Perioperative intensive insulin therapy using artificial endocrine pancreas in patients undergoing pancreatectomy

Perioperative glycemic control is important for reducing postoperative infectious complications. However, clinical trials have shown that efforts to maintain normoglycemia in intensive care unit patients result in deviation of glucose levels from the optimal range, and frequent attacks of hypoglycemia. Tight glycemic control is even more challenging in those undergoing pancreatic resection. Removal of lesions and surrounding normal pancreatic tissue often cause hormone deficiencies that lead to the destruction of glucose homeostasis, which is termed pancreatogenic diabetes. Pancreatogenic diabetes is characterized by the occurrence of hyperglycemia and iatrogenic severe hypoglycemia, which adversely effects patient recovery. Postoperatively, a variety of factors including surgical stress, inflammatory cytokines, sympathomimetic drug therapy, and aggressive nutritional support can also affect glycemic control. This review discusses the endocrine aspects of pancreatic resection and highlights postoperative glycemic control using a closed-loop system or artificial pancreas. In previous experiments, we have demonstrated the reliability of the artificial pancreas in dogs with total pancreatectomy, and its postoperative clinical use has been shown to be effectiveand safe, without the occurrence of hypoglycemic episodes, even in patients after total pancreatectomy. Considering the increasing requirement for tight perioperative glycemic control and the recognized risk of hypoglycemia, we propose the use of an artificial endocrine pancreas that is able to monitor continuously blood glucose concentrations with proven accuracy, and administer automatically substances to return blood glucose concentration to the optimal narrow range.

Heat generation properties in AC magnetic field for composite powder material of the Y3Fe5O12-nSiC system prepared by reverse coprecipitation method

Composite powder material of the Y3Fe5O12–nSiC system was synthesized by a reverse coprecipitation method to study its heat generation property in an AC magnetic field. For Y3Fe5O12 (n = 0), the maximum heat generation ability of 0.45 W·g−1 in an AC magnetic field (370 kHz, 1.77 kA·m−1) was obtained for the sample calcined at 1100 °C. The SiC addition helped to suppress the particle growth for Y3Fe5O12 at the calcination temperature. The heat generation ability was improved by the addition of the SiC powder, and the maximum value of 0.93 W·g−1 was obtained for the n = 0.3 sample calcined at 1250 °C. The heat generation ability and the hysteresis loss value were proportional to the cube of the magnetic field (H 3). The heat generation ability (W·g−1) of the Y3Fe5O12–0.3SiC sample calcined at 1250 °C could be expressed by the equation 4.5×10−4 · f · H 3 using the frequency f (kHz) and the magnetic field H (kA·m−1).

Postoperative hyperglycemia is common in critically ill patients, even in those without a prior history of diabetes mellitus. It is well known that hyperglycemia induced by surgical stress often results in dysregulation of liver metabolism and immune function, impairing postoperative recovery. Current evidence suggests that maintaining normoglycemia postoperatively improves surgical outcome and reduces the mortality and morbidity of critically ill patients. On the basis of these observations, several large randomized controlled studies were designed to evaluate the benefit of postoperative tight glycemic control with intensive insulin therapy. However, intensive insulin therapy carries the risk of hypoglycemia, which is linked to serious neurological events. Recently, we demonstrated that perioperative tight glycemic control in surgical patients could be achieved safely using a closed-loop glycemic control system and that this decreased boththe incidence ofinfection at the site of the surgical incision, without the appearance of hypoglycemia, and actual hospital costs. Here, we review the benefits and requirements of perioperative intensive insulin therapy using a closed-loop artificial endocrine pancreas system in hepatectomized patients. This novel intensive insulin therapy is safe and effectively improves surgical outcome after hepatic resection.

通过真球气泡法测定了不同价数和浓度的无机盐体系下十二烷基硫酸钠(SDS)表面活性剂溶液的表面张力和表面扩展黏度;通过Ross-Miles法测定了相应同一溶液的发泡力和泡沫稳定性;结果表明:SDS溶液的表面张力值随着各种无机盐浓度的增加而减小至不变,此时的无机盐浓度作为相对浓度100%来考察时,发现临界胶束浓度(CMC)值与添加盐的种类无关,只随无机盐的相对浓度的增加而减少;即CMC值随着无机盐的相对浓度的增加从不含无机盐时的8.3 mmol/L均收敛至最小值3.0 mmol/L;表面张力值和对应的发泡力值呈负线性关系,由此推断表面张力值可以评价发泡力;表面扩展黏度值和对应的泡沫稳定性值呈正线性关系,由此推断表面扩展黏度可以评价泡沫稳定性;无机盐种类对SDS表面活性剂发泡力和泡沫稳定性的影响力随阳离子价数(Na^(+)、Cu^(2+)、Fe^(3+))增加而增加。

采用真球气泡法测定了不同价态(Na^(+)、Cu^(2+)、Fe^(3+))和浓度的无机盐体系下十二烷基硫酸钠(SDS)表面活性剂溶液的表面张力、表面扩展黏度和临界胶束浓度(cmc),加入无机盐之后,SDS的cmc值由8 mmol/L降至3 mmol/L,达到最低表面张力的SDS浓度值(C_(t))和最大表面扩展黏度时的SDS浓度值(C_(v))均小于未添加时对应的SDS浓度,且Ct=Cv=cmc=3 mmol/L。在cmc值时,溶液表面张力为37.29 mN/m,加入NaCl后溶液表面张力为27.53 mN/m;加入CuCl_(2)后溶液表面张力为23.44 mN/m;加入Fe(NO_(3))_(3)后溶液表面张力为21.35 mN/m。在cmc值时,溶液的表面扩展黏度为1.70 mN·s/m,加入NaCl后溶液扩展黏度为2.21 mN·s/m;加入CuCl_(2)后溶液扩展黏度为2.58 mN·s/m;加入Fe(NO_(3))_(3)后溶液扩展黏度为2.75 mN·s/m。阳离子的加入使得SDS降低水的表面张力值的效果增强,文章定义M(表面活性剂溶液表面张力值变化的指标)去判断无机盐阳离子对于SDS溶液的作用效果,并分析验证SDS分子呈长条形结构,推定无机盐阳离子提高SDS降低水表面张力值作用效果的机理为:负电荷在SDS分子的纵截面的顶部,阳离子通过正电荷吸引SDS的负电荷端,使得纵截面横躺在溶液表面,增加了SDS分子在表面的覆盖面积,从而增大了SDS降低水的表面张力值的作用效果。

In this work,we quantitatively studied the intertube coupling of different(n,m)-sorted semiconducting single-wall carbon nanotubes(SWCNTs)on their photoluminescence(PL)efficiencies by precisely tuning the ratio of(9,4)and(6,5)SWCNTs in the mixture.A significant decrease in the PL intensity of(9,4)SWCNTs was observed after mixing with(6,5)species when fixing the(9,4)concentration,which was confirmed to be caused by the absorption of incident photons and reabsorption of the emitted photons by the added(6,5)*** contrast,a similar decrease in the PL intensity of(6,5)SWCNTs was also observed after mixing with the larger-diameter(9,4)*** from that of(9,4)SWCNTs,the PL decrease of(6,5)SWCNTs was found to originate not only from photon absorption and reabsorption by the(9,4)species but also from one-way exciton energy transfer(EET)from the(6,5)SWCNTs to the larger-diameter(9,4)*** the experimental results and numerical simulations further demonstrated that increasing the concentration of mixed(9,4)SWCNTs would enhance the effects of photon absorption and reabsorption and EET on the PL intensity of(6,5)SWCNTs quantified by the decrease ratio of the(6,5)PL ***,the influence of EET was found to be always weaker than that of photon absorption and *** proposed that the observed EET between isolated SWCNTs in a surfactant solution is derived from their proximity due to Brownian motion.

通过真球气泡法测定了相同总浓度下、不同复配比的阴离子型表面活性剂十二烷基硫酸钠(SDS)表面张力,探究加入阳离子型表面活性剂十二烷基三甲基溴化铵(DTAB)后复配体系的相互作用。结果表明,无论何种复配体系,在任意总浓度下,其表面张力的最低值都处于相同的复配比例,故认为有一个简易得到复配体系最低表面张力的方法,即在总浓度一定时,找到表面张力值最低时的复配比例,并在此比例下求得cmc值;复配体系泡沫性能优劣的排序与其表面活性大小的排序一致,故泡沫最佳性能的获取方法与复配体系最小表面张力值获取方法一致;并通过建立模型,定义且计算了表面张力系数(K)值。

Postoperative pancreatic fistula (POPF) is one of the most severe complications after pancreatic surgeries. POPF develops as a consequence of pancreatic juice leakage from a surgically exfoliated surface and/or anastomotic stump, which sometimes cause intraperitoneal abscesses and subsequent lethal hemorrhage. In recent years, various surgical and perioperative attempts have been examined to reduce the incidence of POPF. We reviewed several well-designed studies addressing POPF-related factors, such as reconstruction methods, anastomotic techniques, stent usage, prophylactic intra-abdominal drainage, and somatostatin analogs, after pancreaticoduodenectomy and distal pancreatectomy, and we assessed the current status of POPF. In addition, we also discussed the current status of POPF in minimally invasive surgeries, laparoscopic surgeries, and robotic surgeries.

Management of extensive bone and soft tissue defects, which occur after severe trauma of lower extremities and always lead to an unacceptable amputation in some cases, continues to challenge reconstructive surgeons. When performing lower extremity amputation, preservation of the knee joint has been put into a higher priority. The benefit of belowknee amputation over aboveknee ones concentrates on a more normal gait with less energy expenditure during ambulation when a functioning knee joint is present. 1-3
