Diagnostic utility of small-caliber and conventional endoscopes for gastric cancer and analysis of endoscopic false-negative gastric cancers

AIM: To analyze the diagnostic utility of a small-caliber endoscope(SC-E) and clinicopathological features of false-negative gastric cancers(FN-GCs). METHODS: A total of 21638 esophagogastroduodenoscopy(EGD) gastric cancer(GC) screening examinations were analyzed. Secondary endoscopic examinations(n = 3352) were excluded because most secondary examinations tended to be included in the conventional endoscopy(C-E) group. Detection rates of GCs and FN-GCs were compared between SC-E and C-E groups. FN-GC was defined as GC performed with EGD within the past 3 years without GC detection. Macroscopic types, histopathological characteristics and locations of FN-GCs were compared with firstly foundgastric cancers(FF-GCs) in detail. RESULTS: SC-E cases(n = 6657) and C-E cases(n = 11644), a total of 18301 cases, were analyzed. GCs were detected in 16(0.24%) SC-E cases and 40 C-E(0.34%) cases(P = 0.23) and there were 4 FN-GCs(0.06%) in SC-E and 13(0.11%) in C-E(P = 0.27), with no significant difference. FN-GCs/GCs ratio between SC-E and C-E groups was not significantly different(P = 0.75). The comparison of endoscopic macroscopic types of FN-GCs tended to be a less advanced type(P = 0.02). Histopathologically, 70.6% of FN-GCs were differentiated and 29.4% undifferentiated type. On the other hand, 43.0% of FF-GCs were differentiated and 53.8% undifferentiated type, so FN-GCs tended to be more differentiated type(P = 0.048). CONCLUSION: The diagnostic utility of SC-E for the detection of GCs and FN-GCs was not inferior to that of C-E. Careful observation for superficially depressed type lesions in the upper lesser curvature region is needed to decrease FN-GCs.

Patient blinding with blunt tip placebo acupuncture needles:comparison between 1 mm and 2 mm skin press

Objective： To investigate the influence of the depth of skin press in blunt tip placebo acupuncture needles on patient blinding and its relationship to needle ***： Forty healthy volunteers were enrolled as subjects for patient blinding. Four acupuncturists applied the following needles randomly at three points in each forearm： 0.18 mm and 0.25 mm diameter penetrating needles inserted to a depth of 5 mm, and 0.18 mm and 0.25 mm diameter skin-touch needles depressing the skin at the acupoint to a depth of 1 mm and 2 mm from the skin surface. The subjects reported their guesses at the nature of needles they received, and rated needle pain and de qi. A blinding index was calculated to define the success of blinding for ***： The blinding status of subjects for 1 mm press needles of 0.18 mm diameter was ＂random guess＂,but ＂unblinded＂ for 1 mm press needles of 0.25 mm diameter. For 2 mm press needles of both diameters,the blinding status was ＂opposite guess＂ and the blinding status for penetrating needles of both diameters was ＂unblinded.＂ The percentages of ＂felt pain＂ with 2 mm press needles of both diameters were similar to that with penetrating needles, but those were not similar for 1 mm press needles. The frequency of de qi occurrence with 2 mm press needles of 0.18 mm diameter was similar to that of penetrating needles of both ***： Placebo needles of 2 mm press made more subjects guess that the needles penetrated the skin than 1 mm press needles. The use of small diameter needles increased patient blinding.

Inhibitory effects of carbon dioxide insufflation on pneumoperitoneum and bowel distension after percutaneous endoscopic gastrostomy

This study addresses the hypothesis that stagnation of soybean yield on the farm can be improved by selection of a physiological trait favoring carbon assimilate partitioning to terminally placed pods versus genotypes having axillary pods at close plant spacing.13C was fed to source-sink units comprising a leaf,axillary/terminal pods,and petioles at upper and lower positions of the stem axis in two soybean cultivars,namely Shakujo and Enrei,at different densities of *** cultivars differ significantly in architecture,Shakujo bearing a few hundreds of pods in close succession to one another in a terminally placed raceme,in contrast to Enrei having axillary *** yield per plant was higher in Enrei than in Shakujo at low density,but Shakujo out-yielded Enrei at close *** density decreased yield per plant and altered the pattern of assimilate partitioning significantly within the plants for both *** high density more assimilates moved to the upper parts at the cost of the lower *** terminally placed pods of Shakujo were advantaged to receive assimilates under density *** benefit was accrued to pod filling of Enrei,however,under this condition.

Non-Contact Terahertz Inspection of Water Content in Concrete of Infrastructure Buildings

Concrete is the most widely used for construction materials in the world. Water content of concrete is an important parameter in terms durability of concrete structures. Terahertz (THz) waves, for which concrete is a porous and absorbable material, have been studied in order to establish a new non-contact inspection technology for maintenance of concrete structures. In this study, THz transmittance and reflectance of concrete in drying process were measured with a 60 GHz GUNN diode and absorption coefficient is analyzed for concrete with various water contents. It is shown that quantitative detection below 10% is possible for the water content at surface area of concrete.

The ecological process-based method provides a detailed assessment of belowground compartment as one of the major compartment of carbon *** method is determined by the balance between net primary production（NPP）of vegetation and heterotrophic respiration of soil. A Method to Evaluate Abrasion of Shoe-Sole Using 3D Scanning Technique

A study was carried out to evaluate abrasion of shoe-sole for subjects with different running gait. A 3 dimensional （3D） scanning approach together with a commercial software, CloudCompare Mesh Cloud Comparison was utilized for this study. In CloudCompare, a grid system and colored scale was applied to identify the region and extend of abrasion of the shoe-sole. This study clearly showed the extent of abrasion on regions of shoe-sole identified from the colored scale. This paper was done with kind support from Asics Institute for Sports Science, Kobe, Japan and Institute for Sport Research （ISR-NTU）.

Napier grass （Pennisetum purpureum Schumach）, a C4 tropical plant, has high dry matter productivity and sustainability without the need for renovating every year due to good overwintering ability and the absence of serious pathogens or insects injury. This grass is suitable for multipurpose use, providing forage, bioethanol feedstock and phytoremediation of cadmium （Cd） contaminated soil in Kyushu, Japan. When two cultivars of Napier grass （Wruk wona, WK and Merkeron, ME） were grown in Cd-contaminated soils, dry matter yield and the concentration and uptake of herbage Cd were higher in WK than in ME, and soil Cd concentration was reduced by 14% in a single season of cultivation. A dwarf variety of late-heading type （DL）, Napier grass had superior sustainability for more than 5 years in Miyazaki, Japan, and has gradually increased in cultivation areas for both cut-and-carry and grazing herbage production in cooperation with regional communities. Through the extension in several Kyushu areas, DL Napier grass required temperatures above -6.2 ~C for sustainability in winter, based on its threshold response to the lethal minimum temperature. DL Napier grass pasture can be utilized by grazing beef cattle even in early spring with temperate Italian ryegrass inter-sown into its inter-row spaces. Organic digested animal manure or sun-dried leguminous leaves as green manure can replace chemical fertilizers to reduce energy consumption and allow comparable productivity.
