Seminal vesicle （SV） amyloidosis is a well-documented histological entity, but it is observed infrequently. Its incidence is on the rise, which is probably related to the increasing use of prostate biopsies to investigate patients with elevated serum prostate-specific antigen levels. Here, we report seven cases of incidental SV amyloidosis over a 3-year period and consider their relationship to the previously suggested aetiological factors. Based on our series, we conclude that incidental localized SV amyloidosis observed in diagnostic prostate biopsies does not warrant formal investigations for systemic amyloidosis.

Lack of investment for magnetic resonance(MR)fusion systems is an obstacle to deliver targeted prostate biopsies within the prostate cancer diagnostic *** developed a coordinate-based method to support cognitive targeted prostate biopsies and then performed an audit on cancer detection and the location of *** each patient,the prostate is considered as two separate hemiprostates,and each hemiprostate is divided into 4×4×4 *** unit is therefore defined by a three-dimensional *** prospectively applied our coordinates approach to target 106 prostatic lesions in 93 *** 45(of 106;42.5%)lesions positive for cancer,27 lesions(60.0%)harbored clinically significant *** density was significantly higher in patients with proven cancer(median:0.264 ng ml^(-2))when compared to the noncancer group(median:0.145 ng ml-2;P=0.003,Wilcoxon ranksum test).Lesions with Prostate Imaging-Reporting and Data System(PIRADS)score of 5 were found to have a cancer incidence of 65.2%,while PI RADS 4 and 3 lesions have a lower risk of cancer detection,as expected,at 37.3%and 31.3%,*** probability of a lesion being cancerous in our series significantly decreases as we go from the“apex-to-base”dimension(odds ratio[OR]:2.62,95%confidence interval[Cl]:1.55-4.44,P=0.00034).Our analysis also indicates that the probability of cancer decreases as the prostate volume increases(OR:1.03,95%Cl:1.01-1.05,P=0.00327).Based on this feasibility study,the use of coordinates to guide cognitive targeted prostate biopsies warrants future validation study in additional centers.

Objective： To further investigate the in vitro effects of an osteoprotective herbal formula ＂ELP＂（Herba Epimedii, Fructus Ligustri Lucidi and Fructus Psora/eae） using seropharmacological approach. Methods： Rats were fed with ELP or its individual component herbs for 2 days. The serum containing the post-absorbed ingredients of the herbal items were collected for cell culture using UMR106 cell, RAW264.7 cell and mesenchymal stem cell （MSC） isolated from the bone marrow of the rats. The effects of the herbal-containing serum on cell toxicity were detected by 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium bromide assay; bromodeoxyuridine assay was conducted to measure the cell proliferation of UMR106 cell and MSC; cell activity was measured using colorimetric method, and mRNA expression of runt-related transcription factor 2 （Runx2）, alkaline phosphatase （ALP） and osteopontin （OPN） of UMR106 and MSC as well as matrix metalloproteinase 9 （MMP-9）, tartrate-resistant acid phosphatase （TRAP） and cathepsin K of RAW264.7 were analyzed using real-time reverse-transcription polymerase chain reaction. Results： ELP and its component serum exhibited no cytotoxic effects on the cells. The ELP-containing serum increased the proliferation of UMR106 cell and MSC by 25.7% and 14.4%, respectively and the alkaline phosphatase activity of MSC was increased by 42.6%. On the contrary, it inhibited the RAW264.7 cell differentiation by 29.2%. ELP serum upregulated the Runx2 expression of UMR and MSC by 1.18 fold and 1.27 fold, respectively. It also upregulated ALP and OPN expression in MSC by 1.69- and 2.12-fold, respectively. On the other hand, ELP serum down-regulated MMP-9 and cathepsin K expression of RAW264.7 cell by 0.46- and 0.36-fold, respectively. Conclusions： The serum of the animals fed with ELP contains active ingredients which are effective in promoting osteogenesis and inhibiting osteoclastogenesis.

目的：本研究观察针刺在治疗原发性失眠中是否具有真正的临床效应或它只是安慰剂的另一种形式,做出单纯针刺和非穿透性的假针刺比较。通过观察患者治疗前后的症状评估是否得改善的情况下, 为针灸疗效提供进一步的证明和确认。方法：使用随机对照的实验方法, 将符合纳入表准的60例失眠患者按照1：1的比例随机分为常规体针针刺的治疗组与非穿刺假针刺的对照组。要求参与者在治疗的一个月前没有接受任何的针灸治疗, 选择的参与者都是进一年内很少接触或从未接受过针灸的参与者。两组失眠患者每周治疗的次数为2-3次, 每次间隔的时间为至少2天完成10次治疗为1个疗程。常规针刺治疗组主要选的穴位是内关、神门、百会、照海、申脉和三阴交;非穿刺假针刺对照组会在非经络穴位上进行运作。治疗结果是在治疗前后由匹兹堡睡眠质量评分量表衡量而做比较。结果：60位参与者当中,没有参与者脱落退出研究。共有60为患者完成治疗而被列入统计分析1.治疗前：有基线数据上两组间无统计学差异(P＞0.05),也在匹兹堡睡眠质量评分量表(PSQI)分数的差异,使得两组患者具有可比性。2.将常规针刺组和非穿刺假刺组治疗前后的匹兹堡睡眠质量评分量表的总分和其中的六项项目,睡眠时间、入睡时间、睡眠质量、睡眠效率、睡眠障碍、日常功能障碍积分进行组内比较, 经过t检验分析, P＜0.05, 差异有统计学意义。在治疗组的治疗前各成分和总分均比治疗后成分高, 研究显示常规针刺的治疗组对失眠在匹兹堡睡眠质量评分量表中的成分都有较好的改善作用。在对照组里,治疗前各成分,除了睡眠效率和睡眠障碍的部分,和总分均比治疗后成分高, 研究显示非穿刺假针的治疗组对失眠在匹兹堡睡眠质量评分量表中的成分也有部分的改善作用。对照组的匹兹堡睡眠质量评分量表治疗后的总分比对照组高, 而在各成分的积分经组间比较治疗组的疗效也优于对照组。3.观察的60例患者中,常规体刺针灸治疗组的临床总有效率为90%：9例治愈,13例显著改善,5例显效,和3例无效总30例患者;非穿刺假针灸的有效率为73.33%：6例治愈,7显著改善,9显效,和8例无效总30例患者.结论：在本临床研究结果发现, 常规针刺组和假针刺对照组对失眠的疗效都取得一定的效果, 都能一定的程度上改善失眠患者的匹兹堡睡眠质量评分量表的总分。治疗组在在治疗后的各成分积分上都比对照组优胜。虽然对照组大部分在治疗后的各成分评分都有好转, 却在睡眠效率和睡眠障碍上不没有改善。针灸是一个非药物治疗失眠的方法, 不但没有依赖性或耐药性的副作用, 也不会对人体造成严重的不良反应, 而且也是一个可以长期使用的一种治疗放法。

目的探讨儿童神经精神性狼疮（NPSLE）的发病有无相关因素或预测指标。方法回顾性分析1990--2006年诊断为SLE的62例中国儿童系统性红斑狼疮（SLE）患者的临床资料，分别比较有NPSLE组和无NPSLE组、早发NPSLE组和晚发NPSLE组、同组患者诊断NPSLE时和诊断SLE时等的临床资料。结果有NPSLE组在诊断SLE时LN存在率、尿检异常率和尿蛋白定量（24h）值明显低于无NPSLE组；早发NPSLE组的平均年龄明显小于晚发NPSLE组，SLE疾病活动指数（SLEDAI）明显高于晚发NPSLE组；晚发NPSLE组在诊断NPSLE时和诊断SLE时临床表现和实验室检查等各项指标差异均无统计学意义。结论在诊断SLE时存在狼疮肾炎（LN）的患者发生NPSLE概率降低；早发NPSLE大多年龄小，疾病活动性高；没有临床指标可以预测NPSLE的发生。

Antralization at the edge of proximal gastric ulcers:Does Helicobacter pylori infection play a role?

AIM: To determine the prevalence of antralization at the edge of proximal gastric ulcers, and the effect ofH. pylori eradication on the mucosal appearances.METHODS: Biopsies were taken from the antrum, body and the ulcer edge of patients with benign proximal gastric ulcers before and one year after treatment. Gastric mucosa was classified as antral, transitional or body type.H. pylori positive patients receivedeither triple therapy, or omeprazole.RESULTS: Patients with index ulcers in the incisura, body or fundus (n=116) were analyzed. Antral-type mucosa was more prevalent at the ulcer edge inH. pylori-positive patients ***-negative patients (93% vs 60%, OR=8.95,95%CI: 2.47-32.4, P=0.001). At one year, there was a significant reduction in the prevalence of antralization (from 93 % to 61%, P=0.004) at the ulcer edge in patients with H. pyloribeing eradicated. However, there was no difference in the prevalence of antralization at the ulcer edge in those with persistent infection.CONCLUSION: H. pylori infection is associated with antralization at the edge of proximal gastric ulcers, which may be reversible in some patients after eradication of the infection.

Objective：This study was to investigate the variables in bone marrow harvesting procedure and individual donor factors which can potentially affect the yield of mesenchymal stromal cells（MSC）.Methods：We determined the yield of MSC from bone marrow under different clinical conditions by comparing the MSC colony numbers from：（1） donors of different ages;（2） healthy donors and patients with leukemia;（3） bone marrow aspirated at different time points during marrow harvesting;（4） bone marrow harvested by different ***：During the process of harvesting,the number of MSC significantly decreased with increase number of aspiration,from 675/ml at the initial decreased to 60/ml after 100 ml bone marrow aspirated,and 50/ml after 200 ml bone marrow *** number of MSC retrieved from leukemia patients（99/ml bone marrow） was significantly lower than that of healthy donors（708/ml bone marrow）.However,there was no significant difference in growth *** was no significant age-related difference of MSC yielded from donors 55 *** there was no significant difference in MSC number between the samples from single end-holed needle and those from multiple-side-hole ***：The optimal bone marrow samples for MSC collection should be obtained earlier in the process of harvesting *** marrow from donors 55 years was equally good as MSC *** autologous MSC from leukemia patients can be utilized for in-vitro MSC expansion.
