Long-term efficacy of infliximab maintenance therapy for perianal Crohn’s disease

AIM:To assess the long-term efficacy of seton drainage with infliximab maintenance therapy in treatment of stricture for perianal Crohn’s disease(CD). METHODS:Sixty-two patients with perianal CD who required surgical treatment with or without infliximab between September 2000 and April 2010 were identified from our clinic’s *** activities of the perianal lesions were evaluated using the modified perianal CD activity index(mPDAI)*** primary endpoint was a clinical response at 12-15 wk after surgery as a shortterm *** endpoints were recurrence as reflected in the mPDAI score,defined as increased points in every major *** clinical responses were classified as completely healed(mPDAI=0),partially improved(mPDAI score decreased more than 4 points),and failure or recurrence(mPDAI score increased or decreased less than 3 points). RESULTS:There were 43 males and 19 females,of whom 26 were consecutively treated with infliximab after surgery as maintenance *** healing was not *** was seen in 10/36(27.8%) patients without infliximab and 4/26(15.4%)patients with infliximab(P=0.25).Partial improvement was seen in 26/36(72.2%)patients without infliximab and 22/26(88.5%)patients with infliximab(P=0.25). Short-term improvement was achieved in 48/62(77.4%) *** the mPDAI score improved significantly with surgery regardless of infliximab,it decreased more from baseline in patients with infliximab(50.0%) than in those without infliximab(28.6%),(P=0.003). Increasing the yield is one of the most important goals of crop *** the beginning of rice cultivation thousands of years ago,many efforts have been made to increase rice *** of the successes in this history was the Green *** tremendous improvement in the 1960s was achieved by increasing rice and wheat production through the development and widespread adoption of semi-dwarf varieties with the extensive use of fertilizers(Hedden,2003).Dwarfism in crops lowers the center of gravity of the plant body,reducing the risk of lodging and enabling farmers to use more fertilizer to increase ***-dwarf mutations in both rice and wheat created during the Green Revolution are related to the plant growth hormone gibberellin(GA).Although other mechanisms cause dwarf mutants,only GA-related semi-dwarf mutants were selected during the Green Revolution(Wang et al.,2021).While GA-related abnormalities only make plants shorter,most cases caused by other mechanisms involve not only dwarfism but also other morphological abnormalities.

Acoustic Absorption Characteristics of Perforated Thin Plate with Air Jets and Cavity

The present paper focuses on the effect of air jets through a perforated thin plate on the characteristics of an acoustic absorption coefficient. We measured the flow rate, internal pressure, acoustic pressure, and transfer function by using an improved acoustic impedance tube. The normal incidence absorption coefficient was calculated from the measured transfer function using transfer function methods. As a result, the frequency characteristics of the acoustic absorption coefficient against the frequency showed a maximum value at the local frequency. The peak frequency of the acoustic absorption coefficient depended on the thickness of the background air space and the thickness of the perforated plate. As the flow rate increased through the micropores, the peak level of the acoustic absorption coefficient also increased until a flow rate of 80?l /min. As the flow rate further increased, the peak level of the acoustic absorption coefficient decreased and that of the high frequency band increased.

Prediction of return of spontaneous circulation in outof-hospital cardiac arrest with non-shockable initial rhythm using point-of-care testing: a retrospective observational study

BACKGROUND:Out-of-hospital cardiac arrest(OHCA) is a public health concern, and many studies have been conducted on return of spontaneous circulation(ROSC) and its prognostic *** thromboelastometry(ROTEM?), a point-of-care testing(POCT) method, has been useful for predicting ROSC in patients with OHCA, but very few studies have focused on patients with non-shockable rhythm. We examined whether the parameters of POCT could predict ROSC in patients with OHCA and accompanying non-shockable ***:This is a single-center, retrospective observational study. Complete blood count,blood gas, and ROTEM POCT measurements were used. This study included patients with nontraumatic OHCA aged 18 years or older who were transported to the emergency department and evaluated using POCT between January 2013 and December 2021. The patients were divided into the ROSC and non-ROSC groups. Prehospital information and POCT parameters were compared using receiver operating characteristic(ROC) curve analysis, and further logistic regression analysis was ***:Sixty-seven and 135 patients were in the ROSC and non-ROSC groups,respectively. The ROC curves showed a high area under the curve(AUC) for K^(+) of 0.77(95%confidence interval [CI]:0.71–0.83) and EXTEM amplitude 5 min after clotting time(A5) of 0.70(95%CI:0.62–0.77). The odds ratios for ROSC were as follows:female sex 3.67(95%CI:1.67–8.04);K^(+)0.64(95%CI:0.48–0.84);and EXTEM A5 1.03(95%CI:1.01–1.06).CONCLUSION:In OHCA patients with non-shockable rhythm, K^(+) level and the ROTEM parameter EXTEM A5 may be useful in predicting ROSC.

Resampling in neural networks with application to spatial analysis

In developing Artificial Neural Networks(ANNs),the available dataset is split into three categories:training,validation and ***,an important problem arises:How to trust the predic-tion provided by a particular ANN?Due to the randomness related to the network itself(architecture,initialization and learning procedure),there is usually no best *** this issue,we provide a framework,which captures the randomness related to the network *** idea is to perform several training and test trials based on the Jackknife resampling *** consists of iteratively deleting a single observation each time from the sample and recomputing the ANN on the rest of the sample ***,interval prediction is available instead of point *** proposed method was applied and tested using pH,Ca and P data obtained by analyzing 118 georeferenced soil *** results,based on the dataset size simulation,showed that 60%reduction in available dataset offers compatible accuracy in relation to full dataset,and therefore a higher cost of sampling in the field would not be *** re-sampling method spatially characterizes the points of greater or lesser accuracy and *** re-sampling method increased the success rate by using interval prediction instead of using the mean as the most probable *** we restrict it to the regression neural network model,the resampling method proposed can also be extended to other modern statistical tools,such as Kriging,Least Squares Collocation(LSC),Convolutional Neural Network(CNN),and so on.

Imbricate structure of the Otakeyama area, Okayama Permian Yoshii Group in the Prefecture, southwest Japan

The Yoshii Group of the Akiyoshi terrane is distributed over the Otakeyama area, Okayama Prefecture, southwest Japan. The Yoshii Group is composed of siliceous rocks and clastics, comprising chert-clastic sequences. The Yoshii Group is divided into four structural units （Units Ot 1, Ot 2, Ot 3, and Ot 4） in structurally ascending order. Lithological similarities and radiolarian age suggest that these units exhibit almost identical ocean plate stratigraphy. These units are structurally-repeated to form an imbricate structure. This structure was possibly formed by off-scrape accretion at a toe of an accretionary complex. Additionally, a review of previous studies indicates that an imbricate structure is common in the Akiyoshi terrane. The presence of an imbricate structure in the Akiyoshi terrane, Permian accretionary complex, indicates the possibility that a specific horizon of Permian pelagic sequences acted as a drcollement zone.
