Maintenance treatment with infliximab for ulcerative ileitis after intestinal transplantation: A case report

BACKGROUND Evidence has been published on the successful applications of the anti-tumor necrosis factor alpha antibody infliximab,such as induction therapy,salvage treatment for acute cellular rejection,and treatment for chronic ulcerative inflammation,in intestinal transplant ***,the optimal protocol for the effective use of infliximab remains largely undetermined due to scarcity of available clinical *** report a continuative application of infliximab as maintenance therapy for recurrent chronic ulcerative ileitis in a recipient of isolated intestinal transplantation(ITx).CASE SUMMARY The patient was a 11-year-old boy with intestinal motility disorder classified as a hypogenic type of intestinal *** patient underwent living-donor related intestinal *** immunosuppression regimen consisted of daclizumab,tacrolimus,and *** he did not show rejection while on tacrolimus monotherapy,routine screening endoscopy showed several ulcerative lesions in the distal end of the graft 2 years after the intestinal *** work up to evaluate the progression of anemia revealed stenosis with ulcerative inflammatory changes and multiple longitudinal ulcers in the *** the endoscopic findings suggested ulcerative lesions in Crohn’s disease,infliximab treatment was *** with infliximab and a small dose of oral prednisolone afforded successful withdrawal of total parenteral nutrition and maintenance of a well-functioning graft without infectious complications for 5 years since the administration of the first dose of *** Infliximab is effective as maintenance therapy for recurrent chronic ulcerative ileitis in an isolated ITx patient.

Readout Properties of Polychromatic Reconstruction for Nondestructive Holographic Data Storage

Polychromatic reconstruction(PCR) is a novel nondestructive readout method that utilizes a spectrally broad light source for the probe beam. The stored image can be completely reconstructed even though the probe wavelength is very different from the recording one. The large spectral width of the polychromatic probe beam also causes adverse effect on the storage density. But this can be overcomed if an additional optical component is inserted in the imaging system. Therefore, PCR has a great potential to achieve nondestructive readout and large storage density simultaneously. In addition, PCR enables us to design a simple memory system because its readout tolerance is quite large as compared to the conventional monochromatic readout. Unique and attractive features of the polychromatic reconstruction method was theoretically and experimentally demonstrated.

Submicron-sized peanut-shaped poly（methyl methacrylate）/polystyrene （PMMA/PS） particles were successfully synthesized by seeded soap-free emulsion polymerization of styrene on the spherical crosslinked PMMA seed particles. The obtained peanut- shaped particles showed a novel internal morphology： PS phase formed one domain which linked to the other domain having PMMA core encased by PS shell.

Intraoperative frozen section diagnosis of bile duct margin for extrahepatic cholangiocarcinoma

AIM To evaluate the usefulness of frozen section diagnosis(FSD) of bile duct margins during surgery for extrahepatic cholangiocarcinoma(CCA). METHODS We retrospectively analyzed 74 consecutive patients who underwent surgery for extrahepatic CCA from 2012 to 2017, during which FSD of bile duct margins was performed. They consisted of 40 distant and 34 perihilar CCAs(45 and 55 bile duct margins, respectively). The diagnosis was classified into three categories: negative, borderline(biliary intraepithelial neoplasia-1 and 2, and indefinite for neoplasia), or positive. FSD in the epithelial layer, subepithelial layer, and total layer was compared with corresponding permanent section diagnosis(PSD) ***, association between FSD and local recurrence was analyzed with special reference to *** Analysis of 100 duct margins revealed that concordance rate between FSD and PSD was 68.0% in the total layer, 69.0% in the epithelial layer, and 98.0% in the subepithelial layer. The extent of remaining biliary epithelium was comparable between FSD and PSD, and more than half of the margins lost > 50% of the entire epithelium, suggesting low quality of the samples. In FSD, the rate of negative margins decreased and that of borderline and positive margins increased according to the extent of the remaining epithelium. Diagnostic discordance between FSD and PSD was observed in 31 epithelial layers and two subepithelial layers. Alteration from borderline to negative was the most frequent(20 of the 31 epithelial layers). Patients with positive margin in the total and epithelial layers by FSD demonstrated a significantly worse local recurrence-free survival(RFS) compared with patients with borderline and negative margins, which revealed comparable local RFS. Patients with borderline and negative margins in the epithelial layer by PSD also revealed comparable local RFS. AIM： To examine the effects of anti-high mobility group box 1 （HIGB1） neutralizing antibody in experimental severe acute pancreatitis （SAP）. METHODS： SAP was induced by creating closed duodenal loop inC3H/HeN mice. SAP was induced immediately after intrapedtoneal injection of anti-HMGB1 neutralizing antibody （200 pg）. Sevedty of pancreatitis, organ injury （liver, kidney and lung）, and bacterial translocation to pancreas was examined 12 h after induction of SAP. RESULTS： Anti-HHGB1 neutralizing antibody significantly improved the elevation of the serum amylase level and the histological alterations of pancreas and lung in SAR Anti-HHGB1 antibody also significantly ameliorated the elevations of serum alanine aminotransferase and creatinine in SAR However, anti-HHGB1 antibody worsened the bacterial translocation to pancreas. CONCLUSION： Blockade of HHGB1 attenuated the development of SAP and associated organ dysfunction, suggesting that HHGB1 may act as a key mediator for inflammatory response and organ injury in SAR

Inverse correlation between reactive oxygen species in unwashed semen and sperm motion parameters as measured by a computer-assisted semen analyzer

This study investigated the correlation between sperm motion parameters obtained by a computer-assisted semen analyzer and levels of reactive oxygen species in unwashed semen. In total, 847 patients, except for azoospermic patients were investigated. At the time of each patient＇s first consultation, semen parameters were measured using SMASTM or CellSoft 3000TM, and production of reactive oxygen species was measured using a computer-driven LKB Wallac Luminometer 1251 Analyzer. The patients were divided into two groups： reactive oxygen species - positive and negative. The semen parameters within each group were measured using one of the two computer-assisted semen analyzer systems and then compared. Correlations between reactive oxygen species levels and sperm motion parameters in semen from the reactive oxygen species - positive group were also investigated. Reactive oxygen species were detected in semen samples of 282 cases （33.3%）. Sperm concentration （P 〈 0.01; P 〈 0.01）, motility （P 〈 0.01; P 〈 0.05）, and progressive motility （P 〈 0.01; P 〈 0.01） were markedly lower in the reactive oxygen species - positive group than in the reactive oxygen species - negative group. Among the sperm motion parameters in the reactive oxygen species - positive group, sperm concentration （P 〈 0.01; P 〈 0.01）, motility （P 〈 0.05; P 〈 0.01）, mALH （P 〈 0.05; P 〈 0.01）, and progressive motility （P 〈 0.05; P 〈 0.01） also showed inverse correlations with the logarithmic transformed reactive oxygen species levels. Therefore, this study demonstrated that excessive reactive oxygen species in semen damage sperm concentration, motility, and other sperm motion parameters.

Ligularia Cass., (Compositae) is a highly diversified genus, and more than 100 species of which are distributed in the eastern Qinghai-Tibet Plateau and adjacent areas. Ligularia species have been studied with respect to secondary metabolites, and many sesquiterpenes of the furanoeremophilane type have been isolated from them. In order to find correlates among these variations, and ultimately understand the diversity-generating mechanism of Ligularia species in the Hengduan Mountains, we initiated an extensive study that uses furanoeremophilanes as a chemical index and the DNA sequence as a genetic index. Furanoeremophilanes have been detected conventionally by Ehrlich' s test, which has been used in a search for novel natural products. As for the DNA sequence, we determined the nucleotide sequence of the atpB-rbcL intergenic region in the present study.

Predicting Optimal Trading Actions Using a Genetic Algorithm and Ensemble Method

Machine learning has been applied to the foreign exchange market for algorithmic trading. However, the selection of trading algorithms is a difficult problem. In this work, an approach that combines trading agents is designed. In the proposed approach, an artificial neural network is used to predict the optimum actions of each agent for USD/JPY currency pairs. The agents are trained using a genetic algorithm and are then combined using an ensemble method. We compare the performance of the combined agent to the average performance of many agents. Simulation results show that the total return is better when the combined agent is used.

Transcatheter arterial chemoembolization with a fine-powder formulation of cisplatin for hepatocellular carcinoma

AIM:To evaluate the efficacy of transcatheter arterial chemoembolization(TACE) using a suspension of a fine-powder formulation of cisplatin(DDPH) for hepatocellular carcinoma(HCC).METHODS:The study population was comprised of 164 patients who were treated by TACE *** these patients,76 underwent TACE using a suspension of DDPH in lipiodol(LPD)(DDPH group),and the remaining 88 underwent TACE with an emulsion of doxorubicin(ADM) with LPD(ADM group).We compared the DDPH group with the ADM group in terms of the objective early response rate,progression free survival(PFS) and overall survival(OS).RESULTS:The objective early response rate in the DDPH group was significantly higher than that in the ADM group(54% vs 24%,P < 0.001).The PFS rate in the DDPH group was also significantly higher than that in the ADM group(P < 0.001).Moreover,the OS in the DDPH group was significantly longer than that in the ADM group(P = 0.002).Although the incidence rate of nausea or vomiting in the DDPH group was higher than that in the ADM group,the ADM group showed a higher incidence rate of the adverse events of hepatic arterial damage and *** other serious complications were observed in either ***:We conclude that TACE using a suspension of DDPH in LPD could be a useful treatment for HCC.
