Evaluation of prognostic factors and scoring system in colonic perforation

AIM: To study the significance of scoring systems assessing severity and prognostic factors in patients with colonic perforation. METHODS: A total of 26 patients (9 men, 17 women; mean age 72.7 ± 11.6 years) underwent emergency operation for colorectal perforation in our institution between 1993 and 2005. Several clinical factors were measured preoperatively and 24 h postoperatively. Acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ), Mannheim peritonitis index (MPI) and peritonitis index of Altona (PIA Ⅱ) scores were calculated preoperatively. RESULTS: Overall postoperative mortality rate was 23.1% (6 patients). Compared with survivors, non- survivors displayed low blood pressure, low serum protein and high serum creatinine preoperatively, and low blood pressure, low white blood cell count, low pH, low PaO2/FiO2, and high serum creatinine postoperatively. APACHE Ⅱ score was significantly lower in survivors than in non-survivors (10.4 ± 3.84 vs 19.3 ± 2.87, P = 0.00003). Non-survivors tended to display high MPI score and low PIA Ⅱ score, but no signif icant difference was identif ied. CONCLUSION: Pre- and postoperative blood pressure and serum creatinine level appear related to prognosis of colonic perforation. APACHE Ⅱ score is most associated with prognosis and scores ≥ 20 are associated with signif icantly increased mortality rate.

BACKGROUND Emergency pancreaticoduodenectomy(EPD)is a rare event for complex periam-pullary *** intraoperative blood loss is correlated with poor post-operative *** SUMMARY Two patients underwent EPD using a no-touch isolation technique,in which all arteries supplying the pancreatic head region were ligated and divided before manipulation of the pancreatic head and *** operative times were 220 and 239 min,and the blood loss was 70 and 270 g,*** patients were discharged on the 14^(th) and 10^(th) postoperative day,***-two patients underwent EPD for the treatment of neoplastic *** mean operative time was 361.6 min,and the mean blood loss was 747.3 *** comp-lication rate was 37.5%.The in-hospital mortality rate was 9.38%.CONCLUSION The no-touch isolation technique is feasible,safe,and effective for reducing intraoperative blood loss in EPD.

Conjugate Heat Transfer Analysis of Film Cooling Flows

The objective of this study is to evaluate the potential of various grids to satisfactorily simulate the development of a cooling film, using a coupled computation that takes into account the full geometry. Detailed computations of a single row of 30 degrees round holes on a flat plate are presented for blowing ratios of 0.764, 1.01 and 1.54. The simulation results are compared well with experimental data. The two-layer model gave more accurate results but consumed much more computational time than the standard wall functions. The k-ε turbulence model with wall functions with appropriate values of y^＋ is suitable for practical use. The results show the importance of the conjugate calculation for accurately describing the influence of the heat transfer within the cooling film.

The Inter Relationship of Mental State between Antepartum and Postpartum Assessed by Depression and Bonding Scales in Mothers

The number of deaths caused by child abuse is increasing, which is one of social concerns. The mental health of mothers might be related to child abuse. The aim of this study was to examine and compare the mental state of mothers in both the antepartum and postpartum period assessed by the Edinburgh Postnatal Depression Scale (EPDS) and Mother-Infant-Bonding-Scale (MIBS), and compare the results. Participants (n = 134) were recruited twice in antepartum medical checkups (20 to 36 weeks of gestation) and postpartum medical checkups (1 month after birth). Information on characteristics of participants was collected from medical records in both periods. Family function and ante-postpartum mental health were assessed by Family APGAR, EPDS, and MIBS. Antepartum depressive state was related to postpartum depressive state (p = 0.015), antepartum bonding was related to bonding in postpartum bonding (p = 0.0001), and antepartum bonding disorder was related to postpartum depressive state (relative risk = 11.7). Worries about costs and poor of family function were related to the mental health of mothers in both the antepartum and postpartum periods. Antepartum depressive state is an indicator of postpartum depression. We suggested that health professionals conduct an evaluation of mother’s mental health and related factors in the antepartum period. The present findings emphasize the importance of antepartum mental health as a predictor of postpartum depression and bonding disorder.

Approach to perinatal mental health and child abuse prevention in Japanese prefectural health centers

The aim of this study was to conduct a nationwide survey in Japan of prefectural health centers, which were responsible for providing guidance to municipalities. The survey was performed in order to clarify the following issues: 1) the current level of support provided by prefectural centers for pre-and post-natal mental health;2) the structures in place for providing consultation services for an “unwanted pregnancy” and the support available for high-risk cases;and 3) the advice available on postpartum maternal psychological screening, and interpretation of results of such screening. Questionnaires were sent by post to 394 prefectural health centers, of which 277 (70.3%) responded. A total of 32% of prefectural health centers confirmed that they had offered support to high-risk cases during pregnancy, and 72% had offered support post-partum. Regarding offering support to high-risk mothers, those prefectural health centers that did provide consultation services (n = 59) reported providing introductions and information about available facilities (P < 0.001) and conducting case conferences (P < 0.002). This was significantly different than prefectural health centers that did not provide consultation services (n = 198). At the prefectural health centers that “follow up on” the results of the mental health screening, psychiatry consultations were reported twice as often as the prefectural health centers that did “not follow up on” the results of mental health screening. These findings indicate that childcare support systems for postpartum mental health and the prevention of child abuse were established. However, the lack of prenatal health and support systems for the prenatal period remains an issue.

Simple Simulation for COVID-19 in Wuhan, Hubei, China to Predict Outbreak in Wuhan, the Initial Case in Wuhan, and the Epidemic in Japan as of 11 February, 2020

Object: Prediction of the COVID-19 epidemic represents a matter of concern not only for public health or medicine but also for Earth’s general population. This study predicts outbreaks in Wuhan and in Japan as of 11 February, 2020.Method: We applied a simple SIR model to data published by Hubei public health authorities. Moreover, into the model, we incorporate mild and asymptomatic cases from experiences of Japanese residents of Wuhan up to the outbreak. Finally, we predict an outbreak in Japan based on 10,000 iterations of a simulation conducted under the assumption of infected people including mild cases visiting Japan according to the estimated distribution of patients in Wuhan since the date on which the initial case occurred to the date when travel from Wuhan to Japan was suspended. Results: Results suggest the basic reproduction number, R0, as 2.84;its 95% confidence interval (CI) was [2.35, 3.33]. The peak is estimated to be reached on March 11. Its 95% CI peak date is 29 February to 27 March. The 95% CI peak date in Japan is 26 April to 2 May. The greatest number of patients at the peak with severe symptoms was estimated as 858.3 thousand. Discussion and Conclusion: Our obtained R0 of 2.84 approximates an earlier estimate. We predicted the greatest number of patients at the peak with severe symptoms as 858.3 thousand in Japan. This number is 63% greater than the highest daily peak of influenza.
