Complex-valued neural networks(CVNNs)have shown their excellent efficiency compared to their real counterparts in speech enhancement,image and signal processing.Researchers throughout the years have made many efforts to improve the learning algorithms and activation functions of CVNNs.Since CVNNs have proven to have better performance in handling the naturally complex-valued data and signals,this area of study will grow and expect the arrival of some effective improvements in the future.Therefore,there exists an obvious reason to provide a comprehensive survey paper that systematically collects and categorizes the advancement of CVNNs.In this paper,we discuss and summarize the recent advances based on their learning algorithms,activation functions,which is the most challenging part of building a CVNN,and applications.Besides,we outline the structure and applications of complex-valued convolutional,residual and recurrent neural networks.Finally,we also present some challenges and future research directions to facilitate the exploration of the ability of CVNNs.

Dynamic ultrasonography for optimizing treatment position in superior mesenteric artery syndrome:Two case reports and review of literature

BACKGROUND Superior mesenteric artery(SMA)syndrome is a rare cause of duodenal obstruction by extrinsic compression between the SMA and the aorta(SMA-Ao).Although the left lateral recumbent position is considered effective in the treatment of SMA syndrome,individual variations in the optimal patient position have been noted.In this report,we present two elderly cases of SMA syndrome that exhibited rapid recovery due to ultrasonographic dynamic evaluation of the optimal position for each patient.CASE SUMMARY Case 1:A 90-year-old man with nausea and vomiting.Following diagnosis of SMA syndrome by computed tomography(CT),ultrasonography(US)revealed the SMA-Ao distance in the supine position(4 mm),which slightly improved in the lateral position(5.7–7.0 mm)without the passage of duodenal contents.However,in the sitting position,the SMA-Ao distance was increased to 15 mm accompanied by improved content passage.Additionally,US indicated enhanced passage upon abdominal massage on the right side.By day 2,the patient could eat comfortably with the optimal position and massage.Case 2:An 87-year-old woman with vomiting.After the diagnosis of SMA syndrome and aspiration pneumonia by CT,dynamic US confirmed the optimal position(SMA-Ao distance was improved to 7 mm in forward-bent position,whereas it remained at 5 mm in the supine position).By day 7 when her pneumonia recovered,she could eat with the optimal position.CONCLUSION The optimal position for SMA syndrome varies among individuals.Dynamic US appears to be a valuable tool in improving patient outcomes.

BACKGROUND Most species of aconite contain highly toxic aconitines,the oral ingestion of which can be fatal,primarily because they cause ventricular arrhythmias.We describe a case of severe aconite poisoning that was successfully treated through venoarterial extracorporeal membrane oxygenation(VA-ECMO)and in which detailed toxicological analyses of the aconite roots and biological samples were performed using liquid chromatography-tandem mass spectrometry(LC-MS/MS).CASE SUMMARY A 23-year-old male presented to the emergency room with circulatory collapse and ventricular arrhythmia after ingesting approximately half of a root labeled,“Aconitum japonicum Thunb”.Two hours after arrival,VA-ECMO was initiated as circulatory collapse became refractory to antiarrhythmics and vasopressors.Nine hours after arrival,an electrocardiogram revealed a return to sinus rhythm.The patient was weaned off VA-ECMO and the ventilator on hospital days 3 and 5,respectively.On hospital day 15,he was transferred to a psychiatric hospital.The other half of the root and his biological samples were toxicologically analyzed using LC-MS/MS,revealing 244.3 mg/kg of aconitine and 24.7 mg/kg of mesaconitine in the root.Serum on admission contained 1.50 ng/mL of aconitine.Beyond hospital day 2,neither were detected.Urine on admission showed 149.09 ng/mL of aconitine and 3.59 ng/mL of mesaconitine,but these rapidly decreased after hospital day 3.CONCLUSION The key to saving the life of a patient with severe aconite poisoning is to introduce VA-ECMO as soon as possible.

Rare-earth silicates are promising environmental barrier coatings(EBCs)that can protect SiC_(f)/Si C_(m)substrates in next-genera tion gas turbine blades.Notably,RE_(2)Si_(2)O_(7)(RE=Yb and Ho)shows potential as an EBC due to its coefficient of thermal expansion(CTE)compatible with substrates and high resistance to water vapor corrosion.The target operating temperature for next-generation tur bine blades is 1400°C.Corrosion is inevitable during adhesion to molten volcanic ash,and thus,understanding the corrosion behavior o the material is crucial to its reliability.This study investigates the high-temperature corrosion behavior of sintered RE_(2)Si_(2)O_(7)(RE=Yb and Ho).Samples were prepared using a solid-state reaction and hot-press method.They were then exposed to volcanic ash at 1400°C for 224,and 48 h.After 48 h of exposure,volcanic ash did not react with Yb_(2)Si_(2)O_(7)but penetrated its interior,causing damage.Meanwhile Ho_(2)Si_(2)O_(7)was partially dissolved in the molten volcanic ash,forming a reaction zone that prevented volcanic ash melts from penetrating the interior.With increasing heat treatment time,the reaction zone expanded,and the thickness of the acicular apatite grains increased The Ca:Si ratios in the residual volcanic ash were mostly unchanged for Yb_(2)Si_(2)O_(7)but decreased considerably over time for Ho_(2)Si_(2)O_(7).Th Ca in volcanic ash was consumed and formed apatite,indicating that RE^(3+)ions with large ionic radii(Ho>Yb)easily precipitated apatit from the volcanic ash.

In glaucoma,a leading cause of blindness,retinal ganglion cells are progressively damaged.Intraocular pressure reduction is the only established treatment for glaucoma(Collaborative Normal-Tension Glaucoma Study Group,1998;Vass et al.,2007).However,in some cases,visual field loss progresses despite sufficiently reduced intraocular pressure(Collaborative Normal-Tension Glaucoma Study Group,1998;Killer and Pircher,2018).While intraocular pressure and age are known risk factors for glaucoma(Ernest et al.,2013),the underlying mechanisms of glaucoma progression are not fully understood.

BACKGROUND Ultrasonic devices are widely used in many surgical fields,including hepatectomy;however,the negative effects of tissue pad degradation of ultrasonic devices,including those in liver surgery,remain unknown.The Harmonic®1100(H-1100)scalpel has advanced heat control technology than previous models,such as the Harmonic®HD1000i(H-HD1000i).We hypothesized that,because of its advanced temperature-control technology,the H-1100 scalpel would show less tissue pad degradation,resulting in superior sealing performance,compared to that with the H-HD1000i scalpel.AIM To elucidate ultrasonic device tissue pad degradation effects on instrument temperature and sealing performance using ex vivo porcine liver/vessel models.METHODS Two different harmonic scalpels were used and compared:A newer model,the H-1100 scalpel,and an older model,the H-HD1000i scalpel.Using ex vivo porcine livers,each instrument was activated until the liver parenchyma was dissected.The device temperature(passive jaw temperature)was measured after every 10 consecutive activations,until 300 transections of the porcine liver were performed.Tissue pad degradation was evaluated after 300 activations.Sealing performance was evaluated using excised porcine carotid vessels;vessel sealing speed and frequency of vessel burst pressure below 700 mmHg were determined after 300 transections of porcine liver parenchyma.RESULTS The temperature of the H-HD1000i scalpel was approximately 10℃higher than that of the H-1100 scalpel,and gradually increased as the number of activations increased.The median passive jaw temperature of the H-HD1000i scalpel was significantly higher than that of the H-1100 scalpel(73.4℃vs 65.1℃;P<0.001).After 300 transections of porcine liver parenchyma,less tissue pad degradation was observed with the H-1100 scalpel than with the H-HD1000i scalpel(0.08 mm vs 0.51 mm).The H-1100 scalpel demonstrated faster vessel-sealing speed(4.9 sec.vs 5.1 sec.)and less frequent vessel burst pressure<700 mmHg(0%vs 40%)after 300 activations than the H-HD1

BACKGROUND Lactose intolerance(LI)is commonly seen in East Asian countries.Several studies showed that lactose or milk loading has been used as a treatment for lactose malabsorption(LM)in Western countries,but there have been no reports regarding this type of treatment in Japan.As lactose or milk loading requires ingestion of large amounts of lactose within a short period,this is considered to be too harsh for Japanese people because of their less habitual milk consumption(175 mL per day in average)than Western people.In this study,we demonstrated lactose tolerance acquisition in a suitable way for Japanese.AIM To examine the efficacy of lactose(cow’s milk)loading treatment in patients with LM.METHODS Individuals with abdominal symptoms induced by milk or dairy products(LI symptoms)were identified with a questionnaire.A 20 g lactose hydrogen breath test(LHBT)was carried out to confirm LM diagnosis and to evaluate co-existence of small intestinal bacterial overgrowth(SIBO).Respondents diagnosed with LM were selected as study subjects and were treated with incremental loads of cow’s milk,starting from 30 mL and increasing up to 200 mL at 4-7 d intervals.After the treatment,changes in symptoms and LM diagnostic value of 20 g LHBT were investigated.Stool samples pre-and post-treatment were examined for changes in intestinal microbiota using 16S rRNA sequencing.Informed consent was obtained prior to each stage of the study.RESULTS In 46 subjects with LI symptoms(10-68 years old,mean age 34 years old)identified with the questionnaire,35(76.1%)were diagnosed with LM by 20 g LHBT,and 6 had co-existing SIBO.The treatment with incremental cow’s milk was carried out in 32 subjects diagnosed with LM(14-68 years old,median age 38.5 years old).The mean period of the treatment was 41±8.6 d.Improvement of symptoms was observed in 29(90.6%;95%confidence interval:75.0%-98.0%)subjects.Although 20 g LHBT indicated that 10(34.5%)subjects had improved diagnostic value of LM,no change was observed in 16(55.2%)subjects.Analysis of the f

Hepatobiliary pancreatic surgeons sometimes encounter an aberrant(or replaced)right hepatic artery(a-RHA,Hiatt types III-VI)during pancreaticoduodenectomy(PD).Arterial flow cutoff to the a-RHA may be associated with a biliary leak or stenosis at the choledocho-jejunostomy and liver ischemia,resulting in ischemic cholangitis,liver abscess,and sepsis(1).
