Purpose: To evaluate long-term skin reactions following breast-conserving therapy by using the melanin-erythema index meter. Patients and Methods: 164 patients were followed for at least three years after breast-conserving therapy. For both the erythema and the melanin indices, the ratio of the irradiated-side index to the non-irradiated-side index was calculated. The time course of index ratios alternation was examined. Influences from additional therapies and patients’ age were also evaluated. Result: Both erythema and melanin index ratios of the breast skin were recovered to pre-radiation level three years after radiotherapy. However, both index ratios of the area administrated with 10-Gy boost irradiation were still high even after five years after radiotherapy. Endocrine therapy, chemotherapy and age had no significant influence on skin color reactions three years after radiotherapy. Conclusion: Quantitative assessment using the melanin-erythema index meter demonstrated that chronic skin reactions following breast conserving therapy had recovered to pre-radiation level for three years after irradiation except for the 10-Gy boost irradiated area.

Sitting position improves consciousness level in patients with cerebral disorders

Cerebral disorders are often associated with disturbance of consciousness. Since the latter could alter the prognosis of physical function, early improvement of consciousness level is important. The present study tested the hypothesis that sitting position improves disturbances of consciousness in patients with cerebral disorders. The subjects were 17 patients with cerebral disorders and disturbances of consciousness. The consciousness level was evaluated in supine position and following a change to sitting position. The consciousness level was evaluated by the Glasgow Coma Scale (GCS). The total, eye opening, and motor GCS scores were significantly higher in the sitting position compared with the supine position. In conclusions, changing from supine to sitting position improves consciousness level in patients with cerebral disorders.

A unicellular cyanobacterium Synechococcus sp. strain PCC 7002 was transformedwith plasmid pQL1, on which β-lactamase gene (bla) and β-galactosidase gene (lacZ) were *** transformant cells released β-lactamase into medium by an abrupt drop of osmotic pressure. This re-sult indicates that this cyanobacterium recognizes and processes the signal sequence of β-lactamase, andaccumulates the enzyme in periplasm. Repeated release of β-lactamase was possible by repeated osmoticshocks without impairing cell viability. On the other hand, most of the β-galactosidase remained in cyto-plasm under the osmotic shock.

The advanced oxidation of 2,4-dinitrophenol (DNP), 2,5-DNP, and 3,4-DNP in aqueous solution has been investigated using a multi-gas, dielectric barrier discharge. Dielectric barrier discharge was operated in the aqueous solution and gas boundary. The degradation was measured by high performance liquid chromatography (HPLC). The acceleration of the advanced-oxidation has been investigated by the combination of the anion exchange polymer membrane. The result indicated that the degradation pathways involve a rapid detachment of the nitro group and a slow opening of the aromatic-ring. The hydroxyl radical and the excited hydroxyl anion are responsible for the primary attack of the DNP with the production of dihydroxy-nitrobenzenes. The attack of hydroxyl radical occurs at the benzene ring carbon activated by the presence of a phenolic OH group and a nitro group. The result suggested that the reaction is dominated by a pseudo-first order kinetic reaction. The degradation process is interpreted using Molecular Orbital Theory.

Alzheimer’s disease (AD) is the common cause of dementia which shows the neuro-pathologies like an accumulation of amyloid-β (Aβ) and degeneration of cholinergic neuron. Olfactory bulbectomized (OBX) mice show some of AD features, so they have been used to research as AD model. Mesenchymal stem cells (MSCs) can differentiate into many kinds of cells, including neuronal cells. In this study, we intranasally administrated the conditioned medium derived from cultured umbilical cord (UC) MSCs. The intranasal administration of the MSCs medium restored the cognitive impairment observed in OBX mice. In addition, the decreased number of choline acetyltransferase-positive cells in the medial septum was restored by the conditioned medium administration. In conclusion, MSCs-derived conditioned medium may have protective effects of cholinergic neurons in the medial septum, thereby rescuing the cognitive impairment of OBX.

High-Speed Active Release End-Effector Motions for Precise Positioning of Adhered Micro-Objects

This paper presents a release method for micro-objects. To improve position accuracy after release, we propose 3D high-speed end-effector motions. The classical release task focuses on the detachment of a micro-object from an end-effector. The technique utilizes merely the vibration of the end-effector regardless of the pattern of movement. To release different sizes of micro- objects and place them precisely at the desired locations in both air and liquid media, in this paper, we propose high-speed motions by analyzing the adhesion force and movement of micro-objects after separation. To generate high end-effector acceleration, many researchers have applied simple vibration by using an additional actuator. However, in our research, 3D high-speed motion with apt amplitude is accomplished by using only a compact parallel mechanism. To verify the advantages of the proposed motion, we compare five motions, 1D motions (in X-, Y-, and Z-directions) and circular motions (clockwise and counterclockwise directions), by changing the frequency and amplitude of the end-effector. Experiments are conducted with different sizes of microbeads and NIH3T3 cells. From these experiments, we conclude that a counterclockwise circular motion can release the objects precisely in air, while 1D motion in the Y direction and two circular motions can detach the objects at the desired positions after release in a liquid environment.

Stimulation of Root Growth Induced by Aluminum in <i>Quercus serrata</i>Thunb. Is Related to Activity of Nitrate Reductase and Maintenance of IAA Concentration in Roots

Aluminum (Al) is the most abundant metal in the earth’s crust. Excess Al3+ released by soil acidification in soil solution is thought to be a growth limiting factor to many cultivated plant species, but it has been reported to stimulate plant growth in some crop and tree species in certain concentration of Al3+. Previously, we had reported that Al treatment enhanced root development, uptake from growth media and in vivo nitrate reductase (NR) activity of roots. NR is one of the key enzymes in nitrogen metabolism and acts at the first step of nitrate assimilation in plants. In this study, we investigated the process of Al-induced root development in an early stage, focusing on the change in in vitro NR activity, and indole-3-acetic acid (IAA) and cytokinins concentration in roots of Quercus serrata seedlings, which were treated for 1 h with Al or Ca. In Al-treated roots, NR activity increased and IAA concentration was maintained at the same level as pretreatment, and indole-3-acetyl-L-aspartic acid (IA-Asp), which is a metabolic intermediate of IAA degradation, was not detected in roots. In Ca-treated roots, NR activity increased, but IAA concentration decreased as IA-Asp concentration increased. Thus, the maintenance of IAA concentration in Al-treated roots seems to result from suppression in the process of IAA decomposition. Al treatment increased the length and number of second lateral roots but Ca treatment did not. We concluded that root development induced by Al in the early stage was related to NR activity and maintenance of IAA concentration.

Interaction between Warfarin and Proton Pump Inhibitors

Background: Interaction between proton pump inhibitors (PPI) and warfarin is controversial. Previous clinical studies have only a short follow-up period. Methods and Results: All patients (n = 716) for whom warfarin was prescribed from November 1, 2010 to October 30, 2011 were extracted from electronic health records. In retrospective analysis for 1 year, PPI were prescribed to 404 patients. Among them, 108 patients were taking warfarin for more than 6 weeks before and after PPI. The profile of these patients was analyzed: 63 patients took lansoprazole;15 patients took omeprazole;30 patients took rabeprazole. No statistical difference was observed among 3 groups in age, body weight, concomitant use of other drugs, and comorbidity. Warfarin dose and INR did not change after PPI. Multivariate stepwise logistic regression analysis revealed that upper quartile of increment of INR was associated with the presence of atrial fibrillation (OR 3.77, 95% CI 1.16 - 12.27). The patients who had warfarin for shorter periods before PPI, or those who had PPI first (n = 141) had similar dose of warfarin and INR. In all patients analyzed (n = 404), including patients whose follow-up periods were shorter than 6 weeks (n = 155), a patient had cerebral bleeding, and 2 patients had cerebral infarction. Conclusions: Unfavorable interaction between warfarin and PPI was negligible in clinical use. Relatively higher INR was achieved after PPI in the presence of atrial fibrillation.

Importance of adequate immunosuppressive therapy for the recovery of patients with "life-threatening" severe exacerbation of chronic hepatitis B

AIM: Hepatitis B virus (HBV) re-activation often occurs spontaneously or after withdrawal of immunosuppressive therapy in patients with chronic hepatitis B. Severe exacerbation, sometimes developing into fulminant hepatic failure, is at high risk of mortality. The efficacy of corticosteroid therapy in "clinically severe" exacerbation of chronic hepatitis B has not been well demonstrated. In this study we evaluated the efficacy of early introduction of high-dose corticosteroid therapy in patients with lifethreatening severe exacerbation of chronic hepatitis ***: Twenty-two patients, 14 men and 8 women,were defined as "severe" exacerbation of chronic hepatitis B using uniform criteria and enrolled in this study. Eleven patients were treated with corticosteroids at 60 mg or more daily with or without anti-viral drugs within 10 d after the diagnosis of severe disease ("early high-dose"group) and 11 patients were either treated more than 10 d or untreated with corticosteroids ("non-early high-dose"group).RESULTS: Mean age, male-to-female ratio, mean prothrombin time (PT) activity, alanine transaminase (ALT)level, total bilirubin level, positivity of HBeAg, mean IgMHBc titer, and mean HBV DNA polymerase activity did not differ between the two groups. Ten of 11 patients of the "early high-dose" group survived, while only 2 of 11 patients of the "non-early high-dose" group survived (P＜0.001). During the first 2 wk after the introduction of corticosteroids, improvements in PT activities and total bilirubin levels were observed in the "early high-dose"group. Both ALT levels and HBV DNA polymerase levels fell in both ***: The introduction of high-dose corticosteroid can reverse deterioration in patients with "clinically lifethreatening" severe exacerbation of chronic hepatitis B,when used in the early stage of illness.
