Pancreatic cancer with a high serum IgG4 concentration

Differentiation between autoimmune pancreatitis and pancreatic cancer is sometimes difficult. It has been reported that serum IgG4 concentrations are significantly elevated and particularly high (>135 mg/dL) in autoimmune pancreatitis. Measurement of serum IgG4 has become a useful tool for differentiating between autoimmune pancreatitis and pancreatic cancer. However, we present a 74-year-old female with a markedly elevated serum IgG4 (433 mg/dL) who underwent pancreaticoduodenectomy for pancreatic cancer. Elevated serum IgG4 levels continued after the resection. On histology, adenocarcinoma of the pancreas accompanied with moderate lymphoplasmacytic infiltration infiltrated the lower bile duct and duodenum, but there were no findings of autoimmune pancreatitis. Although a small metastasis was detected in one parapancreatic lymph node, regional lymph nodes were swollen. Abundant IgG4-positive plasma cells infiltrated the cancerous areas of the pancreas, but only a few IgG4-positive plasma cells were detected in the noncancerous areas. Pancreatic cancer cells were not immunoreactive for IgG4. An abundant infiltration of IgG4-positive plasma cells was detected in the swollen regional lymph nodes and in the duodenal mucosa. We believe that the serum IgG4 level was elevated in this patient with pancreatic cancer as the result of an IgG4-related systemic disease that had no clinical manifestations other than lymphadenopathy.

Clinicopathological characteristics of clinical early gastric cancer in the upper-third stomach

AIM: To elucidate the clinicopathological characteristics of clinically early gastric cancer in the upper-third stomach and to clarify treatment ***: A total of 683 patients with clinical early gastric cancer were enrolled in this retrospective study, 128 of whom had gastric cancer in the upper-third stomach(U group). All patients underwent a double contrast barium examination, endoscopy, and computed tomography(CT), and were diagnosed preoperatively based on the findings obtained. The clinicopathological features of these patients were compared with those of patients with gastric cancer in the middle- and lower-third stomach(ML group). We also compared clinicopathological factors between accurate-diagnosis and under-diagnosis groups in order to identify factors affecting the accuracy of a preoperative diagnosis of tumor ***: Patients in the U group were older(P = 0.029), had a higher ratio of males to females(P = 0.015), and had more histologically differentiated tumors(P = 0.007) than patients in the ML group. A clinical under-diagnosis occurred in 57 out of 683 patients(8.3%), and was more frequent in the U group than in the ML group(16.4% vs 6.3%, P < 0.0001). Therefore, the rates of lymph node metastasis and lymphatic invasion were slightly higher in the U group than in the ML group(P = 0.071 and 0.082, respectively). An under-diagnosis was more frequent in histologically undifferentiated tumors(P = 0.094) and in those larger than 4 cm(P = 0.024). The medianfollow-up period after surgery was 56 mo(range, 1-186 mo). Overall, survival and disease-specific survival rates were significantly lower in the U group than in the ML group(P = 0.016 and 0.020, respectively). Proposal of a Model of a Three-Dimensional Spherical Universe

Despite the existence of several exact solutions to the general theory of relativity, it is still difficult to explain the entire structure of the universe. In this paper, we propose a novel three-dimensional spherical (S3) universe model. According to this model, the universe had a powerful gravity source at its origin, and a part of the gravitational source formed a bubble of spacetime in which the universe was born. The universe expanded explosively immediately after the Big Bang. The energy obtained from the gravity source at the birth of the universe is constant, that is, it remains constant from the birth of the universe to the end. The expansion and contraction of the universe is determined by the passage of coordinate time. The S3 universe observed from outside can be considered to be a two-dimensional spherical surface by projecting it onto a three-dimensional space. On the other hand, the visible universe viewed from inside is observed as a three-dimensional sphere with an arbitrary observation point. The origin of the S3 universe can be seen to be evenly spread in the outer shell of the visible universe. We define visible longitude as the difference between an observation point and its farthest light source. At an initial phase of the expansion of the universe, few stars can be seen, because the visible longitude is small. However, when the expansion of the universe progresses, the number of visible stars also increases, since visible longitude increases. In our S3 universe model, the redshifts observed and reported so far in literature appear to indicate that the expansion of the universe is accelerating. Our S3 universe model can thus lead to a novel exact solution to general relativity.

Investigation of Experimental Con6guration for Electron Bernstein Wave Heating on LHD

Investigation of experimental configuration for the electron Bernstein wave （EBW） heating by using the existing electron cyclotron heating （ECH） antennas on LHD was performed. By using an antenna installed in the lower port, direct oblique launching of the extraordinary （X-） mode from the high magnetic field side （HFS） is available. Since the parallel component of the refractive index （NIF） varies during propagation because of the inhomogeneity of the magnetic field, NH can be zero when the launched X-mode crosses the fundamental electron cyclotron resonance （ECR） layer even NⅡ is noonzero initially. In such a condition, if the electron density is above a certain level the obliquely launched X-mode can pass the fundamental ECR layer without being damped out and can be mode-converted to EBW that is absorbed at the Doppler shifted ECR layer. By using an antenna installed in the horizontal port, oblique launching from the lower magnetic field side （LFS） toward the over-dense plasma is available. Excitation of EBW via the mode conversion process of ordinary mode（O）-extraordinary mode（X）-electron Bernstein wave （B） is expected with the O-mode launching toward an appropriate direction. The O-X-B mode conversion rate and the region of power deposition were surveyed by varying the magnetic field strength and the launching direction. The results of the survey suggest that efficient heating in the core region is difficult by using the existing antenna. Rearrangement of the final mirror of the launching antenna may be needed.

Apparent Accelerated Expansion of the Three-Dimensional Spherical Universe Observed during Its Contraction Phase

In recent years, it has been thought that the expansion of the universe has begun to accelerate. However, there are other views against this. Here we propose a new theory based on the three-dimensional spherical (S3) universe wherein the same observations as the present universe can also be found by the accelerated contraction of the universe. According to our theory, the expansion velocity of the S3 universe slowed down after the Big Bang, and all the kinetic energy was converted into potential energy to reach the great sphere. After that, accelerated contraction begins, and the universe finally converges to an original single point. In the S3 universe, the passage of time (referred to as “proper time”) changes depending on its expansion velocity. The frequency of light emitted from celestial bodies is determined by their proper time on emission, and when the light is observed by observers having different proper time, a redshift or blueshift is observed. Observers in the expansion phase observe redshifts, because the proper time of the observer progresses faster than that of emitted light, but observers in the contraction phase observe an accelerated delay of the proper time, so the progress of the proper time is reversed based upon its order from nearby celestial bodies, a blueshift is observed, and its range of observable distance increases. The results of this early contraction phase are consistent with the observations of the current universe. In conclusion, the S3 universe may be able to explain the geometrical structure of the current universe.

Interactive Fuzzy Approaches for Solving Multiobjective Two-Person Zero-Sum Games

In this paper, we consider multiobjective two-person zero-sum games with vector payoffs and vector fuzzy payoffs. We translate such games into the corresponding multiobjective programming problems and introduce the pessimistic Pareto optimal solution concept by assuming that a player supposes the opponent adopts the most disadvantage strategy for the self. It is shown that any pessimistic Pareto optimal solution can be obtained on the basis of linear programming techniques even if the membership functions for the objective functions are nonlinear. Moreover, we propose interactive algorithms based on the bisection method to obtain a pessimistic compromise solution from among the set of all pessimistic Pareto optimal solutions. In order to show the efficiency of the proposed method, we illustrate interactive processes of an application to a vegetable shipment problem.

0.025-inch vs 0.035-inch guide wires for wire-guided cannulation during endoscopic retrograde cholangio pancreatography:A randomized study

AIM:To compare the clinical outcomes between 0.025-inch and 0.035-inch guide wires(GWs) when used in wire-guided cannulation(WGC).METHODS:A single center,randomized study was conducted between April 2011 and March 2013. This study was approved by the Medical Ethics Committee at our hospital. Informed,written consent was obtained from each patient prior to study enrollment. Three hundred and twenty-two patients with a na?ve papilla of Vater who underwent endoscopic retrograde cholangiopancreatography(ERCP) for the purpose of selective bile duct cannulation with WGC were enrolled in this study. Fifty-three patients were excluded based on the exclusion criteria,and 269 patients were randomly allocated to two groups by a computer and analyzed:the 0.025-inch GW group(n = 109) and the 0.035-inch GW group(n = 160). The primary endpoint was the success rate of selective bile duct cannulation with WGC. Secondary endpoints were the success rates of the pancreatic GW technique and precutting,selective bile duct cannulation time,ERCP procedure time,the rate of pancreatic duct stent placement,the final success rate of selective bile duct cannulation,andthe incidence of post-ERCP pancreatitis(PEP).RESULTS:The primary success rates of selective bile duct cannulation with WGC were 80.7%(88/109) and 86.3%(138/160) for the 0.025-inch and the 0.035-inch groups,respectively(P = 0.226). There were no statistically significant differences in the success rates of selective bile duct cannulation using the pancreatic duct GW technique(46.7% vs 52.4% for the 0.025-inch and 0.035-inch groups,respectively; P = 0.884) or in the success rates of selective bile duct cannulation using precutting(66.7% vs 63.6% for the 0.025-inch and 0.035-inch groups,respectively; P = 0.893). The final success rates for selective bile duct cannulation using these procedures were 92.7%(101/109) and 97.5%(156/160) for the 0.025-inch and 0.035-inch groups,respectively(P = 0.113). This paper presents the flutter derivatives （FDs） extracted from a stochastic system identification （SSI） method under different turbulent flows, The objective of the study is to investigate the effects of oncoming turbulence on the flutter of suspended long-span bridges using a section model wind-tunnel test, Several wind-tunnel tests were performed on a truss bridge deck section with different oncoming turbulent properties involving reduced turbulence intensities and turbulent scales. This study includes an investigation of the effect of oncoming flows on modal dynamic responses. The transient and buffeting response data from the wind-tunnel test are analyzed using the system identification technique in extracting FDs, and the difficulties involved in this method are discussed. The time-domain SSI is applied to extract all FDs simultaneously from one and two degree-of-freedom （1DOF and 2DOF） systems, Finally, the results under different conditions are discussed and conclusions are formed.

Optimizing the Conditions for Residual Stress Measurement Using a Two-Dimensional XRD Method with Specimen Oscillation

In order to optimize the conditions for residual stress measurement using a two-dimensional X-ray diffraction (2D-XRD) in terms of both efficiency and accuracy. The measurements have been conducted on three stainless steel specimens in this study. The three specimens were processed by annealing, a cavitating jet in air and a disc grinder, with each method introducing different residual stresses at the surface. The specimens were oscillated in the ω-direction, representing a right-hand rotation of the specimen about the incident X-ray beam. The range of the oscillation, Δω, was varied and optimum Δω was determined. Moreover, combinations of the tilt angle between the specimen surface normal and the diffraction vector, ψ, with the rotation angle about its surface normal, f, have been studied with a view to find the most optimum condition. The results show that the use of ω oscillations is an effective method for improving analysis accuracy, especially for large grain metals. The standard error rapidly decreased with increasing range of the ω oscillation, especially for the annealed specimen which generated strong diffraction spots due to its large grain size.

Effect of Residual Stress on the Corrosion Behavior of Austenitic Stainless Steel

In this paper we demonstrate that the residual stress introduced by several different surface finishes affects the critical current density for passivation and the passive current density in the anodic polarization curve of austenitic stainless steel and that those critical current densities can be reduced by controlling the residual stress by applying a cavitating jet to the backs of specimens. The results show that the current density either increased or decreased depending on the surface finish, and that was decreased by introducing compressive residual stress for all surface finishes.
