Clarifying the Strengths-Oriented Attitude among Nurses in Psychiatric Hospitals in Japan

Japanese psychiatric mental health systems are being steadily improved. Psychiatric nurses’ attitude towards strengths of persons with mental disability is critical in order to promote de-institutionalization of patients. However, nurses’ attitudes, particularly their Strengths-Oriented Attitude (SOA) has not been well studied and explained. The purpose of this study was to clarify the SOA of nurses working in psychiatric hospitals in Japan. There were 1148 Psychiatric Registered Nurses (PRN) and Psychiatric Practical Nurses (PPN) employed at 17 psychiatric hospitals in Japan who consented to participate in the study. The self-administered questionnaire was mailed and returned between October 2013 and January 2014. The subjects’ SOA was evaluated using the Strengths-Oriented Attitude Inventory (SOAI) developed by the authors based on the work by Rapp and Goscha. The actual SOA and its relationship to individual characteristics were also analyzed to clarify the SOAI. Factor analysis revealed four components of the SOAI: Factor 1 = “Nursing practice based on the assessment which focused on social resources”;Factor 2 = “Nursing practice based on the holistic assessment of person with mental disability”;Factor 3 = “Support in community which respected the person’s way of living”;and Factor 4 = “Emphasize of the strengths of individual with mental disability.” Results indicated a high orientation toward Factors 2 and 4, but a low orientation toward Factors 1 and 3. There were no significant differences between age, years of psychiatric nursing experience, academic background, and subjects’ SOA. However, differences were observed in gender, current workplace, work position, type of license, and work experience. The subjects showed low orientation toward social resources and respected the patient’s way of life. Nurses working in psychiatric mental health institutions should receive continuing education courses designed to increase their orientation toward Factors 1 and 3.

An Autonomous Incremental Learning Algorithm for Radial Basis Function Networks

In this paper, an incremental learning model called Resource Allocating Network with Long-Term Memory (RAN-LTM) is extended such that the learning is conducted with some autonomy for the following functions: 1) data collection for initial learning, 2) data normalization, 3) addition of radial basis functions (RBFs), and 4) determination of RBF cen-ters and widths. The proposed learning algorithm called Autonomous Learning algorithm for Resource Allocating Network (AL-RAN) is divided into the two learning phases: initial learning phase and incremental learning phase. And the former is further divided into the autonomous data collection and the initial network learning. In the initial learning phase, training data are first collected until the class separability is converged or has a significant dif-ference between normalized and unnormalized data. Then, an initial structure of AL-RAN is autonomously determined by selecting a moderate number of RBF centers from the collected data and by defining as large RBF widths as possible within a proper range. After the initial learning, the incremental learning of AL-RAN is conducted in a sequential way whenever a new training data is given. In the experiments, we evaluate AL-RAN using five benchmark data sets. From the experimental results, we confirm that the above autonomous functions work well and the efficiency in terms of network structure and learning time is improved without sacrificing the recognition accuracy as compared with the previous version of AL-RAN.

Sendai Water Pipeline Response to the 2011 Tohoku Earthquake

Damage caused by the 2011 Tohoku Earthquake （Mw 9.0） to transmission and distribution pipelines in Sendai City is summarized. The locations of the pipeline repairs are discussed relative to earthquake intensity, geomorphologic conditions and landform change in the developed areas of hilly land. Repair rate （repairs/km） is summarized according to pipe material and presence or absence of artificial landform change. The following findings were obtained：（1） More than 80% of the repairs took place in pipelines installed in higher lands such as hill and terrace areas consisting of hard soils; （2） Nearly all the pipe repairlocations in hill areas are where landform change was made through land development tor residential purposes over me past several decades; （3） The aforementioned repair rate was more than 3.6 times than that of other lowland areas where no landform change occurred. The heaviest concentrations of pipe repairs of vinyl chloride pipes and ductile iron pipes were observed within the boundary area between cutting and filling, having a thickness between -2.5 m and 2.5 m. Approximately 78% of the all pipe repairs occurred outside of areas where severe ground failures took place. A mechanism of pipe damage in the areas free of severe ground failure was discussed.

High-Speed Rail Station Interconnectivity and Ridership

The objective of this study was to quantify multimodal connectivity of HSR stations and its impact on ridership in four countries: France, Spain, Japan and China. In this study, multimodal connectivity is measured by the number of different modes of transportation connected to HSR stations, the number of installed arrival and departure facilities for each mode, the transfer time from connecting modes to boarding platforms at HSR stations, and the arrival time intervals of public transportation modes. Data were collected from HSR systems of these four countries. The relationship between ridership and the characteristics of multimodal connectivity was identified using regression models developed in this study. All the connectivity variables considered in this study influence ridership in these four countries in different ways. On the whole, bus, subway, and regional railroad services influence ridership significantly. For instance, the more bus services connected to the station, the higher the ridership. This trend is apparent in three of the four countries, France being the exception. Also, subway, light rail, and traditional rail are modes of high-capacity transportation. Their connection to HSR stations always implies high ridership for high-speed rail. The number of facilities also shows significant impacts on HSR ridership. For instance, the more bus and subway stops, and the more bicycle parking and taxi stands, the higher the ridership. Transfer time also has a significant influence.

Prosthetic Ring Endocarditis with Shrunken Mitral Annulus Late After Undersized Mitral Annuloplasty

We report a case of supra-annular mitral valve replacement for a patient with prosthetic ring endocarditis that developed late after undergoing undersized mitral annuloplasty (UMAP). A 58-year-old man had undergone left ventriculoplasty and UMAP with a 26-mm semi-rigid full ring for dilated cardiomyopathy and functional mitral regurgitation. The mitral valve had progressively stenosed and he developed prosthetic ring endocarditis 5 years after the operation. In the surgery, there were brownish fresh vegetations around the anterior mitral leaflet and the prosthetic ring. The mitral annulus was very small, even after removal of the infected ring prosthesis and debridement of the infected tissues. We implanted a 22-mm mechanical prosthesis in the supra-annular position using everting mattress sutures to prevent the valve leaflets from getting stuck by the thickened subannular tissue. Herein, we discuss the surgical procedures for a shrunken mitral annulus.

Single-Proton Pickup Reaction of the Halo Nucleus ^6He on a ^9Be Target at 25 MeV/nucleon

The inclusive differential cross sections of the ^7 Li nucleus in a reaction induced by ^6He on a ^9Be target are measured at an incident energy of 25 MeV/nucleon. Finite-range distorted-wave Born approximation calculations suggest that these ^7 Li particles are formed in a direct single-proton pickup reaction ^9Be（^6He,^7 Li）^8Li. The experimental data can be well reproduced by taking into account of the contributions of both the ground states and the first excited states of ^7Li and ^8Li.

The Direction of Research on Active Aging and Healthy Life Expectancy in Japan

In Japan, 18.1% of the population known as baby-boomers will become the late-stage elderly in 2025, thereby needing a foundation to support this change. The Japanese Ministry of Health, Labour and Welfare is promoting the development of a regional comprehensive system allowing the elderly to continue living in their familiar surroundings. However, a care shortage is inevitable unless elders are able to age in good health, regardless of the system’s level of enhancement. This study aims to review the literature on active aging, clarify trends in clinical operations undertakings and research in Japan, and consider relevant research issues. After combining the search results of “active aging” and “healthy life expectancy,” we used a text mining technique to analyze the abstracts of 120 original articles and 213 reviews, commentaries, and features. Eight categories were extracted from the original articles: health statistics, gender, age, etc. From the reviews, commentaries, and features, 16 categories were extracted: orientation, disease, and living, etc. Cerebrovascular disease and osteoporosis were the most common diseases covered in the original articles;there has been a substantial amount of research on “active aging” and “healthy life expectancy” because they can easily lead to being bedridden and to a decrease in QOL. In the reviews, commentaries, and features, lifestyle-related diseases and menopause rather than cerebrovascular disease and osteoporosis, were extracted. The categorical differences found in the original articles may be due to the possibility that Japanese researchers are publishing their research abroad rather than in Japan or they submit research on topics that are guaranteed to be published at home or abroad. Little research has been conducted using the terms, “active aging” and “healthy life expectancy,” evidenced by the small number of studies generated. Contrast Enhancement of Posterior Mediastinal Ganglioneuromas—Correlation between the Level of Enhancement and Histopathological Features

Purpose: Relationship between CT or MR images and histological findings, especially vascularity, has not been adequately evaluated. The purpose of this study was to investigate correlation between contrast enhancement on CT and MRI and histological findings in posterior mediastinal ganglioneuromas. Materials and Methods: Contrast-enhanced (CE) CT (n = 11) and CE MRI (n = 5) of 12 patients with ganglioneuroma were reviewed. The attenuation, signal intensity, and dynamic enhancement pattern of the tumors were evaluated. The vascularity was histologically evaluated by the numbers of vessels. Results: Enhancement on CE-CT was none, mild, moderate, and high enhancement in 5 (45%), 2 (18%), 3 (27%), and 1 (9%) of the 11 lesions, respectively. Dynamic MRI showed mild, moderate and high enhancement in 3 (60%), 1 (20%) and 1 (20%) cases, respectively. The level of contrast enhancement correlated well only with the number of capillary vessels (r = 0.79, P = 0.0037). Conclusion: The posterior mediastinal ganglioneuromas sometimes show insufficient enhancement particularly on CE-CT. The level of enhancement might correlate with the amount of capillary vessels.
