Morphology Effects of Leading-edge Serrations on Aerodynamic Force Production： An Integrated Study Using PIV and Force Measurements

While the leading-edge serration in owls＇ wing is known to be responsible for low noise gliding and flapping flights, the findings on its aero-acoustic role have been diverse or even controversial. Here we present an experimental study of the morphological effects of leading-edge serrations on aerodynamic force production by utilizing owl-inspired, single-feather, clean and serrated wing models with different serration lengths and spacing, and by combining Particle Image Velocimetry （PIV） and force measurements. Force measurements show that an increase in the length and density of the leading-edge serrations leads to a reduction in the lift coefficient and lift-to-drag ratio at Angles of Attack （AoAs）〈 15° whereas the clean and serrated wings achieve comparable aerodynamic performance at higher AoAs 〉 15°, which owl wings often reach in flight. Furthermore PIV visualization of the flow fluctuations demonstrates that the leading-edge serration-based mechanism is consistent in all serrated wing models in terms of passive control of the laminar-turbulent transition while at AoAs 〉 15° similar suction flow is present at leading edge resulting in a comparable aerodynamic performance to that of the clean wing. Our results indicate the robustness and usefulness of leading-edge serration-inspired devices for aero-acoustic control in biomimetic rotor designs.

Hepatic encephalopathy with status epileptics: A case report

A 62-year-old male with decompensated liver cirrhosis due to hepatitis C virus developed severe hepatic encephalopathy with status epileptic us. The blood ammonia level on admission was more than twice the normal level. Brain computed tomography and magnetic resonance imaging were normal. In addition, electroencephalogram showed diffuse sharp waves, consistent with hepatic encephalopathy. The status epilepticus was resolved after antiepileptic therapy （phenytoin sodium） and treatment for hepatic encephalopathy （Branched chain amino acids）. The blood ammonia level normalized with the clinical improvement and the patient did not have a recurrence of status epilepticus after the end of the antiepileptic treatment. Additionally, the electroencephalogram showed normal findings. Thus, we diagnosed the patient as hepatic encephalopathy with status epilepticus. We consider the status epilepticus of this patient to a rare and interesting finding in hepatic encephalopathy.

AIM: To elucidate the relationship between the frequency of core mutations and the clinical activity of hepatitis B virus (HBV)-related liver disease and to characterize the amino acid changes in the core region of ***: We studied 17 Chinese patients with chronic hepatitis B according to their clinical courses and patterns of the entire core region of ***: Amino acid changes often appeared in the HBV core region of the HBV gene in patients with high values of alanine aminotransferase (ALT) or with the seroconversion from HbeAg to anti-HBe. The HBV core region with amino acid changes had high frequency sites that corresponded to HLA Ⅰ/Ⅱ restricted recognition epitopes reported by some ***: The core amino acid changes of this study occur due to influence of host immune system. The presence of mutations in the HBV core region seems to be important for predicting the clinical activity of hepatitis B in Chinese patients.

2D Numerical Simulation of Liver Cell Proliferation with Angiogenesis for Hepatic Lobule Formation

The liver has the ability to reform and regenerate in our body. However, the mechanisms of reformation or regeneration of the liver have not been elucidated. In this study, we propose an analysis model using a Particle Model to elucidate the mechanism of liver formation. The object of analysis is a hepatic lobule, which is the basic component of the liver. First, a 2-dimensional cell proliferation around one blood vessel was modeled. Second, angiogenesis was added and considered. And finally, the model was applied to the hepatic lobule and the 2D formation of the hepatic lobule was revealed. We used experimentally derived parameters such as diffusivity, oxygen concentration, and oxygen consumption of a cell. The model will be expected to facilitate in developing tissue-engineered liver using regenerative medicine technology.

This study aimed to identify the reasons for transferring athletes to local medical facilities during the Olympic and Paralympic *** on 567 injuries and other illnesses of athletes treated at the on-site clinics were collected from the Tokyo 2020 Organizing *** these,84 athletes who required outpatient care during the Games were registered for this *** the Olympic and Paralympic Games,66(8.3/1000)and 18(7.2/1000)athletes,respectively,consulted external medical *** the Olympic Games,the reasons for these visits included 48 cases(72.7%)of injuries,13(19.7%)cases of illnesses,and 5(7.6%)cases of heat stroke illness(HSI).Of these patients,56(84.9%)were treated as outpatients and 10(15.1%)were hospitalized,while three of these patients required hospitalization for>7 *** the other hand,in the Paralympics Games,there were 7(38.8%)cases of injuries,9(50.0%)other illnesses,1(5.6%)case of HSI,and 1(5.6%)other cases,of which 11(61.1%)were treated as outpatients and 7(38.9%)were hospitalized,but none was hospitalized for>7 *** accounted for 70%of the total cases at the 2021 Olympic Games,but only three(0.05%)were severe cases that required hospitalization for more than 1 *** contrast,in the Paralympic Games,other illnesses accounted for approximately half of the total *** study provides details on the extent of injuries and other illnesses that were transferred to outside facilities,which has not been documented in previous games.

A Case of Advanced Multiple Hepatocellular Carcinomas with Portal Vein Tumor Thrombosis Successfully Treated by Oral Tegafur/Uracil

A case of advanced multiple hepatocellular carcinomas (HCC) with portal vein tumor thrombosis successfully treated by oral tegafur/uracil is reported. A 69-year-old Japanese woman with advanced HCC with tumor thrombosis underwent transcatheter arterial infusion chemotherapy in April 2001. However, 1 year later, the patient experieced a recurrence with advanced multiple HCC with portal vein tumor thrombosis and ascites. Treatment with oral tegafur/uracil was started in May 2002 and resulted in the partial response of liver tumors and the complete improvement of ascites. She remained in good health for about 6 years. This case strongly suggests that oral tegafur/uracil is an effective treatment for some cases of advanced HCC with portal vein tumor thrombosis.

Retrospective Analysis of S-1 plus Bevacizumab as Maintenance Therapy after Induction of S-1 and Oxaliplatin (SOX) plus Bevacizumab as First-Line Chemotherapy in Patients with Metastatic Colorectal Cancer

Background: The SOFT study was a phase III trial designed to validate the non-inferiority of S-1 and oxaliplatin (SOX) plus bevacizumab to mFOLFOX6 plus bevacizumab in terms of PFS in patients with metastatic colorectal cancer (mCRC) who had not previously received chemotherapy. In this study, we retrospectively reviewed cases in which S-1 plus bevacizumab as maintenance therapy after induction of S-1 and Oxaliplatin (SOX) plus bevacizumab as first-line chemotherapy in patients with metastatic colorectal cancer was applied in order to evaluate its efficacy and safety in clinical practice. Material and method: Among the 40 patients with mCRC at the Fuchu Hospital who received SOX plus bevacizumab as a first line treatment between August 2013 and December 2018. The eligible patients had histologically confirmed mCRC. On day 1 of each 3-week period during the study, patients in the SOX plus bevacizumab received a 7.5 mg/kg intravenous infusion of bevacizumab, followed by an intravenous infusion of 130 mg/m2 oxaliplatin. S-1 (40 - 60 mg) was administered orally two times per day from after dinner on day 1 to after breakfast on day 15, followed by a 7-day rest. Results: The median PFS was 15.0 months and median OS was 36.0 months. The response rate (RR: complete pus partial response) was 85.0%, and the disease control rate (DCR: RR plus stable disease) was 92.5%. The most common adverse events with SOX plus bevacizumab were hypertension (50%), neurosensory toxicity (50%), anorexia (32.5%), fatigue (45%), pigmentation (39%), Neutrophil count decreased (30%), and platelet count decreased (40%). The most common grade 3/4 adverse events were neurosensory toxicity (5%) and fatigue (9%). Conclusion: This study revealed that the survival benefit of S-1 and oxaliplatin (SOX) plus bevacizumab in Japanese patients with mCRC was like that observed in previous clinical trials. A case of very large intrahepatic bile duct adenoma followed for 7 years

A 70-year-old man was referred to our hospital due to abnormal liver function. A tumor of 92 mm × 61 mm was detected on ultrasound screening of the left liver lobe. Although the tumor was suspected to be intrahepatic bile duct carcinoma, he had chronic heart disease and was unable to undergo surgery. Therefore, he was followed without further testing. No increase in tumor serum markers or tumor size was observed for the subsequent 7 years. We continued to suspect intrahepatic bile duct carcinoma, and we decided to perform a tumor biopsy. Tumor biopsy findings indicated intrahepatic bile duct adenoma(BDA), which is a rare benign epithelial liver tumor typically ranging from 1 mm to20 mm. We herein report a case of very large BDA followed for 7 years.

BACKGROUND Intra-articular glenohumeral joint injections are essential procedures for treating various shoulder disorders. Fluoroscopy-guided injections have been extensively used;however, they pose a risk of radiation exposure and are expensive and timeconsuming. Recently, it has been suggested that ultrasound-guided injections are accurate and cost effective *** To evaluate the accuracy of ultrasound-guided glenohumeral injections using a posterior approach that is confirmed using magnetic resonance arthrography(MRA).METHODS The study included 179 shoulders of patients with recurrent anterior instability(150 patients;103 and 76 right and left shoulders, respectively;160 males and 19 females;average age = 20.5 years;age range: 14-63 years) who underwent MRA for preoperative diagnosis. They were injected with 12 m L lidocaine(1%) using the ultrasound-guided posterior approach and then underwent magnetic resonance imaging. Two shoulder surgeons, except for the injector, evaluated the transverse relaxation(T2)-weighted images of axial planes and classified the intraarticular condition of injected contrast into three groups based on one of the three following scenarios: no leakage, injection into the glenohumeral joint without leakage;minor leakage, practical intra-articular injection with some leakage outside the posterior rotator cuffs;and major leakage, inaccurate injection with mass leakage without any contrast into the joint. The inter-rater reliability between two assessors was also evaluated by calculating Cohen’s kappa coefficient. The learning curve was assessed regarding the inaccurate injection rate by analyzing Spearman’s rank correlation *** Of the 179 injections, 163 shoulders(91.0%) had no leakage, 10 shoulders(5.6%) had minor leakage, and six shoulders(3.4%) had major leakage.
