Exteriorized colon anastomosis for unprepared bowel: An alternative to routine colostomy

AIm: To see the possibility of avoiding routine colostomy in patients presenting with unprepared bowel. mETHODS: The cohort is composed of 103 patients, of these, 86 patients presented as emergencies (self- inflected and iatrogenic colon injuries, stab wounds and blast injury of the colon, volvulus sigmoid, obstructing left colon cancer, and strangulated ventral hernia). Another 17 patients were managed electively for other colon pathologies. During laparotomy, the involved segment was resected and the two ends of the colon were brought out via a separate colostomy wound. One layer of interrupted 3/0 silk was used for colon anastomosis. The exteriorized segment was immediately covered with a colostomy bag. Between the 5th and 7th postoperative day, the colon was easily dropped into the peritoneal cavity. The defect in the abdominal wall was closed with interrupted nonabsorbable suture. The skin was left open for secondary closure. RESULTS: The mean hospital stay (± SD) was 11.5 ± 2.6 d (8-20 d). The exteriorized colon was successfully dropped back into the peritoneal cavity in all patients except two. One developed a leak from oesophago- jejunostomy and from the exteriorized colon. She subsequently died of sepsis and multiple organ failure (mOF). In a second patient the colon proximal to the exteriorized anastomosis prolapsed and developed severe serositis, an elective ileo-colic anastomosis (to the left colon) was successfully performed. CONCLUSION: Exteriorized colon anastomosis is simple, avoids the inconvenience of colostomy and can be an alternative to routine colostomy. It is suitable where colostomy is socially unacceptable or the facilities and care is not available.

Tailoring mXene Thickness and Functionalization for Enhanced Room‑Temperature Trace NO_(2) Sensing

In this study,precise control over the thickness and termination of Ti3C2TX mXene flakes is achieved to enhance their electrical properties,environmental stability,and gas-sensing *** a hybrid method involving high-pressure processing,stirring,and immiscible solutions,sub-100 nm mXene flake thickness is achieved within the mXene film on the *** control is achieved by defunctionalizing mXene at 650℃ under vacuum and H2 gas in a CVD furnace,followed by refunctionalization with iodine and bromine vaporization from a bubbler attached to the ***,the introduction of iodine,which has a larger atomic size,lower electronegativity,reduce shielding effect,and lower hydrophilicity(contact angle:99°),profoundly affecting *** improves the surface area(36.2 cm^(2) g^(-1)),oxidation stability in aqueous/ambient environments(21 days/80 days),and film conductivity(749 S m^(-1)).Additionally,it significantly enhances the gas-sensing performance,including the sensitivity(0.1119Ωppm^(-1)),response(0.2% and 23%to 50 ppb and 200 ppm NO_(2)),and response/recovery times(90/100 s).The reduced shielding effect of the–I-terminals and the metallic characteristics of mXene enhance the selectivity of I-mXene toward *** approach paves the way for the development of stable and high-performance gas-sensing two-dimensional materials with promising prospects for future studies.

In this paper,we investigate the existence of solutions and analyze the large-time behavior for Gurtin-maccamy population model involving conformable fractional *** a preliminary step,we construct a generic structure of the solution associated with our proposed model by utilizing some basic properties and tools of conformable fractional *** establish the existence of a unique solution of the given model with the given initial *** last,by using the upper and lower solutions for the characteristic equation,we define the upper and lower boundaries for the obtained solution and describe the large-time behavior of the total population.

BACKGROUND There is concern regarding potential long-term cardiotoxicity with systemic distribution of metals in total joint arthroplasty(TJA)*** To determine the association of commonly used implant metals with echocardiographic measures in TJA *** The study comprised 110 TJA patients who had a recent history of high chromium,cobalt or titanium *** underwent two-dimensional,three-dimensional,Doppler and speckle-strain transthoracic echocardiography and a blood draw to measure metal *** and sex-adjusted linear and logistic regression models were used to examine the association of metal concentrations(exposure)with echocardiographic measures(outcome).RESULTS Higher cobalt concentrations were associated with increased left ventricular end-diastolic volume(estimate 5.09;95%CI:0.02-10.17)as well as left atrial and right ventricular dilation,particularly in men but no changes in cardiac *** titanium concentrations were associated with a reduction in left ventricle global longitudinal strain(estimate 0.38;95%CI:0.70 to 0.06)and cardiac index(estimate 0.08;95%CI,-0.15 to-0.01).CONCLUSION Elevated cobalt and titanium concentrations may be associated with structural and functional cardiac changes in some *** studies are warranted to better understand the systemic effects of metals in TJA patients.

With technological advancements in 6G and Internet of Things(IoT), the incorporation of Unmanned Aerial Vehicles (UAVs) and cellularnetworks has become a hot research topic. At present, the proficient evolution of 6G networks allows the UAVs to offer cost-effective and timelysolutions for real-time applications such as medicine, tracking, surveillance,etc. Energy efficiency, data collection, and route planning are crucial processesto improve the network communication. These processes are highly difficultowing to high mobility, presence of non-stationary links, dynamic topology,and energy-restricted UAVs. With this motivation, the current research paperpresents a novel Energy Aware Data Collection with Routing Planning for6G-enabled UAV communication (EADCRP-6G) technique. The goal of theproposed EADCRP-6G technique is to conduct energy-efficient cluster-baseddata collection and optimal route planning for 6G-enabled UAV ***-6G technique deploys Improved Red Deer algorithm-based Clustering (IRDAC) technique to elect an optimal set of Cluster Heads (CH) andorganize these clusters. Besides, Artificial Fish Swarm-based Route Planning(AFSRP) technique is applied to choose an optimum set of routes for UAVcommunication in 6G networks. In order to validated whether the proposedEADCRP-6G technique enhances the performance, a series of simulationswas performed and the outcomes were investigated under different *** experimental results showcase that the proposed model outperformed allother existing models under different evaluation parameters.

Latest advancements made in the processing abilities of smartdevices have resulted in the designing of Intelligent Internet of Things (IoT)environment. This advanced environment enables the nodes to connect, collect, perceive, and examine useful data from its surroundings. Wireless multimedia Surveillance Networks (WmSNs) form a vital part in IoT-assistedenvironment since it contains visual sensors that examine the surroundingsfrom a number of overlapping views by capturing the images *** IoT devices generate a massive quantity of digital media, it is thereforerequired to save the media, especially images, in a secure way. In order toachieve security, encryption techniques as well as compression techniques areemployed to reduce the amount of digital data, being communicated overthe network. Encryption Then Compression (ETC) techniques pave a wayfor secure and compact transmission of the available data to prevent unauthorized access. With this background, the current research paper presentsa new ETC technique to accomplish image security in IoT *** proposed model involves three major processes namely, IoT-based imageacquisition, encryption, and compression. The presented model involves optimal Signcryption Technique with Whale Optimization algorithm (NmWOA)abbreviated as ST-NmWOA. The optimal key generation of signcryptiontechnique takes place with the help of NmWOA. Besides, the presented modelalso uses Discrete Fourier Transform (DFT) and matrix minimization (mm)algorithm-based compression technique. Extensive set of experimental analysis was conducted to validate the effective performance of the proposed *** obtained values infer that the presented model is superior in terms of bothcompression efficiency and data secrecy in resource-limited IoT environment.

Sleep plays a vital role in optimum working of the brain and the *** people suffer from sleep-oriented illnesses like apnea,insomnia,*** stage classification is a primary process in the quantitative examination of polysomnographic *** stage scoring is mainly based on experts’knowledge which is laborious and time ***,it can be essential to design automated sleep stage classification model using machine learning(mL)and deep learning(DL)*** this view,this study focuses on the design of Competitive multi-verse Optimization with Deep Learning Based Sleep Stage Classification(CmVODL-SSC)model using Electroencephalogram(EEG)*** proposed CmVODL-SSC model intends to effectively categorize different sleep stages on EEG ***,data pre-processing is performed to convert the actual data into useful ***,a cascaded long short term memory(CLSTm)model is employed to perform classification *** last,the CmVO algorithm is utilized for optimally tuning the hyperparameters involved in the CLSTm *** order to report the enhancements of the CmVODL-SSC model,a wide range of simulations was carried out and the results ensured the better performance of the CmVODL-SSC model with average accuracy of 96.90%.

mobile communication and the Internet of Things(IoT)technologies have recently been established to collect data from human beings and the *** data collected can be leveraged to provide intelligent services through different *** is an extreme challenge to monitor disabled people from remote *** is because day-to-day events like falls heavily result in *** a person with disabilities,a fall event is an important cause of mortality and post-traumatic ***,detecting the fall events of disabled persons in smart homes at early stages is essential to provide the necessary support and increase their survival *** current study introduces a Whale Optimization algorithm Deep Transfer Learning-DrivenAutomated Fall Detection(WOADTL-AFD)technique to improve the Quality of Life for persons with *** primary aim of the presented WOADTL-AFD technique is to identify and classify the fall events to help disabled *** attain this,the proposed WOADTL-AFDmodel initially uses amodified SqueezeNet feature extractor which proficiently extracts the feature *** addition,the WOADTLAFD technique classifies the fall events using an extreme Gradient Boosting(XGBoost)*** the presented WOADTL-AFD technique,the WOA approach is used to fine-tune the hyperparameters involved in the modified SqueezeNet *** proposedWOADTL-AFD technique was experimentally validated using the benchmark datasets,and the results confirmed the superior performance of the proposedWOADTL-AFD method compared to other recent approaches.
