Atomic force microscopy （AFM） has the advantage of obtaining mechanical properties as well as topographic information at the same time. By analyzing force-distance curves measured over two-dimensional area using Hertzian contact mechanics, Young＇s modulus mapping was obtained with nanometer-scale resolution. Furthermore, the sample deformation by the force exerted was also estimated from the force-distance curve analyses. We could thus reconstruct a real topographic image by incorporating apparent topographic image with deformation image. We applied this method to carbon black reinforced natural rubber to obtain Young＇s modulus distribution image together with reconstructed real topographic image. Then we were able to recognize three regions; rubber matrix, carbon black （or bound rubber） and intermediate regions. Though the existence of these regions had been investigated by pulsed nuclear magnetic resonance, this paper would be the first to report on the quantitative evaluation of the interfacial region in real space.

Endotracheal Tube Displacement during Cardiac Surgery in Infants: A Retrospective Cohort Study of Its Incidence and Predictors

Introduction: With the traction on the trachea and intrathoracic manipulation during cardiac surgery, the position of the endotracheal tube (ETT) might be changed as compared to before surgery. Migration of the ETT during pediatric cardiac surgery is particularly problematic in infants. Methods: In this retrospective cohort study, chest X-rays were taken in the operating room just before and after surgery. The position of the ETT among all infants under 1 year of age who underwent cardiac surgery between December 2017 to December 2019 was evaluated. The displacement of the ETT position was examined by measuring the position of the tube tip from the tracheal bifurcation on a chest X-ray, and the relationship between surgery-related factors (age, height, weight, sex, surgery time, cardiopulmonary bypass, tube size, use of tube cuff) was analyzed. Results: Eighty-eight of the 141 patients were enrolled. There was a significant proximal displacement of the ETT tip during cardiac surgery. The distance from the carina to the tube tip after surgery was long, on average 2.5 mm, compared to that before surgery (P = 0.013). Cephalad displacement of the ETT either ≥5 mm or ≥2.5 mm was found in 28 and in 38 out of 88 infants after surgery, respectively. After performing multivariate analysis, the use of un-cuffed ETT was the sole exploratory variable predictive of tube tip displacement (OR 0.34, 95% CI 0.10 - 0.93 if ≥5 mm;and OR 0.24, 95% CI 0.08 - 0.75 if ≥2.5 mm displacement;P = 0.04 and 0.01, respectively). Conclusion: Proximal displacement of the ETT during cardiac surgery occurs more frequently in infants with un-cuffed ETT.

Optical fibre sensors based on Brillouin scattering have been vigorously studied in the context of structural health monitoring on account of their capacity for distributed strain and temperature measurements.However,real-time distributed strain measurement has been achieved only for two-end-access systems;such systems reduce the degree of freedom in embedding the sensors into structures,and furthermore render the measurement no longer feasible when extremely high loss or breakage occurs at a point along the sensing fibre.Here,we demonstrate real-time distributed measurement with an intrinsically one-end-access reflectometry configuration by using a correlation-domain technique.In this method,the Brillouin gain spectrum is obtained at high speed using a voltage-controlled oscillator,and the Brillouin frequency shift is converted into a phase delay of a synchronous sinusoidal waveform;the phase delay is subsequently converted into a voltage,which can be directly measured.When a single-point measurement is performed at an arbitrary position,a strain sampling rate of up to 100 kHz is experimentally verified by detecting locally applied dynamic strain at 1 kHz.When distributed measurements are performed at 100 points with 10 times averaging,a repetition rate of 100 Hz is verified by tracking a mechanical wave propagating along the fibre.Some drawbacks of this ultrahigh-speed configuration,including the reduced measurement accuracy,lowered spatial resolution and limited strain dynamic range,are also discussed.

Afferent loop obstruction(ALO)is defined as duodenal or jejunal mechanical obstruction at the proximal anastomosis site of a gastrojejunostomy.With advances in chemotherapy,the incidence of malignant ALO is increasing.Malignant ALO can be complicated by ischemia,gangrenous bowel,pancreatitis,and ascending cholangitis.Moreover,the general condition of patients with recurrent cancer is often poor.Therefore,accurate and rapid diagnosis and minimally invasive treatments are required.However,no review articles on the diagnosis and treatment of malignant ALO have been published.Through literature searching,we reviewed related articles published between 1959 and 2020 in the PubMed database.Herein,we present recent advances in the diagnosis and treatment of malignant ALO and describe future perspectives.Endoscopic transluminal self-expandable metal stent(SEMS)placement is considered the standard treatment for malignant ALO,as this procedure is well established and less invasive.However,with the development of interventional endoscopic ultrasound(EUS)in recent years,the usefulness of EUS-guided gastrojejunostomy has been reported.Moreover,through indirect comparison,this approach has been reported to be superior to transluminal SEMS placement.It is expected that a safer and less invasive treatment method will be established through the continued advancement and innovation of interventional endoscopy techniques.

A Probabilistic Liquefaction Hazard Assessment for Urban Regions Based on Dynamics Analysis Considering Soil Uncertainties

Earthquake induced liquefaction is one of the main geo-disasters threating urban regions, which not only causes direct damages to buildings, but also delays both real-time disaster relief actions and reconstruction activities. It is thus important to assess liquefaction hazard of urban regions effectively and efficiently for disaster prevention and mitigation. Conventional assessment approaches rely on engineering indices such as the factor of safety(FS) against liquefaction, which cannot take into account directly the uncertainties of soils. In contrast, a physics simulation-based approach, by solving soil dynamics problems coupled with excess pore water pressure(EPWP) it is possible to model the uncertainties directly via Monte Carlo simulations. In this study, we demonstrate the capability of such an approach for assessing an urban region with over 10 000 sites. The permeability parameters are assumed to follow a base-10-lognormal distribution among 100 model analyses for each site. A dynamic simulation is conducted for each model analysis to obtain the EPWP results. Based on over 1 million EPWP analysis models, we obtained a probabilistic liquefaction assessment. Empowered by high performance computing, we present for the first time a probabilistic liquefaction hazard assessment for urban regions based on dynamics analysis, which consider soil uncertainties.

Intracranial hemorrhage in patients treated with bevacizumab:Report of two cases

Treatment with bevacizumab,an antiangiogenic agent,in patients with metastatic or unresectable colorectal cancer was approved less than 4 years ago in Japan.Bevacizumab improves the survival of patients with metastatic colorectal cancer;however,it may lead to complications such as bleeding,which are sometimes fatal.Bevacizumab should be administered only after careful consideration because the potential risks of therapy outweigh its benefits.Therefore,pharma-ceutical companies do not recommend bevacizumab therapy for patients with brain metastases.While some reports support the cautious use of bevacizumab,others report that it is not always necessary to prohibit its use in patients with metastases to the central nervous system(CNS),including the brain.Thus,bevacizumab therapy in colorectal cancer patients with brain metastases is controversial,and it is unclear whether brainmetastases are a risk factor for intracranial hemor-rhage during anti-vascular endothelial growth factor(VEGF)therapy.We report a 64-year-old man and a 65-year-old man with recurrent colorectal cancer with-out brain metastases;these patients developed multifocal and solitary intracranial hemorrhage,respectively,after the administration of bevacizumab.Our findings suggest that intracranial hemorrhage can occur even if the patient does not have brain metastases prior to bevacizumab treatment and also suggest that brain metastases are not a risk factor for intracranial hemor-rhage with bevacizumab treatment.These findings also question the necessity of excluding patients with brain metastases from clinical trials on anti-VEGF therapy.

Application of Polymerizable Surfactant in the Preparation of Polystyrene/Nano-Fe_3O_4 Composite

Monooctadecyl maleate, as a polymerizable surfactant, was synthesized by the mono-esterification of maleic anhydride and octadecanol, and was utilized to surface-modify nano-Fe3O4 particles. A polymerizable magnetic fluid was obtained by directly dispersing modified nano-Fe3O4 particles into styrene monomer, and the polystyrene/nano-Fe3O4 composite was prepared through free radical polymerization of polymerizable magnetic fluid. The structure and dispersion status in different dispersion phases of modified nano-Fe3O4 particles were studied by Fourier transform infrared （FTIR） spectrometry, X-ray diffraction （XRD） and transmission electron microscopy （TEM）, respectively. The experimental results show that the nano-Fe3O4 particles modified by monooctadecyl maleate with the size of about 7-10 nm can be uniformly dispersed into styrene and fixed in the composite during the procedure of polymerization. Thermogravimetric analysis （TGA） and vibrating sample magnetometry （VSM） indicate that the thermal stability of polystyrene/nano-Fe3O4 composite is improved compared to that of pure polystyrene, and the composite is a sort of superparamagnetic materials.

Prospective controlled study on the effects of polyethylene glycol in capsule endoscopy

AIM:To prospectively confirm whether a small amount of polyethylene glycol(PEG)ingested after swallowing endoscopy capsule improves image quality and completion rate.METHODS:Forty-four consecutive patients referred to us for capsule endoscopy(CE)were randomized to two groups.All patients were restricted to clear fluids for 12 h before the examination.Patients in group A(22 cases)received no additional preparation,while those in group B(20 cases)ingested 500 mL of PEG within a 2 h period starting 30 min after swallowing the capsule.Clear fluids and meals were allowed 2 h and 4 h after capsule ingestion,respectively.Image quality was assessed as the percentage of visualized bowel surface area as follows:1:90%.The small bowel record was divided into five segments by time,and the score for each segment was evaluated.All CE examinations were performed with the Pillcam SB capsule endoscopy sys-tem(Given Imaging Co.Ltd.,Yoqnem).RESULTS:This study ended in December 2009,because sample size was considered large enough.A total of 44 patients were enrolled.Two patients in group B were excluded from the analysis because small bowel images could not be obtained from these patients;one had a full stomach,while the other presented with a massive gastric bleed.Thus,22 patients from group A and 20 patients from group B completed the study.There was no significant difference in age(P=0.22),sex(P=0.31),and indication for CE.No significant adverse events occurred in any of the study patients.In group A,image quality deteriorated as the capsule progressed distally.However,in group B,image quality was maintained to the distal small bowel.In each of the five segments,the visibility score was significantly higher in group B than in group A(segment 1:4.3± 0.7vs 4.7±0.5,P=0.03;segment 2:4.2±0.9vs 4.8 ±0.4,P=0.01;segment 3:4.0±1.0 vs 4.6±0.7,P =0.04;segment 4:3.6±1.1 vs 4.5±0.6,P=0.003;segment 5:2.7±1.0vs 4.4±0.8,P=0.00004).Thus,the use of PEG during CE examination significantly improved image quality

Response of Rice to Insect Elicitors and the Role of OsJAR1 in Wound and Herbivory-Induced JA-lle Accumulation

Plants produce jasmonic acid （JA） and its amino acid conjugate, jasmonoyI-L-isoleucine （JA-Ile） as major defense signals in response to wounding and herbivory. In rice （Oryza sativa）, JA and JA-Ile rapidly increased after mechanical damage, and this increase was further amplified when the wounds were treated with oral secretions from generalist herbivore larvae, lawn armyworms （Spodoptera mauritia）, revealing for the first time active perception mechanisms of herbivore-associated elicitor（s） in rice. In the rice genome, two OsJAR genes can conjugate JA and lie and form JA-Ile in vitro; however, their function in herbivory- induced accumulation of JA-Ile has not been investigated. By functional characterization of TOS17 retrotransposon-tagged Osjarl plants and their response to simulated herbivory, we show that OsJAR1 is essential for JA-Ile production in herbivore-attacked, field-grown plants. In addition, OsJAR1 was required for normal seed development in rice under field conditions. Our results suggest that OsJAR1 possesses at least two major functions in rice defense and development that cannot be complemented by the additional OsJAR2 gene function, although this gene previously showed overlapping enzyme activity in vitro.
