AIM：To evaluate the antifibrogenic effects of butyrate or phenylbutyrate,a chemical derivative of butyrate,in human pterygium ***：Human pterygium fibroblasts obtained from patient pterygium tissue were treated with butyrate or phenylbutyrate for *** ofα-smooth muscle actin,collagen I,collagen III and matrix metalloproteinase-1m RNA was measured by quantitative real-time reverse transcription polymerase chain reaction,and acetylated histone was evaluated by Western ***：Butyrate inhibitedα-smooth muscle actin,type III collagen and matrix metalloproteinase-1 expressions,and phenylbutyrate inhibited types I and III collagen and matrix metalloproteinase-1 expressions without changing cell viability as well as both of these increased histone *** results suggested that butyrate and phenylbutyrate suppress fibrosis through a mechanism involving histone deacetylase ***：This indicates that butyrate or phenylbutyrate have antifibrogenic effects in human pterygium fibroblasts and could be novel types of prophylactic and/or therapeutic drugs for pterygium,especially phenylbutyrate,which does not have the unpleasant smell associated with butyrate.

Enhancement of the thermoelectric performance of half-metallic full-Heusler Mn_(2)VAl alloys via antisite defect engineering and Si partial substitution

A half-metallic full-Heusler Mn_(2)VAl alloy is a potential p-type thermoelectric material that can directly generate electricity from waste heat via the Seebeck *** practical use,the Seebeck coefficient S of Mn_(2)VAl should be increased while maintaining a high electrical conductivity s from its half-metallic ***,we achieved this objective through antisite defect ***,it was predicted that the S was maximized by regulating partial density of states of majority-spin sp-electrons through the control of the fraction of antisite defect,f_(AD),between V and Al atoms in Mn_(2)***-mentally,a significant increase in S and a slight decrease in s were observed for an Mn_(2)VAl sample with an optimal fAD=33%,enhancing the thermoelectric power factor PF by 2.7 times from an Mn_(2)VAl sample with fAD=14%.Furthermore,we combined the antisite defect engineering with a partial substitution *** Mn_(2)V(Al_(0.96)Si_(0.04))sample with fAD=33%exhibited the highest PF=4.5×10^(-4)W·m^(-1)·K^(-2)at 767 K among the *** maximum dimensionless figure-of-merit zT of the Mn_(2)V(Al_(0.96)Si_(0.04))sample with f_(AD)=33%was measured to be 3.4×10^(-2)at 767 K,which is the highest among the p-type half-metallic full-Heusler alloys.

A dye-sensitized photocatalyst combining Pt-loaded TiO_(2)and Ru(Ⅱ)tris-diimine sensitizer(RuP)was constructed and its activity for photochemical hydrogen evolution was compared with that of Pt-intercalated HCa_(2)Nb_(3)O_(10)*** the sacrificial donor ethylenediaminetetraacetic acid(EDTA)disodium salt dihydrate was used,RuP/Pt/TiO_(2)showed higher activity than RuP/Pt/HCa_(2)Nb_(3)O_(10).In contrast,when NaI(a reversible electron donor)was used,RuP/Pt/TiO_(2)showed little activity due to back electron transfer to the electron acceptor(I3-),which was gener-ated as the oxidation product of I-.By modification with anionic polymers(sodium poly(styrenesulfonate)or sodium polymethacrylate)that could inhibit the scavenging of conduction band electrons by I3-,the H_(2)production activity from aqueous NaI was improved,but it did not exceed that of RuP/Pt/HCa_(2)Nb_(3)O_(10).Transient absorption measurements showed that the rate of semiconductor-to-dye back electron transfer was slower in the case of TiO_(2)than HCa_(2)Nb_(3)O_(10),but the electron transfer reaction to I3-was much *** results indicate that Pt/TiO_(2)is useful for reactions with sacrificial reductants(e.g.,EDTA),where the back electron transfer reaction to the more reducible product can be ***,more careful design of the catalyst will be nec-essary when a reversible electron donor is employed.

This study aimed to identify the reasons for transferring athletes to local medical facilities during the Olympic and Paralympic *** on 567 injuries and other illnesses of athletes treated at the on-site clinics were collected from the Tokyo 2020 Organizing *** these,84 athletes who required outpatient care during the Games were registered for this *** the Olympic and Paralympic Games,66(8.3/1000)and 18(7.2/1000)athletes,respectively,consulted external medical *** the Olympic Games,the reasons for these visits included 48 cases(72.7%)of injuries,13(19.7%)cases of illnesses,and 5(7.6%)cases of heat stroke illness(HSI).Of these patients,56(84.9%)were treated as outpatients and 10(15.1%)were hospitalized,while three of these patients required hospitalization for>7 *** the other hand,in the Paralympics Games,there were 7(38.8%)cases of injuries,9(50.0%)other illnesses,1(5.6%)case of HSI,and 1(5.6%)other cases,of which 11(61.1%)were treated as outpatients and 7(38.9%)were hospitalized,but none was hospitalized for>7 *** accounted for 70%of the total cases at the 2021 Olympic Games,but only three(0.05%)were severe cases that required hospitalization for more than 1 *** contrast,in the Paralympic Games,other illnesses accounted for approximately half of the total *** study provides details on the extent of injuries and other illnesses that were transferred to outside facilities,which has not been documented in previous games.

Challenge to ABO blood type barrier in living donor liver transplantation

ABO incompatible living donor liver transplantation has the potential to expand the donor pool for patients with end stage liver diseases on the expense of challenges to overcome immunological barriers across blood *** is a profound impact of age on incidence and severity of antibody mediated rejection(AMR).Even children older than 1 year have chances of AMR;children aged 8 years or older have risks of hepatic necrosis similar to adult liver *** mechanism of AMR is based on circulatory disturbances secondary to inflammation and injury of the vascular endothelium caused by an antibody-antigen-complement *** strategy to overcome ABO blood type barrier is based on both pre-transplant desensitization and adequate treatment of this ***,rituximab is the standard means of desensitization but unfortunately an insufficient aid to treat *** of low incidence(less than 5%in the rituximab era),in practice of AMR only some case reports about the treatment of clinical AMR are available in the *** experiences revealed that the proteasome inhibitor,bortezomib might be a promising treatment based on its capacity to deplete plasma cell *** ABO blood type barrier has been counteracted in 95%of patients by applying“rituximab-desensitization”,many issues,such as prediction of high-risk patients of infection and AMR and secure treatment strategies for evoked AMR,remain to be resolved.

Tumor size discrepancy between endoscopic and pathological evaluations in colorectal endoscopic submucosal dissection

BACKGROUND Tumor size impacts the technical difficulty and histological curability of colorectal endoscopic submucosal dissection(ESD);however,the preoperative evaluation of tumor size is often different from histological *** influential factors on failure to obtain an accurate tumor size evaluation could help prepare optimal conditions for safer and more reliable *** This was a retrospective study conducted at a single institution.A total of 377 lesions removed by colorectal ESD at our hospital between April 2018 and March 2022 were *** first assessed the difference in size with an absolute per-centage of the scaling ***,we compared the clinicopatho-logical characteristics of the correct scaling group(>-33%and33%),which was further subdivided into the underscaling group(-33%or less of the discrepancy)and overscaling group(33%or more of the discrepancy),*** secondary outcome measures,parameters on size estimation were compared between the underscaling and correct scaling groups,as well as between the overscaling and correct scaling ***,multivariate analysis was performed in terms of the following relevant parameters on size estimation:Pathological size,location,and possible influential factors(P<0.1)in the univariate *** The mean of absolute percentage in the scaling discordance was 21%,and 91 lesions were considered to be incorrectly estimated in *** incorrect scaling was significantly remarkable in larger lesions(40 mm vs 28 mm;P<0.001)and less experience(P<0.001),and these two factors were influential on the underscaling(75 lesions;P<0.001).Conversely,compared with the correct scaling group,16 lesions in the overscaling group were significantly small(20 mm vs 28 mm;P<0.001),and the small lesion size was influential on the overscaling(P=0.002).CONCLUSION Lesions indicated for colorectal ESD tended to be underestimate

Autoimmune hepatitis triggered by acute hepatitis A

The patient was a 57-year-old woman presenting with jaundice as the chief complaint. She began vomiting on July 10, 2003. Jaundice was noted and admitted to our hospital for thorough testing. Tests on admission indicated severe hepatitis, based on： aspartate aminotransferase （AST）, 1 076 IU/L; alanine aminotransferase （ALT）, 1 400 IU/L; total bilirubin （TB）, 20.9 mg/dL; and prothrombin time rate （PT%）, 46.9%. Acute hepatitis A （HA） was diagnosed based on negative hepatitis B surface antigen and hepatitis C virus RNA and positive immunoglobulin （Ig） M HA antibody, but elevation of anti-nuclear antigen （×320） and IgG （3 112 mg/dL） led to suspicion of autoimmune hepatitis （AIH）. Plasma exchange was performed for 3 d from July 17, and steroid pulse therapy was performed for 3 d starting on July 18, followed by oral steroid therapy. Liver biopsy was performed on August 5, and the results confirmed acute hepatitis and mild chronic inflammation. Levels of AST and ALT normalized, so dose of oral steroid was markedly reduced. Steroid therapy was terminated after 4 mo, as the patient had glaucoma. Starting 3 mo after cessation of steroid therapy, levels of AST and ALl began to increase again. Another liver biopsy was performed and AIH was diagnosed based on serum data and biopsy specimen. Oral steroid therapy was reinitiated. Levels of AST and ALT again normalized. The present case was thus considered to represent AIH triggered by acute HA.
