Risk factors for bleeding after endoscopic submucosal dissection for gastric lesions

AIM:To assess risk factors for bleeding after gastric endoscopic submucosal dissection(ESD) and to develop preventive ***:This retrospective study was performed in a tertiary referral center.A total of 328 patients underwent ESD for 398 gastric neoplasms between July 2007 and June *** main outcome was association between post-ESD bleeding and the following:age;sex;comorbidities;daily use of medicine potentially related to gastric injury/bleeding;location,size,and histological depth of lesions;ulceration;experience of operator coagulating the ulcer floor,and duration of *** also determined the relationship between the location of post-ESD bleeding and risk factors for ***:Univariate analysis revealed significant risk factors:tumor location [odds ratio(OR),2.86;95% CI:1.21-6.79,P=0.024],coagulator experience(OR,4.29;95% CI:1.43-12.86,P=0.009),and medicine potentially related to gastric injury/bleeding(OR,2.80;95% CI:1.14-6.90,P=0.039).Multivariate logistic regression analysis confirmed significant,independent risk factors:tumor in lower third of stomach(OR,2.47;95% CI:1.02-5.96,P=0.044),beginner coagulator(OR,3.93;95% CI:1.29-11.9,P=0.016),and medicine(OR,2.76;95% CI:1.09-6.98,P=0.032).We classif ied cases of post-ESD bleeding into two groups(bleeding at the ulcer margin vs bleeding at the center) and found that bleeding at the margin occurred more frequently with beginner coagulators compared with experts(OR,16.00;95% CI:1.22-210.59,P=0.040).CONCLUSION:Beginner coagulators,tumor in the antrum,and medicines were significant risk factors for post-ESD *** at the ulcer margin frequently occurred with beginner operators.

Prospective controlled study on the effects of polyethylene glycol in capsule endoscopy

Gender differences of low-dose aspirin-associated gastroduodenal ulcer in Japanese patients

AIM:To clarify the gender differences about the clini-cal features and risk factors of low-dose aspirin (LDA) (81-100 mg daily)-associated peptic ulcer in Japanese ***: There were 453 patients under treatment with LDA (298 males, 155 females) who underwent esophagogastroduodenoscopy at the Department of Gastroenterology and Hepatology of Hiratsuka City Hospital between January 2003 and December 2007. They had kept taking the LDA or started treatmentduring the study period and kept taking LDA during the whole period of observation. Of these, 119 patients (87 males, 32 females) were diagnosed as having LDA-associated peptic ulcer. We examined the clinical factors associated with LDA-associated peptic ulcer in both ***: A history of peptic ulcer was found to be the risk factor for LDA-associated peptic ulcer common to both sexes. In female patients, age greater than 70 years (prevalence ORs 8.441, 95% CI: 1.797-33.649, P = 0.0069) was found to be another significant risk fac-tor, and the time to diagnosis as having LDA-associat-ed peptic ulcer by endoscopy was significantly shorter than that in the male patients (P = 0.0050). CONCLUSION: We demonstrated gender differences about the clinical features and risk factors of LDA-asso-ciated peptic ulcer. Special attention should be paid to aged female patients taking LDA.

Light-emitting diodes(LEDs)based on perovskite semiconductor materials with tunable emission wavelength in visible light range as well as narrow linewidth are potential competitors among current light-emitting display technologies,but still suffer from severe instability driven by electric ***,we develop a stable,efficient and highcolor purity hybrid LED with a tandem structure by combining the perovskite LED and the commercial organic LED technologies to accelerate the practical application of *** LED and organic LED with close photoluminescence peak are selected to maximize photon emission without photon reabsorption and to achieve the narrowed emission *** designing an efficient interconnecting layer with p-type interface doping that provides good opto-electric coupling and reduces Joule heating,the resulting green emitting hybrid LED shows a narrow linewidth of around 30 nm,a peak luminance of over 176,000 cd m^(−2),a maximum external quantum efficiency of over 40%,and an operational half-lifetime of over 42,000 h.

Pancreatic paraganglioma with draining vessels

A pancreatic paraganglioma is a rare neoplasm that is difficult to distinguish from a pancreatic neuroendocrine tumour. Here we present a case of pancreatic paraganglioma that was surgically resected following preoperative diagnosis of a pancreatic neuroendocrine tumour. Careful evaluation of the endoscopic ultrasonography findings revealed abundant draining ves-sels,which could have led to a correct preoperative diagnosis of pancreatic paraganglioma.

A preliminary study on rectal dose reduction associated with hyaluronic acid implantation in brachytherapy for prostate cancer

Objectives: Hydrogel spacer (HS) was developed to reduce rectal toxicities caused by radiotherapy, but has been reported to cause major adverse events. Our institute has attempted to introduce a hyaluronic acid (HA) as an alternative spacer. This study aimed to compare rectal doses and geometric distributions between the HS and HA implantation in prostate ***: HS and HA were inserted in 20 and 18 patients undergoing high-dose brachytherapy, respectively. The rectum spacer volumes injected were 10 mL and 22 mL, respectively. In the treatment planning system, 13.5 Gy was administered with common catheter positions. The rectal dose indices were assessed between the spacer groups for dosimetry evaluation. Distances between the prostate and rectum and configurations of the spacers were ***: The mean doses irradiated to 0.1 and 2 mL of the rectum were 10.45 Gy and 6.71 Gy for HS, and 6.73 Gy and 4.90 Gy for HA (p<0.001). The mean minimum distances between the prostate and rectum were 1.23 cm and 1.79 cm for HS and HA, respectively (p<0.05). Geometrical configuration comparisons revealed that HA has a higher ability to expand the space than ***: The rectal dose reduction ability of HA is significantly greater than that of HS, suggesting its potential as a new spacer.

【目的】探讨醒斑银弄蝶Carterocephalus alcina Evans,1939与其近似种的关系。【方法】广泛采集醒斑银弄蝶及其近似种样本,基于形态特征、COI条形码及地理分布进行关系分析。【结果】黄斑银弄蝶*** Lee,1962应是醒斑银弄蝶***的个体变异,故为其新异名;产自北京的2个银弄蝶标本与其近似种间的最小平均遗传距离为3.9%,在系统树上独立成支,前翅中室基部和后翅前缘中部有斑且后翅M1-2室斑延伸至外缘,为其独有;醒斑银弄蝶、北京支系、银弄蝶***和黄翅银弄蝶***呈高度支持的单系支。【结论】黄斑银弄蝶是醒斑银弄蝶的新异名;产自北京的近似于醒斑银弄蝶的种群是一个新种,即长斑银弄蝶Carterocephalus longimaculatus Hou,Fan&Li ***.。

Direct measurement of the three-dimensional distribution of leaf area density and light conditions in a mature oak stand by the cube method

Although the distributions of foliage and light play major roles in various forest functions,accurate,nondestructive measurement of these distributions is difficult due to the complexity of the canopy *** evaluate the foliage and light distributions directly and nondestructively in a mature oak stand,we used the cube method by dividing the forest canopy into small cubes(50 cm per side)and directly measured leaf area density(LAD,the total one-sided leaf area per unit volume,i.e.,cube)and relative irradiance(RI)within each *** distribution of LAD and of RI was highly heterogeneous,even at the same canopy *** heterogeneity reflected the presence of foliage clusters associated with multiple forking *** relationship between cumulative LAD at the canopy surface and average RI followed the Beer-Lambert *** mean light extinction coefficient(K)was ***,K was overestimated by more than double(0.80)when calculated based on the classical method using RI at the forest *** overestimation was caused by the lower RI due to light absorption by nonleaf plant parts below the *** findings on the complex foliage and light distributions in canopy layers should help improve the accuracy of RI and K measurements and thus more accurate predictions of environmental responses and forest functions.

Effect of mechanical ball milling on the electrical and powder bed properties of gas-atomized Ti-48Al-2Cr-2Nb and elucidation of the smoke mechanism in the powder bed fusion electron b eam melting process

Smoke is unexpected powder-splashing caused by electrostatic force and is one of the main problems hindering the process stability and applicability of the powder bed fusion electron beam(PBF-EB)*** this study,mechanical stimulation was suggested to suppress smoke of gas-atomized(GA)Ti-48Al-2Cr-2Nb powder using Al_(2)O_(3) and WC ball *** deformation mechanism of the GA powder depending on the ball milling media was discussed based on the developed particle morphology distribu-tion map and contact mechanics *** was revealed that the rapid decrement of flowability and packing density after WC ball milling owing to the formation of angular fragments by the brittle *** variation of surface and electrical properties by mechanical stimulation was investigated via XPS,TEM,and Impedance *** electrical resistivity of the ball-milled powders gradually decreased with increasing milling duration,despite the increased oxide film thickness,and the capacitive response disappeared in Al-60 and WC-30 via metal-insulator *** could be due to the accumulation of strain and defects on the oxide film via mechanical *** smoke mechanism of ball-milled powders was discussed based on the percolation *** the smoke experiment,smoke was more suppressed for WC-10 and WC-20 than that for Al-40 and Al-50,respectively,despite the longer charge dissipation *** could be due to the high probability of contact with conductive *** the Al-60 and WC-30 powders,smoke was further restricted by the formation of a percolation cluster with metal-like electrical *** believe that this study will contribute to a better understanding of the smoke mechanism and process optimization of the PBF-EB.
