Climate change with increasing temperature is making a significant impact on human health, including more heat-related diseases, and increasing the burden on the healthcare system. Although many studies have explored the association between increasing temperatures and negative health outcomes, research on the associated costs of heat-related diseases remains relatively sparse. Furthermore, estimations of future costs associated with heat-attributable hospital healthcare have not been well explored. This study used a distributed lag nonlinear model to estimate heat-attributable hospital healthcare costs in Perth, Western Australia. Using 2006–2012 as the baseline, future costings for 2026–2032 and 2046–2052 were estimated under RCP2.6, RCP4.5, and RCP8.5. Higher temperatures were found to be associated with increased hospital healthcare costs. The total hospital costs attributable to heat over the baseline period 2006–2012 was estimated to be 79.5 million AUD, with costs for mental health hospitalizations being the largest contributor of the heat-related conditions examined. Costs are estimated to increase substantially to 125.8–129.1 million AUD in 2026–2032, and 174.1–190.3 million AUD by midcentury under climate change scenarios. Our findings of a notable burden of heat-attributable healthcare costs now and in the future emphasize the importance of climate change adaptation measures to reduce the adverse health effects of increasing temperatures and heat exposure on the people of Perth.

AIM:To evaluate pre-lung transplant acid reflux on p H-testing vs corresponding bolus reflux on multichannel intraluminal impedance(MII) to predict early allograft ***:This was a retrospective cohort study of lung transplant recipients who underwent pretransplant combined MII-p H-testing at a tertiary care center from January 2007 to November 2012. Patients with pre-transplant fundoplication were excluded. Time-to-event analysis was performed using a Cox proportional hazards model to assess associations between measures of reflux on MII-p H testing and early allograft injury. Area under the receiver operating characteristic(ROC) curve(c-statistic) of the Cox model was calculated to assess the predictive value of each reflux parameter for early allograft injury. Six p H-testing parameters and their corresponding MIImeasures were specified a priori. The p H parameters were upright,recumbent,and overall acid reflux exposure; elevated acid reflux exposure; total acid reflux episodes; and acid clearance time. The corresponding MII measures were upright,recumbent,and overall bolus reflux exposure; elevated bolus reflux exposure; total bolus reflux episodes; and bolus clearance ***:Thirty-two subjects(47% men,mean age:55 years old) met the inclusion criteria of the study. Idiopathic pulmonary fibrosis(46.9%) repres e n t e d t h e m o s t c o m m o n p u l m o n a r y d i a g n o s i s leading to transplantation. Baseline demographics,pre-transplant cardiopulmonary function,number of lungs transplanted(unilateral vs bilateral),and posttransplant proton pump inhibitor use were similar between reflux severity groups. The area under the ROC curve,or c-statistic,of each acid reflux parameter on pre-transplant p H-testing was lower than its bolus reflux counterpart on MII in the prediction of early allograft injury. The need to increase bio-oil yield from biomass and enhance its fuel properties has driven research into optimizing the pyrolysis *** study investigated the influence of three key process parameters—temperature,heating rate,and nitrogen flow rate—on the pyrolysis of elephant grass(Pennisetum purpureum)in a fixed-bed *** surface methodology was used to study the im-pact of the aforementioned variables on bio-oil yield to improve its production *** analysis of the biomass revealed 79.24 wt%volatile matter,14.22 wt%fixed carbon,and 5.86%ash,with ultimate analysis showing 45.44%carbon,5.59%hydrogen,and 40.95%*** high volatile matter content and favourable carbon and hydrogen percentages indicate that elephant grass is a vi-able energy source due to its potential for high bio-oil yield and energy *** resulting bio-oil exhibited a higher heating value of 20.9 MJ/kg,indicating its suitability for various heating applications.A second-order regression model was developed for bio-oil yield,with optimal conditions identified as a temperature of 550°C,a heating rate of 17°C/min,and a nitrogen flow rate of 6 ml/*** study achieved an optimal bio-oil yield of 59.03 wt%,and the model’s high R²value of 0.8683 from analysis of variance analysis confirmed its predictive *** research highlights elephant grass as a sustainable feedstock for bio-oil production,offering valuable insights into optimizing pyrolysis conditions to enhance bio-oil yield,thus advancing biofuel technology.

Dear Editor,Phaeochromocytoma of thelurinary bladder is a rare tumour that originates from chromaffin tissue of the sympathetic nervous system situated within the urinary bladder wall[1].These are tumours of the sympathetic nervous tissue and the symptom profile will depend on the secretory function[1].These account for less than 0.05%of all bladder tu-mours and less than 1%of all phaeochromocytoma[1].As phaeochromocytoma of the urinary bladder is such a rare condition,only limited literature is available to direct clinical decision making.

We report here how a heterotopic penetrating peptic ulcer progressed to cause small bowel obstruction in a pa- tient with multiple previous negative investigations. The clinical presentation, radiographic features and patho- logical findings of this case are described, along with the salient lessons learnt. The added value of wireless cap- sule endoscopy （WCE） in such circumstances is debated.
