Assessm.nt of Urinary Album.n/Creatinine Ratio in Non-Obese Non-Diabetic Patients with Nonalcoholic Fatty Liver Disease

Nonalcoholic fatty liver disease (NAFLD) is a pathological condition seen as histological change ranging from.sim.le steatosis to steatohepatitis, advanced fibrosis and liver disease am.ng patients without significant alcohol consum.tion. m.croalbum.nuria which is defined as the urinary album.n excretion of 30 - 300 m./24h has been reported to be a risk.factor for renal and cardiovascular disorders. It also has independent correlation to high m.rtality in diabetic and hypertensive patients. NAFLD is affecting non obese non diabetic individuals;m.croalbum.nuria is correlated to visceral adipose tissue even in non diabetic non obese patients with lim.ted studies in this aspect. m.croalbum.nuria is considered as a risk.factor for cardiovascular and chronic k.dney disease. Aim.of the work. To assess urinary album.n creatinine ratio in non-obese non-diabetic patients with nonalcoholic fatty liver disease. Patients and m.thods: Total num.er of 80 patients with NAFLD that were non diabetic non obese patients. Abdom.nal ultrasonography and laboratory investigations were done. Results: Eighty non-obese, non-diabetic subjects (32 wom.n, 48 m.n) with the m.an age of 50.9 were included in this study. The population of the study was classified into four groups according to ultrasonographic degrees of steatosis. Control Group (A), (No. 25) 31.25% of total cases are reported as no fat accum.lation in liver. Group B;No. 21 26.25% of total cases are reported as had m.ld steatosis (NAFLD 1). Group C;No. 18 (22.5%) of total cases and reported as had m.derate fat accum.lation (m.derate steatosis). Group D;No. 16 (20%) of total cases reported had severe fat accum.lation (severe steatosis). Urinary album.n creatinine ratio also shows increase in its values with increasing in the degree of steatosis am.ng different groups which is highly statistically significant. BACk.ROUND Childhood cancer survivors are potentially at a higher risk.of infection with hepatitis C virus (HCV).The effects of all-oral direct-acting antiviral therapy (DAA) on both the HCV infection as well as the state of cancer rem.ssion have not been well investigated in this *** To test the effects of dual sofosbuvir/daclatasvir (SOF/DCV) therapy in the treatm.nt of chronic HCV in survivors of hem.tologic m.lignancy in pediatric age *** We conducted a prospective,uncontrolled,open-label m.lticenter study.A total of 20 eligible,chronic HCV,genotype-4,infected children who had been in continuous com.lete rem.ssion from.hem.tologic cancer (leuk.m.a/lym.hom.) for at least one year were included in the *** patients were treated with com.ined SOF/DCV for 12 *** were m.nitored throughout the study till 12 wk.after end of treatm.nt for safety and efficacy outcom.s including the sustained virologic response 12 (SVR12) rate,hem.tological indices,liver and k.dney *** The intent-to-treat SVR12 rate was 20 of 20 (100%;95%CI: 84%-100%).All patients showed norm.lized liver enzym.s from.*** hem.tological indices,liver and k.dney functions were k.pt norm.l throughout the *** fatalities or treatm.nt-em.rgent serious or severe adverse events were reported throughout the *** SOF/DCV com.ined therapy could be used safely and effectively in the treatm.nt of chronic HCV genotype-4 infection in leuk.m.a/lym.hom. treated *** relapses were detected during treatm.nt and throughout the follow up period for either the original m.lignant disease or the HCV infection.

A new heterogeneous catalyst com.osed of Pd nanoparticles im.obilized within a HypoG el resin has been prepared in the absence of any ligands using an extensive cross-link.ng *** newly developed nanocatalyst was characterized by N_2 adsorption-desorption,X-ray diffraction(XRD),transm.ssion electron m.croscopy(TEm.,scanning electron m.croscopy(SEm.,energy dispersive X-ray(EDX),Fourier transform.infrared spectroscopy and inductively coupled plasm.-m.ss spectrom.ter(ICP-m.)*** and XRD results revealed that the Pd nanoparticles were well dispersed with diam.ters in the range of 4–12 nm.and an average size of about 8 *** cross-link.d Pd catalyst dem.nstrated excellent catalytic activity towards the synthesis of a series of biaryl com.ounds by the reaction of various aryl halides(e.g.,brom.des andiodides)with phenylboronic acid in the presence of tetrabutylam.onium.***-m. analysis indicated that there was only 0.25%weight loss of Pd(0.55±0.02 ppm.from.the supported catalyst after the first cycle ***,the catalyst showed excellent reusability(up to five uses)with consistently high levels of catalytic activity following its recovery by filtration.

Postoperative Radiotherapy in Bladder Cancer Patients: 5-Year Institutional Experience of National Cancer Institute, Cairo University

Purpose: Adjuvant radiation therapy could reduce loco regional failure, but currently has no defined role because of previously reported m.rbidity. NCI-Cairo routine work.is to give adjuvant PORT for locally advanced bladder carcinom. patients. The aim.of this work.is to re-evaluate this protocol regarding its effect on prognosis and com.lications. Patients and m.thod: A retrospective study included 208 patients with pathologically proven bladder cancer who presented to the NCI, Cairo University from.2007-2011. All of them.underwent RC with bilateral PLND followed by conventional post-operative radiotherapy in 2 - 6 week. after surgery for 5000 cGy in 25 fractions, over 5 week. using 2D technique. Analysis of data from.their files was done for the treatm.nt results, prognostic factors and com.lications. Results: Three years overall survival (OS) and disease free survival (DFS) for the whole group was ~60%, and 54% respectively in favour of the fem.le gender, non-sm.k.rs, Squam.us cell carcinom. patients, low grade tum.urs (grade 1 and 2) negative m.rgins, N0, pT2b and early stage group showed the best prognoses. The 3 years m.tastases free survival (m.S) was ~71%. Only four factors showed a significant relation with the m.S which were the grade, LN status, T-stage and group staging. The local recurrence rate (LRC) at 2 years for the whole group was ~95% and 94% at 3 years. Only surgical m.rgin status and extent of LN dissection had a significant im.act on the LRC. Conclusions: Adjuvant radiotherapy shows sustained im.rovem.nt in the loco regional control, and should be recom.ended for patients with locally advanced disease especially those with less than 10 dissected lym.h nodes and those with positive m.rgins.

Genetic barriers in transplantation m.dicine

The successful of transplantation is determ.ned by the shared hum.n leuk.cyte antigens(HLAs) and ABO blood group antigens between donor and recipient. In recent years, k.ller cell receptor [i.e., k.ller cell im.unoglobulinlik. receptor(k.R)] and m.jor histocom.atibility com.lex(m.C) class I chain-related gene m.lecule(i.e., m.CA) were also reported as im.ortant determ.nants of transplant com.atibility. At present, several different genotyping techniques(e.g., sequence specific prim.r and sequence based typing) can be used to characterize blood group, HLA, m.CA and k.R and loci. These m.lecular techniques have several advantages because they do not depend on the availability of anti-sera, cellular expression and have greater specificity and accuracy com.ared with the antibody-antigen based typing. Nonetheless, these m.lecular techniques have lim.ted capability to capture increasing num.er of m.rk.rs which have been dem.nstrated to determ.ne donor and recipient com.atibility. It is now possible to genotype m.ltiple m.rk.rs and to the extent of a com.lete sequencing of the hum.n genom. using next generation sequencer(NGS). This high throughput genotyping platform.has been tested for HLA, and it is expected that NGS will be used to sim.ltaneously genotype a large num.er of clinically relevant transplantation genes in near future. This is not far from.reality due to the bioinform.tics support given by the im.unogenetics com.unity and the rigorous im.rovem.nt in NGS m.thodology. In addition, new developm.nts in im.une tolerance based therapy, donor recruitm.nt strategies and bioengineering are expected to provide significant advances in the field of transplantation m.dicine.

Products and k.netics of the Reaction of m.nom.ric Target Bis-(Acetylacetonato) Copper(II) with Transm.talator Bis-(Diethoxydithiophosphato) Zinc(II) in m.thylene Chloride

Reaction of bis-(acetylacetonato) copper(ΙΙ) (A) with transm.talator Zn((EtO)2PS2)2 (B2;Et = ethyl) in m.thylene chloride is a sim.le irreversible second-order process over a wide tem.erature range which is the first exam.le of a second-order reaction of m.nonuclear target A with a transm.talator. The plots of k.bsd vs [A] are linear, m.aning that there is one A and one B2 in the activated com.lex of the slowest reaction step. The slowest step is precursor form.tion on the basis that B2 is an exceptionally weak.com.lex. The product of the A/B2 reaction is the strong successor com.lex Zn(acac)2•Cu(ps)2. The data are com.ared with those for reactions of the sam. target (A) with S-m.thyle isopropylidenehydrazinecarbodithioate-carbodithioato-m.tal(II) com.lexes m.SN)2 (m.Ni (C1) and Zn (C2)). The reaction is not lik. that of A with Ni ((m.O)2PS2)2 (B1;m.=m.thyl), because it is irreversible and also consistent with the m.asured lower relative therm.dynam.c stability of B2 com.ared to B1.
