Bleb needling outcomes for failed trabeculectomy blebs in Asian eyes: a 2-year follow up

·AIm: To describe the outcomes of bleb needling in primary glaucoma in an Asian tertiary eye centre over a2 y period. To compare the success rates between primary angle-closure glaucoma(PACG) and primary open angle glaucoma(POAG). Lastly, to identify factors associated with success of bleb needling.·mETHODS: This was a retrospective review of 227 patients who underwent bleb needling between June2009 and June 2011 in Singapore National Eye *** 5-fluorouracil(5-FU) augmented bleb needling was performed either at the slit lamp or in the operating theatre. Repeat bleb needlings were performed as necessary. Complete success was defined as maintenance of intraocular pressure(IOP) ≥6 mm Hg and ≤21 mm Hg, in the absence of further surgery or use of antiglaucoma medication. Qualified success met the above criteria with or without use of antiglaucoma medications.· RESULTS: One hundred and seventy-five eyes completed the two-year follow up. Sixty-nine percent of participants had POAG and 31% had PACG. The mean interval between filtering surgery and bleb needling was299.9 ±616.4d for POAG and 167.1 ±272.2d for *** needling attempts were 1.9±1.4 and 2±1.6 for POAG and PACG respectively. In general, there was a statistically significant reduction of IOP ranging from21.9% to 26.8% from month 1 through to month 24. The complete success rates at month 6 were 70.0% for POAG and 65.7% for PACG. At month 12, this decreased to62.2% for POAG and PACG and at month 24, 57.9% for POAG and 63.0% for PACG respectively. The qualified success rates at month 6 for POAG and PACG were23.8% and 29.9% respectively, 32.2% and 29.2% at month12, and 34.7% and 29.6% at month 24. The success ratesbetween POAG and PACG were not significantly different(P >0.05 for complete and qualified success at months 6,12 and 24). Background:To examine the cosmetic outcome of femtosecond laser-assisted pterygium surgery(FLAPS)with conjunctival autograft(CAG)and its potential predictive ***:This was a prospective interventional case series(NCT02866968).We included 29 patients(29 eyes)with primary pterygium who underwent *** outcome was graded by two graders(an ophthalmology resident and an experienced ophthalmologist)using Hirst classification system(1-4=excellent-poor).Weighted Cohen’s kappa analysis was performed to examine the intra-and inter-rater *** relationship between cosmetic outcome and various factors were determined by Spearman’s correlation coefficients(r).Results:The preoperative severity of pterygium(Tan grading system)was mild/atrophic(7%),moderate/intermediate(62%),and severe/fleshy(31%).An ultrathin CAG(mean thickness of 74.5±9.8μm)was fashioned *** excellent cosmetic outcome of FLAPS(median±IQR)was observed at 3 months(1.0±1.0)and remained similar at 6months(1.0±0.0)and 12months(1.0±0.0)*** final follow-up,27(93%)patients achieved good-to-excellent cosmetic outcome,with 1(3%)patient having a poor outcome due to incomplete pterygium *** kappa analysis of Hirst grading system showed excellent intra-rater(κ=0.86-0.95)and inter-rater reliability(κ=0.84-0.88).There was a weak and borderline significant correlation between good cosmetic outcome and reduced postoperative CAG thickness(r=0.38,P=0.06)but not with age,gender,preoperative pterygium severity,or intraoperative CAG ***:FLAPS can result in an excellent cosmetic outcome,which may be attributed to the beneficial effect of an ultrathin CAG.

The active Sagaing fault in myanmar defines the boundary between the Indian Plate and the Eurasian Plate and causes seismic damage in the major cities of myanmar. Small bodies of serpentinite occur along the fault. We for the first time investigated the highly sheared serpentinite bodies in the Sheinmagar area and Yega Inn area along the Sagaing fault. Extensively sheared/brecciated serpentinites and related rocks, such as talc and/or chlorite-bearing rocks contains small rock fragments of serpentinites. Serpentine texture and mineral chemistry indicate that the protolith of these serpentinites were mainly harzburgite with minor amounts of dunite, some of which are cut by gabbroic veins. No shape-preferred orientation of the antigorite is present, indicating that the serpentinization was occurred under relatively static conditions. Protolith and serpentine minerals are similar to those of the jadeitite-bearing serpentinites in the north of the Sagaing fault(the Jade mine belt). Chemical variations of spinels in the studied area are within the compositional range of forearc peridotites and those in the mantle section of nearby ophiolites. After the formation of antigorite serpentinite under static conditions, these serpentinites were subsequently, but locally deformed, probably due to the activity of the Sagaing fault, resulting in the formation of serpentinite schist/brecciated rock. The presence of the less-deformed antigorite serpentinite in the sheared/brecciated zone indicates the strain localization mainly along the surrounding serpentine-talc(±chlorite) schistose rocks, which is probably formed by the reaction between serpentinite and country rocks. Further studies are needed to better understand whether the distribution of serpentinized peridotites cause variations in the activity of the Sagaing fault.

BACKGROUND Type 1 diabetes(T1D)is a complex disease with a higher incidence in Europeans than other *** Colombians Living in medellin(CLm)is admixed with ancestry contributions from Europeans,Native Americans(NAT)and Africans(AFR).AIm Our aim was to analyze the genetic admixture component at candidate T1D loci in Colombian individuals with the *** Seventy-four ancestry informative markers(AIms),which tagged 41 T1D candidate loci/genes,were tested by studying a cohort of 200 Northwest Colombia diseased individuals.T1D status was classified by testing for glutamic acid decarboxylase(GAD-65 kDa)and protein tyrosine-like antigen-2 autoantibodies in serum *** loci/genes included hla,INS,PTPN22,CTLA4,IL2RA,SUmO4,CLEC16A,IFIH1,EFR3B,IL7R,NRP1 and RNASEH1,amongst *** 1,000 genome database was used to analyze data from 94 individuals corresponding to the reference *** the data did not comply with a normal distribution,medians were compared between groups using the mann-Whitney *** Both T1D patients and individuals from CLm displayed mainly European ancestry(61.58 vs 62.06)followed by Native American(27.34 vs 27.46)and to a lesser extent the AFR ancestry(10.28 vs 10.65)***,compared to CLm,ancestry of T1D patients displayed a decrease of NAT ancestry at gene EFR3B(24.30 vs 37.10)and an increase at genes IFIH1(32.07 vs 14.99)and IL7R(52.18 vs 39.18).Also,for gene NRP1(36.67 vs 0.003),we observed a non-AFR contribution(attributed to NAT).Autoimmune patients(positive for any of two auto-antibodies)displayed lower NAT ancestry than idiopathic patients at the mHC region(20.36 vs 31.88).Also,late onset patients presented with greater AFR ancestry than early onset patients at gene IL7R(19.96 vs 6.17).An association analysis showed that,even after adjusting for admixture,an association exists for at least seven such AIms,with the strongest findings on chromosomes 5 and 10(gene IL7R,P=5.56×10-6 and gene NRP1,P=8.

Epstein-Barr virus(EBV) infection occurs by distinct mechanisms across different cell types. EBV infection of B cells in vitro minimally requires 5 viral glycoproteins and 2 cellular proteins. By contrast, infection of epithelial cells requires a minimum of 3 viral glycoproteins, which are capable of interacting with one or more of 3 different cellular proteins. The full complement of proteins involved in entry into all cell types capable of being infected in vivo is unknown. This review discusses the events that occur when the virus is delivered into the cytoplasm of a cell, the players known to be involved in these events, and the ways in which these players are thought to function.

Hematopoietic cell transplantation for Crohn’s disease;is it time?

AIm： To review all studies in the literature that have assessed Hematopoietic cell transplantation （HCT） and Crohn＇s disease （CD） with the ultimate aims of determining if this is a viable treatment option for those with CD. A secondary aim was to review the above literature and determine if the studies shed further light on the mechanisms involved in the pathogenesis of CD. mETHODS： An extensive medline search was performed on all articles from 1970 to 2005 using the keywords; bone marrow transplant, stem cell, hematopoietic cell, Crohn＇s disease and inflammatory bowel disease. RESULTS： We identified one case in which a patient developed CD following an allogeneic HCT from a sibling suffering with CD. Evidence for transfer of the genetic predisposition to develop CD was also identified with report of a patient that developed severe CD following an allogeneic HCT. Following HCT it was found that the donor （that had no signs or symptoms of CD） and the recipient had several haplotype mismatches in hla class 111 genes in the IBD3 locus including a polymorphism of NOD2/CARD15 that has been associated with CD. Thirty three published cases of patients with CD who underwent either autologous or allogeneic HCT were identified. At the time of publication 29 of these 33 patients were considered to be in remission. The median follow-up time was seven years, and twenty months for allogeneic and autologous HCT respectively. For patients who underwent HCT primarily for treatment of their CD there have been no mortalities related to transplant complications. CONCLUSION： Overall these preliminary data suggest that both allogeneic and autologous HCT may be effective in inducing remission in refractory CD. This supports the hypothesis that the hematolymphatic cells play a key role in CD and that resetting of the immune system may be a critical approach in the management or cure of CD.

measles virus （mV） is highly contagious pathogen, which causes a profound immunosuppression, resulting in high infant mortality. This virus infects dendritic cells （DCs） following the binding of mV hemagglutinin （mV-H） to CD150 receptor and alters DC functions by a mechanism that is not completely understood. We have analyzed the effect of mV-H interaction with CD150-expressing DCs on the DC signaling pathways and consequent phenotypic and functional changes in the absence of infectious context. We demonstrated that contact between CD150 on human DCs and mV-H expressed on membrane of transfected CHO cells was sufficient to modulate the activity of two major regulatory pathways of DC differentiation and function： to stimulate Akt and inhibit p38 mAPK phosphorylation, without concomitant ERK1/2 activation. Furthermore, interaction with mV-H decreased the expression level of DC activation markers CD80, CD83, CD86, and hla-DR and strongly downregulated IL-12 production but did not modulate IL-IO secretion. moreover, contact with mV-H suppressed DC-mediated T-cell alloproliferation, demonstrating profound alteration of DC maturation and functions. Finally, engagement of CD150 by mV-H in mice transgenic for human CD150 decreased inflammatory responses, showing the immunosuppressive effect of CD150-mV-H interaction in vivo. Altogether, these results uncover novel mechanism of mV-induced immunosuppression, implicating modulation of cell signaling pathways following mV-H interaction with CD150-expressing DCs and reveal anti-inflammatory effects of CD150 stimulation.

AIm： To investigate the prognostic significance of PECAm-1, ICAm-3 and hla-DR antigens in patients with primary non-Hodgkin＇s gastric lymphoma. mETHODS： We immunohistochemically studied PECAm-1, ICAm-3 and hla-DR antigen expression in 36 B-cell mALT-type primary gastric lymphoma patients. Ten non-malignant and ten healthy gastric tissue specimens were used as controls. Clinicopathological and survival data were correlated with the staining results. RESULTS： hla-DR antigen expression was detected in 33 gastric lymphoma patients （91.7%） and 6 nonmalignant patients （54.5%）. PECAm-1 stained tumor cells of 10 patients （27.8%）, endothelial cells of 9 patients （25%） and inflammatory infiltrate of 4 patients （40%） with benign gastric disease. ICAm-3 expression was observed on the tumor cells of 17 patients （47.2%）, while 5 non-malignant patients （50%） were stained positive as well. None of the healthy controls was stained for any of the genes studied. In the multivariate analysis, hla-DR antigen and PECAm-1 were proved to be statistically significant independent prognostic factors associated with a favourable and an unfavourable prognosis respectively （P= 0.009 and P= 0.003）. In the univariate analysis, PECAm-1（＋）/ICAm-3（-） and hla-DR（-）/ICAm-3（-） patients exhibited a significantly decreased overall survival compared to those with the exactly opposite gene expression patterns （P=0.0041 and P= 0.0091, respectively）. Those patients who were hla-DR（＋）/ICAm-3（＋）/PECAm-I（-）（n = 8） had a significantly higher survival rate compared to the rest of the group （n = 24）（P= 0.0289）. CONCLUSION： PECAm-1, ICAm-3 and hla-DR are representative markers of tumor expansion potential and host immune surveillance respectively. Their combined use may help us to identify high-risk patients who could benefit from more aggressive therapeutic protocols.
