Backgrounds ATP is the major energy source for myotube contraction,and is quickly produced to compensate ATP consumption and to maintain sufficient ATP *** is consumed mainly in cytoplasm and produced in mitochondria during myotube *** understand the mechanism of ATP homeostasis during myotube contraction,it is essential to monitor mitochondrial ATP at single-cell level,and examine how ATP is produced and consumed in ***:We established C2C12 cell line stably expressing fluorescent probe of mitochondrial ATP,and induced differentiation into myotubes・We gave electric pulse stimulation to the differentiated myotubes,and measured mitochondrial *** constructed mathematical model of mitochondrial ATP at single-cell level,and analyzed kinetic parameters of ATP production and ***:We performed hierarchical clustering analysis of time course of mitochondrial ATP,which resulted in two *** 1 showed strong transient increase,whereas cluster 2 showed weak transient *** modeling at single-cell level revealed that the ATP production rate of cluster 1 was larger than that of cluster 2,and that both regulatory pathways of ATP production and consumption of cluster 1 were faster than those of cluster *** 1 showed larger mitochondrial mass than cluster 2,suggesting that cluster 1 shows the similar property of slow muscle fibers,and cluster 2 shows the similar property of fast muscle *** Cluster 1 showed the stronger mitochondrial ATP increase by larger ATP production rate,but not smaller *** 1 might reflect the larger oxidative capacity of slow muscle fiber.

Prevention of fatal hepatic complication in schistosomiasis by inhibition of CETP

Schistosoma japonicum,once endemic all the East Asia,remains as a serious public health problem in certain *** egg embryonation in the liver causes granulomatosis and eventually fatal cirrhosis,so that prevention of this process is one of the keys to reduce its *** embryonation requires cholesteryl ester from HDL of the host blood for egg yolk formation,and this reaction is impaired from the abnormal large HDL in genetic cholesteryl ester transfer protein(CETP) *** CETP was expressed in mice that otherwise lack this protein,granulomatosis of the liver was shown increased compared to the wild type upon infection of Schistosoma *** CETP deficiencies accumulated exclusively in East Asia,from Indochina to Siberia,so that Shistosomiasis can be a screening factor for this ***36 related protein(CD36RP) was identified as a protein for this reaction,cloned from the cDNA library of Schistosoma japonicum with 1880-bp encoding 506 amino *** antibody against the extracellular loop of CD36 RP inhibited cholesteryl ester uptake from HDL and suppressed egg embryonation in ***,inhibition of CETP is a potential approach to prevent liver granulomatosis and thereby fatal liver cirrhosis in the infection of Schistosoma japonicum.

A substantial incidence of silent short segment endoscopically suspected esophageal metaplasia in an adult Japanese primary care practice

AIM: To determine the incidence and characteristics of endoscopically suspected esophageal metaplasia (ESEM) in a primary adult care institution. METHODS: Eight hundred and thirty two consecutive individuals (mean age, 67.6 years) undergoing upper gastrointestinal endoscopy between January 2009 and December 2010 were included in this study. The diagnosis of ESEM was based on the criteria proposed by the Japan Esophageal Society, and was classified as long segment ESEM (3 cm or more) or short segment ESEM (< 3cm). Short segment ESEM was further divided into circumferential and partial types. Age, gender, hiatus hernia, esophagitis, gastroesophageal reflux disease (GERD)-suggested symptoms, and antacid medications were recorded as background factors. Esophagitis was graded according to the Los Angeles classification. Hiatus hernia was divided into absent and at least partially present. RESULTS: Long and short segment ESEM were found in 0 and 184 (22.1%) patients, respectively (mean age of short segment ESEM patients, 68.3 years). Male gender and hiatus hernia were shown to be significant factors affecting short segment ESEM by both univariate (P = 0.03 and P = 9.9x10-18) and multivariate [Odds ratio (OR) = 1.45; P = 0.04, and OR = 43.3; P = 1.5x10-7)] analyses. Two thirds of patients with short segment ESEM did not have GERD-suggested symptoms. There was no correlation between short segment ESEM and GERD-suggested symptoms. CONCLUSION: The incidence of short segment ESEM in our community practice seems higher than assumed in Asian countries. As GERD-suggested symptoms are a poor predictor of ESEM, endoscopists should bear in mind that silent short segment ESEM does exist and, in fact, was found in the majority of our patients.

Background:Surgery for perihilar cholangiocarcinoma(PHCC)remains a challenging procedure with high morbidity and *** Academic Medical Center(Amsterdam UMC)and Memorial Sloan Kettering Cancer Center proposed a postoperative mortality risk score(POMRS)and post-hepatectomy liver failure score(PHLFS)to predict patient *** study aimed to validate the POMRS and PHLFS for PHCC patients at Hokkaido ***:Medical records of 260 consecutive PHCC patients who had undergone major hepatectomy with extrahepatic bile duct resection without pancreaticoduodenectomy at Hokkaido University between March 2001 and November 2018 were evaluated to validate the PHLFS and ***:The observed risks for PHLF were 13.7%,24.5%,and 39.8%for the low-risk,intermediate-risk,and high-risk groups,respectively,in the study cohort.A receiver-operator characteristic(ROC)analysis revealed that the PHLFS had moderate predictive value,with an analysis under the curve(AUC)value of *** rates based on the POMRS were 1.7%,5%,and 5.1%for the low-risk,intermediate-risk,and high-risk groups,*** ROC analysis demonstrated an AUC value of ***:This external validation study showed that for PHLFS the threshold for discrimination in an Eastern cohort was reached(AUC>0.6),but it would require optimization of the model before use in clinical practice is *** POMRS were not applicable in the eastern *** external validation is recommended.

Questionnaire and Intervention Study on Effects of Drinking Cows’ Milk at Breakfast on the Circadian Typology and Mental Health of Japanese Infants Aged 1 - 6 Years

This study examines relationship between drinking cows’ milk at breakfast and several mental and physical characteristics (the diurnal type, sleep habits and mental condition as anger, out of control of emotion, irritation and depression) of Japanese small children aged 1 - 6 years old. A questionnaire study and an intervention one were performed in this study. An integrated questionnaire was administered, in July 2014 to 1112 participants attending one of 10 nursery school and 1 kindergarten located in Kochi (33°N, 133°E), Japan, and 582 parents (mostly mothers) which answered it instead of children (rate of answer: 51.9%). Intervention was done to 111 children attending the kindergarten. Seventy six parents answered the questionnaire which was administered 3 months after the intervention days of 21 (rate of answer: 51.9%). There are two contents of intervention, one is the distribution of cows’ milk for 21 days to be drunk at breakfast and another is the distribution of leaflet entitled “Go to bed early! Get up early! and Take nutritionally rich breakfast and cows’ milk!” just before the intervention. Just before the intervention, letter was distributed to 111 parents who were asked for their children to follow the contents of the leaflet and drink the cows’ milk distributed every day for the 21 days. Small children who drink cows’ milk at breakfast more than once per week and take nutritionally rich breakfast more than 4 times per week are more morning-typed than the other three groups in which children fit into one or none of the two issues of taking morning cows’ milk and rich breakfast (p p = 0.004). Also there was positive correlation between the implementation value for the 21 days and the diurnal type score 3 months later (r = 0.301, p = 0.018). Drinking cows’ milk at breakfast seems to be effective for small children to become more morning-typed through two sets of syntheses from tryptophan via serotonin into melatonin in the evening.

Hilar cholangiocarcinoma with intratumoral calcification: A case report

This report describes a rare case of hilar cholangiocarcinoma with intratumoral calcification that mimicked hepatolithiasis. A 73-year-old man presented to a local hospital with a calcified lesion in the hepatic hilum. At first,hepatolithiasis was diagnosed,and he underwent endoscopic stone extraction via the transpapillary route. This treatment strategy failed due to biliary stricture. He was referred to our hospital,and further examination suggested the existence of cholangiocarcinoma. He underwent left hepatectomy with caudate lobectomy and extrahepatic bile duct resection. Pathological examination revealed hilar cholangiocarcinoma with intratumoral calcification,while no stones were found. To the best of our knowledge,only one case of calcified hilar cholangiocarcinoma has been previously reported in the literature. Here,we report a rare case of calcified hilar cholangiocarcinoma and reveal its clinicopathologic features.

Background. The extent of paraaortic lymph node (PAN) metastasis parellels that of distant metastases in patients with biliary carcinoma. Accurate preoperative assessment of PAN metastasis has a crucial impact on surgical indications. In this retrospective study, we evaluated whether computed tomography (CT) scans were useful for diagnosing PAN metastases and excluding patients with PAN metastases from an indication for surgery. Methods. Between March 1999 and November 2003, 57 patients with biliary carcinoma underwent radical lymphadenectomy or surgical biopsy of PANs. Nine of these patients were diagnosed as having PAN metastasis microscopically. All patients had undergone abdominal CT scans before surgery. To diagnose PAN metastases, we used the following diagnostic criteria. (1) Size; when lymph nodes were greater than 12 mm, 10 mm, 8 mm, or 6 mm in longo or short- axis diameter, the nodes were considered metastatic. (2) Shape and size; when the axial ratio of a lymph node was greater than 0.5, 0.7, 1.0, and the maximum diameter of the long or short axis was greater than 12 mm, 10 mm, 8 mm, or 6 mm, the node was considered metastatic. (3) Internal structure; if the internal structure of a PAN was heterogeneous, the node was considered metastatic. A positive predictive value was calculated for each included criterion when patients numbered ten or more. Results. Positive predictive values using the above criteria ranged from 13% to 36% . Only one patient had PANs with heterogeneous internal structures. Conclusions. We were unable to determine surgical indications based on the morphological criteria revealed by a CT scan.
