Short-and long-term results of endoscopic ultrasoundguided transmural drainage for pancreatic pseudocysts and walled-off necrosis

AIM To evaluate the short-and long-term results of endoscopic ultrasound-guided transmural drainage(EUS-GTD) for pancreatic fluid collection(PFC) and identify the predictive factors of treatment outcome for walled-off necrosis(WON) managed by EUS-GTD *** We investigated 103 consecutive patients with PFC who underwent EUS-GTD between September 1999 and August 2015. Patients were divided into four groups as follows: WON(n = 40), pancreatic pseudocyst(PPC; n = 11), chronic pseudocyst(n = 33), and others(n = 19). We evaluated the short-and long-term outcomes of the treatment. In cases of WON, multiple logistic regression analyses were performed to identify the predictor variables associated with the treatment success. In addition, PFC recurrence was examined in patients followed up for more than 6 mo and internal stent removal after successful EUS-GTD was *** In this study, the total technical success rate was 96.1%. The treatment success rate of WON, PPC, chronic pseudocyst, and others was 57.5%, 90.9%, 91.0%, and 89.5%, respectively. Contrast-enhanced computed tomography using the multivariate logistic regression analysis revealed that the treatment success rate of WON was significantly lower in patients with more than 50% pancreatic parenchymal necrosis(OR = 17.0; 95%CI: 1.9-150.7; P = 0.011) and in patients with more than 150 mm of PFC(OR = 27.9; 95%CI: 3.4-227.7; P = 0.002).The recurrence of PFC in the long term was 13.3%(median observation time, 38.8 mo). Mean amylase level in the cavity was significantly higher in the recurrence group than in the no recurrence group(P = 0.02).CONCLUSION The reduction of WON by EUS-GTD alone was associated with the proportion of necrotic tissue and extent of the cavity. The amylase level in the cavity may be a predictive factor for recurrence of PFC.

Efficacy of multiple biliary stenting for refractory benign biliary strictures due to chronic calcifying pancreatitis

AIMTo investigate endoscopic therapy efficacy for refractory benign biliary strictures (BBS) with multiple biliary stenting and clarify predictors. METHODSTen consecutive patients with stones in the pancreatic head and BBS due to chronic pancreatitis who underwent endoscopic therapy were evaluated. Endoscopic insertion of a single stent failed in all patients. We used plastic stents (7F, 8.5F, and 10F) and increased stents at intervals of 2 or 3 mo. Stents were removed approximately 1 year after initial stenting. BBS and common bile duct (CBD) diameter were evaluated using cholangiography. Patients were followed for ≥ 6 mo after therapy, interviewed for cholestasis symptoms, and underwent liver function testing every visit. Patients with complete and incomplete stricture dilations were compared. RESULTSEndoscopic therapy was completed in 8 (80%) patients, whereas 2 (20%) patients could not continue therapy because of severe acute cholangitis and abdominal abscess, respectively. The mean number of stents was 4.1 ± 1.2. In two (20%) patients, BBS did not improve; thus, a biliary stent was inserted. BBS improved in six (60%) patients. CBD diameter improved more significantly in the complete group than in the incomplete group (6.1 ± 1.8 mm vs 13.7 ± 2.2 mm, respectively, P = 0.010). Stricture length was significantly associated with complete stricture dilation (complete group; 20.5 ± 3.0 mm, incomplete group; 29.0 ± 5.1 mm, P = 0.011). Acute cholangitis did not recur during the mean follow-up period of 20.6 ± 7.3 mo. CONCLUSIONSequential endoscopic insertion of multiple stents is effective for refractory BBS caused by chronic calcifying pancreatitis. BBS length calculation can improve patient selection procedure for therapy.

Development of electric cart for improving walking ability — application of control theory to assistive technology

This paper explains the development of an electric cart that helps the elderly maintain or improve their physical strength. Unlike commercially available ones, it has a pedal unit that provides some exercise for a user in training his lower limbs. An impedance model describes the feeling of pushing the pedals. The largest pedal load is determined based on a pedaling experiment. An H∞ controller is designed for each of the largest pedal load and virtually no load. A control law, which is based on the concept of dynamic parallel distributed compensation, is designed using the rating of perceived exertion of a driver as a criterion to choose a pedal load between the largest and almost zero. Five university students and twelve elderly people participated experiments to verify the system design and the validity of the system.

3D attitude measurement by means of spread spectrum modulated ultrasonic wave

Local position and attitude measurement methods are necessary for the navigation of autonomous robots and the other applications. In this paper, a 3D attitude measurement method is proposed. This method uses the spread spectrum modulated ultrasonic wave for the measurement of the pseudo-distance of each pathway between transmitter and receiver elements that are arranged in an array. The relative attitude between transmitter and receiver unit could be calculated from the relationship between these pseudo-distances. The use of spread spectrum modulation enables the high accuracy, noise tolerance, and coexistence of multiple transmitter devices by code division multiple access (CDMA) between transmitter and receiver elements. A pair of transmitter and receiver prototype for proposed method was manufactured, and the fundamental performance was examined in experiments. As a result, accuracy of σ=0.78 mm in the pseudo-distance measurement of each pathway between transmitter and receiver elements was confirmed, and about 0.01 rad resolution in the attitude measurement was also confirmed.

Selection of Suitable Maximum-heart-rate Formulas for Use with Karvonen Formula to Calculate Exercise Intensity

The Karvonen formula, which is widely used to estimate exercise intensity, contains maximum heart rate, H Rmax, as a variable. This study employed pedaling experiments to assess which of the proposed formulas for calculating H Rmaxwas the most suitable for use with the Karvonen formula. First, two kinds of experiments involving an ergometer were performed: an all-in-one-day experiment that tested eight pedaling loads in one day, and a one-load-per-day experiment that tested one load per day for eight days.A comparison of the data on 7 subjects showed that the all-in-one-day type of experiment was better for assessing H Rmaxformulas,at least for the load levels tested in our experiments. A statistical analysis of the experimental data on 47 subjects showed two of the H Rmaxformulas to be suitable for use in the Karvonen formula to estimate exercise intensity for males in their 20 s. In addition, the physical characteristics of a person having the greatest impact on exercise intensity were determined.

Fundamental study on short range measurement using RF time of flight based on Vernier effect

Recognizing the position of moving objects, facilities or important equipments has been regarded as a hot issue in the future environment which needs ubiquitous characteristics. Furthermore, it is indispensably needed to acquire location data in order to realize an infrastructure of sensor network system. Namely, distance data are the preliminary information to complete localization of a moving object in local area. Even though there have been several researches on this topic, there are however still much amount of problems in getting precisely exact distance data. Out of them, the research on using TOF (Time-of-flight) is very few as well as could be thought of as a cutting-edge technique. Moreover, it is very difficult to get minutely RF signal's TOF for getting short distance data; that is, very complicated signal processing skills are requested for realizing it. In this paper, we present a new methodology of RF short ranging technique based on vernier effect through using of heterogeneous operating clocks.

2-D localization system utilizing M-sequence encoded laser beam scanning

In this paper, as the active position measurement system, a novel method for a two-dimensional localization system using M-sequence signal is proposed. The feature of this system is to realize a position measurement only by scanning the encoded laser beams from fixing points to a measurement field, and by observing it. First, both the system configuration and encoding method were practically considered, and an order and an initial value of M-sequence signal were selected for encoding. This method is based on discrete angle measurement therefore the resolution has its own limitation. To overcome this limitation, analogue phase shift detecting method was introduced. System design was performed and applied to position measurement experiments. The experimental results show that the measurement error of the analogue method is 1/3 of that of the discrete method.

Acoustic CT system for temperature distribution measurement

In this paper, a measurement method for cross-sectional temperature distribution is addressed. A novel method based on an acoustic CT technique is proposed. Specifically, the temperature distributions are estimated using the time of flight data of several ultrasonic propagation paths. The times of the flight data contain both temperature and wind effect, and the method to select only temperature component is introduced. A filtered back projection method is applied to reconstruct the temperature distributions from the time of flight data. An experimental system was designed and fabricated to realize simultaneous temperature and wind velocity distribution measurements. Through this system, the effectiveness of the proposed measurement method is confirmed.

This paper proposes a novel method for analyzing a textile fabric structure to extract positional information regarding each yarn using three-dimensional X-ray computed tomography(3D CT) *** relationship among the yarns can be reconstructed using the extracted yarn positional *** this paper,a sequence of points on the center line of each yarn of the sample is defined as the yarn positional information,since the sequence can be regarded as the representative position of the *** sequence is extracted by tracing the *** yarn is traced by estimating the yarn center and direction and correlating the yarn part of the 3D CT image with a 3D yarn model,which is moved along the estimated yarn *** trajectory of the center of the yarn model corresponds to the positional information of the *** application of the proposed method is shown by experimentally applying the proposed method to a 3D CT image of a double-layered woven ***,the experimental results for a plain knitted fabric show that this method can be applied to even knitted fabrics.
