Role of robotic-assisted pancreatic surgery:lessons learned from our initial experience

BACKGROUND:Minimally invasive surgery has achieved worldwide acceptance in various fields, however, pancreatic surgery remains one of the most challenging abdominal procedures. In fact, the indication for robotic surgery in pancreatic disease has been controversial. The present study aimed to assess the safety and feasibility of robotic pancreatic ***:We retrospectively reviewed our experience of robotic pancreatic resection done in Sanchinarro University Hospital. Clinicopathologic characteristics, and perioperative and postoperative outcomes were recorded and ***:From October 2010 to April 2016, 50 patients underwent robotic-assisted surgery for different pancreatic pathologies. All procedures were performed using the da Vinci robotic system. Of the 50 patients, 26 were male and 24 female. The average age of all patients was 62 years. Operative time was 370 minutes. Among the procedures performed were 16 pancreaticoduodenectomies(PD), 23 distal pancreatectomies(DP), 11 tumor enucleations(TE). The mean hospital stay was 17.6 days in PD group, 9.0 days in DP group and 8.4 days in TE group. Pancreatic fistula occurred in 10 cases(20%), 2 after PD, 3 after DP, and 5 after TE. Four patients had postoperative transfusion in PD group and one in DP group. Conversion to open laparotomy occurred in four patients(8%). No serious intraoperative complications were observed. CONCLUSIONS:From our early experience, robotic pancreatic surgery is a safe and feasible procedure. Further experience and follow-up are required to confirm the role of robotic approach in pancreatic surgery.

Background:Pancreas divisum is a congenital embryological disease caused by a lack of fusion between the ventral and dorsal pancreatic ducts in the early stages of *** acute pancreatitis,chronic pancreatitis or chronic abdominal pain are the main clinical syndromes at presentation and occur in only 5%of the patients with pancreas *** review aimed to discuss diagnosis and treatment strategies in patients with symptomatic pancreas *** sources:We report a literature review from 1990 up to January 2018 to explore the various diagnostic modalities and surgical techniques and results reported in the surgical treatment of pancreas ***:There are limited reports available on this topic in the *** analyzed and described the main indications in the treatment of pancreas divisum,focusing on surgical treatment and a discussion of the different ***,we report the results from our experience in two cases of pancreas divisum treated by pancreatic head resection with segmental duodenectomy(the Nakao procedure).Conclusions:Pancreas divisum is a common pancreatic malformation in which only a few patients develop a symptomatic *** treatment is needed in case of endoscopic drainage failure and in cases complicated with chronic pancreatitis and local *** techniques,of greater or lesser complexity,have been proposed.

The back pain is the most common injury in human activities where heavy objects must be lifted or must be suspended for a long time.A weight lifting exoskeleton also known as force augmentation exoskeleton is designed to reduce the strain on the back and the limbs and reduce the risk to suffer *** the other hand,different kinds of controllers have been implemented to achieve whit this goal,for example,a conventional PD Control,PD Control with Gravity Compensation,PD Control with Adaptive Desired Gravity Compensation and PD Control with Robust *** paper aims to evaluate and compare the performance from the previously cited controllers used to reduce the strain in the back,through the implementation of each controller in a three Degrees Of Freedom(DOF)exoskeleton powered by pneumatic muscle actuators;some numerical simulations as well as experimental trials have been conducted and three different performance indices were used in order to determine the effectiveness of each one with respect to the simple PD controller when the mass to be lifted is unknown.

Effects of Exogenous Abscisic Acid and Water Stress on the Growth Response of Subterranean Clover of Different Genotypes

The response of subterranean clover ( Trifolium subterraneum L.) to different abscisic acid (ABA) concentrations (10 -6 , 10 -5 , 10 -4 mol/L) in nutrient solution and to water stress were studied in growth room. Six cultivars of contrasting yield capacity were compared. Plants remained growing in Hoagland solution or pots until at least four full_developed leaves appeared. The ABA was then applied and the fresh weight, leaf number and length of the largest root were measured at 1, 4, 7 and 11 d after ABA treatment. The above parameters were also measured at 15 d under water stress. In all the tested genotypes ABA caused similar reduction in these growth parameters, as well as a significant decrease of leaf water potential which was dependent on ABA concentration. The average growth reduction after 11 d under 10 -4 mol/L ABA coincided with the range of these crops under water stress in pot experiments. On average of the different genotypes, leaf number, area of full_developed leaf and the dry weight per plant decreased by about 50% whereas the root/shoot ratio increased by 80%.The genotype variation and ranking for this treatment were rather similar to the same genotypes in pot experiments. The genotypes, Clare, Nuba and Seaton Park, showed the best results under both control and ABA treated conditions and water stress conditions. The similarity between the response to ABA in nutrient solution and to water stress opens the possibility to use this approach as a way to quantify the drought resistance of subterranean clover genotypes.

Since the beginning of the pandemic caused by the new Coronavirus (SARS- CoV-2), critically ill patients care has been challenging. It is necessary to perform the best available practice and protect the health team, preserving human resources and rationalizing costs. Changes in tracheostomies institutional protocols are necessary according to each health care services reality. This is particular important in developing countries. The aim of this study is to establish a practical and sucint guideline to minimize controversies regarding the proper timing to perform elective tracheostomies in critically ill coronavirus infected patients.

The targeted area for plant reforestation in the state of Michoacan is made up of eroded areas and pasture induced areas, both adding to 525,260 ha and representing 10.9% of the total surface of the state. The regions with the largest areas in need of reforestation are the Costa with 185,396 ha (29%), Oriente with 106,836 (17%) and Tierra Caliente with 96,284 (15%). Commercial forest plantations are an alternative for reducing the pressure on natural forests and creating options for sustainable and diversified development, changing the land use from fragmented agricultur and livestock purposes to forestry. The object of this study was to determine potential areas for the establishment of commercial forest plantations of Tabebuia rosea (Bertol.) DC. in the state of Michoacan, Mexico, by using geographic information systems. The outlining of potential areas was performed with the IDRISI 32 and ArcView programs, according to the environmental requirements of T. rosea. These environmental requirements include precipitation, temperature, soil, altitude, land use and slope. The analysis resulted in two maps where the potential areas for the establishment of commercial forest plantations with this species were shown, which turned out a total potential area of 54,320 ha, distributed in 47,566 ha considering a slope of 0% - 15% for non-manual forest plantations, and 6754 ha considering a slope of 15% - 30% for manual forest plantations. The ecologic conditions in Michoacan are favorable for successful establishment and development of commercial forest plantations of T. rosea.

Classification method is a formula, logical description generalizing characteristics of objects of related area. Nowadays, billions of smart objects are immersed in the environment, sensing, interacting, and cooperating with each other to enable efficient services. When we think about IoT, classification is a major challenge particularly if our technology is international level applicable. So, this limitation needs clear and deep analysis of the existing classification matrixes and gives some future directions depending on the different researches in the area. The paper surveys the current state-of-art in the classification of IoT. First, we try to explain commonly existing classification matrixes;Second, cooperation of different methods defending on classification matrixes used. Then analyses challenges that IoT faced from classification angle and finally we give some direction for future IoT classification.

＜正＞ Multiple alphal-adrenoceptors seem to contract rat tail artery. AlphalA- predominates and either alpha1B-,alpha1D-,or alpha1L-is the second population. We characterized alphal-adrenoceptors with selective agonists/antagonists in tail artery of young Wistar rat. Tail artery was exposed to A61603 （N-[5-（4.5- dihy-dro- 1H- imidazol - 2 - yl） - 2 - hydroxy - 5. 6. 7. 8 - tetrahydronaphthalen- 1 - yl] methanesulfonamide） or to phenylephrine （alphalA- and alphal- agonists ）, and to prazosin （alphal- antagonist）, the alphal A-antagonists 5 - methy-lurapidil,RS 100329 （5-methyl - 3 - [3 - [4 - [2 - （2,2,2,-trifluoroethoxy） phenyl] - 1-piperazinyl] propyl] - 2,4-（1H）-pyrimidinedione）,RS 17053 （N- [2（2-cyclopropylmethoxy） ethyl] - 5-chloro-a, a-dimethyl - 1H-indole - 3-ethylamine）, and the alpha1D-antagonist BMY 7378 （8-[2-[4-（2-methoxyphenyl） - 1-piperazinyl] ethyl] - 8 - azaspiro[4.5] decane - 7,9 -dione） . A61603 showed 100 fold higher affinity over PHE. Prazosin,RS100329,5-MU,and RS17053 displaced agonists with high affinity indicating alphal A-adrenoceptors while PHE activated alphal B-adrenoceptors since BMY 7378 had low affinity.

AIM: To determine if efforts to improve antiretroviral therapy(ART) adherence minimizes the negative impact of depression on human immunodeficiency virus(HIV) outcomes. METHODS: A cross-sectional study of a clinic-based cohort of 158 HIV seropositive(HIV+) African Americans screened for major depressive disorder(MDD) in 2012. CD4 T lymphocyte(CD4+) counts were obtained from these individuals. Self-report on adherence to ART was determined from questionnaire administered during clinic visits. The primary outcome measure was conditional odds of having a poorer CD4+ count( 350 cells/mm^3 CD4+ T lymphocyte endorsed significantly greater ART adherence compared to the group with < 350 cells/mm3 CD4+ T lymphocyte count(P < 0.004). Prevalence of MDD was 39.5% and 66% of individuals with MDD took antidepressants. Poor CD4+ T lymphocyte count was associated with poor ART adherence and MDD. Adjusting for ART adherence, age, sex and education, which were potential confounders, the association between MDD and poor CD4+ T lymphocyte remained significant only in the untreated MDD ***: Therefore, CD4+ count could be a clinical marker of untreated depression in HIV+. Also, mental health care may be relevant to primary care of HIV+ patients.
