Upon glucose elevation, pancreatic beta-cells secrete insulin in a Ca2＋-dependent manner. In diabetic animal models, different aspects of the calcium signaling path- way in beta-cells are altered, but there is no consensus regarding their relative contributions to the development of beta-cell dysfunction. In this study, we compared the increase in cytosolic Ca2＊（[Ca2＊]~） via Ca2＋ influx, Ca2＊ mobilization from endoplasmic reticulum （ER） calcium stores, and the removal of Ca2＋ via multiple mechanisms in beta-cells from both diabetic db/db mice and non- diabetic C57BL/6J mice. We refined our previous quan- titative model to describe the slow [Ca2＋]i recovery after depolarization in beta-cells from db/db mice. According to the model, the activity levels of the two subtypes of the sarco-endoplasmic reticulum Ca2＋-ATPase （SERCA） pump, SERCA2 and SERCA3, were severely down-reg- ulated in diabetic cells to 65% and 0% of the levels in normal cells. This down-regulation may lead to a reduc- tion in the Ca2＋ concentration in the ER, a compensatory up-regulation of the plasma membrane Na＋/Ca2＋ exchanger （NCX） and a reduction in depolarizationevoked Ca2＋ influx. As a result, the patterns of glucosestimulated calcium oscillations were significantly different in db/db diabetic beta-cells compared with normal cells. Overall, quantifying the changes in the calcium signaling pathway in db/db diabetic beta-cells will aid in the development of a disease model that could provide insight into the adaptive transformations of beta-cell function during diabetes development.

High Frequency Transcricoid Jet Ventilation Prior to Neck Surgery—A 10-Year Retrospective Case Series Analysis

Difficult airway is still a challenging field in anesthesia, emergency departments and intensive care unit and is the cause of severe patient injury such as irreversible hypoxic brain damage or even death. Therefore, safer techniques are needed to maintain the airway in patients, who are difficult to intubate. The aim was to analyze the safety of the technique of transcricoid high frequency jetventilation (TCHFJV) by the investigation of complications. We performed a 10-year retrospective analysis (1/2009-1/2019) of patients subjected to TCHFJV at the hospital of the Ludwigs-Maximilians-University, Munich, Germany. TCHFJV was applied before anesthesia induction in awake, spontaneously breathing patients, with suspected or known difficult airway, scheduled for head and neck surgery. During the 10-year study period, we identified 39,477 patients, who underwent neck surgery. Of these, 1489 (3.8%) patients were managed with TCHFJV (1090 males, 399 females;62 ± 13 years, BMI 25 ± 0.2 kg?m-2, ASA class 3 ± 0.5). TCHFJV was successful in 1479 (99%) patients. Subsequent endotracheal intubation was performed in most cases (93%). Occasionally, emergency cricothyroidotomy (n = 2) or tracheotomy (n = 6) was necessary following TCHFJV. TCHFJV-related complications occurred in 1.5% of the cases. A majority (83%) of the complications were judged to be non-severe (grade 1, 2A) and 17% were severe (grade 3B), while there were none that were very severeC or lethalD. This retrospective study indicates that TCHFJV in experienced hands is a relatively safe alternative airway management strategy for patients undergoing neck surgery.

Purpose: Assessment of contractile function is a major challenge in patients with left ventricular dysfunction, especially during cardiac surgery. The initial tangent of the femoral arterial pressure increase (tanin) has recently been described to be an estimate of left ventricular (LV) contractility. To confirm these findings tanin was compared to various indices of LV performance in patients undergoing cardiac surgery. Methods: Data from 17 patients were evaluated retrospectively. Myocardial performance was estimated by the echocardiographic indices ejection fraction (EF), shortening fraction (FS), circumferential fiber shortening velocity (Vcf), the parameters of pulse contour analysis area under the curve (AUC) and tanin. Measurements were taken before and after cardiopulmonary bypass (CPB). Results: Tanin increased significantly (813 ± 216 mmHg/s vs. 1490 ± 450 mmHg/s, p < 0.05) after CPB, as well as Vcf (0.89 ± 0.14 circ/s vs. 1.47 ± 0.27 circ/s, p < 0.05) and EF (65% ± 7% vs. 74% ± 6%, p < 0.05). FS did not change (40.7% ± 7% vs. 46.5% ± 5%, p = 0.30). AUC significantly dropped after CPB (435 ± 54 mmHg*s vs. 263 ± 27 mmHg*s). Tanin and Vcf correlated strongly (r = 0.70, p < 0.001), while tanin showed only weak correlation with EF (r = 0.36, p = 0.037). There was no significant correlation with FS (r = 0.31, p = 0.079). Tanin and AUC correlated inversely (r = -0.62, p < 0.001). Conclusions: While showing little or no correlation with EF and FS respectively, tanin correlated well with the less preload-dependent parameter Vcf, thus suggesting that tanin may be used as an easily accessible estimate of LV contractility during cardiac surgery.
