Background: Medical staff provide care to spouses of terminal cancer patients through trial and error by meeting their various support needs and spousal factors regarding their anticipatory grief. Studies on the association between spousal characteristics and anticipatory grief have been inconclusive;additionally, there has been insufficient research on support needs for anticipatory grief of spouses. This study aimed to explore the spousal characteristics and social support needs predicting anticipatory grief in spouses of patients with cancer at the end of life. Methods: This was a cross-sectional study. Eligible spouses (n = 102) completed a self-report questionnaire in two hospitals with palliative care units in Japan. The questionnaire included demographic information, a tool assessing social support needs of spouses, and the Anticipatory Grief Scale for Family Caregivers. Results: Simple regression analyses indicated that patient age, chemotherapy, no treatment, ECOG PS3, children aged under 20 years, total score of “social support needs regarding the disease and treatment of the patients” and subscale scores (“medical condition and cure,” “daily life and social support,” and “intimacy and employment”), and total score of “social support needs of the spouses” and subscale scores (“family psychological issues and social support” and “intimacy, employment, and society”) were significant variables (all p p Conclusions: Patients having no experience of “chemotherapy” and higher “social support needs of the spouses” in Model 1, and greater spousal needs of “family psychological issues and social support” in Model 2 were significant predictors of severe anticipatory grief. Medical staff should pay attention to these risk factors that predict anticipatory grief among spouses.

Analyses of Various MHD Phenomena in EPM Application to Steelmaking Processes by Finite Volume Method

Applied technologies of magnetohydrodynamics(MHD)have been widely used in steelmaking based on their characteristics of clean and remote control of liquid ***,phenomena are quite complex and coupled among hydrodynamics,electromagnetism and thermodynamics,so numerical simulations play important roles in various applications of MHD for equipments design and control parameters *** the simulation,it is preferable to use one package calculation rather than coupled analyses such as the one between finite element method(FEM)and finite difference method(FDM)with the data conversion caused by different *** the resolution,a three-dimensional finite volume approach for MHD is established based on scalar and vector potential and magnetic induction equation treated as transport *** this paper,results of the analyses of various MHD phenomena in EPM application to steelmaking process are described with some examples such as plasma heating and electromagnetic flow control tools.

Effect of Helicobacter pylori cdrA on interleukin-8 secretions and nuclear factor kappa B activation

AIM： To investigate genetic diversity of Helicobacter pylori （H. pylorl） cell division-related gene A （cdrA） and its effect on the host ***： Inactivation of H. py/ori cdrA, which is involved in ceil division and morphological elonga- tion, has a role in chronic persistent infections. Ge- netic property of H. pylori cdrA was evaluated using polymerase chain reaction and sequencing in 128 （77 American and 51 Japanese） clinical isolates obtained from 48 and 51 patients, respectively. Enzyme-linked immunosorbent assay was performed to measure in- terleukin-8 （IL-8） secretion with gastric biopsy speci- mens obtained from American patients colonized with cdrA-positive or -negative strains and AGS cells co- cultured with wild-type HPK5 （cdrA-positive） or its de- rivative HPKT510 （cdrA-disruptant）. Furthermore, the cytotoxin-associated gene A （cagA） status （transloca- tion and phosphorylation） and kinetics of transcription factors [nuclear factor-kappa B （NF-~：B） and inhibition kappa B] were investigated in AGS cells co-cultured with HPK5, HPKT510 and its derivative HPKSCA （cagA- disruptant） by western blotting analysis with immuno- precipitation. RESULTS： Genetic diversity of the H. pylori cdrA gene demonstrated that the cdrA status segregated into two categories including four allele types, cdrA-positive （al- lele types, I and 11 ） and cdrA-negative （allele types; 111 and IV） categories, respectively. Almost all Japanese isolates were cdrA-positive （ 1 ： 7.8% and 11 ： 90.2%）, whereas 16.9% of American isolates were cdrA-positive （11） and 83.1% were cdrA-negative （nl： 37.7% and IV： 45.5%）, indicating extended diversity of cdrA in individual American isolates. Comparison of each isolate from different regions （antrum and corpus） in the stomach of 29 Americans revealed that cdrA status was identical in both isolates from different regions in 17 cases. Appropriate Heart Rate in a Patient with Repaired Tetralogy of Fallot

Appropriate heart rate in a failing pulmonary ventricle remains unknown, particularly in congenital heart diseasewith unique hemodynamics. A 71-year-old male with repaired tetralogy of Fallot and a pacemaker for a sinusnode dysfunction suffered from heart failure symptoms with preserved left ventricular function. Simply changingthe pacemaker’s lower rate from 60 to 75 bpm, New York Heart Association classification improved from III to II,and hemodynamic parameters drastically improved. We regarded this case as informative. Appropriate heart ratecould be higher in congenital patients with failing right and non-failing left ventricles than in adults with malfunctioningLV.

Correlation Study Between Suspended Particulate Matter and DOAS Data

Continuous data of aerosol optical thickness monitored using differential optical absorption spectroscopy （DOAS） are correlated with the concentration of ground-measured suspended particulate matter （SPM）. A high correlation is found between the DOAS and the ground SPM data, making it possible to calculate the mass extinction efficiency of the aerosols in the atmosphere. It is found that the value of mean mass extinction efficiency （MEE） varies over a range of 2.6-13.7 m^2 g^-1, with smaller and larger values occurring for size distributions dominated by coarse and fine particles, respectively.

Late Quaternary Geomorphology of the Tonami Plain and Activity of the Tonami-Heiya Fault Zone, Toyama Prefecture, Central Japan

The Holocene alluvial fans and flood plains formed by the Sho and Oyabe Rivers spread out in the Tonami plain from the central through the northern parts in the Toyama Prefecture, central Japan. Along the foot of the surrounding mountains and hills, higher, middle and lower terraces of late Pleistocene-Holocene in age are distributed. These terraces have been displaced, by the reverse dip-slip activities of Tonami-heiya fault zone in a sense of upheaval in the mountains side, even during the Holocene time. We examined stratigraphic cross section utilizing borehole data and geomorphologically analyzed 5 m-DEM data in order to elucidate the fault trace of the Isurugi fault which Tonami-heiya fault zone. As the results, the northern segment of Isurugi fault seems to run along the northeastern foot of Hodatsu Hills and extends underground through the lower-most Oyabe River into the Toyama Bay. Consequently, its total length reaches about 30 km. In the southern segment, a continuous fault scarplet was recognized to cut across the lower dissected fans. The slip-rate of Isurugi fault is estimated to be 0.31 - 0.64 m/kyr. In the Hokuriku region, reverse faulting and related folding with strike in a NE-SW direction have occurred during the late Quaternary. The hinge line of block movement due to the activities of the Tonami-heiya fault zone is revealed to have shifted from the mountain side into the plain side within the Holocene time. In conclusion, the Quaternary folding and faulting associated with the crustal warping at a wavelength of about 20 km is currently in progress, causing both the subsidence of Tonami plain and the upheaval of surrounding mountains and hills.

Autopsy study of anatomical features of the posterior gastric artery for surgical contribution

AIM： To investigate features of the posterior gastric arten/（PGA） with respect to incidence, location and size by using autopsy subjects. METHODS： Autopsies were performed on 72 cadavers of adults with no history of abdominal operations. The localization of the PGA, the distance between the root of the splenic artery and the origin of the PGA, and the external diameter of the PGA were examined. RESULTS： The PGA was recognized in all patients. In 70 （97.2%） cadavers, the PGA branched from the splenic artery, and one female in this group had two PGAs. In 1 （1.4%） patient, the PGA originated from the root of the celiac trunk and in another （1.4%） patient, the PGA branched from the superior polar artery. Overall, the PGA extended for a length of 5.8-12.2 （mean, 8.4） cm from the root of the splenic artery, and the external diameter of the PGA was 1.2-3.2 （mean, 2） mm. CONCLUSION： The anatomical features of the PGA can be readily observed and characterized by autopsy. This study has provided valuable information on the features of the PGA useful in the planning of surgical treatment.

Decrease of reactive-oxygen-producing granulocytes and release of IL-10 into the peripheral blood following leukocytapheresis in patients with active ulcerative colitis

AIM: To investigate the clinical efficacy of leukocytapheresis (LCAP) in patients with active ulcerative colitis (UC), and to elucidate the mechanisms by determining the changes in the cytokine levels in the peripheral blood and of the functions of the peripheral blood leukocytes in these patients. METHODS: The subjects were 19 patients with active UC, with a mean clinical activity index (CAI) of 9.2. The LCAP was conducted using Cellsorba E. In each session of LCAP, 2-3 L of blood at the flow rate of 30-50 mL/min was processed. The treatment was carried out in approximately 1-h sessions, once a week, for 5-10 wk. Blood samples for determination of the cytokine levels were collected from the inflow side of the column (site of dehematization; at the start of LCAP) and outflow side of the column (at the end of LCAP). Blood samples for the determination of reactive-oxygen-producing cells were collected from the peripheral blood before and after LCAP. RESULTS: LCAP resulted in clinical improvement in all the 19 patients of UC recruited for this study. Remission (CAI: ≤4) was noted in 15 (79%) of the 19 patients. The blood level of the pro-inflammatory cytokine IL-6 was found to be decreased following treatment by LCAP, and the level of the anti-inflammatory cytokine IL-10 at the outflow side of the LCAP column was found to be significantly elevated as compared to that at the inflow side of the column. The reactive-oxygen-producing granulocytes in the peripheral blood of UC patients was increased as compared to that in healthy persons and the increase was found to be decreased following treatment by LCAP. CONCLUSION: LCAP exerted a high therapeutic efficacy in patients with active UC. Our findings suggest that LCAP is associated with enhanced production of the inhibitory cytokine IL-10 to indirectly inhibit the functions of the inflammatory leukocytes, and that inflammation is also considerably attenuated by the direct removal of reactive-oxygen-producing neutrophils fro

Improved version of BTOPMC model and its application in event-based hydrologic simulations

In this paper, a grid-based distributed hydrological model BTOPMC （Block-wise use of TOPMODEL） is introduced, which was developed from the original TOPMODEL. In order to broaden the model＇s application to arid regions, improvement methodology is also implemented. The canopy interception and soil infiltration processes were incorporated into the original BTOPMC to model event-based runoff simulation in large arid regions. One designed infiltration model with application of time compression approximation method is emphasized and validated for improving model＇s performance for event hydrological simulations with a case study of Lushi River basin.
