Meta-analysis of lymph node metastasis in Siewert type Ⅰ and Ⅱ T1 adenocarcinomas

AIM To evaluate the incidence of lymph node metastasis(LNM) and its risk factors in patients with Siewert type Ⅰ and type Ⅱ pT1 adenocarcinomas.METHODS We enrolled 85 patients [69 men, 16 women; median age(range), 67(38-84) years] who had undergone esophagectomy or proximal gastrectomy for Siewert type Ⅰ and type Ⅱ pT1 adenocarcinomas. Predictive risk factors of LNM included age, sex, location of the tumor center, confirmed Barrett's esophageal adenocarcinoma, tumor size, macroscopic tumor type, pathology, invasion depth, presence of ulceration, and lymphovascular invasion. Multivariate logistic regression analysis was used to identify factors predicting LNM. We also evaluated the frequencies of LNM for Siewert type Ⅰ and type Ⅱ pT1 adenocarcinomas in meta-data analysis.RESULTS LNMs were found in 11 out of 85 patients(12.9%, 95%CI: 5.8-20.0). Only 1 of the 15 patients(6.6%, 95%CI: 0.0-19.2) who had a final diagnosis of pT1a adenocarcinoma had a positive LNM, whereas 10 ofthe 70 patients(14.2%, 95%CI: 6.0-22.4) with a final diagnosis of pT1b adenocarcinoma had positive LNM. Furthermore, only one of the 30 patients(3.3%, 95%CI: 0.0-9.7) with a tumor invasion depth within 500 μm from muscularis mucosae had positive LNM. Poor differentiation and lymphovascular invasion were independently associated with a risk of LNM. In meta-data analysis, 12 of the 355 patients(3.3%, 95%CI: 1.5-5.2) who had a final diagnosis of pT1a adenocarcinoma had a positive LNM, whereas 91 of the 438 patients(20.7%, 95%CI: 16.9-24.5) with a final diagnosis of pT1b adenocarcinoma had positive LNM. CONCLUSION We consider endoscopic submucosal dissection(ESD) is suitable for patients with Siewert type Ⅰ and type Ⅱ T1 a adenocarcinomas. For patients with T1b adenocarcinoma, especially invasion depth is within 500 μm from muscularis mucosae with no other risk factor for LNM, diagnostic ESD could be a treatment option according to the overall status of patients and the presence of comorbidities.

BACKGROUND Neuropathic pain management should aim at improving quality of life and daily living activities of patients;therefore,emphasis should be placed on pain management including understanding the pain patterns during daily activity.Therefore,lifestyle guidance should be based on a detailed understanding of pain;however,previous studies commonly evaluated pain intensity at a single point in time.We report a case on patient education intervention based on the relationship between pain circadian rhythms and detailed physical activity during the day.CASE SUMMARY A man in his 60s,who suffered a brachial plexus injury in a traffic accident,presented with neuropathic pain.Early assessment of the importance of daily living activities to the patient,pain rhythmicity,and physical activity,was performed.The early assessments showed that the pain intensity was lower on days when more light-intensity physical activity(LIPA)was performed,than on days when less LIPA was performed.Consequently,patient education focused on methods to decrease the pain intensity that tended to worsen in the afternoon,and encouraged behavioral changes by suggesting the patient to take walks,which could be used to maintain LIPA in the afternoon.On reassessment,the afternoon LIPA,which had been the focus of attention,had increased and a change was noted in the circadian rhythm of pain.CONCLUSION Patient education based on a composite assessment elicited positive results in relation to the pain circadian rhythm and physical activity.

Partial Purification and Characterization of the Rat Parotid Gland Protein Kinase Catalyzing Phosphorylation of Matured Destrin at Ser-2

Destrin, also called actin-depolymerizing factor (ADF), exists in resting parotid tissue as phosphorylated (inactive) and dephosphorylated (active) forms, and β-adrenergic stimulation of this tissue induces dephosphorylation of destrin. It is suggested that destrin dephosphorylation is involved in cortical F-actin disruption observed in parallel with β-agonist-induced amylase secretion. At present, the phosphorylation/dephosphorylation mechanism of destrin in parotid tissue is not known. We previously detected, in a crude rat parotid extract, a constitutively active protein kinase catalyzing phosphorylation of destrin;however, its identification has been hampered by difficulty in its enrichment. The purpose of this study was to explore a simple purification method(s) for this enzyme. To this end, we first developed a high-throughput dot-blot assay for the kinase with an anti-phosphodestrin antibody and then studied its purification by column chromatography on several media. We found that the kinase could be partially purified by sequential chromatography on DEAE-cellulose, phenyl-Sepharose, and hydroxyapatite columns. In each chromatography, however, the kinase could be eluted, at the cost of resolution, only by sharp increases in the elution power of the eluent;gradual increases in the elution power resulted in unacceptably poor recovery. We confirmed that enzymatic properties of the kinase were not basically altered during the purification. Further purification of the kinase was achieved by native polyacrylamide gel electrophoresis (PAGE), which resolved the kinase activity into two bands and separated the activity from most proteins (the kinase activity after PAGE was detected with destrin-coated polyvinylidene difluoride membranes and the anti-phosphodestrin antibody). The two bands seem to constitute the major destrin-phosphorylating activity in the resting rat parotid gland. We here report its partial purification and characterization together with the detection methods.

The Distribution of the Oceanic Sea Skaters, Halobatesinside and outside the Kuroshio

The purpose of this study was to clarify the distribution of oceanic Halobates in the area of the Kuroshio flowing near the southern shore in the direction of 100° - 120°, and also to compare the population density of Halobates between the area within or outside the area of the Kuroshio and also among seasons. This study was carried out during 8 cruises by R/V TANSEIMARU. The Kuroshio area south of the southern Japan coast (30°00'N - 35°00'N, 130°25'E - 141°04'E) was dominated by H. sericeus, and the averaged population-density of this species was significantly higher inside the Kuroshio than outside this current. On the Kuroshio, H. sericeus was dominant with the population density of 16,396.4 km-2 ± 66,138.4 [26] (Mean ± SD [n]), whereas the density of H. germanus was 8,581.9 km-2 ± 24,443.2 [26]. The two oceanic sea skaters, H. sericeusand H. germanus showed significant seasonal variation in the population density, with significantly higher density in October than other months, whereas there was no such significant October peak in the cosmopolitan oceanic sea skater, H. micans. The results of this study may suggest that H. sericeus could use the Kuroshio as a transportation tool to distribute a wide latitude area of from 10°N to 40°N in the western tropical, subtropical and temperate area in the Pacific Ocean.

This study aims, first, to examine the limit for tolerance to lower salinity by an oceanic sea skater, Halobates micans, and , second, to make it clear whether exclusively fresh water Halobatinae species, Metrocoris histrio has salinity tolerance. Adults of H. micans were collected using Neuston Net from the starboard side of R/V MIRAI on a fixed station at 8°S, 80°E, whereas those of M. histrio were collected from a small pond filled with a spring fresh water in Kochi (33°N, 133°E), Japan. Time in survival was measured in starved condition under several salinity conditions: 0‰, 2‰, 4‰, 6‰, 8‰, 9‰ and 10‰ for H. micans;0‰, 5‰, 10‰, 12.5‰, 15‰ for M. histrio. Half of adults were in coma due to lower salinity under 10‰ and time in survival was less than 10 hours under less than 4‰ for H. micans. Time in survival was half at 5‰ of 80 hours on average at 0‰ as a control and less than 10 hours at 10‰ or higher salinity for M. histrio. Relatively flexible osmo-regulation ability by H. micans would be related to wide variety of salinity condition of surface oceanic water, whereas very limited tolerance even to lower salinity of 5‰ may be permitted by the no chances to be exposed to brackish water in natural habitats of M. histrio. This study showed that salinity tolerance of Halobatinae species would reflect, directly, the salinity condition of their habitats.
