibition of IL-2 signaling in responding T cells was mediated by upregulation of Suppressor Of Cytokine Signaling 3 (SOCS3). This mechanism could be mimicked by pharmacological Janus Kinase-1 (JAK1) inhibition. Collectively, our results identify an unrecognized mechanism that prevents the development of unwanted Th2 cell responses and associated diseases and outline potential preventive interventions.

Use of orthotics with orthotic sandals versus the sole use of orthotics for plantar fasciitis:Randomised controlled trial

Response to endoscopic therapy for biliary anastomotic strictures in deceased versus living donor liver transplantation

BACKGROUND:Endoscopic therapy has been successful in the management of biliary complications after both deceased donor liver transplantation(DDLT) and living donor liver transplantation(LDLT).LDLT is thought to be associated with higher rates of biliary complications,but there are few studies comparing the success of endoscopic management of anastomotic strictures between the two *** study aims to compare our experience in the endoscopic management of anastomotic strictures in DDLT versus ***:This is a retrospective database review of all liver transplant patients undergoing endoscopic retrograde cholangiopancreatography(ERCP) after liver *** frequency of anastomotic stricture and the time to develop and to resolve anastomotic stricture were compared between DDLT and *** response of anastomotic stricture to endoscopic therapy was also ***:A total of 362 patients underwent liver transplantation between 2003 and 2011,with 125 requiring ERCP to manage biliary ***-three(9.9%) cases of DDLT and 8(27.6%) of LDLT(P=0.01) were found to have anastomotic *** comparing DDLT and LDLT,there was no difference in the mean time to the development of anastomotic strictures(98±17 vs 172±65 days,P=0.11),likelihood of response to ERCP [22(66.7%) vs 6(75.0%),P=0.69],mean time to the resolution of anastomotic strictures(268±77 vs 125±37 days,P=0.34),and the number of ERCPs required to achieve resolution(3.9±0.4 vs 4.7±0.9,P=0.38).CONCLUSIONS:Endoscopic therapy is effective in the majority of biliary complications relating to liver *** strictures occur more frequently in LDLT compared with DDLT,with equivalent endoscopic treatment response and outcomes for both groups.

microhabitat use by white-footed mice Peromyscus leucopus in forested and old-field habitats occupied by morrow's honeysuckle Lonicera morrowii

We quantified microhabitat use by white-footed mice Peromyscus leucopus in forest and old-field habitats occupied by morrow＇ s honeysuckle Lonicera morrowii, an invasive exotic shrub imported from Japan. microhabitat characteristics were compared between trails used by mice （ n = 124） and randomly selected trails （ n = 127） in 4 study plots located at Fort Necessity National Battlefield, Farmington, Pennsylvania, USA. We compared 10 mierohabitat variables between used and random trails using non-metric multidimensional scaling （NmDS） and classification and regression tree （CART） analysis. Trails used by mice were statistically different from randomly selected trails in both forested plots （ P 〈 0. 008） and old-field plots （ P 〈 0. 001 ）. In the forested plots, trails of white-footed mice were more often associated with a greater percent cover （% cover） of coarse woody debris （CWD） than were randomly selected trails. In the old-field plots, mouse trails were commonly characterized by having a lower % cover of exotic herbaceous vegetation, a greater % cover of shrubs, and a greater % cover of morrow＇ s honeysuckle than randomly selected trails. Our study indicates that white-footed mice do not move randomly and prefer areas of high structural complexity, thereby showing significant microhabitat preference. The preference of white-footed mice for areas with a relatively high percent cover of morrow＇s honeysuckle could 1 ） be a factor in the aggressive nature of the exotic honeysuckle shrub＇s spread throughout the Battlefield or 2） cause the shrub to spread even faster into adjacent areas not yet occupied by morrow＇ s honeysuckle

Florida Turkey Nest Site Selection and Success

Changing landscapes and land-use practices are altering habitat for Florida wild turkeys (meleagris gallopavo osceola). However, an understanding of habitat determinants of nest success is lacking for this unique turkey subspecies, potentially limiting conservation success. We examined female wild turkey nest site selection and nest success at microhabitat and patch levels using logistic regression in an Information-Theoretical framework in Florida, 2008-2010. We captured and radio-equipped adult female turkeys, and followed birds to nests. Nests were monitored to document success, and habitat was measured at multiple levels at nest and random sites. Females selected nest sites in dense vegetation (i.e., increased saw palmetto cover [Serenoa repens] and higher palm stem densities) that may have provided lateral and vertical cover for concealment at the microhabitat level (i.e., area within 7 m of the nest), while selecting for a more open habitat (i.e., decreasing hardwood and conifer stem densities) at the patch level (i.e., area within 28 m of the nest). Similarly, successful nests were in more dense vegetation at the nest site (i.e., increased saw palmetto cover) in an otherwise more open habitat (i.e., lower basal area) than unsuccessful nests. Habitat management that creates patches of dense shrub vegetation such as saw palmetto within an open landscape may be best for Florida wild turkey nesting habitat and success.
