Lithium Associated Hyperparathyroidism: An Evidence Based Surgical Approach

Background: long-term lithium use in psychiatric patients may lead to lithium associated hyperparathyroidism (LAH). Although anecdotal case reports have appeared, an evidence based algorithm for management of LAH is lacking. Methods: A comprehensive literature search was performed (1973-2010) using PubMed with keywords;“lithium” “hypercalcemia” “hyperparathyroidism” “sestamibi” “intra-operative parathyroid hormone (IOPTH) monitoring” “parathyroidectomy” and “medical management”. All English language publications addressing etiology and clinical management issues concerning LAH were critically analyzed. Results: Lithium associated hyperparathyroidism occurs in 4.3% - 6.3% of chronic lithium users compared to the general population which has an incidence of 0.5% - 1%. 194 cases of LAH have been reported which includes 10 patients (5%) treated medically and 170 patients (88%) who underwent parathyroidectomy. No details were available for 14 patients (7%). Among parathyroidectomy patients, 104 (59%) had adenomatous disease and 66 (39%) had multiglandular hyperplasia. Preoperative localization studies were utilized in only 22 patients (13%) and IOPTH monitoring was reported in only 3 studies (32 patients, 19%). Among surgical patients, bilateral neck exploration (BNE) was the most common approach performed in 162 patients (95%);focused neck exploration was utilized in only 8 patients (5%). Parathyroidectomy normalized LAH biochemical changes in nearly all patients (90% - 97%) in the early post-operative period, but recurrent hyperparathyroidism occurred in 8% - 42% of patients. Conclusion: LAH is an under appreciated and poorly understood endocrine disorder. LAH has a higher incidence of multiglandular disease and bilateral neck exploration is mandatory in majority for disease control. Nonsurgical approaches may be useful in select patients on short-term lithium therapy.

A Study of Temperature and Aging Effects on Eigenfrequencies of Concrete Bridges for Health Monitoring

This paper discusses the influence of environmental factors and of normal material aging on the eigenfrequencies of concrete bridges based on monitoring data registered during 4 years of a specific bridge. It is a new composite steel-concrete bridge built in 2006 in Luxembourg. The measurements are analyzed and compared to literature data. The final objective is the use of real monitored eigenfrequencies for structural health monitoring and damage detection based on identification of stiffness losses in practical applications. Therefore, it is very important to identify and compensate for outdoor influences namely temperature, excitation force level and normal aging effects, like creep and shrinkage of concrete and their impact on material properties. The present paper aims at describing these effects in order to separate them from damage effects. It is shown that temperature change rates and temperature gradients within the bridge have an influence on the eigenfrequencies. Hence the key idea for assessment from the full database is to select only measurements with small temperature differences and slow temperature change rates.

FFCDH: Solution to Enable Face-to-Face Conversation between Deaf and Hearing People

A real time communication between deaf and hearing people is still a barrier that isolates the deaf people from the hearing world. Over ninety percent of deaf children are born to hearing parents. However, most of them can only learn how to communicate using sign language at school. One of the reasons is that the hearings parents have neither enough time nor support to learn sign language to communicate and support their children. Not surprisingly, the deaf finds difficulties in the oral-only education. Since many other hearing pupils do not even know about the existence of sign language, they cannot communicate directly with the deaf without a sign language interpreter. Therefore, to enable a face-to-face conversation between deaf and hearing people, it is important not only to sustain real time conversation between the deaf and their hearing counterparts but also to equip the hearing with basics of sign language. However, speech to sign conversion remains a challenge due to dialectal and sign language variation, speech utterance and lack of sign language written form. In this paper, a solution named Face-to-Face Conversation Deaf and Hearing people—FFCDH is proposed to address above issues. FFCDH supports real time conversation and also allows the hearing to learn the signs with the same meaning as the deaf understand. Moreover, FFCDH records the speech of the hearing and converts it into signs for the deaf. It also provides deaf with an option to adjust volume of their speech by displaying volume of their voice. The performance of the system in supporting the deaf has been evaluated by using a real test-bed. The obtained results show that English and Japanese daily conversation phrases can be recognized with over 90 percent accuracy on average. The average coherent of simple content is over 94 percent. Hip Tuberculosis at Stage IV: Outcomes and Some Conditions for Total Hip Replacement

Background: Treatment of hip disorders during active hip tuberculosis has long been a controversial issue. Some authors have reported performing hip replacement with different strategies with very good outcomes. This study aimed to describe the surgical outcomes and necessary conditions for total hip replacement in active hip tuberculosis. Methods: We conducted a quasi-experimental study. The study enrolled 40 patients with 42 active tuberculosis hips at stage IV treated by total hip replacement from October 2016 to December 2019 at the National Lung Hospital. We followed up with the patients for at least 12 months, evaluated surgical outcomes, and investigated the factors associated with these outcomes by logistic regression analysis. Results: Surgical outcomes showed that 37 patients (88.1%) had excellent hip function, no abscesses, and no sinus tract formation. Four cases (9.5%) had sinus tract formations. One case (2.4%) had good hip function. Binary logistic regression models revealed that sinus tract formation was associated with preoperative tuberculosis infection syndrome. The average time to obtain antituberculosis drug treatment preoperatively was 4.6 weeks. Conclusion: Total hip replacement for active hip tuberculosis is a practical and promising treatment method. Surgeons should consider improving patients’ conditions before performing total hip replacement, administering antitubercular drugs, and arthrotomy to eliminate all abscesses, and decrease the risk of tuberculosis infection syndrome and the inflammatory response.

long-range and fast transport of coherent excitons is important for the development of high-speed excitonic circuits and quantum computing ***,most of these coherent excitons have only been observed in some low-dimensional semiconductors when coupled with cavities,as there are large inhomogeneous broadening and dephasing effects on the transport of excitons in their native states in ***,by confining coherent excitons at the 2D quantum limit,we first observed molecular aggregation-enabled‘supertransport’of excitons in atomically thin two-dimensional(2D)organic semiconductors between coherent states,with a measured high effective exciton diffusion coefficient of ~346.9 cm^(2)/s at room *** value is one to several orders of magnitude higher than the values reported for other organic molecular aggregates and low-dimensional inorganic *** coupling to any optical cavities,the monolayer pentacene sample,a very clean 2D quantum system(~1.2 nm thick)with high crystallinity(J-type aggregation)and minimal interfacial states,showed superradiant emission from Frenkel excitons,which was experimentally confirmed by the temperature-dependent photoluminescence(PL)emission,highly enhanced radiative decay rate,significantly narrowed PL peak width and strongly directional in-plane *** coherence in monolayer pentacene samples was observed to be delocalised over~135 molecules,which is significantly larger than the values(a few molecules)observed for other organic thin *** addition,the supertransport of excitons in monolayer pentacene samples showed highly anisotropic *** results pave the way for the development of future high-speed excitonic circuits,fast OLEDs,and other optoelectronic devices.

Efficacy and safety of tenofovir in chronic hepatitis B: Australian real world experience

AIMTo evaluate the long-term treatment outcomes of tenofovir therapy in patients in a real world Australian tertiary care setting. METHODSWe performed a retrospective analysis of treatment outcomes among treatment-naïve and treatment-experienced patients receiving a minimum 3 mo tenofovir therapy through St Vincent’s Hospital Melbourne, Australia. We included patients receiving tenofovir [tenofovir disoproxil fumarate (TDF)] monotherapy, as well as patients treated with TDF in combination with a second antiviral agent. Patients were excluded if they demonstrated human immune-deficiency virus/hepatitis C virus/hepatitis delta virus coinfection or were less than 18 years of age. We considered virological and biochemical response, as well as safety outcomes. Virological response was determined by measurement of hepatitis B virus (HBV) DNA using sensitive assays; biochemical response was determined via serum liver function tests; histological response was determined from liver biopsy and fibroscan; safety analysis focused on glomerular renal function and bone mineral density. The primary efficacy endpoint was complete virological suppression over time, defined by HBV DNA RESULTSNinety-two patients were identified who fulfilled the enrolment criteria. Median follow-up was 26 mo (range 3-114). Mean age was 46 (24-78) years, 64 (70%) were male and 77 (84%) were of Asian origin. 55 (60%) patients were treatment-naïve and 62 patients (67%) were HBeAg-negative. Complete virological suppression was achieved by 45/65 (71%) patients at 12 mo, 37/46 (80%) at 24 mo and 25/28 (89%) at 36 mo. Partial virological response (HBV DNA 20-2000 IU/mL) was achieved by 89/92 (96.7%) of patients. Multivariate analysis showed a significant relationship between virological suppression at end of follow-up and baseline HBV DNA level (OR = 0.897, 95%CI: 0.833-0.967, P = 0.0046) and HBeAg positive status (OR = 0.373, 95%CI: 0.183-0.762, P = 0.0069). Automatic Abnormal Electroencephalograms Detection of Preterm Infants

Many preterm infants suffer from neural disorders caused by early birth complications. The detection of children with neurological risk is an important challenge. The electroencephalogram is an important technique for establishing long-term neurological prognosis. Within this scope, the goal of this study is to propose an automatic detection of abnormal preterm babies’ electroencephalograms (EEG). A corpus of 316 neonatal EEG recordings of 100 infants born after less than 35 weeks of gestation were preprocessed and a time series of standard deviation was computed. This time series was thresholded to detect Inter Burst Intervals (IBI). Temporal features were extracted from bursts and IBI. Feature selection was carried out with classification in one step so as to select the best combination of features in terms of classification performance. Two classifiers were tested: Multiple Linear Regressions and Support Vector Machines (SVM). Performance was computed using cross validations. Methods were validated on a corpus of 100 infants with no serious brain damage. The Multiple Linear Regression method shows the best results with a sensitivity of 86.11% ± 10.01%, a specificity of 77.44% ± 7.62% and an AUC (Area under the ROC curves) of 0.82 ± 0.04. An accurate detection of abnormal EEG for preterm infants is feasible. This study is a first step towards an automatic analysis of the premature brain, making it possible to lighten the physician’s workload in the future.

Treatment-Induced Acute Leukaemia after Major Response to Cyclophosphamide-Based Metronomic Chemotherapy in Refractory Heavily Pre-Treated Prostate Cancer

Background: metronomic chemotherapy is based on antiangiogenic and immunologic mechanisms obtained by the administration of traditional cytotoxic drugs at lower concentration without rest periods. The low dosage induces fewer or no side effect compared to classic maximum tolerated dose administration (MTD). At present, no treatment related acute leukaemia was reported in cyclophosphamide-based metronomic chemotherapy (CMC). Case: We report the case of an 81-year-old man considered as having castration and chemo-refractory metastatic prostate cancer. CMC was started. Objective response was observed in this heavily pre-treated patient with progression free survival lasting more than 30 months. No toxicity was observed in this period and his autonomy was maintained. Finally, our patient developed a chemotherapy-induced acute myeloid leukaemia at 36th month of CMC. Conclusion: Even CMC is a well-tolerated treatment;secondary acute leukaemia is related to cumulative dose of cyclophosphamide. The benefit and the risk of long-term exposure to cyclophosphamide should be carefully balanced.
