S-1 induced hepatic steatosis in patients with pancreatic cancer:Retrospective analysis

To determine whether S-1 induces hepatic steatosis in patients being treated for pancreatic cancer. METHODSThis retrospective study evaluated 22 patients who received oral S-1 as a first-line treatment for pancreatic cancer between January 2008 and July 2015 at the Ishikawa Prefectural Central Hospital. Patients underwent abdominal computed tomography (CT) scans before chemotherapy and within 3 mo from the start of treatment. CT numbers of the liver and spleen were measured before and after S-1 administration. Steatosis was diagnosed when the ratio of the CT number of the liver to that of the spleen (liver/spleen ratio) was 2 (range, 18-27 kg/m2). Of the 22 patients, six (27%) regularly consumed alcohol, and five (23%) had liver metastases. The mean ratio of CT number of the liver to the spleen was significantly higher before administration of S-1 (1.27 vs 1.09, P = 0.012) compared with after. Of the 22 patients, five (23%) had hepatic steatosis and 17 (77%) did not. The pretreatment demographic and clinical characteristics of these two groups showed no significant differences. The relationship between liver/spleen ratio and alanine transaminase activity in these patients. A statistically significant inverse correlation was observed (r = -0.417, P < 0.027). CONCLUSIONOf the 22 patients with pancreatic cancer, five (23%) experienced S-1 induced hepatic steatosis. Care should be taken during S-1 treatment of patients with pancreatic cancer.

After surgery for Crohn's disease (CD), early endoscopic lesions are frequently observed despite no symptomatic recurrence. The severity of lesions found at postoperative endoscopy is reported to be a strong predictive factor for future clinical recurrence. If endoscopic lesions in the early postoperative period can be reduced with medications, symptomatic recurrence will likely be delayed and decreased. Before the introduction of biologic therapies, various medications were used for the maintenance of clinical remission after surgery; however, few demonstrated consistent efficacy. Infliximab is a recombinant anti-tumor necrosis factor-α antibody. Although infliximab is one of the most effective medications in the management of CD, its efficacy for early endoscopic lesions after surgery has not yet been assessed. The author and colleagues recently conducted a prospective study in order to investigate the impact of infliximab on early endoscopic lesions after resection for CD. We found that infliximab therapy showed clear suppressive effects on clinical and endoscopic disease activity in patients with early endoscopic lesions after resection.

The authors consider Maxwell＇s equations for an isomagnetic anisotropic and inhomogeneous medium in two dimensions, and discuss an inverse problem of determining the permittivity tensor （ε1,ε2,ε2,ε3 ） and the permeability μ in the constitutive relations from a finite number of lateral boundary measurements. Applying a Carleman estimate, the authors prove an estimate of the Lipschitz type for stability, provided that ε1,ε2,ε3,μ satisfy some a priori conditions.

The effects of the protic and aprotic polar solvents on the emission spectrum of the naphthalene-triethyl-amine system in THF were studied under conditions of steady-state illumination. The fluorescence spectrum of the naphthalene-triethylamine system consists of two emission bands, the fluorescence band of naphthalene(band A, 329 nm) and the emission band of the exciplex(band B, 468 nm). The intensities of both the emission bands decrease with increasing the solvent polarity. The intensity of band B also decreases due to the hydrogen-bonding interaction between triethylamine and protic solvent, while that of band A increases. It is thus suggested that the quenching of naphthalene fluorescence by triethylamine in THF occurs through the charge transfer and electron transfer reactions. The spectral changes upon the increase of solvent polarity can be explained by the dependences of the equilibrium constant between exciplex and ion-pair and the rate constant for the electron transfer reaction from triethylamine to the excited naphthalene on the relative permittivity of solvent. It is shown that the formation of intermolecular hydrogen-bonding between triethylamine and protic solvent suppresses the quenching reaction by the decrease in free amine. Acetonitrile has only a polar effect and trichloroacetic acid only a hydrogen-bonding(or protonation) effect, while alcohols have both the effects. The effects of alcohols could be separated into the effects of solvent polarity and intermolecular hydrogen-bonding interaction quantitatively.

The redox active ionic liquid, 1-ethyl-4-(methoxycarbonyl)pyridinium bis(trifluoromethanesulfonyl)amide(RIL), was synthesized from its iodide form by an anion exchange reaction of Li(NTf_2) with viscos liquid(η= 122 cP at 25C) and characterized by NMR, IR, and elemental analysis. The compound showed reversible redox couples at -0.65 V and -1.48 V vs. Ag/AgCl and worked as an electron mediator in the B_(12) complex/[Ru(bpy)_3]Cl_2 photosensitizer catalytic system under visible light irradiation. The catalytic efficiency in the RIL was higher than those in DMF, MeOH, and the redox inactive ionic liquid, 1-butyl-3-methyl imidazolium bis(trifluoromethylsulfonyl)amide.

Influences of Incidence Angle on 2D-Flow and Secondary Flow Structure in Ultra-Highly Loaded Turbine Cascade

An increase of turbine blade loading can reduce the numbers of blade and stage of gas turbines. However, an increase of blade loading makes the secondary flow much stronger because of the steep pitch-wise pressure gradient in the cascade passage, and consequently deteriorates the turbine efficiency. In this study, the computations were performed for the flow in an ultra-highly loaded turbine cascade with high turning angle in order to clarify the effects of the incidence angle on the two dimensional flow and the secondary flow in the cascade passage, which cause the profile loss and the secondary loss, respectively. The computed results showed good agreement with the experimental surface oil flow visualizations and the blade surface static pressure at mid-span of the blade. The profile loss was strongly increased by the increase of incidence angle especially in the positive range. Moreover, the positive incidences not only strengthened the horseshoe vortex and the passage vortex but also induced a new vortex on the end-wall. Moreover, the newly formed vortex influenced the formation of the pressure side leg of horseshoe vortex.

The effective thermal conductivity of composites with ellipsoidal fillers is analyzed by using a homogenization method that is able to represent the microstructure *** this study,various parameters such as the volume fraction,shape,and distribution of the filler are quantitatively estimated to understand the mechanisms of heat transfer in the ***,thermal boundary resistance between resin and filler is important for obtaining composites with higher thermal ***,the anisotropy of the effective thermal conductivity arises from contact between filler in the case of ellipsoidal filler and produces lower thermal ***,the filler network and thermal resistance are essential for the heat transfer in composites because the path of thermal conduction is improved by contact between neighboring filler particles.

We explain various facets of the THSR(Tohsaki-Horiuchi Schuck-Ropke)wave *** first discuss the THSR,wave function as a wave function of cluster-gas state,since the THSR,wave function was originally introduced to elucidate the 3a-condensate-like character of the Hoyle state(O2^+ state)of *** briefly review the cluster-model studies of the Hoyle state in 1970’s in order to explain how there emerged the idea to assign the a condensate character to the Hoyle *** then explain that the THSR wave function can describe very well also non-gaslike ordinary cluster states with spatial localization of *** fact means that the dynamical motion of clusters is of nonlocalized nature just as in gas-like states of clusters and the localization of clusters is due to the inter-cluster Pauli principle which is against the close approach of two *** nonlocalized cluster dynamics is formulated by the container model of cluster *** container model describes gas-like state and non-gaslike states as the solutions of the Hill Wheeler equation with respect to the size parameter of THSR wave function which is just the size parameter of the *** we notice that fact that the THSR wave function with the smallest value of size parameter is equivalent to the shell-model wave function,we see that the container model describes the evolution of cluster structure from the ground state with shell-model structure up to the gas-like cluster state via ordinary non-gaslike cluster *** the description of various cluster structure,more generation of THSR wave function have been introduced and we review some typical examples with their actual applications.

Multi-stage ignition and/or double NTC(negative temperature coefficient)behavior resulted from the low-temperature oxidation of ether compounds are still not clearly *** have investigated the oxidation mechanism of a stoichiometric DEE(diethyl ether)/air mixture by using a micro flow reactor with a controlled temperature profile to see the detail of low-temperature weak flame *** simulation was also performed to understand the chemical kinetics mechanism of observed weak flame *** measurement showed separated weak flame in the temperature range of 600 K-800 *** simulation also qualitatively reproduced this separated weak flame,and showed four peak of heat *** the reaction flow analysis,it was found that(1)O-O bond scission reaction of keto-hydroperoxide produced by DEE,(2)O-O bond scission reaction of CH3O2H,CH3CO3H,and C2H5O2H,(3)O-O bond scission reaction of H2O2,and(4)H+O2=O+OH are key chain branching reactions to explain the multi-stage oxidation.
