The journey to implement cancer genomic medicine(CGM)in oncology practice began in the 1980s,which is considered the dawn of genetic and genomic cancer *** the time,a variety of activating oncogenic alterations and their functional significance were unveiled in cancer cells,which led to the development of molecular targeted therapies in the 2000s and *** CGM is still a relatively new discipline and it is difficult to predict to what extent CGM will benefit the diverse pool of cancer patients,the National Cancer Center(NCC)of Japan has already contributed considerably to CGM advancement for the conquest of *** back at these past achievements of the NCC,we predict that the future of CGM will involve the following:1)A biobank of paired cancerous and non-cancerous tissues and cells from various cancer types and stages will be *** quantity and quality of these samples will be compatible with omics *** biobank samples will be linked to longitudinal clinical information.2)New technologies,such as whole-genome sequencing and artificial intelligence,will be introduced and new bioresources for functional and pharmacologic analyses(e.g.,a patient-derived xenograft library)will be systematically deployed.3)Fast and bidirectional translational research(bench-to-bedside and bedside-to-bench)performed by basic researchers and clinical investigators,preferably working alongside each other at the same institution,will be implemented;4)Close collaborations between academia,industry,regulatory bodies,and funding agencies will be established.5)There will be an investment in the other branch of CGM,personalized preventive medicine,based on the individual's genetic predisposition to cancer.

目的;F-fluciclovine(anti-;F-FACBC)氨基酸代谢显像诊断脑肿瘤的效能较好,而;FFDG诊断脑肿瘤存在局限性。该研究通过Ⅱa期临床研究来评价anti-;F-FACBC在脑胶质瘤患者肿瘤中的分布和安全性。方法 5例脑胶质瘤拟行手术切除的患者术前静脉注射anti-;F-FACBC后行PET显像,同时行钆增强T;加权（CE-T;W）MRI。根据CE-T;MRI和anti-;F-FACBC PET显像结果,取术中标本行组织病理学分析。比较MRI和PET/CT图像上的肿瘤范围。Anti-;F-FACBC显像时绘制肿瘤和正常脑组织的时间-放射性曲线,在肿瘤、对侧正常脑组织和小脑部位勾画ROI,并计算上述部位SUV。根据患者主观症状、客观体征、心电图、生命体征、实验室检查结果和不良反应发生情况评价anti-;F-FACBC的安全性。结果所有患者的胶质瘤病灶均摄取anti-;F-FACBC,CE-T;W MRI也可显示病灶,但前者显像的病灶范围大于后者。组织病理学结果证实,有2例患者的病灶在anti-;F-FACBC显像的病灶范围内,但在CE-T;W MRI显示的范围外。2例患者出现轻度不良反应:1例头部隐痛到轻度头痛,1例全身不适;上述症状均自行缓解。轻度头痛和anti-;F-FACBC的因果关系无法排除。该研究还发现,肿瘤与正常组织存在较高的T/N比值。结论 Anti-;F-FACBC PET/CT显像可以显示胶质瘤的病灶范围,其准确性比CE-T;W MRI高。Anti-;F-FACBC用于脑胶质瘤患者显像有较好的安全性。

Cancer statistics in Japan Cancer is the most common cause of death in Japan based on Statistics 2021~*** statistics were first gathered,infectious diseases,such as tuberculosis,and cerebrovascular disease have been the main causes of death in *** surpassed cerebrovascular disease as the main cause of death in 1981,and the number of cancer deaths has *** 38,000 people died of cancer in *** National Cancer Center(NCC)reported that the 5-year survival rate for patients with cancer was improving(62%for males and 66.9%for females)in a population-based cancer registry.

Surgical outcome comparisons of multifocal IOLs of Lentis Comfort LS-313 MF15 and Tecnis Eyhance DIB00V

AIM:To compare the surgical outcomes of a multifocal intraocular lens(IOL;Lentis Comfort LS-313 MF15)with those of an enhanced monofocal IOL(Tecnis Eyhance DIB00V).METHODS:This retrospective study included patients who underwent cataract surgery with LS-313 MF15 or Eyhance IOL *** regarding patient demographics,surgical records,and ophthalmic examination before the cataract surgery and one and three months postoperatively were *** acuities,refractive values,defocus curves,contrast sensitivities and subjective symptoms were ***:Among the 71 eyes(47 patients)included in this study,32 eyes(20 patients)underwent LS-313 MF15 IOL implantation,and 39 eyes(27 patients)underwent Eyhance IOL *** significant differences were observed in age,axial length,or refractive error between the two groups ***,the distancecorrected and uncorrected distance visual acuities one month postoperatively did not differ between the groups,and both groups had sufficient visual acuities at the distances of 5,1 m,70,50,and 30 *** ophthalmic data,including subjective symptoms based on the 14-item Visual Function Index Questionnaire,monocular defocus curves,contrast sensitivities,and halo and glare,did not differ between the groups three months ***,both groups had good *** spherical equivalent one month postoperatively was significantly myopic in the LS-313 MF15 group compared with that in the Eyhance group(P=0.033);however,this difference was not observed three months postoperatively(P=0.471).CONCLUSION:Comparison of the surgical outcomes of LS-313 MF15 with those of Eyhance with different optical properties reveal that both IOLs show good postoperative outcomes,with no significant differences being noted between the two IOLs.

We have read the position paper by *** Freeman and the members of the Pancreas Nutrition Committee of the North American and the European Societies for Pediatric Gastroenterology,Hepatology,and Nutrition(NASPGHAN and ESPGHAN)(1).That paper focused on chronic pancreatitis(CP)in children and endoscopic procedures about which NASPGHAN published a second position paper(2).There are clinical practice guidelines for the management of CP in adults but not for CP in children(3).

Characteristics of In-Hospital Patients with Congenital Heart Disease Requiring Rapid Response System Activations: A Japanese Database Study

Objectives:This study aimed to study the characteristics of in-hospital deterioration in patients with congenital heart disease who required rapid response system activation and identify risk factors associated with 1-month ***:We retrospectively analysed data from a Japanese rapid response system registry with 35 participating *** included consecutive patients with congenital heart disease who required rapid response system activation between January 2014 and March *** regression analyses were performed to examine the associations between 1-month mortality and other patient-specific ***:Among 9,607 patients for whom the rapid response system was activated,only 82(0.9%)had congenital heart *** few patients with congenital heart disease were being treated at the cardiology and cardiovascular surgery departments(12.3%and 9.9%,respectively).Moreover,the incidences of rapid-response events after intensive care unit discharge or surgery were low(6.8% and 12.2%,respectively).The most common reason for rapid response system activation was respiratory dysfunction(desaturation:35.4%,tachypnoea:25.6%,and new dyspnoea:19.5%).Rapid response system interventions and intensive care unit transfers were required for 65.9% and 20.7%of patients,*** mortality rate was 1.2% at the end of the rapid response system intervention and 11.0% after 1 ***,decreased respiratory rate and decreased heart rate at rapid response system activation were associated with increased 1-month *** adjusted odds ratio was 1.10(95% confidence interval 1.02–1.19)and 1.02(95% confidence interval,1.00–1.04) for respiratory rate and heart rate,***:Rapid response systems were rarely activated after cardiac surgery and intensive care unit discharge,which were situations with a high risk of sudden deterioration in patients with congenital heart ***,encouraging the use of the rapid response sys

One new furanocembranoid diterpene,11-hydroxy-Δ^(12(13))-pukalide(1),along with six known secondary metabolites,11-acetoxy-Δ^(12(13))-pukalide(2),13α-acetoxypukalide(3),pukalide(4),3α-methoxyfuranocembranoid(5),Δ9(15)-africanene(6),and methyl(5′E)-5-(2′,6′-dimethylocta-5′,7′-dienyl)furan-3-carboxylate(7)were isolated from the Okinawan soft coral Sinularia *** chemical structures were elucidated based on spectroscopic analysis(FTIR,NMR,and HRESIMS),and the relative stereochemistry of 1 was determined by NOESY experiments and acetylation,which yielded derivative *** addition,compounds 1 and 7 exhibited toxicity in the brine shrimp lethality test.

A Switching Controller System for a Wheeled Mobile Robot

A wheeled mobile mechanism with a passive and/or active linkage mechanism for rough terrain environment is developed and evaluated. The wheeled mobile mechanism which has high mobility in rough terrain needs sophisticated system to adapt various environments. We focus on the development of a switching controller system for wheeled mobile robots in rough terrain. This system consists of two sub-systems： an environment recognition system using link angles and an adaptive control system. In the environment recognition system, we introduce a Self-Organizing Map （SOM） for clustering link angles. In the adaptive controllers, we introduce neural networks to calculate the inverse model of the wheeled mobile robot. The environment recognition system can recognize the environment in which the robot travels, and the adjustable controllers are tuned by experimental results for each environment. The dual sub-system switching controller system is experimentally evaluated. The system recognizes its environment and adapts by switching the adjustable controllers. This system demonstrates superior performance to a well-tuned single PID controller.

A trial of simple and easy health monitoring for power poles by using ‘ipod’

At present,the regular inspection of power poles is just a visual inspection based on the inspector’s ***,overlook of the deteriorated poles can come *** order to avoid an accident due to the overlook of the deteriorated poles,the authors propose a simple and easy health monitoring method for power poles using‘i Pod’.Since the applicability of‘i Pod’as a vibration sensor in this purpose was not clear,it was examined in the following ***,the authors proposed a tool to fix the‘i Pod’to a power pole,and the performance of‘i Pod’as a vibration measurement device was compared with a conventional acceleration *** a result,it was confirmed that‘i Pod’could give almost the same response with the conventional sensor in terms of the observed predominant ***,vibration measurements were conducted at power poles in a *** a result,the observed predominant frequency corresponded to the calculated 2nd natural frequency of power *** some power poles were supported by some additional wires from the ground,the measurements were successful at almost all the ***,the authors confirmed that the‘i Pod’based health monitoring is possible.
