Objectives: We describe a novel rehabilitation tool for patients with homonymo us hemianopia based on a visual search (VS) paradigm that is portable, inexpensi ve, and easy to deploy. We hypothesised that by training patients to improve the efficiency of eye movements made in their blind field their disability would be alleviated. Methods: twenty nine patients with homonymous visual field defects (HVFD) without neglect practised VS paradigms in 20 daily sessions over one mont h. Search fields comprising randomly positioned target and distracter elements, differing by a single feature, were displayed for three seconds on a dedicated t elevision monitor in the patients'homes. Improvements were assessed by examinin g response time (Rt), error rates in VS, perimetric visual fields (VFs) and visu al search fields (VSFs), before and after treatment. Functional improvements wer e measured using objective visual tasks which represented activities of daily li ving (ADl) and a subjective questionnaire. Results: As a group the patients had significantly shorter mean Rt in VS after training (P trated a variety of mechanisms to account for this . Improvements were confined to the training period and maint ained at follow up. three patients had significantly longer Rt after training. t hey had high initial error rates which improved with training. Patients performe d ADl tasks significantly faster after training and reported significant subject ive improvements. there was no concomitant enlargement of the VF, but there was a small but significant enlargement of the VSF. Conclusion: Patients can improve VS with practice. this usually involves shorter Rts, but occasionally a longer Rt in a complex speed- accuracy trade-off. these changes translate to improved overall visual functi on, assessed objectively and subjectively, suggesting that they represent robust training effects. the underlying mechanism may involve the adoption of compensa tory eye movement strate

Characterisation and risk assessment of venous thromboembolism in gastrointestinal cancers

AIM to characterise venous thromboembolism(VtE) in gastrointestinal cancer and assess the clinical ut.l.ty of risk stratification scoring. MEtHODS We performed a retrospective analysis using electronic patient records of 910 gastro-oesophageal(GO) cancer and 1299 colorect.l.cancer(CRC) patients referred to a tertiary cancer centre to identify the incidence of VtE, its relationship to chemotherapy and impact on *** risk scores were calculated using the Khorana index. Patients were classified as low risk(0 points), intermediate risk(1 to 2 points) or high risk(3 points). Data was analysed to determine the sensitivity of the Khorana score to predict VtE. RESUltS the incidence of VtE was 8.9% for CRC patients and 9.7% for GO cancer patients. Pulmonary emboli(PE) were more common in advanced than in localised CRC(50% vs 21% of events respectively) and lower limb deep vein thrombosis(DVt) were more common in localised than in advanced CRC(62% vs 39% of events respectively). the median time to VtE from cancer diagnosis was 8.3 mo for CRC patients compared to 6.7 mo in GO cancer. In localised CRC median time to VtE was 7.1 mo compared with 10.1 mo in advanced CRC. In contrast in GO cancer, the median time to VtE was 12.5 mo in localised disease and 6.8 mo in advanced disease. No survival difference was seen between patients with and without VtE in this cohort. the majority of patients with CRC in whom VtE was diagnosed had low or intermediate Khorana risk score(94% for localised and 97% in advanced CRC). In GO cancer, all patients scored either intermediate or high risk due to the primary site demonstrating a limitation of the risk assessment score in discriminating high and low risk patients with GO cancers. Additional risk factors were identified in this cohort including surgery, chemotherapy or hospit.l.admission. Millipede forming-a new met.l.forming approach

Millipede Forming is a novel met.l.forming method that has been proposed and developed *** can be applied to overcome fundament.l.limitations in conventional roll forming,and,in addition,used for increment.l.forging and integrated into a strip casting or extrusion process for downstream thickness reduction. In this new method,there are two main principles:①the use of an optimal transitional surface or space linking of the incoming material to the outgoing product,and②implementation of a method to feed the material through the forming device so that it conforms with this ideal space and the required deformation is imposed on it by the tooling. the system described here is called "Millipede forming" because deformation is accomplished in a large number of small steps.A prototype has been built to demonstrate the working principle applied to the replacement of a roll forming process;it can be used to produce simple profiles within a forming length an order of magnitude shorter than the conventional process. In this paper,the working principle of "Millipede forming" is introduced and some fundament.l.research on forming a channel section and a circular tube is *** the success of the small scale prototype, research on a full scale pilot machine has been *** is expected that this process can be developed and extended as a useful alternative to conventional roll forming,increment.l.forging and within a strip casting or other novel strip material(*** met.l.) system.

Functional-trait ecology of the plateau pika Ochotona curzoniae in the Qinghai-tibetan Plateau ecosystem

Understanding a species’functional traits allows for a directed and productive perspective on the role a species plays in nature,and thus its relative importance to conservation *** functionaltrait ecology of the plateau pika Ochotona curzoniae is examined to better understand the resilience and sustainability of the high alpine grasslands of the Qinghai-tibetan Plateau(QtP).the key functional traits of plateau pikas are their abundance and behavior of digging extensive burrow *** pikas have been poisoned over a significant part of their original geographic distribution across the QtP,allowing comparison of ecological communities with and without *** all mammalian and avian carnivores,most of which are obligate predators on pikas,have been lost in regions where pikas have been *** endemic birds on the QtP nest in pika burrows;when pikas are poisoned,burrows collapse,and these birds are greatly reduced in *** to the biopedturbation resulting from their burrows,regional plant species richness is higher in areas with pikas than *** presence of pika burrows allows higher rates of infiltration during heavy monsoon rains compared to poisoned areas,possibly mitigating runoff and the potential for serious downslope erosion and ***,the functional traits of plateau pikas enhance native biodiversity and other important ecosystem functions;these traits are *** plateau pikas are not natural colonizers,active reintroduction programs are needed to restore pikas to areas from which they have been poisoned to restore the important functional ecological traits of pikas.

Objective: to assess the feasibility of testing for chlamydia directly on a s ingle liquid based specimen (thinPrep test) collected for cervical screening. Me thod: Cervical smears were taken using a Cervex spat.l. and rinsed in the liquid based cyt.l.gy collection vial. Following this, the conventional sample for chl amydia testing was taken from the endocervix using an Abbott Collection kit. Cyt ological specimens were prepared using an automated slide processor. Residual ce llular material and the conventional samples were sent to the laboratory where b oth were tested for chlamydia by ligase chain reaction (lCR). the manufacturer’ s protocol for lCR urine testing was modified to substitute 1 ml of Preserv Cyt suspension. Results: 581 women had both swab and cyt.l.gy suspension tested for Chlamydia trachomatis with lCR. there were 19 concordant positive and 562 conco rdant negative reports. the stability of chlamydia in the cyt.l.gy suspension wa s maintained for at.l.ast 5 months. Conclusion: the findings lead us to conclude that samples collected for liquid based cyt.l.gy using the thinPrep test collec tion vial provide a potential platform for chlamydia screening, though the study established several issues to be addressed to make this a practical proposition .

thyroid Function and Depression in HIV-1 Infection

thyroid abnormalities have been reported in persons with HIV infection, although data have been inconsistent with respect to its frequency and association with specific medications. the purpose of this study was to explore thyroid system response to thyroid releasing hormone stimulation in persons with and without HIV infection and determine the extent to which their response was associated with depression. As part of a larger study of neuroendocrine response persons with HIV-1 infection, control and HIV-1 infected individuals were evaluated. Participants' response to tRH stimulation was evaluated via tSH, tot.l.t3, and t4 levels at baseline and 15, 30, 45, and 60 minutes after tRH stimulation. Participants with HIV infection had a more robust response to tRH stimulation as measured by higher levels of tSH, lower levels of t4 and modestly higher levels of t3. Depressed persons had a reduced tSH response to stimulation and lower levels of both t4 and t3, although the effect of depression on t4 was not statistically significant. these results suggest that tSH response to tRH-stimulation may be exaggerated in individuals with HIV infection but reduced in those with depression. they also suggest that the effects of depression and HIV infection may interact, and may provide a partial explanation for observed thyroid abnormalities in HIV-infected individuals. Results thus provide a partial explanation for findings on thyroid and depression in those affected by HIV infection.
